Based on two years’ ethnographic research on doctor-patient relations in urban China, this paper examines the causes of patients’ mistrust of physicians. I identify the major factors at the societal, institutional, and interpersonal levels that lead to patients’ mistrust of physicians. First, I set the context by describing the extent of mistrust at the societal level. Then, I investigate the institutional sources of mistrust. I argue that the financing mechanism of public hospitals and physicians’ income structures are the most crucial (...) factors in inducing patients’ mistrust. Hospitals’ heavy reliance on self-finance has basically caused public hospitals to run like private hospitals, resulting in blatant conflicts of interest between hospitals and patients. Related to this is physicians’ reliance on bonuses and commissions as part of their regular incomes, which has inevitably resulted in overtreatment and, hence, mistrust from the patients. At the interpersonal level, I describe how individual physicians’ attitudes toward and interaction with patients may also affect patients’ sense of trust or mistrust in physicians. In conclusion, I discuss the ethical implications of the mistrust problem, and suggest changes at the institutional and interpersonal levels to mitigate the problem. (shrink)

This article examines how distrust drives exchange. We propose a theoretical framework integrating the literature of trust into cultural sociology and use a case of patients giving hongbao (red envelopes containing money) to doctors in China to examine how distrust drives different forms of unofficial exchange. Based on more than two years’ ethnography, we found that hongbao exchanges between Chinese patients and doctors were, ironically, bred by the public’s generalized distrust in doctors’ moral ethics. In the absence of institutional assurance, (...) Chinese patients drew on the cultural logic of particularism and its associated cultural repertoire to induce fidelity from their physicians. They mobilized interpersonal networks to function as assurance and presented hongbao as a return of favor to the doctors. This form of exchange is gifting-oriented. Alternatively, if there were no interpersonal networks to rely on, they proactively offered hongbao to doctors at arm’s length in an attempt to personalize the relationship to seek assurance and abate their anxieties. This form of exchange is bribery-oriented. Both forms of exchange co-existed when there was one-way generalized distrust manifested from patients to doctors. When doctors also developed generalized distrust in patients, arm’s length exchanges declined, leaving embedded exchanges as the dominant form. Our study asserts the central role of culture in constituting exchange behaviors and the importance of institutions in shaping the form of exchange. It contributes to the midrange theory of trust, generating a number of hypotheses for future research on the relationships among culture, institutions, distrust, assurance, and illicit exchange. Based on selected writings on women's experiences of and reflections on dress and travel published in the Hong Kong feminist journal Nuliu, this paper discusses the politics of female subjectivity in relation to the everyday. The context of the discussion is the changing actualization of the well-known feminist slogan ‘the personal is political’ within the local feminist movement in Hong Kong between the 1980s and the 1990s. The paper aims to create a new paradigm for analysing agency – the key (...) concept in subject formation – by critiquing the ideology of choice, which is a liberal value system that connects people's imagination with the notion of personal ‘liberation’. As demonstrated by the examples of dress and travel, the everyday is a site of both possibilities and conflict for women, who generate new strategies or tactics to negotiate not only with institutions, structures and policies, but also with ‘interpellations’, ‘temporality’, ‘spatiality’, ‘performativity’, ‘symbolism’ and ‘psyche’. In my paper, I argue that a first-person perspective (the perspective of a patient/recipient of an action) pertaining to response analysis is significant in Confucianism given the deeply personal nature of Confucianism. It matters whether oneself or others is the patient of an action because Confucianism as a virtue theory emphasizes self-reflection and reflexivity of one’s response in self-cultivation. Moreover, as an account of role-ethics, Confucianism calls attention to one’s particular relationship with others—one reacts differently in kind, not just in (...) degrees, depending on whether one’s or others’ family is at risk. I also appeal to Thomas Nagel’s framework of objectivity vs subjectivity to help elucidate the Confucian position on the value of an individual’s particular perspective, distinguishing the person who is the patient from other people. Finally, I revisit the mirror and the one body analogy discussed by Shun and highlight their point of impartiality rather than objectivity as implied by Shun. (shrink)

Today it commands considerable attention in current debates on "Asian values" raging in classrooms and boardrooms in both East Asia and the West. This volume, which represents the work of fifteen respected scholars of early Chinese thought and culture, is an especially timely effort to bring the Mencius under fresh scrutiny. Making use of recently (...) excavated manuscripts, the contributors approach the Mencius from novel perspectives, challenge established interpretations, and confront anew issues that continue to attract and divide students of this classic text. T'ang Chün-i's early work Ai-ching chih fu-yin (Gospel of love) has been much neglected by T'ang scholars. This essay argues that this text is not a caprice, and that it marks an important stage in T'ang's life and studies. Furthermore, in the history of Chinese philosophy, it is probably the first book ever written on the philosophy of love.

The Tao Tê Ching is probably the world's second most translated and annotated book , yet it remains among the most enigmatic. Of its eighty-one chapters, no one denies that the most important is the first, and many scholars go further to claim that it is the key to the whole work: if it is understood fully, all the rest may be seen to be implied. Unfortunately, the first chapter also happens to be the most ambiguous. But even so, (...) after so much attention can there be anything left to say? It seems to me that an important point has been missed or at least obscured, and that the popularity of certain translations has made this obscuration more prevalent recently. To correct this, I shall offer below a line-by-line explication of this crucial passage. The following interpretation first demonstrates the parallel structure of the first eight lines as signifying two different ways of experiencing: lines one, three, five and seven refer to the experience of Tao, and lines two, four, six and eight to our more usual way of experiencing the world. I shall suggest that the difference between these ways is the difference between our familiar dualistic experience and a much less common nondualistic way of experiencing in which there is no bifurcation between subject and object. Second, we shall see that the parallel structure unfolds dialectically: each succeeding pair of lines elaborates upon the issues that naturally arise in response to the preceding pair. In the process of showing this, I shall take sides on the two main controversies over this chapter: first, whether it should be interpreted cosmologically or ontologically/epistemologically , and second, whether lines, five and six should be punctuated to translate yü as ‘desire/intention’. My main thesis is that the traditional understanding of yü as ‘desire’ or ‘intention’ is an essential part of the meaning of the chapter. This is by no means an original claim, but why it is so important does not seem to have been noticed before and provides the reason for this paper. Wing-tsit Chan's criticism of such translations, that ‘intention interrupts the thought of the chapter’, 1 is thus a serious misreading of the text. (shrink)

The primary function of smooth muscle cells is to contract and alter the stiffness or diameter of hollow organs such as blood vessels, the airways and the gastrointestinal and urogenital tracts. In addition to purely structural functions, smooth muscle cells may play important metabolic roles, particularly in various inflammatory responses. In cell culture, these cells have been shown to be metabolically dynamic, synthesizing and secreting extracellular matrix proteins, glycosaminoglycans and a wide variety of cell–cell signaling proteins, such as interleukins, chemokines (...) and peptide growth factors. Secreted cell signaling proteins participate in the inflammatory response of smooth muscle‐containing organs, and some can also stimulate smooth muscle migration, proliferation and contraction. The cellular signaling pathways controlling synthesis of these signaling proteins are similar to those used by cells mediating innate immunity and may contribute to pathogenesis of diverse diseases including atherosclerosis, asthma, inflammatory bowel diseases and preterm labor. Appreciating the role of smooth muscle cells in these diseases may lead to better understanding of the beneficial effects of anti‐inflammatory drugs as well as identification of new targets for anti‐inflammatory therapy. The paper presents a perspective on our relation to our environment that is inspired by Confucian thought and that stands in contrast to certain common strands in contemporary philosophical discussions. It conceptualizes our relation to what we encounter on a day-to-day basis primarily in terms of the way we experience and respond to situations, rather than to the objects affected in the situations. From this perspective, the contemporary philosophical distinction between a first- and a third-person point of view is often (...) not suited for describing our responses to situations. Instead, our responses to situations can vary along different dimensions, including the way we direct our attention, our thoughts and sentiments, our motivations, etc., and these variations admit of degrees. Such variations depend on various factors, such as the different ways in which one relates to the object affected in a situation and the kind of person one is. and cannot be adequately described in terms of a shift from one point of view to another. The paper discusses two examples, anger and compassion, to illustrate this alternative perspective, but the discussion can be extended to other areas such as gratitude. (shrink)

Documents the problems and possibilities for women's studies, and exposes the resistance to women's initiatives, authority and autonomy. Qualitative and quantitative methods were used to investigate how shopping behaviors and environment influence dietary intake and weight status among homeless Minnesotans living in food deserts. Seven focus groups (n = 53) and a quantitative survey (n = 255), using the social cognitive theory as the theoretical framework, were conducted at two homeless shelters (S1 and S2) in the Twin Cities area. Heights, weights, and 24-h dietary recalls were also collected. Food stores within a five-block radius of the shelters were (...) evaluated for the price and availability of foods and compared to the Thrifty Food Plan’s market basket prices (MBP). Results showed that almost 80% of the sample was overweight or obese, with women consuming less than the recommended level for the fruits, vegetables, and milk food groups and excess for the fats/oils/sweets food group. Focus groups and participant surveys indicated that the shelter infrastructure and surrounding community influenced the types of foods available, food store access, and the foods purchased and consumed. Participants relied on food assistance programs, including food stamps, to supplement their food supply; however, some felt the high food prices at neighboring stores limited food choice and that food stamps did not adequately cover food costs. Results from the food store survey found the majority of food prices exceeded Midwest or national MBP. To promote healthier dietary intake and weight status, community-based interventions and city planners should aim to increase access to food through improved food availability and food access within the shelter environment and surrounding community. Pp. xii + 330.Modern scholarship on the Tao Te Ching has tended to focus on questions of authorship and the intended meaning of the text, often working from both the unquestioned assumption that matters of origination are of primary historical importance and (...) the quasi-theological bias that the original textual meaning is somehow the "right" or authoritative one. But the role of the Tao Te Ching in Chinese history and culture, as well as that of its purported author, is a much more complex affair. The text has been the subject of more than seven hundred commentaries and has been appropriated by vast networks of scholars, government officials, self-conscious religious communities, and laypeople, while Lao Tzu himself has undergone a fascinating history of deification and aesthetic representation. With Lao-tzu and the Tao-te-ching, editors Livia Kohn and Michael LaFargue take steps toward filling in some of the complicated (and much overlooked) post-canonical history by assembling essays on subjects ranging from the ritual uses of the text to modern hermeneutic experiments. The volume includes nine new essays, as well as three older ones reprinted or translated from earlier sources.The book is organized in four sections—"Ancient Myths," "Chinese Interpretations," "Modern Readings," and "Critical Methods"—although there is noticeable [End Page 625] overlap among the parts. The twelve essays are of varying quality and of varying use for different readers. Some of the material is highly technical and presupposes a sinological audience, while other material rehearses data that should already be familiar to scholars of ancient Chinese philosophy but may be tremendously useful to those who are interested in the text but lacking the scholarly background. One case of the latter is Julia Hardy's "Influential Western Interpretations of the Tao-te-ching," which does not especially break new ground but is a wonderful historical synopsis of Western scholarship on the text, as it meticulously identifies the key interpretive voices and sorts out their positions. Hardy gives only brief attention to the earliest eighteenth- and nineteenth-century scholarship—omitting, for example, any mention of Victor von Strauss' 1870 German rendition—which might actually have been most edifying for specialists. LaFargue and Julian Pas' "On Translating the Tao-te-ching" is essentially a primer on hermeneutic problems relevant to the text that serves as an intelligent corrective to the uncoordinated body of Taoist materials available to novice readers.What may be most clear from several of the essays taken in tandem is that the popular Western "mystical" reading of the text not only is one of many historical interpretations but is also very much heir to Wang Pi's intellectual lineage. In "A Tale of Two Commentaries: Ho-shang-kung and Wang Pi on the Lao-tzu," Alan K. L. Chan notes how such standard tenets as the spiritualized metaphysical understanding of Tao, the relationship between nonbeing and creativity, and the identification of naturalness and nonaction with spontaneity originate with Wang Pi but are nowhere to be found in the earlier Ho-shang-kung reading. The latter employs typical Han dynasty correlative cosmological interpretive methods, referring to naturalness as the preservation of energies in the physical body and to "the One" not as an abstract metaphysical oneness but as "the original substance of life itself, energy in its most pure and potent form" (p. 93). According to Isabel Robinet's "Later Commen-taries: Textual Polysemy and Syncretistic Interpretations," the panoply of text interpretations falls into roughly three categories—"technical or specialist readings," "philosophical exegeses," and "inner alchemy interpretations," which she then proceeds to divide further into "philological" and "ideological" readings. This is all further complicated when one considers the Tao Te Ching not as a strictly philosophical text but as a collection of polemical aphorisms, as LaFargue does in "Recovering the Tao-te-ching's Original Meaning: Some Remarks on Historical Hermeneutics," or as a ritual, liturgical or meditative aid, as Kohn does in "The Tao-te-ching in Ritual."Lao-tzu and the...
