Firearm injury in the United States is a public health crisis in which physicians are uniquely situated to intervene. However, their ability to mitigate harm is limited by a complex array of laws and regulations that shape their role in firearm injury prevention. This piece uses four clinical scenarios to illustrate how these laws and regulations impact physician practice, including patient counseling, injury reporting, and the use of court orders and involuntary holds. Unintended consequences on clinical practice of laws intended (...) to reduce firearm injury are also discussed. Lessons drawn from these cases suggest that physicians require more nuanced education on this topic, and that policymakers should consult front-line healthcare providers when designing firearm policies. Computer simulations show that an unstructured neural-network model [Shultz, T. R., & Bale, A. C. (2001). Infancy, 2, 501–536] covers the essential features␣of infant learning of simple grammars in an artificial language [Marcus, G. F., Vijayan, S., Bandi Rao, S., & Vishton, P. M. (1999). Science, 283, 77–80], and generalizes to examples both outside and inside of the range of training sentences. Knowledge-representation analyses confirm that these networks discover that duplicate words in the sentences are nearly identical and that (...) they use this near-identity relation to distinguish sentences that are consistent or inconsistent with a familiar grammar. Recent simulations that were claimed to show that this model did not really learn these grammars [Vilcu, M., & Hadley, R. F. (2005). Minds and Machines, 15, 359–382] confounded syntactic types with speech sounds and did not perform standard statistical tests of results. (shrink)

Norwegian health professionals, elderly people and family members experience ethical problems involving end-of-life decision making for elders in the context of the values of Norwegian society. This study used ethical inquiry and qualitative methodology to conduct and analyze interviews carried out with 25 health professionals, six elderly people and five family members about the ethical problems they encountered in end-of-life decision making in Norway. All three participant groups experienced ethical problems involving the adequacy of health care for elderly Norwegians. Older (...) people were concerned about being a burden to their families at the end of their life. However, health professionals wished to protect families from the burden of difficult decisions regarding health care for elderly parents at the end of life. Strategies are suggested for dialogue about end-of-life decisions and the integration of palliative care approaches into health care services for frail elderly people. (shrink)

The federal government is funding a sea change in health care by investing in interventions targeting social determinants of health, which are significant contributors to illness and health inequity. This funding power has encouraged states, professional and accreditation organizations, health care entities, and providers to focus heavily on social determinants. We examine how this shift in focus affects clinical practice in the fields of oncology and emergency medicine, and highlight potential areas of reform.

Social categorization is an early emerging and robust component of social cognition, yet the role that social categories play in children's understanding of the social world has remained unclear. The present studies examined children's explanations of social behavior to provide a window into their intuitive theories of how social categories constrain human action. Children systematically referenced category memberships and social relationships as causal-explanatory factors for specific types of social interactions: harm among members of different categories more than harm among members (...) of the same category. In contrast, they systematically referred to agents' mental states to explain the reverse patterns of behaviors: harm among members of the same category more than harm among members of different categories. These data suggest that children view social category memberships as playing a causal-explanatory role in constraining social interactions. (shrink)

Marjorie Lewis draws on her own experience of breaking through the glass ceiling to become the first woman President of the United Theological College of the West Indies. Through this, she considers the theological and biblical perspectives on gender equality, internalized and unrecognized inequality, naming and exorcising abuse in institutional relationships and strategies to survive and thrive. At the heart is a rejection of the notion that all suffering is to be embraced unchallenged as part of the Christian experience (...) and that it is the particular calling of women. Physicians play a critical role in preventing and treating firearm injury, although the scope of that role remains contentious and lacks systematic definition. This piece aims to utilize the fundamental principles of medical ethics to present a framework for physician involvement in firearm violence. Physicians' agency relationship with their patients creates ethical obligations grounded on three principles of medical ethics — patient autonomy, beneficence, and nonmaleficence. Taken together, they suggest that physicians ought to engage in clinical screening and treatment related (...) to firearm violence. The principle of beneficence also applies more generally, but more weakly, to relations between physicians and society, creating nonobligatory moral ideals. Balanced against physicians' primary obligations to patient agency relationships, general beneficence suggests that physicians may engage in public advocacy to address gun violence, although they are not ethically obligated to do so. A fourth foundational principle — justice — requires that clinicians attempt to ensure that the benefits and burdens of healthcare are distributed fairly. (shrink)

No student or colleague of Marjorie Grene will miss her incisive presence in these papers on the study and nature of living nature, and we believe the new reader will quickly join the stimulating discussion and critique which Professor Grene steadily provokes. For years she has worked with equally sure knowledge in the classical domain of philosophy and in modern epistemological inquiry, equally philosopher of science and metaphysician. Patient engagement in healthcare is increasingly discussed in the literature, and initiatives engaging patients in quality improvement activities, organizational design, governance, and research are becoming more and more common and have even become mandatory for certain health institutions. Here we discuss a number of ethical challenges raised by this engagement from patients from the perspectives of research, organizational/quality improvement practices, and patient experiences, while offering preliminary recommendations as to how to address them. We identified three broad categories of ethical issues (...) that intersect between the different types of patient engagement: (1) establishing a shared vision about goals of patient engagement and respective roles; (2) the process and method of engaging with patients; and (3) practical aspects of patient engagement. To explain these issues, we build from our personal, professional, and academic experiences, as well as traditions such as pragmatism and hermeneutics that stress the importance of participation, empowerment, and engagement. Patient engagement can be highly valuable at numerous levels, but particular attention should be paid to the process of engaging with patients and related ethical issues. Some lessons from the literature on the ethics of participatory research can be translated to organizational and quality improvement practices. (shrink)

Can an assessment of individuals’ narcissism help explain the quality of a respondent’s ethical judgment? How is the relationship between religiosity and ethical judgment moderated by the effects of narcissism? With a sample of 385 undergraduate business majors, this study uses a taxonomic approach to examine the effects of intrinsic and extrinsic religiosity as well as orthodox Christian beliefs on ethical judgment. Three distinct clusters were identified: Skeptics, Nominals, and Devouts. Surprisingly, of the three clusters, Nominals and Devouts were the (...) only groups impacted by narcissism, although Skeptics overall demonstrate the worst ethical judgment. (shrink)

Children adopt an inherence-based view of some social categories, viewing certain social categories as reflecting the inherent features of their members. Thinking of social categories in these terms contributes to prejudice and intergroup conflict. Thus, understanding what leads children to apply inherence-based views to particular categories could provide new direction for efforts to reduce these negative social phenomena.

Neuroconstructivist modeling can be usefully extended with algorithms that build their own topology and recruit existing knowledge, effectively constructing a hierarchy of network modules. Possible benefits include allowing abilities to emerge naturally, in a way that affords objective study, deeper insights, and more rapid progress, and provides more serious consideration of the implications of constructivism.
