Effective communication and interpersonal skills have long been recognized as fundamental to the delivery of quality health care. However, there is mounting evidence that the pressures of communication in high stress work areas such as hospital emergency departments present particular challenges to the delivery of quality care. A recent report on incident management in the Australian health care system cites the main cause of critical incidents, as being poor and inadequate communication between clinicians and patients. This article presents research that (...) describes and analyses spoken interactions between health care practitioners and patients in one ED of a large, public teaching hospital in Sydney, Australia. The research aimed to address the challenges and critical incidents caused by breakdowns in communication that occur between health practitioners and patients and by refining and extending knowledge of discourse structures, to identify ways in which health care practitioners can enhance their communicative practices thereby improving the quality of the patient journey through the ED. The research used a qualitative ethnographic approach combined with discourse analysis of audio-recorded interactions. Some key findings from the analysis of data are outlined including how the absence of information about processes, the pressure of time within the ED, divergent goals of clinicians and patients, the delivery of diagnoses and professional roles impact on patient experiences. Finally, the article presents an in-depth linguistic analysis on interpersonal and experiential patterns in the discursive practices of patients, nurses and doctors. (shrink)

Recent work has shown that linguistic and visual contexts jointly modulate linguistic expectancy and, thus, the processing effort for a expected critical word. According to these findings, uncertainty about the upcoming referent in a visually-situated sentence can be reduced by exploiting the selectional restrictions of a preceding word, which then reduces processing effort on the critical word. Interestingly, however, no such modulation was observed in these studies on the expectation-generating word itself. The goal of the current study is to investigate (...) whether the reduction of uncertainty simply does not modulate processing effort-or whether the particular subject-verb-object sentence structure used in these studies accounts for the observed pattern. To test these questions, the current design reverses the functional roles of nouns and verbs by using sentence constructions in which the noun reduces uncertainty about upcoming verbs, and the verb provides the disambiguating and reference-resolving piece of information. Experiment 1 and Experiment 2 both replicate the effect found in previous work, albeit on the verb in the current study. Results on the noun, where uncertainty is reduced and expectations are generated in the current design, were mixed and were most likely influenced by design decisions specific to each experiment. These results show that processing of the reference-resolving word—whether it be a noun or a verb—reliably benefits from the prior linguistic and visual information that lead to the generation of concrete expectations. Resumen Según el diagnóstico sobre la academia contemporánea del pensador italiano Piero Coda, la teología, disciplina escindida de la filósofa desde la modernidad, se ha reducido a una mera apologética de la fe. En este marco, la Trinidad se vacía de contenido ontológico, dejando de ser principio de interpretación de la realidad. En el presente artículo analizaré las consecuencias antropológicas que posee el hecho de que la creación, específicamente del hombre, sea imago Trinitatis en el pensamiento de Edith Stein. (...) En la primera sección, presentaré brevemente cómo Stein aborda el misterio y las relaciones intra-trinitarias utilizando las imágenes del amor y de la vida. Luego, presentaré brevemente el desarrollo de la noción de persona y como ésta es resignificada a la luz del dogma trinitario para explicar en qué sentido Dios es persona. En la segunda sección, presentaré cómo se da la imagen del Dios Trino en la creación. Este será el primer paso, dado que para Stein el discurso cosmológico es solo la antesala del discurso antropológico. Finalmente, analizaré cómo se da la imagen del Dios trinitario en la persona humana y por ende cómo esta noción es resignificada y enriquecida.According to the diagnosis on the contemporary academy of the Italian thinker Piero Coda, theology, having been cut off from philosophy since modernity, has been reduced to mere apologetics. Within this framework, the doctrine of the Trinity is emptied of any ontological content, ceasing to be a principle of interpretation of reality. In this article I will analyse the anthropological consequences of the fact that creation, specifically of man, is imago Trinitatis in the thought of Edith Stein. In the first section, I will briefly present how Stein approaches mystery and intra-Trinitarian relationships using the images of love and life. Then, I will offer a quick description of the development of the notion of person and how it is re-signified in light of the Trinitarian dogma as to explain in what sense God is a person. In the second section, I will present how the image of the Triune God appears in Creation, a first step in cosmological discourse that will lead, for Stein, to the anthropological discourse. Finally, I will analyse how the image of the Trinitarian God is present to the human person and therefore how this notion is re-signified and enriched. (shrink)

This essay evaluates the role of gender in the human person. According to the Thomistic account, gender is an accident of the material body. I suggest that the Thomistic account is in need of revision and examine a modified Thomistic account presented by John Finley, which establishes gender as an inseparable accident stemming from the soul. Finley’s account, which strives to remain faithful to Thomism, is closer to the essentialist position offered by personalist philosopher Edith Stein. I present (...) class='Hi'>Stein’s account of gender as essential by focusing on two poignant claims made by Stein: that gender relates to the essence of the person as it unfolds from a spiritual core and that human nature is comprised of a dual species, male and female. Edith Stein came to phenomenology after beginning her university studies in psychology. She struggled with the inability of psychology to justify and delineate its founding principles. She found in Edmund Husserl, though his sustained criticisms of psychologism, the possibility of a phenomenological ground for psychology. This article demonstrates how Stein, drawing from but also distancing herself from Husserl, justifies the possibility of a phenomenological psychology framed within a personalist structure of subjectivity and sociality.

The division of cognitive labor is fundamental to all cultures. Adults have a strong sense of how knowledge is clustered in the world around them and use that sense to access additional information, defer to relevant experts, and ground their own incomplete understandings. One prominent way of clustering knowledge is by disciplines similar to those that comprise the natural and social sciences. Seven studies explored an emerging sense of these discipline‐based ways of clustering of knowledge. Even 5‐year‐olds could cluster knowledge (...) in a manner roughly corresponding to the departments of natural and social sciences in a university, doing so without any explicit awareness of those academic disciplines. But this awareness is fragile early on and competes with other ways of clustering knowledge. Over the next few years, children come to see discipline‐based clusters as having a privileged status, one that may be linked to increasingly sophisticated assumptions about essences for natural kinds. The posterior parietal cortex (PPC) is the most likely site where egocentric spatial relationships are represented in the brain. PPC cells receive visual, auditory, somaesthetic, and vestibular sensory inputs; oculomotor, head, limb, and body motor signals; and strong motivational projections from the limbic system. Their discharge increases not only when an animal moves towards a sensory target, but also when it directs its attention to it. PPC lesions have the opposite effect: sensory inattention and neglect. The PPC does not seem (...) to contain a “map” of the location of objects in space but a distributed neural network for transforming one set of sensory vectors into other sensory reference frames or into various motor coordinate systems. Which set of transformation rules is used probably depends on attention, which selectively enhances the synapses needed for making a particular sensory comparison or aiming a particular movement. The wife of a childhood friend was a close match and gave him one of her kidneys. Before his transplant, aerobic exercise was difficult; a few months after transplant, (...) Slakter could ride his bike, hike, and work out with a personal trainer. He no longer had to weigh food or gauge his potassium consumption, a concern for people with kidney failure. For Slakter, “life post-transplant has been a significant improvement.”Individuals or family members of those who have received an organ transplant from a living or deceased donor share their stories in this issue of Narrative Inquiry in Bioethics. The symposium is titled “Receiving the Gift of Life: Stories from Organ Transplant Recipients.” Organ transplantation can extend an individual’s life by decades. It can also greatly improve an individual’s quality of life, as was Slakter’s experience. At the same time, organ transplantation is expensive and stressful. Recipients can experience complications, including organ rejection, and there are few appealing alternatives.After her heart transplant, author Leilani R. Graham required four open-heart surgeries, ECMO, and an intra-aortic balloon pump. She experienced delirium, necrotizing pneumonia, and severe muscular atrophy that required her to re-learn how to walk. She struggled with weight gain, explosive mood swings, hair loss, and was terrified that her body would reject the new heart. For Graham, “[Transplant] was not the miracle [she] was hoping for.” Graham says she felt unprepared for and inadequately informed about what to expect after her transplant, which left her “battered, beaten, and bruised” with a 12-inch line from neck to navel and “bullet holes” up and down her torso from the chest tubes. When reflecting on her experience post-transplant, Graham says, “It wasn’t the physical trauma that hurt the most. It was that no one seemed surprised [by the aftereffects of transplant].” Seemingly no one, that is except Graham, who calls on transplant providers to “prioritize empathy.”The symposium editors are Jason T. In their research, the authors performed qualitative analysis on 52 works of literature and identified 254 excerpts of figurative language with themes of non-physical or psychological suffering. The authors found 13 salient themes: brokenness, diminishment, disorientation, drowning, emptiness, imprisonment, battle, darkness, isolation, invisibility, lifelessness, punishment, and torture. In this article, we draw upon the notion that employees’ work characteristics are an important pathway through which leaders influence employee well-being and propose a theoretical framework that integrates perspectives on leadership, occupational stress, and job design. Based on this integrative approach, we developed the health- and development-promoting leadership behavior questionnaire (HDLBQ) for assessing job demands emanating from and job resources provided through the leader. Validation of the measure in German, French, and English using an overall sample of 2,934 employees (...) demonstrated adequate psychometric properties. An examination of the factorial structure revealed three higher-order factors: demanding, development-oriented, and support-oriented leadership. Multigroup confirmatory factor analysis indicated structural equivalence across the three language versions of the HDLBQ. Correlations with employee well-being were moderate, and the HDLBQ explained unique variance in employee well-being beyond that explained by transformational leadership. Suggestions for applications of the HDLBQ and approaches to enhance employee well-being at the workplace are discussed. (shrink)

Aristotle presents four causes in Posterior Analytics 2.11, but where we expect matter we find instead the confusing formula, ‘what things being the case, necessarily this is the case’, and an equally confusing example. Some commentators infer that Aristotle is not referring to matter, others that he is but in a non-standard way. I argue that APo. 94a20-34 presents not matter, but determination by general features or facts, including facts about something’s genus. In the present paper, we explore the impact of singing for girls in rural Bangladesh. Previous findings in this field-based interview study (with 18 girls) have demonstrated that singing can act as a driving force in young girls’ psychological individuation processes, implying increased agency and autonomy. A critical question, however, is to what extent the village girls will manage to maintain a feeling of agency as they pass through puberty. How do they navigate between their own wish to continue singing (...) and pressure from cultural norms (such as getting married)? Using Margareth Archer’s morphogenetic approach and Arjun Appadurai’s approach to culture as a capacity as analytical tools, we connect different modes of reflexivity to the girls’ capacity to aspire. We discuss specific mechanisms that emerge as relevant for the girls/young women as they navigate their way through the Bengali rural society. (shrink)

La integración de la tecnología en el ámbito del derecho penal plantea desafíos significativos debido a la falta de regulación de la inteligencia artificial (IA), lo que podría comprometer los derechos fundamentales. Este estudio se centra en abordar estos desafíos y proponer soluciones mediante un análisis exhaustivo de la situación actual y las posibles implicaciones de la IA en el sistema jurídico penal. El objetivo principal de esta investigación es analizar los problemas, desafíos y oportunidades que surgen con la implementación (...) de la inteligencia artificial en el ámbito penal. Se busca identificar la necesidad de una regulación específica para la IA en el derecho penal, con el fin de proteger los derechos fundamentales de las personas y garantizar un sistema jurídico justo y equitativo. Para abordar esta problemática, se adoptó un enfoque metodológico cualitativo. Se llevó a cabo una revisión bibliográfica detallada para examinar los antecedentes, problemas y desafíos asociados con la IA en los sistemas jurídicos, especialmente en el ámbito penal. Además, se realizaron análisis de criterios y opiniones de expertos en derecho para obtener una comprensión más profunda de la situación y las posibles soluciones. Los resultados de este estudio revelan la necesidad urgente de establecer una normativa específica para regular la aplicación de la inteligencia artificial en el ámbito penal. Se destaca que la falta de regulación representa un riesgo significativo para los derechos fundamentales, especialmente en lo que respecta a la configuración de delitos con el apoyo de la IA. Además, se identifican diferentes dimensiones que deben abordarse en la regulación, incluyendo principios y reglas básicas del derecho, supervisión humana de los sistemas de IA y la promoción de la innovación responsable en este campo. Este artículo subraya la importancia de establecer una regulación efectiva para la inteligencia artificial en el ámbito penal. Esta regulación no solo protegerá los derechos fundamentales de las personas, sino que también promoverá un uso responsable y progresivo de la IA como una herramienta valiosa para los profesionales del derecho. A substantial portion of the knowledge base of psychology is based on subjective reports with a risk of information bias. The objective of the present study was to elucidate one contextual source of variance and potential bias in subjective reports: the influence of affective state at the time of responding to questionnaires. Employees were subjected to mood-induction procedures in the laboratory. Neutral, positive, and negative moods were induced by combinations of pictures from the international affective picture set and music. The (...) subjects responded to questions on visual analog scales in order to optimize sensitivity and attenuate short-term memory effects. Most subjects exhibited significant affective-state inductions with no change in arousal. The analyses took affective response to the manipulation into account. Only four of 20 questions were somewhat influenced by induced affective state: job overload, social support from co-workers, satisfaction with getting to develop personally, and an item measuring agreeableness. In general, responding to questions of work that were phrased for valence was little or insignificantly influenced by induced affective state. (shrink)

Grafts of embryonic neural tissue into the brains of adult patients are currently being used to treat Parkinson's disease and are under serious consideration as therapy for a variety of other degenerative and traumatic disorders. This target article evaluates the use of transplants to promote recovery from brain injury and highlights the kinds of questions and problems that must be addressed before this form of therapy is routinely applied. It has been argued that neural transplantation can promote functional recovery through (...) the replacement of damaged nerve cells, the reestablishment of specific nerve pathways lost as a result of injury, the release of specific neurotransmitters, or the production of factors that promote neuronal growth. The latter two mechanisms, which need not rely on anatomical connections to the host brain, are open to examination for nonsurgical, less intrusive therapeutic use. Certain subjective judgments used to select patients who will receive grafts and in assessment of the outcome of graft therapy make it difficult to evaluate the procedure. In addition, little long-term assessment of transplant efficacy and effect has been done in nonhuman primates. Carefully controlled human studies, with multiple testing paradigms, are also needed to establish the efficacy of transplant therapy. (shrink)

The present paper analyses how top managers construct the meaning of sustainable human resource management and its responsibility areas and how they identify and prioritize stakeholders in sustainable HRM. The empirical data were collected as part of the Finnish HR Barometer inquiry. A qualitative analysis reveals four dimensions of sustainable HRM: Justice and equality, transparent HR practices, profitability, and employee well-being. It also reveals four broader responsibility areas: Legal and ethical, managerial, social, and economic. Contrary to the prior green HRM (...) literature, ecological responsibility was largely ignored. The study also reveals a wide range of stakeholders, specifically, owners, managers, employees, customers, and employee representatives, as well as their special roles and requirements for sustainable HRM. These findings contribute to the literature of sustainable HRM by illustrating the dimensions and broader responsibilities of sustainable HRM as seen by top managers. Their conceptions of sustainable HRM are largely neglected in the prior literature despite their crucial role in legitimating HRM and thus sustainable HRM in companies. These results also contribute to the theory of stakeholder salience in the sustainable HRM context from the viewpoint of top managers. (shrink)

This article explores the concept of cultural competence and its relevance as an organizational resource in ethical disputes. Empirically, we aim to reveal the cultural competences that a global forest industry company, StoraEnso, and a global environmental nongovernmental organization (NGO), Greenpeace, utilized in forestry conflicts during 1985–2001. Our study is based on data which were collected from corporate and NGO communication outlets and which have gone through a detailed discourse-semiotic analysis. Our reinterpretation of the discourses identified three cultural competences: (1) (...) the ability to understand changing consumer preferences and values, (2) the ability to utilize culturally determined positions of expertise, and (3) the ability to maintain trust and credibility in the community through open communicative practices. We argue that these competences are relevant in industry–NGO disputes for both parties. However, maintaining them all simultaneously is a difficult task, since various discourses which aim at upholding them can sometimes have contradictory effects. (shrink)
