Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making. Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments. DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system. The metrology literature neglects a strong empirical measurement tradition in economics, which is different from the traditions as accounted for by the formalist representational theory of measurement. This empirical tradition comes closest to Mari's characterization of measurement in which he describes measurement results as informationally adequate to given goals. In economics, one has to deal with soft systems, which induces problems of invariance and of self-awareness. It will be shown that in the empirical economic measurement tradition both problems have been (...) on the agenda for a long while, and that the proposed solutions to these problems provide clues for the directions in which one could develop a measurement theory that takes account of soft systems. This article investigates the meaning of doing good and doing well in positive business. It examines the relationship between the two expressions and discusses their relevance, shedding new light on the significance of ‘positive’ in positive business and positive organizational scholarship. Thus, this article illuminates the ultimate end of positive states and practices. ‘Positive’ primarily represents values and assumptions. These lead to the creation of beneficial situations and marked improvements, which put individuals and organizations on an upward trajectory toward achieving (...) excellent functioning, assuring profitability in addition to sustainability and social well-being. Doing well, with reference to economic performance and profits, is partly constitutive of doing good. This presents a new approach regarding the role of economic outcomes in positive business and POS. It means that financial criteria do not have priority. Profits are considered vital and necessary, but the final ‘raison d'être’ of positive states and practices is the overall well-being of the stakeholders. (shrink)

Empirical bioethics is commonly understood as integrating empirical research with normative-ethical research in order to address an ethical issue. Methodological analyses in empirical bioethics mainly focus on the integration of socio-empirical sciences and normative ethics. But while there are numerous multidisciplinary research projects combining life sciences and normative ethics, there is few explicit methodological reflection on how to integrate both fields, or about the goals and rationales of such interdisciplinary cooperation. In this paper we will review some drivers for the (...) tendency of empirical bioethics methodologies to focus on the collaboration of normative ethics with particularly social sciences. Subsequently, we argue that the ends of empirical bioethics, not the empirical methods, are decisive for the question of which empirical disciplines can contribute to empirical bioethics in a meaningful way. Using already existing types of research integration as a springboard, five possible types of research which encompass life sciences and normative analysis will illustrate how such cooperation can be conceptualized from a methodological perspective within empirical bioethics. We will conclude with a reflection on the limitations and challenges of empirical bioethics research that integrates life sciences. (shrink)

Communities of Deaf people consider themselves a minority with its own language, Sign Language, and culture. Two basic characteristics of Deaf Culture, its orientation on community and the awareness of an own Deaf worldview different from hearing people's worldview, are often not recognized by Christian Churches. This article tries to find a theological answer on this dramatic situation, formulating a theological foundation of the legitimacy of Deaf people's experience of God's presence in the Deaf community, and the thesis that God's (...) presence can take place even in a very secular environment. The legitimacy of the experience of God's presence in the community is supported by statements of exegetes, Fathers of the Church, spiritual theologians, and Church historians about Jesus' presence amongst two or three. This presence needs of a new Pneumatic religious culture, which is based on the quality of relationship. (shrink)

In our days, the creedal phrase 'I believe in God the Father almighty' is interpreted primarily along Trinitarian lines: It is applied to God as the Father of Jesus Christ. Here I argue that it has a dual background: in Jesus' prayer practice, in which He consistently addressed God as 'Father', and in the Hellenistic habit of referring to the Creator as 'Father'. I discuss Jesus' use of the term 'Father' against its Old Testament background, and argue that it primarily (...) points to the intimacy of Jesus' relationship with His father. Against the Hellenistic background, however, the metaphor 'Father' means 'he who brings forth effortlessly'. Finally, I discuss some gender issues connected with the use of the term 'Father' for God. I dispute that consciousness is generated by core circuitry in the forebrain, with predominance of motor areas, as Cotterillproposes in Enchanted Looms and other theorists do also. Ipropose instead that conscious contents are the momentary modeof action of the integrated cortical field, expressed as a point vector ( dominant focus ), to which, in varying degree, allsectors of the network contribute. Consciousness is the brain''saccess to its own activity space, and is identical with the moment''sdominant mode of activity. The dominant (...) focus is generally weightedtoward enactively encoded percepts. Anticipation and preparation,perception and action, inextricably interdigitate. I also dispute the view of Cotterill and others that consciousnesshas unique agency, which bestowed adaptive advantage when the brain evolved. Being identical with the activity of the network,consciousness can have no additional agency, and it can offerno adaptive advantages beyond those that characterize the network. (shrink)

In the social sciences we hardly can create laboratory conditions, we only can try to find out which kinds of experiments Nature has carried out. Knowledge about Nature's designs can be used to infer conditions for reliable predictions. This problem was explicitly dealt with in Haavelmo's (1944) discussion of autonomous relationships, Friedman's (1953) as-if methodology, and Simon's (1961) discussions of nearly-decomposable systems. All three accounts take Marshallian partitioning as starting point, however not with a sharp ceteris paribus razor but with (...) the blunt knife of negligibility assumptions. As will be shown, in each account reflection on which influences are negligible, for what phenomena and for how long, played a central role. (shrink)

As Gray insists, his comparator model proposes a brute correlation only – of consciousness with septohippocampal output. I suggest that the comparator straddles a feedback loop that boosts the activation ofnovelrepresentations, thus helping them feature in present or recollected experience. Such a role in organizing conscious contents would transcend correlation and help explain how consciousness emerges from brain function.

BackgroundFunctional changes after spinal cord injury are related to changes in cortical plasticity. These changes can be measured with electroencephalography and has potential to be used as a clinical biomarker.MethodIn this longitudinal study participants underwent a total of 30 sessions of robotic-assisted gait training over a course of 6 weeks. The duration of each session was 30 min. Resting state EEG was recorded before and after 30-session rehabilitation therapy. To measure gait, we used the Walking Index for Spinal Cord Injury (...) Scale, 10-Meter- Walking Test, Timed-Up-and-Go, and 6-Min-Walking Test. Balance was measured using Berg Balance Scale.ResultsFifteen participants with incomplete SCI who had AIS C or D injuries based on American Spinal Cord Injury Association Impairment Scale classification were included in this study. Mean age was 35.7 years and the mean time since injury was 17.08 months. All participants showed clinical improvement with the rehabilitation program. EEG data revealed that high beta EEG activity in the central area had a negative correlation with gait and balance measured at baseline, in a way that greater high beta EEG power was related to worse clinical function at baseline. Moreover, improvement in gait and balance had negative correlations with the change in alpha/theta ratio in the parietal area.ConclusionIn SCI, functional impairment and subsequent improvement following rehabilitation therapy with RAGT correlated with the change in cortical activity measured by EEG. Our results suggest that EEG alpha/theta ratio may be a potential surrogate marker of functional improvement during rehabilitation. Future studies are necessary to improve and validate these findings as a neurophysiological biomarker for SCI rehabilitation. (shrink)
