Indeed, it is Aquinas's gradual integration of satisfaction as a motive for the Incarnation subordinate to love (166) that enables Aquinas aptly to locate satisfaction within the Christian life (cp. 47, 136, 142, 166) and accounts for Cessario's subtitle. Third, I am not clear on Ccssario's (or my own!) (...) stand on how we ought sort out the respects in which Thomas's and our perspectives merge (cp. xiv). In his conclusion Cessario is cautious, suggesting that "if the term ' satisfaction ' cannot be restored to current usage, then certainly the substance of Saint Thomas's understanding of satisfaction can and should be." (256). But, given Cessario's own persuasive display of the way satisfactio pervades the very texture of Aquinas's theology, I am not sure we can have the "substance" without the term-or vice-versa. Clearly Cessario thinks the merging is substantive. But he himself teaches us that supporting this claim involves supporting claims for exegetical, liturgical ecclesial, and other practices. Those of us who think that satisfactio is a description rather than "the guiding model or key notion" (xx, my emphasis) for thinking Christ crucified will need to see such support worked out in relation to the competing sotcriologies Cessario mentions (267) as well as ecumenical discussions of justification which were understandably outside the scope of this book (151). But a single book can only accomplish a single thing-and this book clearly succeeds in showing us how Thomas can be brought to bear on contemporary theological issues. How We Know is a collection of six papers delivered in 1984 at the twentieth Nobel Conference, held at Gustavus Adolphus College in St. Peter, Minnesota. Two words that came to my mind as I worked through the papers were "fascinating" and "frightening". The Conference dealt with cognitive science and a number of the authors have come up with interesting, provocative, and often orig'inal observations about how the human mind operates when it hies to remember and to understand. In his Introduction Michael Shafto, who is a Scientific Officer in the Personnel and Training Research Programs of the United States Office of Naval Research, notes that cognitive science as the work of a coalition of thinkers is barely a decade old, and he reminds the reader that the methods BOOK REVIEWS 59.7 of cognitive science are those of experimental psychology, physiology, and computer science. Though the Confermce was supposedly aimed at nonspecialists, the reader unfamiliar with cognitive science might find some of the papers a bit difficult. Certainly the backgrounds of the authors are very impressive. The first four papers in the collection are by Gerald Edelman, Brenda Milner, Roger C. Schank, and Herbert A. Simon respectively. Edelman is the Director and Scientific Chairman of the Neurosciences Research Program at Rockefeller University; Milner is Professor of Psychology in the Department of Neurology at McGill University: Schank is Professor and Chairman of the Computer Science Department at Yale; and Simon is Professor of Computer Science and Psychology at Carnegie-Mellon University. Calling his theory of the hum:m brain "Neural Darwinism," Edelman claims that the brain is Darwinian. Taking Darwin's theory of natural selections and applying it to the brain, Edelman argues that during ontogeny and behavior those groups of neurons are chosen that are adaptive for the organism. Milner explores the neurophysiological basis of that special moment when a new experience affects the brain. Insisting that memory is not merely a passive reproi!ucer but rather actively selective, Milner, relying on her studies of patients. with loss of memory, suggests that the frontal lobes of the brain hnve a special role in memory. Between 1873 and 1876, Felix Klein published a series of papers that he later placed under the rubric anschauliche Geometrie in the second volume of his collected works (1922). The present study attempts not only to follow the course of this work, but also to place it in a larger historical context. Methodologically, Klein’s approach had roots in Poncelet’s principle of continuity, though the more immediate influences on him came from his teachers, Plücker and Clebsch. In the 1860s, Clebsch reworked (...) some of the central ideas in Riemann’s theory of Abelian functions to obtain complicated results for systems of algebraic curves, most published earlier by Hesse and Steiner. These findings played a major role in enumerative geometry, whereas Plücker’s work had a strongly qualitative character that imbued Klein’s early studies. A leitmotif in these works can be seen in the interplay between real curves and surfaces as reflected by their transformational properties. During the early 1870s, Klein and Zeuthen began to explore the possibility of deriving all possible forms for real cubic surfaces as well as quartic curves. They did so using continuity methods reminiscent of Poncelet’s earlier approach. Indeed, I would say I have frequently been obsessed with them. I am 52 years old, and date the onset of my psychiatric symptoms—my long-term misery—very specifically: 11:00 pm Pacific Standard Time, August (...) 28, 1965. At almost precisely that time, I started obsessing that I was homosexual and, in fact, turning into a girl. Within days, derealization followed: for instance, the houses in my neighborhood seemed one-dimensional—they seemed as though they were painted, as though I could stick my hand or an object through them. Colors seemed less bright as well—but I hardly had words for all this at the time: it was an overwhelmingly preverbal feeling, but I found some words for it.I was also depressed, and knew I was depressed—that is, I was worried, had trouble sleeping, and had suicidal thoughts and feelings. Within weeks, I asked my parents if I could see a psychiatrist, and I did. I remember asking my psychiatrist if he ever thought about the possibility that everything might be an illusion, and he quoted the refrain from "Row, Row, Row Your Boat": "merrily, merrily, merrily, merrily, life is but a dream." But just asking him produced a chilling feeling that he did not exist and, largely for this reason, I did not mention unreality feelings again to therapists for another 20 years.So, like Dr. Wells' patient Edward, described by James Phillips (2003), I was in trouble; like Edward, I was starting to construct a narrative. I started feeling that I had "to tell my story" at about this time—before then, I had not felt a need to do that. That may be partly because young children take life for granted (at least, I did), and it may be partly because suffering, by its very nature, demands explanation; both reasons were probably operating—certainly, both my parents told me years later that I had seemed emotionally distressed as a child, and my mother told they would have given me Ritalin if it had been available. So, to other people (my parents and my teachers) I was mentally ill in some sense, and I have some pretty clear memories of what I now recognize as depression, especially in the second and third grades; but I did not think of myself as sick and I did not think of myself as suffering—and I did not experience the duality (at least) of selves that I started to experience almost immediately in August 1965.Phillips further points out that the "future Edward" could look back in his narrative, at a later age, and reconstruct his history either as someone with a continuing mental illness, or as someone who had attained a secure, healthy adulthood and saw his experience as adolescent pain in the service of ultimate adult health and integration. I see my story, I am sorry to report, as [End Page 369] one of unremitting basic pain but with the positive dimension that I was able to go on, obtain a good education, work a good deal in my adult life, and father a child. But my narrative—and am I not, as a highly verbal person, almost constrained to think of my life as a narrative?—has been one, as Phillips (following Kleinman) realizes, of suffering. It is, above all else, an illness narrative. I find the approach of J. Melvin Woody (2003)—of privileging performing and performing arts and spatial metaphors over story—less relevant and true to myself, although I see no reason to argue with Mary's preference for such forms. It may be that the temporal discontinuities she has experienced predispose her to see herself in such terms—may, in fact, necessitate doing so. While the prevalence of opioid use disorder among pregnant women has multiplied in the United States in the last decade, buprenorphine treatment for peripartum women with OUD has been administered to reduce risks of repeated cycles of craving and withdrawal. However, the maternal behavior and bonding in mothers with OUD may be altered as the underlying maternal behavior neurocircuit is opioid sensitive. In the regulation of rodent maternal behaviors such as licking and grooming, a series of opioid-sensitive brain regions are (...) functionally connected, including the ventral pallidum. In humans, these brain regions, interact with the supplementary motor area to regulate maternal behaviors and are functionally dysregulated by opioids. It is unclear how these brain regions respond to the emotions of their child for mothers receiving BT. In this functional magnetic resonance imaging pilot study in 22 mothers within the first postpartum year, including six mothers receiving BT and 16 non-OUD mothers as a comparison group, we devised a child face mirroring task in fMRI settings to assess maternal responses to pictures of facial expressions of own child and an unknown child in an empathic mirroring condition and a non-mirroring observation condition. In each condition, faces of neutral, ambiguous, distressed, and joyful expressions of each child were repeatedly displayed in a random order. The response of SMA during empathic mirroring vs. non-mirroring of own child was reduced among BT/OUD vs. CG. Within MBN, the left VP, critical for parental sensitivity, had a similar deficit. This study outlines potential mechanisms for investigating the risks of deficits in the neural responses to actual maternal sensitivity and parenting behavior in mothers with OUD, and potential targets for interventions that reduce stress and augment maternal behavior and child outcome. In 2012, a U.S. Institute of Medicine report called for a different approach to health care: “Left unchanged, health care will continue to underperform; cause unnecessary harm; and strain national, state, and family budgets.” The answer, they suggested, would be a “continuously learning” health system. Ethicists and researchers urged the creation of “learning health organizations” that would integrate knowledge from patient‐care data to continuously improve the quality of care. Our experience with an ongoing research study on atrial fibrillation—a trial known (...) as IMPACT‐AFib—gave us some insight into one of the challenges that will have to be dealt with in creating these organizations. Although the proposed educational intervention study placed no restrictions on what providers and health plans could do, the oversight team argued that the ethical principle of beneficence did not allow the researchers to be “bystanders” in relation to a control group receiving suboptimal care. In response, the researchers designed a “workaround” that allowed the project to go forward. We believe the experience suggests that what we call “bystander ethics” will create challenges for the kinds of quality improvement research that LHOs are designed to do. (shrink)

Moral distress is a major problem for nurses, other clinicians, and the health system itself. But if properly understood and responded to, it is also a promising guide for healthcare improvement. When individuals experience moral distress or burnout, their reports must be seen as crucial data requiring careful attention to the individuals and to the organization. Distress and burnout will often point to important opportunities for system improvements, which may in turn reduce the experience of distress. For this potential virtuous (...) cycle to happen, individuals must be able to articulate their concerns without fear of retribution, and organizational leaders must be able to listen in an undefensive, improvementoriented manner. James E. Bruce explores the relationship between morality and God’s free choices in the thought of Francis Turretin. The first book-length treatment of Turretin’s natural law theory, Rights in the Law provides an important theological backdrop to Early Modern moral and political philosophy. Turretin affirms Thomas Aquinas’s approach to the natural law, calling it the common opinion of the Reformed orthodox, but he develops it, too, by introducing a threefold scheme of right —divine, natural, and positive—to explain how change (...) within the law is possible. For example, God can change the specific day for Sabbath observance from Saturday to Sunday—from positive right—without changing the natural law precept that finite creatures ought to rest. Yet even with respect to the natural law God is still free. God can make a world in which there is no such thing as murder: he can choose not to make a world that contains such a thing as man. What God cannot do is make a murderable man. So God’s free choices determine the natural law insofar as the natural law is constituted by the nature of the things that God has chosen to create. Women's experiences with sexual harassment were analyzed with three types of variables: occupational and workplace sex ratios, organizational policies and procedures for dealing with sexual harassment problems, and women's cultural status. Regression analyses revealed that extent of contact with men was a key predictor of incidence of harassment, number of different types of harrassment, sexual comments, sexual categorical remarks, and sexual materials. Gender predominance was a significant predictor of physical threats and sexual materials. Informational methods were less successful than proactive (...) methods in reducing incidents of sexual harassment. The analyses support two generalizations. The “contact hypothesis” tested and verified by Gutek and her colleagues provides a substantive understanding of our findings on workplace and occupational numerical predominance. Second, organizations that take a variety of steps to address sexual harassment are more apt to be successful in curtailing the problem than those relying mainly on “get out the word” techniques. What are international codes of conduct for? The broad support for such codes masks fundamental differences about their purpose. Corporations see codes of conduct as regimes for regulating their relations with their suppliers in developing countries and—not least—to counter negative publicity. For labor and human rights activists, on the other hand, codes of conduct are levers for forcing positive change in global labor and environmental standards. Here I consider two areas typically covered by codes of conduct—wages and child labor—and identify (...) some of the dangers of using codes to force change. If low wages or child labor are the result of poverty, and can’t be fixed by enlightened corporate policies, then codes will at best leave the underlying problems untouched and at worst will aggravate them. I conclude that we should be cautious about using codes to force higher standards. (shrink)
