Background: Obstetric labor and childbirth are mostly regarded as a physiological process, whereas social, cultural, psychological and transcendental aspects have received less attention. Labor support has been suggested to promote labor progress. The aim of this study was to investigate whether continuous labor support by a midwife promotes labor progress and vaginal delivery.Material and Methods: A randomized controlled study at a university hospital in Sweden in 2015–17. Primiparous women with singleton pregnancy and spontaneous labor onset were randomized to continuous support (...) or standard care during delivery. The primary outcome was the duration of active labor. Secondary outcomes were delivery mode, women's need of labor analgesia and satisfaction with delivery, maternal cortisol levels, and neonatal morbidity.Results: Continuous support was followed by shorter active labor 11.0 ± 5.7 h compared to 13.7 ± 3.9 h with standard care. Women in the continuous support group tended to have lower cortisol levels and low cortisol during the first and second stages of labor were correlated with shorter active labor. Continuous support was followed by spontaneous delivery in 73%, instrumental delivery in 24% and emergency cesarean section in 3% in contrast to standard care which was followed by spontaneous delivery in 62%, instrumental delivery in 24% and cesarean in 14%. The continuous support group received combined analgesic methods more often. Women's satisfaction with delivery and neonatal morbidity were comparable.Conclusion: Continuous labor support was followed by shorter active labor compared to standard care. Women with continuous support had a high rate of vaginal delivery and tended to have lower cortisol levels during all stages of active labor reflecting a lower stress level. Low cortisol was correlated to shorter active labor. Based on these results, we recommend continuous labor support for all primiparous women during active labor. (shrink)

Using different quantum states (e.g., two mode squeezed-state, multipartite GHZ-like-states) as quantum resources, two protocols for "continuous variable (CV) controlled quantum conference" are proposed. These CV protocols for controlled quantum conferences (CQCs) are the first of their kind and can be reduced to CV protocols for various other cryptographic tasks. In the proposed protocols, Charlie is considered the controller, having the power to terminate the protocol at any time and to control the flow of information among the other users by (...) using a parameterised control switch. Based on the information shared by Charlie with the participants of the conference, the control power of Charlie is evaluated and compared to the proposed protocols. The comparison of the efficiency of the proposed protocols has revealed that, under certain constraints, the 4-mode GHZ state-based protocol is more efficient than the two-mode squeezed state-based protocol. The control power Charlie is evaluated for the proposed protocols under certain constraints. A security analysis is also performed, and it is observed that the proposed protocols are secure against a variety of attacks, including but not limited to disturbance attacks, man-in-the-middle attacks, Trojan horse attacks, laser seeding attacks, cloning attacks, beam splitter attacks, and malicious user(s) attacks. This paper focuses on studying containment control problem with switching communication graphs of continuous-time heterogeneous multiagent systems where the control inputs are constrained in a nonconvex set. A nonlinear projection algorithm is proposed to address the problem. We discuss the stability and containment control of the system with switching topologies and nonconvex control input constraints under three different conditions. It is shown that all agents converge to the convex hull of the given leaders ultimately while staying in the nonconvex set (...) under the premise that at least one directed path from leaders to the agents exists in each bounded time interval. Finally, the validity of the results obtained in this paper is verified by simulation. (shrink)

ObjectiveThis study aims to evaluate the effectiveness of the MotivAir program—a phone-based intervention based on Motivational Interviewing principles and techniques—in enhancing adherence to Continuous Positive Airway Pressure therapy among patients with Obstructive Sleep Apnea Syndrome.MethodsA multicenter randomized controlled trial design with random allocation at the level of the individual will be conducted to compare the impact of the experimental program with a control group receiving usual care only in improving selected clinical and psychological parameters in the patients. A minimum sample (...) of 80 participants will be recruited in each center according to the inclusion criteria. After the initial screening, participants will be randomly assigned to either the experimental group or the control condition. The program will last 180 days and will be delivered by a trained nurse. The impact of the MotivAir program on selected primary and secondary outcomes will be measured at baseline, and after 1-, 3-, and 6-month from CPAP initiation.DiscussionParticipants are expected to show an increased level of adherence to CPAP and to acquire the skills and self-confidence necessary to deal with the psychological consequences of their chronic condition. Objective: To assess whether continuous consent, a process in which information is given to research participants at different stages in a trial, and clinician training in that process were effective when used by clinicians while gaining consent to the Total Body Hypothermia (TOBY) trial. The TOBY trial is a randomised controlled trial (RCT) investigating the use of whole-body cooling for neonates with evidence of perinatal asphyxia. Obtaining valid informed consent for the TOBY trial is difficult, but is a good test (...) of the effectiveness of continuous consent. Methods: Semistructured interviews were conducted with 30 sets of parents who consented to the TOBY trial and with 10 clinicians who sought it by the continuous consent process. Analysis was focused on the validity of parental consent based on the consent components of competence, information, understanding and voluntariness. Results: No marked problems with consent validity at the point of signature were observed in 19 of 27 (70%) couples. Problems were found mainly to lie with the competence and understanding of the parents: mothers, particularly, had problems with competence in the early stages of consent. Problems in understanding were primarily to do with side effects. Problems in both competence and understanding were observed to reduce markedly, particularly for mothers, in the post-signature phase, when further discussion took place. Randomisation was generally understood but unpopular. Information was not always given by clinicians in stages during the short period available before parents gave consent. Most clinicians, however, were able to give follow-up information. Discussion: Consent validity was found to compare favourably with similar trials examined in the Euricon study. Conclusion: Adopting the elements of the continuous consent process and clinician training in RCTs should be considered by researchers, particularly when they have concerns about the quality of consent they are likely to obtain by using a conventional process. Previous research has repeatedly demonstrated that providing learners with self-control over their feedback schedules enhances motor skill learning. Increased information processing under self-control conditions has been shown to contribute to these benefits. However, the timing of critical information processing for self-control participants during the acquisition of continuous tasks is unknown. The present study was designed to enhance clarity related to this issue. Participants learned a continuous tracing task under self-control (SC) or yoked (YK) conditions. Groups of participants also completed a (...) secondary cognitive load task either during or after the execution of each primary task trial. Results showed enhanced learning for SC compared to YK participants who did not complete the cognitive load task. However, this benefit was eliminated for SC participants who completed the cognitive load task either during or after the primary task. These findings suggest that effective information processing both during and after continuous task execution is critical for reaping the benefits of self-controlled practice. Further interpretations and implications of these findings as well as suggestions for future research are discussed. (shrink)

In the chemical production process, the temperature of the continuous reactor has nonlinear characteristics such as large inertia. An improved autodisturbance control method is proposed. By improving the tracking differentiator with adjustable parameters, the expanded state observer and the control structure obtained an improved automatic disturbance rejection control model and realized the optimal control of the nonlinear and large-delay systems. On the process control training system, the experiment of the continuous system process flow is compared with the anti-interference of the (...) Smith predictive compensation system, which uses an optimal set of data. The reactor temperature fluctuates, but the self-resistance is improved. The optimal overshoot of the disturbance is close to 0. Compared with the original antidisturbance controller, the antidisturbance time is not affected by the number of iterations, which reduces the high requirements on the accuracy of the parameters. Therefore, the improved automatic disturbance rejection control improves the operation and enhances the ability to resist external disturbance that changes the controlled object. (shrink)

Establishing the basic knowledge, methodology, and technology for a framework for the continuous decoding of hand/arm movement intention was the aim of the ERC-funded project “Feel Your Reach”. In this work, we review the studies and methods we performed and implemented in the last 6 years, which build the basis for enabling severely paralyzed people to non-invasively control a robotic arm in real-time from electroencephalogram. In detail, we investigated goal-directed movement detection, decoding of executed and attempted movement trajectories, grasping correlates, (...) error processing, and kinesthetic feedback. Although we have tested some of our approaches already with the target populations, we still need to transfer the “Feel Your Reach” framework to people with cervical spinal cord injury and evaluate the decoders’ performance while participants attempt to perform upper-limb movements. While on the one hand, we made major progress towards this ambitious goal, we also critically discuss current limitations. (shrink)

Do people sometimes exercise self-control in such a way as to bring it about that they do not act on present-directed motivation that continues to be motivationally strongest for a significant stretch of time (even though they are able to act on that motivation at the time) and intentionally act otherwise during that stretch of time? This paper explores the relative merits of two different theories about synchronic self-control that provide different answers to this question. One is due to Sripada (...) (Noûs 1–38, 2012) and the other to Mele (Irrationality, 1987; Autonomous agents, 1995; Motivation and agency, 2003). Special attention is paid to evidence Sripada offers for an affirmative answer to the question, and some guidance is offered on the project of finding evidence for an affirmative answer. (shrink)

Based on the dual mechanisms of control theory, there are two distinct mechanisms of cognitive control, proactive and reactive control. Importantly, accumulating evidence indicates that there is a developmental shift from predominantly using reactive control to proactive control during childhood, and the engagement of proactive control emerges as early as 5–7 years old. However, less is known about whether and how proactive control at this early age stage is associated with children’s other cognitive abilities such as working memory and math (...) ability. To address this issue, the current study recruited 98 Chinese children under 5–7 years old. Among them, a total of 81 children contributed useable data for the assessments of cognitive control, working memory, and math ability. The results revealed that children at this age period predominantly employed a pattern of proactive control during an AX-Continuous Performance Task. Moreover, the proactive control index estimated by this task was positively associated with both working memory and math performance. Further regression analysis showed that proactive control accounted for significant additional variance in predicting math performance after controlling for working memory. Most interestingly, mediation analysis showed that proactive control significantly mediated the association between working memory and math performance. This suggests that as working memory increases so does proactive control, which may in turn improve math ability in early childhood. Researchers often claim that self-control is a skill. It is also often stated that self-control exertions are intentional actions. However, no account has yet been proposed of the skillful agency that makes self-control exertion possible, so our understanding of self-control remains incomplete. Here I propose the skill model of self-control, which accounts for skillful agency by tackling the guidance problem: how can agents transform their abstract and coarse-grained intentions into the highly context-sensitive, fine-grained control processes required to select, revise and (...) correct strategies during self-control exertion? The skill model borrows conceptual tools from ‘hierarchical models’ recently developed in the context of motor skills, and asserts that self-control crucially involves the ability to manage the implementation and monitoring of regulatory strategies as the self-control exercise unfolds. Skilled agents are able do this by means of flexible practical reasoning: a fast, context-sensitive type of deliberation that incorporates non-propositional representations into the formation and revision of the mixed-format intentions that structure self-control exertion. The literatures on implementation intentions and motivation framing offer corroborating evidence for the theory. As a surprising result, the skill of self-control that allows agents to overcome the contrary motivations they experience is self-effacing: instead of continuously honing this skill, expert agents replace it with a different one, which minimizes or prevents contrary motivations from arising in the first place. Thus, the more expert you are at self-control, the less likely you are to use it. (shrink)

Initially proposed by Julian Rotter in 1966, the year 2016 marks the 50th anniversary of this remarkable breakthrough, subsequently inspiring thousands of research studies in the human sciences - research that has only served to deepen the utility of this amazing concept. Edited by John W. Reich and Frank J. Infurna, Perceived Control: Theory, Research, and Practice in the First 50 Years (...) commemorates this important anniversary by featuring contributions from leading figures of the time - some of whom were there at the very beginning of Rotter's breakthrough - to give readers a valuable historical record and measuring stick to illustrate how far we've come. Other contributors to this volume expertly present contemporary and cutting-edge summaries of the current state of our knowledge all while giving us a roadmap for future developments and directions. What have these developments revealed about basic human strengths and capacities? Why has this concept proven so remarkably effective in illuminating our everyday life in sickness and health? Perceived Control is a fascinating work that incorporates research from Rotter's original concept, and addresses many of the leading comparable concepts that have since evolved: self-efficacy, personal mastery, competence, primary and secondary control, and more specific topics such as health locus of control, learned helplessness, and other heuristic concepts discussed in many different fields of psychology and the allied disciplines. Continuous culture techniques were developed in the early twentieth century to replace cumbersome studies of cell growth in batch cultures. In contrast to batch cultures, they constituted an open concept, as cells are forced to proliferate by adding new medium while cell suspension is constantly removed. During the 1940s and 1950s new devices have been designed—called “automatic syringe mechanism,” “turbidostat,” “chemostat,” “bactogen,” and “microbial auxanometer”—which allowed increasingly accurate quantitative measurements of bacterial growth. With these devices cell growth came under the (...) external control of the experimenters and thus accessible for developing a mathematical theory of growth kinetics—developed mainly by Jacques Monod, Aron Novick and Leo Szilard in the early 1950s and still in use today. The paper explores the development of continuous culture devices and claims that these devices are simulators for standard cells following specific requirements, in particular involving mathematical constraints in the design and setting of the devices as well as experiments. These requirements have led to contemporary designs of continuous culture techniques realizing a specific event-based flow algorithm able to simulate directed evolution and produce artificial cells and microorganisms. This current development is seen as an alternative approach to today’s synthetic biology. (shrink)

Type 1 diabetes (T1D) is a chronic illness that requires intensive lifelong management of blood glucose concentrations by means of external insulin administration. There have been substantial developments in the ways of measuring glucose levels, which is crucial to T1D self-management. Recently, continuous glucose monitoring (CGM) has allowed people with T1D to keep track of their blood glucose levels in near real-time. These devices have alarms that warn users about potentially dangerous blood glucose trends, which can often be shared with (...) ther people. CGM is consistently associated with improved glycemic control and reduced hypoglycemia and is currently recommended by doctors. However, due to the costs of CGM, only those who qualify for hospital provision or those who can personally afford it are able to use it, which excludes many people. In this paper, I argue that unequal access to CGM results in: (1) unjust health inequalities, (2) relational injustice, (3) injustice with regard to agency and autonomy, and (4) epistemic injustice. These considerations provide prima facie moral reasons why all people with T1D should have access to CGM technology. I discuss the specific case of CGM policy in the Netherlands, which currently only provides coverage for a small group of people with T1D, and argue that, especially with additional considerations of cost-effectiveness, the Dutch government ought to include CGM in basic health care insurance for all people with T1D. We advance an account that grounds cognition, specifically decision-making, in an activity all organisms as autonomous systems must perform to keep themselves viable—controlling their production mechanisms. Production mechanisms, as we characterize them, perform activities such as procuring resources from their environment, putting these resources to use to construct and repair the organism's body and moving through the environment. Given the variable nature of the environment and the continual degradation of the organism, these production mechanisms must be regulated by control mechanisms (...) that select when a production is required and how it should be carried out. To operate on production mechanisms, control mechanisms need to procure information through measurement processes and evaluate possible actions. They are making decisions. In all organisms, these decisions are made by multiple different control mechanisms that are organized not hierarchically but heterarchically. In many cases, they employ internal models of features of the environment with which the organism must deal. Cognition, in the form of decision-making, is thus fundamental to living systems which must control their production mechanisms. (shrink)

To counter the pandemic caused by severe acute respiratory syndrome coronavirus 2, some have proposed accelerating SARS-CoV-2 vaccine development through controlled human infection trials. These trials would involve the deliberate exposure of relatively few young, healthy volunteers to SARS-CoV-2. We defend this proposal against the charge that there is still too much uncertainty surrounding the risks of COVID-19 to responsibly run such a trial.

Historical continuities and constraints in the professionalization of nursingThe support of medicine and the state may be crucial to nursing's current professional aspirations for legitimation and implementation of nursing reforms and for new roles for nurses in health care. As such, medicine and the state are in the invidious position of influencing nursing's occupational future. This situation is not new. An historical analysis of the establishment of nursing at Prince Alfred Hospital, Sydney, Australia, at the end of the nineteenth century (...) reveals that the State Government of NSW and the medical profession supported nursing's occupational development, yet set the framework within which this could occur. For instance, the state provided patronage to nursing through recommendations of the 1873 Royal Commission and because it financially backed Prince Alfred Hospital, while the medical profession defined nursing knowledge and practice through its control of the nursing curriculum. Membership of the hospital board provided both medicine and the state with powerful positions over hospital policies that affected nursing. While nursing became established as a distinct occupation for women with the aid of State and medical support, its subordinate position in health care was, and continues to be, constrained by these traditional supporters. This relationship between nursing, medicine and the state has implications for nursing's current professionalization strategies and aspirations. (shrink)

In circulation, T cells are spherical with selectin enriched dynamic microvilli protruding from the surface. Following extravasation, these microvilli serve another role, continuously surveying their environment for antigen in the form of peptide‐MHC (pMHC) expressed on the surface of antigen presenting cells (APCs). Upon recognition of their cognate pMHC, the microvilli are initially stabilized and then flatten into F‐actin dependent microclusters as the T cell spreads over the APC. Within 1–5 min, clathrin is recruited by the ESCRT‐0 component Hrs to (...) mediate release of T cell receptor (TCR) loaded vesicles directly from the plasma membrane by clathrin and ESCRT‐mediated ectocytosis (CEME). After 5‐10 min, Hrs is displaced by the endocytic clathrin adaptor epsin‐1 to induce clathrin‐mediated trans‐endocytosis (CMTE) of TCR‐pMHC conjugates. Here we discuss some of the functional properties of the clathrin machinery which enables it to control these topologically opposite modes of membrane transfer at the immunological synapse, and how this might be regulated during T cell activation. A common criticism of free will or origination theories is that if what we do is not the result of an unbroken sequence of causes and effects, then it must to some degree be the product of chance. But in what sense can a chance act be intentional or deliberate, in what sense can it be based on reasons, and in what sense can a person be held responsible for it? If free and responsible action is incompatible with determinism, must (...) it not equally well be incompatible with indeterminism? Professor McCall says no. He argues that a new idea, that of a controlled indeterministic process, resolves a variety of classical dilemmas and opens the way to a new understanding of the relationship between actions, reasons, causes, and responsibility. Does he succeed? All of this, like a related line of argument by Professor McCann to which you can turn, is a long way from what seems to me the continuing arguableness of determinism and the unavoidableness of the proposition that both incompatibilism and compatibilism about freedom are false. But we all need to remember, with Cromwell, in our own bowels if not by those of Christ, that we may be mistaken. I guess that given the proportion of false to true views in the world, we need to remember it is arguable that we are more likely to be mistaken. (shrink)

Later on, the Pasteur Institute maintained control of these standards in the framework of a commercial monopoly that it established on the veterinary vaccines first sent and then cultivated abroad by the Société de Vulgarisation du Vaccin Charbonneux Pasteur, founded in 1886. The paper dealt with the control on political corruption in Italy, in particular with the reasons why most of the control mechanisms did not work for a long time, allowingfor the development of"tangentopoli". First of all, we briefly discussed the reasons why the controls ''from below"--that is, from citizens or electors--did not function in Italy: the pervasive occupation of the administration and the civil society by the political parties, as well as "secret" agreements between political parties in order to avoid (...) political scandals were discussed.The paper continued by analyzing two types of institutional control: the administrative controls and the judiciary controls. In the second part, we presented some main characteristics of the Italian public administration that hampered internal controls, the informal control of parties over the bureaucrats through clientelism and complicity in corruption being one among them. In the third part, we focused on the peculiar characteristics of a magistracy that enjoyed of a very high degree of format autonomy from the political power, but was "pushed" towards politics for reasons as different as complicity in corruption and the need to "substitute" for the weak policy making capacity of political parties. In the fourth part, we reviewed some reform bills to deal with the control of political corruption. (shrink)

In this view from 2007–2009, the ethical challenges facing a potential global effort to control infectious disease are explored; they provide sobering insight into the challenges of later decades. Despite the devastating pandemic of HIV/AIDS that erupted in the early 1980s, despite the failure to eradicate polio and the emergence of resistant forms of tuberculosis that came into focus in the 1990s, and despite newly emerging diseases like Severe Acute Respiratory Syndrome (SARS) in 2003 and the fearsome prospect of human-to-human (...) avian flu, it is nevertheless a time of some excitement over prospects for effective control of much of infectious disease. Funded by national and international governmental and nongovernmental organizations (NGOs), private foundations, and even popular entertainers, large-scale new efforts are under way to address global killers like AIDS, tuberculosis, and malaria, among others. This “marvelous momentum” can be seen as part of a continuing effort from the time of Jenner onward. Extrapolating from this, we explore the notion of a “comprehensive global effort for the eradication, elimination or control of infectious disease”, with particular attention to the ethical issues that arise. This is to “think big” about disease-control efforts that are now often done in piecemeal ways. This chapter identifies five tracks along which such efforts need to be pursued: (1) national and international organizations and the development of collective will; (2) epidemiological and health care infrastructure; (3) scientific development; (4) religious, social, and cultural considerations; (5) legal and social protections for individuals and groups. Each of these poses significant ethical issues which, we argue, should be viewed in a comprehensive way, to ensure that practice, research, and policy in each of these areas understands the person with communicable infectious disease as both victim and (potential) vector. The abstract justified the need to develop the theory of implementation of the Chinese railway initiative "One Belt, One Road" on Russian territory, focused on the docking of Chinese socialism and modern Russian capitalism. It is shown that the introduction of control, blocking corruption and managing costs in the process of construction of railway and related facilities, will have an impact on the economic efficiency of the construction of OPOP. In order to carry out control, it is necessary to digitize (...) the construction processes, by forming an informational regulatory and legislative framework. A class of models is presented, in the form of continuation monads polymorphic for first-order individuals, that is sound and complete for minimal intuitionistic predicate logic . The proofs of soundness and completeness are constructive and the computational content of their composition is, in particular, a β-normalisation-by-evaluation program for simply typed lambda calculus with sum types. Although the inspiration comes from Danvyʼs type-directed partial evaluator for the same lambda calculus, the use of delimited control operators is avoided. Although the extent to which motivational factors are involved in the production and sustaining of biased or 'irrational' beliefs continues to be a controversial issue in social psychology, even those who urge that such beliefs are often explained by non-motivational tendencies admit that biased beliefs sometimes have motivational sources. Sometimes toe are influenced by motivational pressures in ways proscribed by principles that we accept for belief-acquisition or belief-revision ('doxastic' principles). Many garden-variety instances of self-deception are cases in point. We are (...) not always helpless victims of those pressures, however. This paper examines the nature of doxastic self-control (roughly, a capacity to counteract motivational pressures that incline us to acquire or retain beliefs that would violate our doxastic principles) and explores our prospects for avoiding motivationally biased believing by exercising self-control. Rachlin overlooks that free will determines when and in what direction acts that appear impulsive will occur. Because behavioral patterns continuously evolve, animals are not guaranteed when they will, or how to, maximize larger-later reinforcements. An animal therefore uses self-control to emit free acts to vary behavioral patterns to optimize larger-later rewards.

In quantity discrimination tasks, adults, infants and animals have been sometimes observed to process number only after all continuous variables, such as area or density, have been controlled for. This has been taken as evidence that processing number may be more cognitively demanding than processing continuous variables. We tested this hypothesis by training mosquitofish to discriminate two items from three in three different conditions. In one condition, continuous variables were controlled while numerical information was available; in another, the number was (...) kept constant and information relating to continuous variables was available; in the third condition, stimuli differed for both number and continuous quantities. Fish learned to discriminate more quickly when both number and continuous information were available compared to when they could use continuous information only or number only; there was no difference in the learning rate between the two latter conditions. Our results do not support the hypothesis that processing numbers imposes a higher cognitive load than processing continuous variables. Rather, they suggest that availability of multiple information sources may facilitate discrimination learning. (shrink)

Since the end of the 198os, we have been witnessing a shift in the type of control that is exerted on salaried labor. The disciplinary control of Taylorist provenance has been giving way to a modular control which introduces a new freedom, the freedom of the salaried employee to modulate his or her laboring activity, and, in part, mobility and usage of tithe. But this new type of control is above all a control of the subjective commitment of the employee, (...) an evaluation of performance which tends to equate the employee with a form of micro-capital, entrusted with valorizing itself economically and justifying its continuing employment. But the positive aspect of this mutation should also be noted: the personal commitment of the employee is also a forma of initiative taken in real-life situations, a production of services for various audiences, which produce, in the strong sense of the term, an ethical investment in how labor activity is conducted, along with a new evaluation of productivity. The strictly personal perspective is not the whole picture, for it conceals the emergence - real or potential - of communities of commitment. It is in terms of this mutation that both the usage of time and the modalities of salaried wages need to be rethought. (shrink)

Samuel H. LiPuma and Joseph P. DeMarco argue for a positive right to continuous sedation until death (CSD) for any patient with a life expectancy less than six months. They reject any requirement of proportionality. Their proposed guideline makes CSD an option for a decisional adult patient with an appropriate terminal diagnosis regardless of whether suffering (physical or existential) is present. This guideline purports to “empower” the patient with the ability to control the timing and manner of her death. This (...) extends even to the option to “opt out” of the awareness and experience of dying and to avoid suffering altogether, even if one’s symptoms and suffering could be effectively treated.We respond first with a critique of their terminology. We then turn to some purely practical considerations of how this guideline might be enacted in the current atmosphere of American hospice and palliative care medicine. We close with a consideration of one philosophical concern that might ground the discussion of risks, benefits, and alternatives necessary for informed consent. (shrink)

Continuous deep sedation at the end of life is a practice that has been the topic of considerable ethical debate, for example surrounding its perceived similarity or dissimilarity with physician-assisted dying. The practice is generally considered to be legal as a form of symptom control, although this is mostly only assumed. France has passed an amendment to the Public Health Act that would grant certain terminally ill patients an explicit right to continuous deep sedation until they pass away. Such a (...) framework would be unique in the world. In this paper we will highlight and reflect on four relevant aspects and shortcomings of the proposed bill. First, that the bill suggests that continuous deeps sedation should be considered as a sui generis practice. Second, that it requires that sedation should always be accompanied by the withholding of all artificial nutrition and hydration. In the most recently amended version of the legal proposal it is stated that life sustaining treatments are withheld unless the patient objects. Third, that the French bill would not require that the suffering for which continuous deep sedation is initiated is unbearable. Fourth, the question as to whether the proposal should be considered as a way to avoid having to decriminalise euthanasia and/or PAS or, on the contrary, as a veiled way to decriminalise these practices. The French proposal to amend the Public Health Act to include a right to continuous deep sedation for some patients is a unique opportunity to clarify the legality of continuous deep sedation as an end-of-life practice. Moreover, it would recognize that the practice of continuous deep sedation raises ethical and legal issues that are different from those raised by symptom control on the one hand and assisted dying on the other hand. Nevertheless, there are still various issues of significant ethical concern in the French legislative proposal. (shrink)

Although the study of visual perception has made more progress in the past 40 years than any other area of cognitive science, there remain major disagreements as to how closely vision is tied to general cognition. This paper sets out some of the arguments for both sides and defends the position that an important part of visual perception, which may be called early vision or just vision, is prohibited from accessing relevant expectations, knowledge and utilities - in other words it (...) is cognitively impenetrable. That part of vision is complex and articulated and provides a representation of the 3-D surfaces of objects sufficient to serve as an index into memory, with somewhat different outputs being made available to other systems such as those dealing with motor control. The paper also addresses certain conceptual and methodological issues, including the use of signal detection theory and event-related potentials to assess cognitive penetration of vision. A distinction is made among several stages in visual processing. These include, in addition to the inflexible early-vision stage, a pre-perceptual attention allocation stage and a post-perceptual evaluation, memory-accessing, and inference stage which provide several different highly constrained ways in which cognition can affect the outcome of visual perception. The paper discusses arguments that have been presented in both computer vision and psychology showing that vision is "intelligent" and involves elements of problem solving". It is suggested that these cases do not show cognitive penetration, but rather they show that certain natural constraints on interpretation, concerned primarily with optical and geometrical properties of the world, have been compiled into the visual system. Sadly, the contributions of Daniel Remein, chief rogue at the Organism for Poetic Research as well as editor at Whiskey & Fox , were not able to appear in this version of the proceedings. THE RUINS: THE QUESTION OF DISCIPLINARITY IN THE POST/MEDIEVAL UNIVERSITY” SEPTEMBER 21ST, 2012: SESSION 13 MCLEOD C.322, CURRY STUDENT CENTER NORTHEASTERN UNIVERSITY, BOSTON, MA. Traditionally, a wayzgoose was a celebration at the end of a printer’s year, a night off in the late fall before the work began of printing by candlelight. According to the OED, the Master Printer would make for the journeymen “a good Feast, and not only entertains them at his own House, but besides, gives them Money to spend at the Ale-house or Tavern at Night.” Following in this line, continent. proposes in its publication(s) a night out and a good Feast, away from the noxious fumes of the Academy and into a night of revelry which begins, but does not end, at the alehouse or Tavern. continent. proposes that the thinking of the Academy be freed to be thought elsewhere, in the alleys and doorways of the village and cities, encountered not in the strictly defined spaces of the classroom and blackboard (now white) but anticipated and found where thinking occurs. Historically, academic journals have served a different purpose than the Academy itself. Journals (from the Anglo-Fr. jurnal , "a day," from O.Fr. jornel , "day, time; day's work," hence the journalist as writer of the news of the day ) have served as privileged sites for the articulation and concretization of specific modes of knowledge and control (insemination of those ideas has been formalized in the classroom, in seminar). In contrast, the academic journal is post-partum and has been an old-boys club, an insider trading network in which truths are (re)circulated against themselves, forming a Maginot Line against whatever is new, or the distinctly challenging. All in a Jurnal’s Work will discuss (in part) the ramifications of cheap start-up publications that are challenging the traditional ensconced-in-ivory academic journals and their supporting infrastructures. The panel will be seeking a questioning (as a challenging) towards the discipline of knowledge production/fabrication (of truth[s]) and the event of the Academy (and its publications) as it has evolved and continues to (d)evolve. Issues to be discussed will revolve around the power of academic publishing and its origins, hierarchical versus horizontal academic modules (is there a place for the General Assembly in academia?) and the evolving idea of the Multiversity as a site(s) of a (BABELing) multivocality in the wake of the University of Disaster. A CONTINUOUS ACT Nico Jenkins In Pierre Hadot’s extraordinary book, Philosophy as a Way of Life , the practices of philosophy—that is the exercise of what we can term pre-institionalized love of wisdom, what Philo of Alexandria described as a training towards wisdom—are described as, following the Stoics, “a continuous act, permanent and identical with life itself, which had to be renewed at each instant.” This renewal of thinking, this coming to be of being itself —meeting itself on its own ground, concerns the way philosophy is practiced, and more often, taught, or rather not taught. Hadot continues his thinking with a description of what happens to the structure of thought in the medieval ages as it becomes adopted—co-opted— by the university, and by extension, by the institution of the church. Philosophy becomes no longer a way of living, no longer a praxis as such but becomes a condition that is locked in a theoretical construct, one which was literally removed from life (and in life we read then love, wisdom, being etc, also the home, the market, the field, the street) and secured behind the high walls of the monastery (which were shortly replaced by the high walls of the Academy) where thinking unfortunately rests for the most part today. Hadot writes further that this dangerous movement of removal reduces thinking to a theoretical practice akin to the mythical Ouroborous; “education was thus no longer directed toward people who were to be educated with a view to becoming fully developed human beings, but to specialists, in order that they might learn how to train other specialists.” Thought then is trapped behind the walls of the academy, and with the exception of such thinkers as Spinoza, Descartes, and Liebnitz as well as others who think from an outside in, thought remains, in the form of Kant, Fichte, Hegel, a practice reduced or removed from life and restricted to what Schopenhauer will call “mere fencing in front of the mirror.” This is of course the tendency today and philosophy remains, for the most part, a discourse produced and thought inside the academy and disseminated, in seminated through university presses and of course the academic journal. It seems necessary to me that we have to have a place, a return, to a thinking which is closer to the Stoics, closer to the pre-Socratics, to a thinking which is a practice of becoming, a training to think, to live, to die. this is not south romantic than imperative . This thinking is one of a deep ir responsibility because it is not known, it is not figured out. It requires a risk and what Heidegger calls a leap (for there is no bridge to it). I call it irresponsible because for too long sanctioned thought has had as its premise the idea that thinking has a goal, a direction, a telos ; that it is not an activity but a process which gives a product; that the responsibility of thinking in turn demand an answer. In my mind, only freed from that goal can thinking, in contrast, bask in its own thinking, bask in a state in which the unknown can remain unknown, that mystery can rest as mystery. This is not to say that the world needs no answers, or to promote a Whitmanesque “leaning and loafing” as the only valid practice. It is only to say that for too long, thinking has been validated by the academy, by the answerable, by the already decided. To me, this requires—as an answer— the ir responsibility of thought, what Nancy calls, “a world for which all is not already done (played out, finished, enshrined in a destiny), nor entirely still to do (in the future for always future tomorrows).” This is a thinking not sanctified by the academy, made sacred by the church or the palace but rather it takes place on the périphérie , beyond the ring road, in alleyways behind the marketplace, in cafes stained with the syphilitic patina of irresponsible talk, of loose talk, the kind of talk made loose not only by the tankard and the goblet by the practice and training of attuned thinking. continent. was formed as a collective of thinkers coming out of the European Graduate School (also known as the University of Disaster) three years ago, in an effort to combat, or challenge, the dominant paradigm which isolates thinking from the street, from life, from where perhaps wisdom tends to emerge. We feel that not only is the university herself no longer the privileged site of where true thinking takes place—and where only official thought can take place—but that the very artificiality of the academy denies thinking—at times—authentic, thought. Our goal at continent. is to create a media agnostic publication which is rigorous in its intellectual underpinnings but which will remain permanently beyond and out of reach of the academy. Though many of us butter our bread with academic paychecks (maid!) we attempt to keep continent. as a refuge—and a refugee—from the University in ruins, from the University as a site of preformed and institutionalized pre-set dialogues. In concert with other publications-both cyber and print—as well as various blogs we are attempting to re-dialogize the dialogue of thinking. We are attempting both to speak to— as well as with and against —the university as a site of accreted knowledge. We have not been utterly successful and continue to attempt to define what that role of being perpetually beyond is (while still trying to maintain rigorous intellectual standards) but it is just that, a goal, which is perpetually opening, perpetually unanswered, perhaps even by design, perpetually unanswerable. The principal goal of the brain-computer interface is to translate brain signals into meaningful commands to control external devices or neuroprostheses to restore lost functions of patients with severe motor disabilities. The invasive recording of brain signals involves numerous health issues. Therefore, BCIs based on non-invasive recording modalities such as electroencephalography are safer and more comfortable for the patients. The BCI requires reconstructing continuous movement parameters such as position or velocity for practical application of neuroprostheses. The BCI studies in continuous (...) decoding have extensively relied on extracting features from the amplitude of brain signals, whereas the brain connectivity features have rarely been explored. This study aims to investigate the feasibility of using phase-based connectivity features in decoding continuous hand movements from EEG signals. To this end, the EEG data were collected from seven healthy subjects performing a 2D center-out hand movement task in four orthogonal directions. The phase-locking value and magnitude-squared coherence are exploited as connectivity features along with multiple linear regression for decoding hand positions. A brute-force search approach is employed to find the best channel pairs for extracting features related to hand movements. The results reveal that the regression models based on PLV and MSC features achieve the average Pearson correlations of 0.43 ± 0.03 and 0.42 ± 0.06, respectively, between predicted and actual trajectories over all subjects. The delta and alpha band features have the most contribution in regression analysis. The results also demonstrate that both PLV and MSC decoding models lead to superior results on our data compared to two recently proposed feature extraction methods solely based on the amplitude or phase of recording signals. This study verifies the ability of PLV and MSC features in the continuous decoding of hand movements with linear regression. Thus, our findings suggest that extracting features based on brain connectivity can improve the accuracy of trajectory decoder BCIs. (shrink)

Epithelial stem cells have been identified in integumental structures such as hairs and continuously growing teeth of various rodents, and in the gut. Here we propose the involvement of epithelial stem cells in the continuous tooth replacement that characterizes non‐mammalian vertebrates, as exemplified by the zebrafish. Arguments are based on morphological observations of tooth renewal in the zebrafish and on the similarities between molecular control of hair and tooth formation. Dissection of the molecular cascades underlying the regulation of the epithelial (...) stem cell niche might open perspectives for new regenerative treatment strategies in clinical dentistry. The rationale for pesticide use in agriculture is that costs associated with pesticide pollution are to be justified by its benefits, but this is not so obvious. Valuing the benefits by simple economic analysis has increased pesticide use in agriculture and consequently produced pesticide-induced “public ills.” This paper attempts to explore the research gaps of the economic and social consequences of pesticide use in developing countries, particularly with an example of Nepal. We argue that although the negative sides of agricultural (...) development, for example- soil, water, and air pollution; pest resistance and resurgence; bioaccumulation, bio-magnification; and loss of biodiversity and ecosystem resilience caused by the use of pesticides in agriculture, are “developmental problems” and are “unintentional,” the magnitude may be increased by undervaluing the problems in the analysis of its economic returns. Despite continuous effort for holistic system analyses for studying complex phenomena like pesticides impacts, the development within the academic science has proceeded in the opposite direction that might have accelerated marginalization of the third world subsistence agricultural communities. We hypothesize that, if these adversities are realized and accounted for, the benefits from the current use of pesticides could be outweighed by the costs of pollution and ill human health. This paper also illustrates different pathways and mechanisms for marginalization. In view of potential and overall negative impacts of pesticide use, we recommend alternative ways of controlling pests such as community integrated pest management (IPM) along with education and training activities. Such measures are likely to reduce the health and environmental costs of pesticide pollution, and also enhance the capabilities of third world agricultural communities in terms of knowledge, decision making, innovation, and policy change. This paper presents an active fault-tolerant control strategy for quadrotor helicopters to simultaneously accommodate sensor faults and external disturbances. Unlike most of the existing fault diagnosis and fault-tolerant control schemes for quadrotor helicopters, the proposed fault diagnosis scheme is able to estimate sensor faults while eliminating the effect of external disturbances. Moreover, the proposed fault-tolerant control scheme is capable to eliminate the adverse effect of external disturbances as well by designing a disturbance observer to effectively estimate the unknown external disturbances (...) and integrating with the designed integral sliding-mode controller. In this case, the continuous operation of the quadrotor helicopter is ensured while avoiding the unexpected control chattering. In addition, the stability of the closed-loop system is theoretically proved. Finally, the effectiveness and advantages of the proposed scheme are validated and demonstrated through comparative numerical simulations of the quadrotor helicopter under different faulty and uncertain scenarios.
