IntroductionBrain tumours frequently cause language impairments and are also likely to co-occur with localised abnormal slow-wave brain activity. However, it is unclear whether this applies specifically to low-grade brain tumours. We investigate slow-wave activity in resting-state electroencephalography in low-grade glioma and meningioma patients, and its relation to pre- and postoperative language functioning.MethodPatients with a glioma infiltrating the language-dominant hemisphere and patients with a meningioma with mass effect on this hemisphere underwent extensive language testing before and 1 year after surgery. EEG (...) was registered preoperatively, postoperatively, and once in healthy individuals. Slow-wave activity in delta- and theta- frequency bands was evaluated visually and quantitatively by spectral power at three levels over the scalp: the whole brain, the affected hemisphere, and the affected region.ResultsGlioma patients had increased delta activity and increased theta activity before and after surgery. In these patients, increased preoperative theta activity was related to the presence of language impairment, especially to poor word retrieval and grammatical performance. Preoperative slow-wave activity was also related to postoperative language outcomes. Meningioma patients showed no significant increase in EEG slow-wave activity compared to healthy individuals, but they presented with word retrieval, grammatical, and writing problems preoperatively, as well as with writing impairments postoperatively.DiscussionAlthough the brain-tumour pathology in low-grade gliomas and meningiomas has a different effect on resting-state brain activity, patients with low-grade gliomas and meningiomas both suffer from language impairments. Increased theta activity in glioma patients can be considered as a language-impairment marker, with prognostic value for language outcome after surgery. (shrink)

Background Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. Methods We compared preferences of (...) parents/patients at enrolment, documented by a questionnaire, with preferences developed during follow-up by an interview-study to investigate equipoise of child-participants and parents in the BeSt-for-Kids-study. This trial in new-onset Juvenile Idiopathic Arthritis-patients consists of three strategies. One strategy comprises initial treatment with a biological disease-modifying-antirheumatic-drug, currently not standard-of-care. Semi-structured interviews were conducted with 23 parents and 7 patients, median 11 months after enrolment. Results Initially most parents and children were not in equipoise. Parents/patients who refused participation, regularly declined due to specific preferences. Many participating families preferred the biological-first-strategy. They participated to have a chance for this initial treatment, and would even consider stopping trial-participation when not randomized for it. Their conviction of superiority of the biological-first strategy was based on knowledge from internet and close relations. According to four parents, the physician-investigator preferred the biological-first-strategy, but the majority stated that she had no preferred strategy. In phase 2, preferences tended to change to the treatment actually received. Conclusions Lack of equipoise during enrolment did not reduce study recruitment, mainly due to the fact that preferred treatment was only available within the study. Still, when developing a trial it is important to evaluate whether the physicians’ research question is in line with preferences of the patient-group. By exploring so-called ‘informed patient-group’-equipoise, successful recruitment may be enhanced and bias avoided. In our study, lack of equipoise during trial-participation did not reduce retention in those assigned to a less favoured option. We observed a change for preference towards treatment actually received, possibly explained by comparable outcomes in all three arms. (shrink)

This book examines the outlook for Latin American entrepreneurs in the new global environment. Using case studies from across the region, the book highlights liberalization measures nations are adopting to facilitate small and medium size enterprise (SME) creation and growth, and existing barriers that are threatening SME sector gains.

How could it happen that the very processes intended to assure ethical research in Australia might, themselves, undermine good research practice?This paper describes one PhD candidate’s recent experiences of multicentre review for a Human Research Ethics Committee approved, low/negligible risk, qualitative study, at the crossroad of health services research and organisational research.A retrospective review of international literature about multi-centre review processes revealed that many of these experiences were not unique and might have been expected, notwithstanding Australian efforts at harmonisation of (...) multi-centre review. This paper examines not only the burden of red-tape that was applied to a small doctoral study, but also the way in which the red-tape threatened the anonymity of potential study participants and risked exposing them to undue pressure and distress.These experiences support the view that harmonisation initiatives have not yet developed as the Australian National Health and Medical Research Council may have hoped and that further attention is needed to harmonise research governance processes. This study, based on the self-determination theory, investigates the link between university students' social peer and teacher integration and intrinsic motivation development. Both integration contexts are expected to contribute to the student's development, either additive or compensatory. The analyses rely on a nationally representative sample of 7,619 German university students and cover the time between the 3rd and 5th semesters in a longitudinal design. Person-centered analytical tools were applied to tap interindividual differences in the motivational trajectories as well as in (...) integration profiles. Latent transition analyses revealed distinct links between the motivational trajectories and the integration profiles, pointing to additive effects of teacher and peer integration. Positive trajectories were more likely in the Highly than in Moderately Integrated profiles. The two profiles pointing to below-average integration levels showed the same probabilities for rather negative trajectories. The results are discussed against the backdrop of self-determination theory and additive vs. compensatory effects of teacher and peer integration, proposing a threshold model. (shrink)

In this paper, we provide a detailed critical review of current approaches to ecthesis in Aristotle’s Prior Analytics, with a view to motivate a new approach, which builds upon previous work by Marion & Rückert (2016) on the dictum de omni. This approach sets Aristotle’s work within the context of dialectic and uses Lorenzen’s dialogical logic, hereby reframed with use of Martin-Löf's constructive type theory as ‘immanent reasoning’. We then provide rules of syllogistic for the latter, and provide proofs (...) of e-conversion, Darapti and Bocardo and e-subalternation, while showing how close to Aristotle’s text these proofs remain. Le doute cartésien – théorique, généralisé, hyperbolique et volontaire – reprend et renforce tout d’abord les raisons sceptiques traditionnelles de douter, afin de mettre pour la première fois en doute tout le sensible, mais aussi de montrer que ces raisons buttent sur les naturae simplicissimae et la certitude de la mathesis universalis. Descartes forge alors un nouvel argument sceptique : le « Dieu qui peut tout », qui permet de penser que, lorsque je me rends à l’évidence des natures simples, (...) je peux me tromper du point de vue d’une autre rationalité et d’une évidence supérieure à la mienne. Mais le doute n’est pas seulement chez Descartes un moment et un moyen provisoires qu’il s’agirait de vaincre, et qui serait effectivement vaincu par la certitude du cogito. Puisqu’il ne dévoile aucune chose, n’énonce aucune proposition et n’engage pas la moindre prédication, le doute laisse en effet la parole ne dire qu’elle-même et se manifester ce qui y parle, c’est-à-dire celui qui parle, ego. On peut donc soutenir que c’est directement du doute, véritable premier principe cartésien, que provient l’existence. (shrink)

The attitude of biologists to the history of their discipline varies. For some, a hazy knowledge of the recent past is all that is necessary to provide an explanatory basis for their work. They take it for granted that everything of value from the less recent past has been appropriately incorporated into present-day thinking. Other biologists see history as an integral part of their research: the historical roots of accepted facts and theories help in the evaluation of present positions. These (...) biologists bring to history their specialized knowledge, which can be an advantage, but often they also bring an agenda that biases what they investigate and how they present it. We illustrate this by describing our own foray into history, which was motivated by findings in cell biology that suggested that some accepted views about heredity and evolution were wrong. (shrink)

A Sydney-based megachurch with global reach, well-known for its “prosperity gospel” of financial acquisition, has developed an additional strand: a detailed theology of consumption. The affinity between a theology of guilt-free—indeed, obligatory—consumption and late capitalism goes some way towards explaining the attraction this minority strand of Christianity holds for politicians, including those without personal religious commitments, in a secular electorate.
