The Toronto experience suggests that there may be several general lessons for academic health sciences complexes to learn from the Olivieri/Apotex affair regarding the ethics, independence, and integrity of clinical research sponsored by for profit enterprises. From a local perspective, the OAA occurred when there already was a focus on the complex and changing relationships among the University of Toronto, its medical school, the fully affiliated teaching hospitals, and off campus faculty because of intertwined interests and responsibilities. The OAA became (...) a catalyst that accelerated various systemic reforms, particularly concerning academic/industry relations. In this article, the evolving governance framework for the Toronto academic health sciences complex is reviewed and these policy and process reforms discussed. These reforms have created collaborative activity among research ethics boards and contract research offices of the partner institutions, and allowed the joint university/hospital ethics centre to play a role in governance and policy, while respecting the missions and mandates of the involved institutions. Although few of the policies are dramatically innovative, what is arguably novel is the elaboration of an overarching governance framework that aims to move ethics to a central focus in the academic complex. Time alone will tell how sustainable and effective these changes are. (shrink)

Aim of the study was to analyze the posttraumatic stress disorder risk nurses, detecting the relationship between distress experience and personality dimensions in Italian COVID-19 outbreak. A cross-sectional study was conducted based on 2 data detection. Mental evaluation was carried out in Laboratory of Clinical Psychology on n.69 nurses in range age 22–64 years old. Measurement was focused on symptoms anxiety, personality traits, peritraumatic dissociation and post-traumatic stress for all participants. No online screening was applied. Comparisons within the various demographic (...) characteristics demonstrated few significant differences between groups on DASS-21, PDEQ, and ISE-R scores. Correlation analysis was performed among PDEQ, DASS-21, BFI-10 and IES-R and confirmed between anxiety and peritraumatic dissociation and post-traumatic stress; then anxiety is positively correlated to agreeableness variable of BFI-10 test. The emotional distress was protracted overtime but in long-term personality traits resulted mediator facing subjective stress. Our finding drew details for protective and predictive risk factors as well as mental health issues of nurses dealing with pandemic: healthcare workers faced the protracted challenge caring COVID-19 patients over and over again: in short time the impact was relevant, and the prolonged exposition to the stressor was tackled by personal resources such as personality traits. Par la justification rationnelle des réclamations, l'argumentation engage la responsabilité des sujets de droit, infléchissant l'éthique vers une idée procédurale de la société juste. Mais comment articuler la visée substantielle à la voie procédurale? Comment (...) assurer, sur la voie du discours, le principe de reconnaissance? Dans cette entreprise, l'éthique de la reconstruction revêt une signification décisive. C'est à introduire ce concept original que sont consacrées ces réflexions. A substantial body of evidence suggests that autobiographical recollection and simulation of future happenings activate a shared neural network. Many of the neural regions implicated in this network are affected in patients with bipolar disorder , showing altered metabolic functioning and/or structural volume abnormalities. Studies of autobiographical recall in BD reveal overgeneralization, where autobiographical memory comprises primarily factual or repeated information as opposed to details specific in time and in place and definitive of re-experiencing. To date, no study has examined (...) whether these deficits extend to future event simulation. We examined the ability of patients with BD and controls to imagine positive, negative and neutral future events using a modified version of the Autobiographical Interview that allowed for separation of episodic and non-episodic details. Patients were selectively impaired in imagining future positive, negative, and neutral episodic details; simulation of non-episodic details was equivalent across groups. (shrink)

After nearly 20 years of work by activists, fair trade, a movement establishing alternative trading organizations to ensure minimal returns, safe working conditions, and environmentally sustainable production, is now gaining steam, with increasing awareness and availability across a variety of products. However, this article addresses several major remaining challenges: (a) a lack of agreement about what fair trade really means and how it should be certified; (b) uneven awareness and availability across different areas, with marked differences between some parts of (...) Europe and North America that reflect more fundamental debates about distribution; (c) larger questions about the extent of the potential contribution of fair trade to development under the current system, including limitations on the number and types of workers affected and the fair trade focus on commodity goods. In some quarters within philosophy of medicine, more particularly in the phenomenological approaches, naturalism is looked upon with suspicion. This paper argues, first, that it is necessary to distinguish between two expressions of this attitude towards naturalism: phenomenological approaches to illness disagree with naturalism regarding various theoretical claims and they disapprove of naturalism on an ethical level. Second, this paper argues that both the disagreement with and the disapproval of naturalism are to a large extent confused. L'interprétation actuelle de Kant face à la Révolution française se présente sous forme d'un choix : ou bien Kant serait un libéral selon les tendances de la philosophie politique du jour, ou bien il serait un révolutionnaire qui avancerait masqué. L'ouvrage de Christian Ferrié qui repose sur une connaissance précise des inédits a le mérite de sortir de cette alternative. Spectateur enthousiaste de la Révolution en France, Kant réfute pourtant le droit de rébellion et rejette la méthode révolutionnaire. La pensée (...) politique de Kant semble partagée entre une adhésion au dessein républicain de la Révolution et des principes réformistes que cet événement bouleverse. Comment être alors sans contraction réformiste et révolutionnaire? Cet Ouvrage tenté de répondre à cette question sans esquiver les difficultés d'une position entre réforme et révolution. Car Kant paraît osciller entre une réflexion sur l'histoire qui établit de facto l'efficacité des révolutions et une doctrine normative du droit qui préconise la voie de la réforme. L'ambivalence apparente de sa position disparaît dès qu'elle est réinscrite dans le contexte des controverses de son temps en Allemagne. Kant défend la cause de la France en révolution contre l'offensive d'un réformisme antirévolutionnaire qui veut améliorer la société d'ordres pour mieux la sauver. C'est ainsi que Kant est conduit à inventer un réformisme révolutionnaire, inédit en ce qu'il préconise en principe la voie réformatrice tout en reconnaissant en fait la nécessité du moment révolutionnaire. La révolution devient la condition des réformes révolutionnaires qui l'accomplissent. Loin de proposer une synthèse positive, Kant conçoit plutôt un champ de tensions dynamiques entre réforme et révolution. Porté par l'enthousiasme que suscite la Révolution française, Kant donne à penser l'articulation de ces deux moments du politique au sein d'une nouvelle et "difficile" politique de l'émancipation qui parvient à penser une sorte de réformisme en révolution. S'engage la bataille idéologique : puis-je ou non disposer de mon existence? Des deux côtés, on invoque la dignité humaine, que ce soit pour réclamer le droit de "mourir dans la dignité" ou pour justifier l'interdiction (...) de toute forme d'euthanasie au nom du "caractère sacré" de la vie humaine. La "dignité humaine" devrait-elle justifier tout et son contraire? Il importe plutôt d'en élucider la notion et le fondement, en commençant par faire le clair sur les interprétations concurrentes et leurs implications pratiques. L'approche pluridisciplinaire de l'ouvrage favorisera une lecture critique au sein de laquelle différents points de vue s'inscrivent dans des moments distincts : Moment d'attestation, celui des praticiens aux prises avec la réalité des hôpitaux et des unités de soins palliatifs. Ils expérimentent au jour le jour le drame et la tragédie des passages de la phase curative à la phase palliative puis à la phase terminale ; Moment d'information, d'analyse exégétique et critique, touchant notamment à l'état du droit ; Moment de problématisation. De quoi et vers quoi? Tout logiquement, en direction des fondements philosophiques, c'est-à-dire de ce qui justifie que l'on prenne telle ou telle décision importante concernant les soins, l'arrêt des soins, la fin de vie, voire l'arrêt de la vie. By the time George Wilton Field concluded his work at the marine laboratory his initial scientific concerns had forced him directly into local politics. He pleaded with little success with the community of South Kingstown, and with no success with the town of Narragansett, to create and maintain a permanent breach:Is it not possible for the acute business sense and the broad philanthropy of the community to sweep aside petty, local, and personal jealousies which are now blocking practical progress for (...) the establishment of a permanent breach at Point Judith Pond? It is truly criminal neglect which permits fifteen hundred acres of valuable water-farming area to lie practically idle and rapidly deteriorate with each passing year.... In the opinion of the writer the Point Judith Pond and those of similar type could be made the seat of oyster, clam, crab, herring, white perch, and striped bass fisheries.30In the summer of 1899 Field was invited to teach a summer course on echinoderms at the MBL in Woods Hole, and to conduct summer research in a laboratory of the U.S. Fish Commission, also located at Woods Hole. When the summer was over, he remained there. Whether he had intentions of returning to resume his position in Rhode Island is unclear. At this point all correspondence with the Agricultural Experiment Station ceases, and Field's last report is a brief statement in the annual report of the experiment station for 1900 wherein he laments the variety of experiments he has not been able to carry to conclusion, such as a study of the artificial fertilization of water analogous to the method of chemically fertilizing the land for crops.The correspondence reveals that the enthusiasm Field brought to Point Judith Pond in 1896 was gradually sapped by his own fragile health, by three years' exposure to the local politics surrounding the southern Rhode Island fishing industry, and by a college administration determined to remove the stench of his invertebrates. He sought a refuge in the sheltered world of pure research at the U.S. Fish Commission Laboratory, where he set out to investigate the “Origin of Sex” using, as his animal models, squid and toadfish.On November 14, 1899, the Board of Managers of the college ordered the director of the experiment station to dispose of the marine laboratory at Point Judith Pond.31 How long the laboratory at Buttonwood Point survived in the institutional memory of the University of Rhode Island is open to question. The current Graduate School of Oceanography, in the event, traces its history back to 1937, not 1896.Nevertheless, Field and his one-room marine station established a precedent of land-grant marine research that other state colleges would follow, including Rhode Island itself, which reestablished its marine station, this time permanently, at South Ferry in 1937. In his brief research career in Rhode Island, George Wilton Field had discovered the same coastal attributes that would lead later to the creation of one of the world's major marine research centers at the University of Rhode Island's Graduate School of Oceanography.And, in a measure of triumph for his work, little more than a year after Field left for Woods Hole and his laboratory was dismantled, the town of South Kingstown voted the funds necessary to begin the construction of a permanent breachway.32 Whether Field's scientific reasoning and the conclusions of his marine research played any part in finally deciding the thirty-year-old debate in the affirmative will probably never be known. Selon l'idéologue Jeremy Rifkin, elles annoncent la fin du capitalisme au profit d'un monde de gratuité et de souci de l'autre. N'est-ce pas, tout à l'inverse, vers un hyperlibéralisme, vénal et dérégulateur, que nous nous dirigeons? Certaines perspectives ouvertes par les innovations technoscientifiques sont enthousiasmantes, d'autres effrayantes. Ces discussions sont également l'occasion d'en revenir aux fondateurs de la pensée politique contemporaine et de vérifier ainsi l'actualité de la réflexion d'Emmanuel Kant, de Friedrich Hegel, de Karl Marx, d'Alexis de Tocqueville, de John Stuart Mill ou, plus près de nous, de Sigmund Freud et de Max Weber pour penser les problèmes politiques et spirituels de notre temps. Mais ce qui ne laisse pas d'étonner, c'est l'écho qu'elle rencontre auprès d'un grand public avide de lectures philosophiques. Il convient de s'interroger sur cet engouement, ou plutôt d'interroger les philosophes à ce sujet, eux qui sont sans doute les mieux placés pour y répondre. " Précédés d'une introduction à l'œuvre des auteurs interviewés, les six entretiens réunis dans ce livre font le point à la fois sur la pensée de chacun d'eux et sur la situation générale de la philosophie française actuelle. What happens when the meaning of life based on a divine revelation no longer makes sense? Does the quest for transcendence end in the pursuit of material success and self-absorption? Luc Ferry argues that modernity and the emergence of secular humanism in Europe since the eighteenth century have not killed the search for meaning and the sacred, or even the idea of God, but rather have transformed both through a dual process: the humanization of the divine and the divinization of (...) the human. Ferry sees evidence for the first of these in the Catholic Church's attempts to counter the growing rejection of dogmatism and to translate the religious tradition into contemporary language. For political and literary theorists working on race, Ralph Ellison’s Invisible Man is a canonical text. Most political theorists approach the novel through what this essay calls a “martyr reading,” though martyrdom is just one of several political strategies explored in the work. This essay highlights an alternative in Ellison’s repertoire. The “trickster reading” developed here better accounts for several key scenes in the novel and also shows the limits of martyrdom as a technique of democratic politics. While other democratic (...) theorists have identified Ellison with redemptive or deliberative aims, the novel’s references to Homer’s Odyssey, with its own trickster-hero, license a trickster reading of Invisible Man. Tricksters do not take on suffering, loss, or sacrifice in the hope of redemption nor are they necessarily committed to the virtues of deliberation. Instead, they evade sacrifice by resorting routinely to irony, cunning, and refusal. This reading demonstrates the importance of these techniques to Ellison’s vision and to our politics today. One of the many faces of Islam in Indonesia is the abangan Muslims or the abangans. As one of the most populous Muslim groups in the country, it is important to know them. To understand Indonesian Islam or Muslims, one cannot overlook them. The article argues that, amid recent escalating Muslim–Christian tension in the country, this majority Muslim group can play a significant role in enhancing Muslim–Christian relations in the future, on account of their worldview that emphasizes and maintains cosmic (...) harmony and balance of all existence. Their open and syncretic attitudes toward other religions may foster religious tolerance and coexistence. These are attested in the author’s personal engagement with them. It is suggested that Christians be more inclusive toward them, but, at the same time, be critical in engaging their worldview. These attitudes are necessary for building a mutual and peaceful Muslim–Christian relationship in the region. (shrink)

Indonesia is recognized as a nurse exporting country, with policies that encourage nursing professionals to emigrate abroad. This includes the country’s adoption of international principles attempting to protect Indonesian nurses that emigrate as well as the country’s own participation in a bilateral trade and investment agreement, known as the Indonesia–Japan Economic Partnership Agreement that facilitates Indonesian nurse migration to Japan. Despite the potential trade and employment benefits from sending nurses abroad under the Indonesia–Japan Economic Partnership Agreement, Indonesia itself is suffering (...) from a crisis in nursing capacity and ensuring adequate healthcare access for its own populations. This represents a distinct challenge for Indonesia in appropriately balancing domestic health workforce needs, employment, and training opportunities for Indonesian nurses, and the need to acknowledge the rights of nurses to freely migrate abroad. Hence, this article reviews the complex operational and ethical issues associated with Indonesian health worker migration under the Indonesia–Japan Economic Partnership Agreement. It also introduces a policy proposal to improve performance of the Indonesia–Japan Economic Partnership Agreement and better align it with international principles focused on equitable health worker migration.
