Background and Purpose The decision to perform decompressive craniectomy for patients with malignant MCA syndrome can be ethically complex. We investigated factors that clinicians consider in this decision-making process. Methods A survey including clinical vignettes and attitudes questions surrounding the use of hemicraniectomy in malignant MCA syndrome was distributed to 203 neurosurgeons, neurologists, staff and residents, and nurses and allied health members specializing in the care of neurological patients. These were practicing health care providers situated in an urban setting in (...) Canada where access to health care is covered by national policy and is a human right. Results Eighty-eight participants responded to survey. All participants unanimously supported the procedure for young and healthy patients. Advanced age, the presence of aphasia, comorbid medical conditions, and poor baseline functional status were dominant factors associated with increasing reluctance to offer surgical decompression. Patients’ previously expressed wishes were also an important consideration. Eighty-six percent of respondents agreed that withholding surgery is ethically justified if the outcome is perceived as futile. Conclusions Health care providers use similar factors to determine if aggressive management is justified given anticipated burden on patient quality of life and extended impact on society. This convergence can be harnessed beneficially for the transparent communication of medical options in the ethically complex setting of decompressive hemicraniectomy post-stroke. (shrink)

This essay draws a new picture of the science of bacteria in its 'golden age', circa 1880-1900: the organization of its knowledge and practice, its germ theory of disease, the difference between its two major research traditions, and, above all, its place in life science in this period that bristled with theories and debates over inheritance, variation, selection, evolution and that witnessed the transition from natural history to laboratory biology. Pasteur and Koch's science acquired this biological dimension not despite being (...) outside academic biology, nor despite the limitations of its applied, medical matrix, but rather because of that framework. The very practices of vaccine development constituted, at the same time, a new biological model of bacterial species and variation, which aligned them with other living things. Finally, the new picture reveals unsuspected continuity to later microbiology and molecular biology. In illuminating the self-perceptions of these sciences in relation to the past, it situates and opens a critical perspecive on writings by bacteriologists such as Ludwik Fleck, François Jacob and René Dubos, which have widely informed how we understand science. Should engineering ethics be taught? Despite the obvious truism that we all want our students to be moral engineers who practice virtuous professional behavior, I argue, in this article that the question itself obscures several ambiguities that prompt preliminary resolution. Upon clarification of these ambiguities, and an attempt to delineate key issues that make the question a philosophically interesting one, I conclude that engineering ethics not only should not, but cannot, be taught if we understand “teaching engineering ethics” to mean (...) training engineers to be moral individuals (as some advocates seem to have proposed). However, I also conclude that there is a justification to teaching engineering ethics, insofar as we are able to clearly identify the most desirable and efficacious pedagogical approach to the subject area, which I propose to be a case study-based format that utilizes the principle of human cognitive pattern recognition. It is no accident that the book Benjamin wrote as a reader of himself, A Berlin Childhood, also begins with the description of a park, that of the Tiergarten zoo. However great the difference may seem between this collection of short prose pieces and Proust's three-thousand-page novel when viewed from the outside, Benjamin's book illustrates [his] fascination... A sentence in his book points to the central experience of Proust's work: that almost everything childhood was can be withheld from a person (...) for years, suddenly to be offered him anew as if by chance. "Like a mother who holds the new-born infant to her breast without waking it, life proceeds for a long time with the still tender memory of childhood" . Also reminiscent of Proust is the description of the mother who, on evenings when guests are in the house, comes in to see her child only fleetingly to say good night; so, too, is that of the boy attentively listening to the noises which penetrate into his room from the courtyard below and thus from a foreign world. The studied elevation of the newly invented telephone to the level of a mythical object is anticipated in Proust as well. And the relationship to and influence of the earlier work can be demonstrated even in the use of metaphor. But little is gained by this approach, and it would not be easy to refute the objection that such similarities lie in the authors' common raw material: childhood, the fin de siècle epoch, and the attempt to bring them both into the present. His many influential works include Theorie des modernen Dramas , Versuch über das Tragische , and a five-volume collection of his lectures. not lie in the conceptual distinctions but in the perceived functions of metaphors and whether in the concrete case they are judged positive or negative. The ongoing debates reflect these concerns quite clearly~ namely that metaphors are judged on the basis of supposed dangers they pose and opportunities they offer. These are the criteria of evaluation that are obviously dependent on the context in which the transfer of meaning occurs. Our fundamental concern is indeed the transfer itself~ its prospects and (...) its limits. Looking at possible functions of metaphors is one approach to under standing and elucidating sentiments about them. The papers in this volume illustrate, by quite different examples, three basic functions of metaphors: illustrative, heuristic~ and constitutive. These functions rep resent different degrees of transfer of meaning. Metaphors are illustrative when they are used primarily as a literary device, to increase the power of conviction of an argument, for example. Although the difference between the illustrative and the heuristic function of metaphors is not great, it does exist: metaphors are used for heuristic purposes whenever "differences" of meaning are employed to open new perspectives and to gain new insights. In the case of "constitutive" metaphors they function to actually replace previous meanings by new ones. Sabine Maasen in her paper introduces the distinction between transfer and transforma tion. According to the conceptualist view in the philosophy of perception, we must possess concepts for all the objects, properties and relations which feature in our perceptual experiences. In this paper, I investigate the possibility of developing an argument against the conceptualist view by appealing to the notion of attention. In Part One, I begin by setting out an apparently promising version of such an argument, a version which appeals to a link between attention and perceptual demonstrative concept possession. In Part (...) Two, however, I show how the conceptualist can challenge what appears to be the key premise of the argument, and I go on to describe, in Part Three, an important further difficulty which we face if we attempt to overcome this challenge in a particular way. My conclusion will be that the conceptualist's challenge to the argument is convincing and hence that the argument remains inconclusive. (shrink)

In one, international relations is a Hobbesian state of nature in which moral judgments are entirely inappropriate, and in the other, states are analogous to persons in domestic society in having rights of autonomy that insulate them from external moral assessment and political interference. Beitz postulates that a theory of international politics should include a revised principle of state autonomy based on the justice (...) of a state's domestic institutions, and a principle of international distributive justice to establish a fair division of resources and wealth among persons situated in diverse national societies. This book proposes a new paradigm for the interpretation of Plato's early and middle dialogues. Rejecting the usual assumption of a distinct 'Socratic' period in the development of Plato's thought, this view regards the earlier works as deliberate preparation for the exposition of Plato's mature philosophy. Differences between the dialogues do not represent different stages in Plato's own thinking but rather different aspects and moments in the presentation of a new and unfamiliar view of reality. Once the fictional character of (...) the Socratic genre is recognised, there is no reason to regard Plato's early dialogues as representing the philosophy of the historical Socrates. The ArgumentIn Merton's early work in the sociology of science three theses are identified: economic and military influence in shaping early modern science; the “Puritan spur” to scientific activity; the critical role of a democratic social order for the support of science. These themes are located in the contemporary economic crisis of the 1930s, the rise of Nazism and fascism, and the emerging radical and Marxist political activism of scientists in the United States and the United Kingdom. Merton's interaction with (...) this context is critical for understanding his choice of problems and issues for the nascent sociology of science. The enunciation of the four-part normative structure of science is closely identified with the political ideals of this context. Merton's transition from an interest in problems of science in society to his postwar concern with the social and organizational structures of science and the social behavior of scientists is framed against the anticommunist and anti-Marxist thrust of immediate postwar politics in the United States. Charles Darwin's On the Origin of Species is unquestionably one of the chief landmarks in biology. The Origin (as it is widely known) was literally only an abstract of the manuscript Darwin had originally intended to complete and publish as the formal presentation of his views on evolution. Compared with the Origin, his original long manuscript work on Natural Selection, which is presented here and made available for the first time in printed form, has more abundant examples and illustrations (...) of Darwin's argument, plus an extensive citation of sources. In a series of recent papers, Emily Katz has argued that on Aristotle's view mathematical sciences are in an important respect no different from most natural sciences: They study sensible substances, but not qua sensible. In this paper, I argue that this is only half the story. Mathematical sciences are distinctive for Aristotle in that they study things ‘from’, ‘through’ or ‘in’ abstraction, whereas natural sciences study things ‘like the snub’. What this means, I argue, is that natural sciences must (...) study properties as they occur in the subjects from which they are originally abstracted, even where they reify these properties and treat them as subjects. The objects of mathematical sciences, on the other hand, can be studied as if they did not really occur in an underlying subject. This is because none of the properties of mathematical objects depend on their being in reality features of the subjects from which they are abstracted, such as bodies and inscriptions. Mathematical sciences are in this way able to study what are in reality non-substances as if they were substances. (shrink)
