Objectives: To gather information about the practices and attitudes of providers of maternity care with respect to informed consent for newborn screening .Methods: A questionnaire concerning information provision and parental consent for NBS was sent to all 1036 registered lead maternity carers in New Zealand.Results: 93% of LMC in New Zealand report giving parents information concerning NBS, most frequently after delivery and in the third trimester . The majority of LMC currently obtain some form of consent for NBS from parents (...) and consider this to be the ideal approach . Despite this a significant minority of LMC reported considering that NBS should be mandatory. Of those in our survey who believed that NBS should be mandatory, paradoxically most still believed that some form of parental consent should be obtained; of those who believed testing should not be mandatory, only a small proportion would accept parental refusal without question.Conclusions: When the results of this survey are considered in conjunction with existing evidence there appears to be a consensus that good quality information in the prenatal period should be an integral part of any NBS programme. The issue of consent is more complex and there is less agreement on the preferred degree of parental involvement in decisions to allow babies to undergo NBS. A policy that both strongly recommends NBS but also allows parental choice appears to be most consistent with the views of LMC in this survey. (shrink)

There is a growing consensus that results generated through multiplex genetic tests, even those produced as a part of research, should be reported to providers and patients when they are considered “actionable,” that is, when they could be used to inform some potentially beneficial clinical action. However, there remains controversy over the precise criterion that should be used in identifying when a result meets this standard. In this paper, we seek to refine the concept of “actionability” by exploring one proposed (...) use for genetic test results. We argue that genetic test results indicating that a patient is at risk for developing a chronic health condition should not be considered actionable if the only potential value of that result is to motivate patients to make changes in their health behaviors. Since the empirical research currently available on this question is equivocal, we explore relevant psychological theories of human motivation to demonstrate that current theory does not support the assumption that information about genetic risk will be motivating to most patients in their attempts to make changes in health behaviors. Pharmacogenetics and disease genetics studies are becoming increasingly prevalent, leading to a growing body of information on genetic associations for drug responsiveness and disease susceptibility with the potential to improve health care. Much of these data are presently characterized as exploratory (non‐validated or hypothesis‐generating). There is, however, a trend for research participants to be permitted (...) access to their personal data if they so choose. Researchers, sponsors, patient advocacy groups, ethics committees and regulatory authorities are consequently confronting the issue of whether, and how, study participants might receive their individual results. Noted international ethico‐legal guidelines and public policy positions in Europe and the United States are reviewed for background. The authors offer ‘Points‐to‐Consider’ regarding returning research results in the context of drug development trials based on their knowledge and experience. These considerations include: the clinical relevance of data, laboratory qualifications, informed consent procedures, confidentiality of medical information and the competency of persons providing results to participants. The discussion is framed as a benefit‐to‐risk assessment to balance the potential positive versus negative consequences to participants, while maintaining the integrity and feasibility of conducting genetic research studies. Language comprehension requires a simulation that uses neural systems involved in perception, action, and emotion. A review of recent literature as well as new experiments support five predictions derived from this framework. 1. Being in an emotional state congruent with sentence content facilitates sentence comprehension. 2. Because women are more reactive to sad events and men are more reactive to angry events, women understand sentences about sad events with greater facility than men, and men understand sentences about angry events with (...) greater facility than women. 3. Because it takes time to shift from one emotion to another, reading a sad sentence slows the reading of a happy sentence more for women than men, whereas reading an angry sentence slows the reading of a happy sentence more for men than for women. 4. Because sad states motivate affiliative actions and angry states motivate aggressive action, gender and emotional content of sentences interact with the response mode. 5. Because emotion simulation requires particular action systems, adapting those action systems will affect comprehension of sentences with emotional content congruent with the adapted action system. These results have implications for the study of language, emotion, and gender differences. (shrink)

Different styles of social interaction are one of the core characteristics of autism spectrum disorder. Social differences among individuals with ASD often include difficulty in discerning the emotions of neurotypical people based on their facial expressions. This review first covers the rich body of literature studying differences in facial emotion recognition in those with ASD, including behavioral studies and neurological findings. In particular, we highlight subtle emotion recognition and various factors related to inconsistent findings in behavioral studies of FER in (...) ASD. Then, we discuss the dual problem of FER – namely facial emotion expression or the production of facial expressions of emotion. Despite being less studied, social interaction involves both the ability to recognize emotions and to produce appropriate facial expressions. How others perceive facial expressions of emotion in those with ASD has remained an under-researched area. Finally, we propose a method for teaching FER [FER teaching hierarchy ] based on recent research investigating FER in ASD, considering the use of posed vs. genuine emotions and static vs. dynamic stimuli. We also propose two possible teaching approaches: a standard method of teaching progressively from simple drawings and cartoon characters to more complex audio-visual video clips of genuine human expressions of emotion with context clues or teaching in a field of images that includes posed and genuine emotions to improve generalizability before progressing to more complex audio-visual stimuli. Lastly, we advocate for autism interventionists to use FER stimuli developed primarily for research purposes to facilitate the incorporation of well-controlled stimuli to teach FER and bridge the gap between intervention and research in this area. Facial expressions of emotion play an important role in human social interactions. However, posed expressions of emotion are not always the same as genuine feelings. Recent research has found that facial expressions are increasingly used as a tool for understanding social interactions instead of personal emotions. Therefore, the credibility assessment of facial expressions, namely, the discrimination of genuine (spontaneous) expressions from posed (deliberate/volitional/deceptive) ones, is a crucial yet challenging task in facial expression understanding. With recent advances in computer vision and (...) machine learning techniques, rapid progress has been made in recent years for automatic detection of genuine and posed facial expressions. This paper presents a general review of the relevant research, including several spontaneous vs. posed (SVP) facial expression databases and various computer vision based detection methods. In addition, a variety of factors that will influence the performance of SVP detection methods are discussed along with open issues and technical challenges in this nascent field. (shrink)

It is often said that the health promoter must be an expert in generalisation and Debates and Dilemmas in Promoting Health is an illustration of the vast range of issues influencing practice and policy health promotion today.The publication is part of The Open University undergraduate course “Promoting Health: Skills, Perspectives and Practice”.
