This article discusses the misalignment of the drug innovation model in the US with broader societal goals. The paper calls for a reconfiguration of this model to prioritize the common good and ensure equitable access to health innovations. The article stresses the importance of adopting a mission-oriented approach to shape the drug market, including reforming intellectual property rights.

Enhancing research and development and ensuring equitable pricing and access to cutting-edge treatments are both vital to a biopharmaceutical innovation system that works in the public interest. However, despite delivering numerous therapeutic advances, the existing system suffers from major problems: a lack of directionality to meet key needs, inefficient collaboration, high prices that fail to reflect the public contribution, and an overly-financialized business model.

In lieu of an abstract, here is a brief excerpt of the content:138 In 2010 the city of Colorado Springs was strapped for cash. Government officials announced that they would either have to raise revenue through increased taxation or cut public services—­ in some cases rather severely—­ including, perhaps, police and fire protection, and even more basic bits of municipal infrastructure. The city shut down one-­ third of residential streetlights and closed public restrooms. Citi­ zens were outraged, but a majority (...) of voters had recently defeated a proposed tax increase. It’s tempting to suppose that the city was trying to teach its citizens a lesson: if you want public services, you have to be willing to pay for them. Colorado Springs is, and has been for some time now, a died-­in-­the-­wool small-­government, anti-­tax-­increase, fiscally conservative town. As Colorado Springs resident Bo Sharifi put it, “As soon as I hear government [say] ‘Oh, we need more money’... I guess I kind of automatically assume there’s probably some other things you could probably cut before firemen, policemen, city lights and that sort of thing.”1 On the surface, that seems reasonable. So... what comes to mind? How about food stamps? What about WIC? Of course these are federal services, not funded by Coloradans alone. There’s government, and then there’s government. Well, how about... well... what? Doesn’t the government spend money on all sorts of things we could very easily do without? Perhaps. Isn’t it part of our job to know or to decide which things these are? If we don’t trust government, even more obviously we don’t trust it with our money, though we pretty clearly have no idea how public funds are actually allocated or spent. Bo’s wife Sara viewed this as extortion. “By shoveling out a hundred dollars to turn on a streetlight that we kind of felt was supposed to be on anyway, that it was giving them what they wanted,” she said. They who? One resident wrote the city of Colorado Springs a check for $300 to turn the lights on for his entire neighborhood. City Councilwoman Jan Martin pointed out to him that the total cost to the individual Colorado Springs taxpayer to keep the streetlights on—­and to fund the maintenance of public parks and medians, as well as community centers and pre-­recession-­level police and fire services—­by approving the tax increase would have been: $200. He replied that he would never vote for a tax increase. In other words, as the reporter who covered the story (Robert Smith) observed, he would rather pay more to keep the streetlights on in his own neighborhood than to pay for all city services combined. Smith remarked that “Colorado Springs was stepping away from one of the things that we take for granted in most American cities: that we’re all in it together—­ at least when it comes to basic services.”2 As near as I can tell, this is the moral of Steven Cassedy’s exceptionally timely Connected: that we’re all in it together. For much of the book Cassedy merely traces the history of the emergence in the consciousness of ordinary Americans of the concept of the social network, but he is not so much talking about the notion of a network society as such, as I understand him. Or, at any rate, the network society as such is not really his main concern. He is talking about the idea of social, political, national, and ultimately global and universal human interconnectedness that was a distinct product of the complex networked structures (both concrete and abstract) that came to constitute modern American life in the twentieth century (and of course life pretty much everywhere ), and about our understanding of ourselves as so networked.3 On this... Abstract:Jessica Elkayam asks Mariana Ortega about the influence both Latina feminisms and Martin Heidegger have had on the development of Ortega's mestiza theory.

In times of crises, public health leaders may claim that trials of public health interventions are unethical. One reason for this claim can be that equipoise—i.e. a situation of uncertainty and/or disagreement among experts about the evidence regarding an intervention—has been disturbed by a change of collective expert views. Some might claim that equipoise is disturbed if the majority of experts believe that emergency public health interventions are likely to be more beneficial than harmful. However, such beliefs are not always (...) justified: where high quality research has not been conducted, there is often considerable residual uncertainty about whether interventions offer net benefits. In this essay we argue that high-quality research, namely by means of well-designed randomized trials, is ethically obligatory before, during, and after implementing policies in public health emergencies (PHEs). We contend that this standard applies to both pharmaceutical and non-pharmaceutical interventions, and we elaborate an account of equipoise that captures key features of debates in the recent pandemic. We build our case by analyzing research strategies employed during the COVID-19 pandemic regarding drugs, vaccines, and non-pharmaceutical interventions; and by providing responses to possible objections. Finally, we propose a public health policy reform: whenever a policy implemented during a PHE is not grounded in high-quality evidence that expected benefits outweigh harms, there should be a planned approach to generate high-quality evidence, with review of emerging data at preset time points. These preset timepoints guarantee that policymakers pause to review emerging evidence and consider ceasing ineffective or even harmful policies, thereby improving transparency and accountability, as well as permitting the redirection of resources to more effective or beneficial interventions. Draws from Latina feminism, existential phenomenology, and race theory to explore the concept of selfhood. This original study intertwining Latina feminism, existential phenomenology, and race theory offers a new philosophical approach to understanding selfhood and identity. Focusing on writings by Gloría Anzaldúa, María Lugones, and Linda Martín Alcoff, Mariana Ortega articulates a phenomenology that introduces a conception of selfhood as both multiple and singular. Her Latina feminist phenomenological approach can account for identities belonging simultaneously to different worlds, including immigrants, (...) exiles, and inhabitants of borderlands. Ortega’s project forges new directions not only in Latina feminist thinking on such issues as borders, mestizaje, marginality, resistance, and identity politics, but also connects this analysis to the existential phenomenology of Martin Heidegger and to such concepts as being-in-the-world, authenticity, and intersubjectivity. The pairing of the personal and the political in Ortega’s work is illustrative of the primacy of lived experience in the development of theoretical understandings of who we are. In addition to bringing to light central metaphysical issues regarding the temporality and continuity of the self, Ortega models a practice of philosophy that draws from work in other disciplines and that recognizes the important contributions of Latina feminists and other theorists of color to philosophical pursuits. Plato’s Parmenides is one of the main sources to reconstruct Zeno’s ideas. It has usually been held that Plato presents him there as a follower of Parmenides, whose book intends to defend his master’s argument against those who try to ridicule it. The aim of this paper is to demonstrate that Plato never presents Zeno as an advocate of monism, but rather as an expert on the antilogikètékhne, which is used by him to examine and refute pluralism as well as (...) monism. In order to prove this hypothesis, I will consider first the testimony of the Phaedrus, and specially Phdr. 261d6–8 (dk 29 a 13), where Plato explains some characteristics of the antilogiké that are essential to understand the goals of Zeno’s method. Then I will analyze some passages of the Parmenides (Prm. 127a7-d5, 128a1-e4, dk 29 a 11–12; 127d6-128a1; 135d3-6; 136a4-b6; 136e1-4) to show how this method is presented as an intellectual gymnastic, which could be considered as a propaedeutic for the acquisition of truth and knowledge. It's time that philosophers read Gloria Anzaldúa as a philosopher. Scholars have been hinting at it for some time, but in describing her they still tend to choose the terms “theorist,” “feminist,” and “thinker” instead of “philosopher.” Anzaldúa fits into all of these categories, but from her notes, we know that Anzaldúa also thought of herself as a philosopher. In 2002, for instance, she called herself a “feminist‐visionary‐spiritual‐activist‐poet‐philosopher fiction writer.” This essay argues that we should grant Anzaldúa's wish to be (...) considered a philosopher in addition to the other appellations, and focuses on works that she read and wrote in the first half of her scholarly career, including the publication of Borderlands/La Frontera in 1987. The evidence for the claim that Anzaldúa thought of herself as a philosopher is mostly archival, but some of it also appears in her published works. Once philosophers explicitly begin to recognize Anzaldúa as a philosopher, comparisons between her and other already recognized philosophers will become fuller and bear more fruit. (shrink)

Features of threatening cues and the associated context influence the perceived imminence of threat and the defensive responses evoked. To provide additional knowledge about how the directionality of a threat (i.e. directed-towards or away from the viewer) might impact defensive responses in humans, participants were shown pictures of a man carrying a gun (threat) or nonlethal object (neutral) directed-away from or towards the participant. Cardiac and electrodermal responses were collected. Compared to neutral images, threatening images depicting a gun directed-towards the (...) participant induced sustained bradycardia and an increased electrodermal response, interpreted as immobility under attack. This defensive immobility reaction is evoked by high perceived threat and inescapable situations and indicates intense action preparation. Pictures of guns directed-away from the participant induced shorter bradycardia and no significant modulation of the electrodermal response compared to neutral pictures, which might be consistent with the perception of a less threatening situation. The results show that the directionality of threat stimuli is a key factor that prompts different patterns of defensive responses. (shrink)

Jurisdiction, Scale and Governance: Chronotopes of Law develops a post-metaphysical framework for analyzing the spatio-temporal workings of law and other forms of governance. In this regard, it does not seek merely to combine analyses of legal temporality carried out by anthropologists with analyses of law and space carried out by geographers and socio-legal scholars. Adding two metaphysical abstractions together does not produce anything but somewhat more complex, but equally metaphysical, abstractions. And, in this book Mariana Valverde develops an anti-metaphysical theoretical approach to law that aims not to theorize the world in general, but rather to be useful to researchers who seek to shed light on the actual workings of law and other forms of governance. : The aim of this essay is to analyze the notion of "loving, knowing ignorance," a type of "arrogant perception" that produces ignorance about women of color and their work at the same time that it proclaims to have both knowledge about and loving perception toward them. The first part discusses Marilyn Frye's accounts of "arrogant" as well as of "loving" perception and presents an explanation of "loving, knowing ignorance." The second part discusses the work of Audre Lorde, Elizabeth Spelman, (...) and María Lugones in their attempts to deal with the issue of arrogant perception within feminism, and examines how Lugones's notion of "'world'-traveling" may help us deal with "loving, knowing ignorance." Ultimately, the author suggests that we need to become aware of instances of "loving, knowing ignorance," especially if we are to stay true to Third Wave feminism's commitment to diversity. (shrink)

But our ideas about anger still come from ancient Greek and Roman philosophers. Their stories about how anger works and why it (...) is bad have been dominant throughout history, and they are not kind to angry women in the twenty-first century. They have created a perilous environment for us, existentially speaking, by painting anger as irrational, crazy, and ugly. They have left us no way to handle our anger that does not amount to trying to control, suppress, or eradicate it. In this short paper, I contrast a philosophy of anger left to us by ancient Western philosophy with a contemporary one offered by Latina feminist María Lugones. Lugones offers a philosophy of anger that assumes it has something to teach us about our perilous environment.1Part I: Anger Seen in the LightPlato compared passions like anger to a hard-to-control, hot-blooded, black-skinned horse that must be reined in by the "charioteer" of reason (Phaedrus, ln. 253E). He thought we should use self-control to contain our anger, and he was not alone. The Roman Stoic Seneca, who described anger similarly, once told a story about Plato getting livid (Potegal and Novaco 9–24). Instead of beating one of his slaves, Plato froze, his hand drawn back in striking position. A friend asked Plato what he was doing. "I am making an angry man expiate his crime," Plato replied (Seneca, De Ira, Book III, section 12). Plato's freezing was his way of acknowledging that rage is weakness. Seneca formulated this scene into a principle: the only appropriate time to express anger is when you are not angry. Otherwise, you are a slave to your emotion. [End Page 23]I became angry during quarantine. I had been promised a year without teaching or administrative responsibility, but everything changed when the schools went remote. Since my kids were home anyway, I decided to home-school them. But I found myself becoming angry almost daily, and it was upsetting. To make sense of it, I went back to my philosophical sources.I consulted Epictetus, the Stoic philosopher whose Handbook I used to read every year. For the fifteenth time, he told me that "an uneducated man blames others; a partially educated man blames himself. A fully educated man blames no one" (13). While I could not control my circumstances in 2020—Epictetus granted that I did not have the power to end a pandemic or reopen schools—I could control my bursts of anger. Instead of blaming my spouse and kids for my troubles, I should blame myself for expecting life to be easier. Better yet, I should blame no one and accept the new normal gracefully. I also reread the Meditations of Marcus Aurelius, the second-century Roman emperor and Stoic who believed that yielding to anger is a sign of weakness (Potegal and Novaco 16). Marcus reformulated one of the central tenets of Stoicism: "Disturbance comes only from within—from our own perceptions" (38). His advice? Lower your expectations. Remember that the only person you can change is yourself. To that end, expect people to irritate you daily and you will be ready for it (17). For me, that meant remembering that having kids meant having messes. But expecting a mess did not clean the table every night, load or unload the dishwasher, or vacuum the crumb-riddled floor. Marcus did not send his servants to clean my house. So, I left him for Aristotle.Aristotle's soul was not a charioteer with horses, but it was tripartite: feelings, predispositions, active conditions. Feelings are hard to change, Aristotle thought, so let's not waste too much energy trying. Predispositions just name the likelihood of feeling a particular feeling. Como resultado, se identifica allí un proceso desarticulación-rearticulación de consensos sobre lo posible y lo deseable en relación con la propia crisis (causas, alcances, soluciones) y con la configuración del tiempo de lo común (en (...) dos niveles, el de la narrativa histórica y el del tiempo ontológico). Finalmente, se ofrece una clave interpretativa que vincula el proceso de pérdida de legitimidad de la palabra política que caracterizó al periodo con el carácter anti-política que comporta la forma inevitabilista de concebir el tiempo de lo común. El objetivo de este artículo es estudiar el carácter enigmático de la escritura de Heráclito de Éfeso tomando en consideración la tradición de los enigmas en Grecia antigua. Compararé algunos de sus aforismos con los acertijos compuestos por Cleobulina de Lindos para mostrar que en uno y otro caso la función del enigma es similar: ocultar lo evidente haciendo uso de diversas estrategias lingüísticas con vistas a ampliar tanto el conocimiento sobre el mundo como el conocimiento de sí.

The current study aims to make an overall semiotic analysis of translation strategies used to reproduce the imagery and relevant cultural features in John Fowles’ “The Collector.” Regarding literary translation as a cross-cultural dialogue aimed to achieve both artistic and aesthetic effects contributes much to analyzing the semiotic features of the translated discourse and deciphering the relevant socio-cultural information decoded in the source language text. Therefore, it has been decided that translation is a communicative act that facilitates the transfer of (...) meaning in a cross-cultural perspective and focuses on reproducing the source language semiosphere. The semiotic approach is well-established in translation studies with the subtlety of methods and criteria. Therefore, it was considered that the analysis of lexico-semantic, grammatical and stylistic translation strategies would usefully supplement and extend the scope of literary text research. In the current study, comparing the source and target language texts has shown that the translator employs various translation transformations to transmit the author’s pragmatic intent and socially relevant information in the target language. It is in Momus and the Intercenales that this "somber Alberti" is present. ABSTRACT The aim of the present article is to rebuild picturebooks’ identity as polyphonic cultural objects based on a brief historical trajectory. Dialogue established between text and reader in contemporary picturebook’s production is broadened by the presence of different semiosis that allow multiple voices to be heard, since they act as meaning enhancers. The Bakhtinian concept of polyphony is herein held at the picturebook reflections scope to set contrast in children’s literature, which has a moralizing nature and is substantiated from (...) a monological perspective. The books by Angela Lago, who is a representative of the Brazilian children’s literature, are herein analyzed to disclose multimodal aspects found in this cultural object (book). In conclusion, picturebooks open a window of possibilities for open minded and creative readings, as well as for different readers, due to their polysemic and polyphonic nature. (shrink)

Buscamos aquí explicitar los desarrollos en torno al tema de la maternidad y de la gestación de la filósofa argentina Lucía Piossek Prebisch a partir de su escrito: “Aislamiento y comunicación…” (1966), en términos de una fenomenología de la maternidad. Nuestro objetivo consiste en rastrear varias presentaciones sobre esta temática realizadas por la filósofa y posteriormente publicadas entre los años 1960 y 1980 y hasta 2015, destacando las implicancias conceptuales en sus escritos, el vínculo con la fenomenología y las continuaciones (...) que permitiría su pensamiento. En este sentido, nuestra hipótesis es que las tesis de Piossek Prebisch constituyen un antecedente fundamental dentro del campo de la fenomenología de la maternidad, no solo por lo temprano de su aparición, sino, y sobre todo, por la profundidad y originalidad de su pensamiento. Background Non-adherence to medication is associated with increased risk of relapse in patients with bipolar disorder. Objectives To validate patient-evaluated adherence to medication measured via smartphones against validated adherence questionnaire; and investigate characteristics for adherence to medication measured via smartphones. Methods Patients with BD evaluated adherence to medication daily for 6–9 months via smartphones. The Medication Adherence Rating Scale and the Rogers’ Empowerment questionnaires were filled out. The 17-item Hamilton Depression Rating Scale, the Young Mania Rating Scale and the Functional (...) Assessment Short Test were clinically rated. Data were collected multiple times per patient. The present study represents exploratory pooled reanalyses of data collected as part of two randomised controlled trials. Findings During the study 90.50% of the days were evaluated as ‘medication taken’, 6.91% as ‘medication taken with changes’ and 2.59% as ‘medication not taken’. Adherence to medication measured via smartphones was valid compared with the MARS. Younger age and longer illness duration were significant predictors for non-adherence to medication. Decreased affective symptoms measured with smartphone-based patient-reported mood and clinical ratings as well as decreased empowerment were associated with non-adherence. Conclusions Smartphone-based monitoring of adherence to medication was valid compared with validated adherence questionnaire. Younger age and longer illness duration were predictors for non-adherence. Increased empowerment was associated with adherence. Clinical implications Using smartphones for empowerment of adherence using patient-reported measures may be helpful in everyday clinical settings. The present review on dyadic coping (DC) aims at providing a critical integration of both the conceptual and empirical DC literature and overcoming the limitations of past reviews by (a) describing, comparing, and integrating all the DC models, (b) presenting and integrating findings from studies based on DC models, and (c) suggesting directions for further research. After discussing each DC model, we advance a conceptual integration of all models, which serves as the framework to organize the review of the empirical literature. This integration includes the following DC dimensions: (a) Stress Communication, (b) Positive DC by One Partner (supportive DC, empathic responding, delegated DC, active engagement), (c) Positive Conjoint DC (common, collaborative, communal, mutual responsiveness); (d) Negative DC by One Partner (protective buffering, overprotection, and hostility/ambivalence), and (e) Negative Conjoint DC (common negative DC, disengaged avoidance). Developmental, relational, and contextual variables are included as factors shaping DC. To be included in the empirical review, articles had to be published in or a peer-reviewed journal in English and/or German before 2017 and include an original empirical study guided by one of the DC models. The review included 139 studies and, with the exception of CM whose findings were discussed separately, findings were presented for overall DC and each of the dimensions identified in the conceptual integration. Findings were grouped also according to whether the stressor related or not to a medical or mental health condition. Demographic and cultural factors affecting DC were discussed. Overall, the empirical review suggests that in Western couples, positive individual and conjoint DC forms, taken together or separately, have individual and relational benefits for couples coping with stress in general and/or mental health or medical stressors. Research on DC can be expanded to include other populations and stressors and use improved designs. (shrink)

This original study intertwining Latina feminism, existential phenomenology, and race theory offers a new philosophical approach to understanding selfhood and identity. Focusing on writings by Gloría Anzaldúa, María Lugones, and Linda Martín Alcoff, Mariana Ortega articulates a phenomenology that introduces a conception of selfhood as both multiple and singular. Her Latina feminist phenomenological approach can account for identities belonging simultaneously to different worlds, including immigrants, exiles, and inhabitants of borderlands. Ortega’s project forges new directions not only in Latina feminist (...) thinking on such issues as borders, mestizaje, marginality, resistance, and identity politics, but also connects this analysis to the existential phenomenology of Martin Heidegger and to such concepts as being-in-the-world, authenticity, and intersubjectivity. The pairing of the personal and the political in Ortega’s work is illustrative of the primacy of lived experience in the development of theoretical understandings of who we are. In addition to bringing to light central metaphysical issues regarding the temporality and continuity of the self, Ortega models a practice of philosophy that draws from work in other disciplines and that recognizes the important contributions of Latina feminists and other theorists of color to philosophical pursuits. Its beginning showed tension comparable to that of the 1950s and early 1960s. As of the mid-1980s, however, there was a détente, the end of the Cold War and the collapse of the bipolar international power system. Movies, like other forms of cultural production, were tools for propaganda and mobilization throughout the Cold War. The purpose of this paper is to analyze how the Cold War was shown in the 1980s movies with the highest box-office sales in the US. The Communist enemy is examined in these feature films according to the Reaganist rhetoric of the period. In today's conditions of Ukraine's development, strengthening its legal foundation and establishing a democratic civil society is an urgent problem of the person's ability to exercise the rights and freedoms guaranteed by the constitution and other legislative acts. Recently, in the world, cases of human rights violations in connection with religious, racial, linguistic and other differences have become more frequent and more frequent in the world. The ambiguity, the antagonistic nature of the definitions and the relation between the concepts of (...) various social groups, such as the national, religious minority, etc., often leads to speculation and aggravation of the ethnopolitical situation. I argue that, in Plato’s Laws, demotic virtues (δημόσιαι ἀρεταί, 968a2) are the virtues that non-divine beings can attain. I consider two related questions: what demotic virtues are and how they relate to divine virtue. According to my interpretation, demotic virtues are an attainable – but unreliable – type of virtue that non-divine beings can improve through knowledge. These virtues are not perfect; only divine beings possess perfect virtue. However, this does not mean that perfect virtue plays no part in (...) the ethical lives of non-divine beings. O aumento da participação feminina no mercado de trabalho formal e informal, decorrente dos movimentos feministas, começou a ameaçar em larga escala um dos pilares da desigualdade, o provimento econômico e autonomia sobre a vida das mulheres, dessa forma, as estruturas sociais se reorganizam para reproduzir a lógica de poder opressora. Por meio de ambientes organizacionais, diversos e inclusivos, especialmente no que se refere a gênero, as mulheres poderão experienciar a valorização social, a influência no poder de decisão, o pertencimento (...) e engajamento no grupo de trabalho, a autenticidade e reconhecimento de diversidade (sentimento de interação justa), e essa experiência impactará a vida profissional e concomitantemente a pessoal/privada. O objetivo principal é mapear como é abordado a violência contra a mulher no ambiente de trabalho na literatura nacional. O método utilizado será a revisão de literatura integrativa, definida como o compêndio de literatura existente objetivando a sumarização e discussão de um corpo de conhecimento relacionado ao tema. Em suma, os resultados desta revisão integrativa reforçam a necessidade de ação conjunta por parte da comunidade para combater a violência contra a mulher no ambiente de trabalho, pois ao reconhecer que se trata de um problema social que afeta a sociedade como um todo, é possível fortalecer os laços de solidariedade e incentivar ações coletivas para estabelecer um ambiente de trabalho seguro e inclusivo. How does everyday, inauthentic Dasein dominated by das Man become authentic? The aim of this article is to answer this and other questions about Dasein's authenticity by carrying out an analysis of the 'call of conscience'. This analysis, in turn, provides insights about Dasein's possibility for ethical existence. We will see that even though there are some puzzling issues in Heidegger's explanation of Dasein in its everydayness and its authenticity, the Heideggerian Existential Analytic is not 'anti-ethical' as some have claimed. We will also see that the Existential Analytic's commitment to the ontic raises doubts about Heidegger's claim that the Analytic is prior to any ethics and examine how reading Being and Time as a project of 'metontology' may dissolve these doubts. Finally, the paper offers a suggestion about the importance of 'authentic historicality' in a factical account of the moral life. (shrink)

Aristotle presents two different approaches to riddle in the Poetics and the Rhetoric. In this paper, I intend to argue that, despite meaningful differences, these two views on riddle are not contradictory, but rather complementary. Taken together, they provide a valuable explanation of the structure, as well as the cognitive function, of riddle.
