We had an ideal of evidence-based, rational, ethical and equitable health care and set out to serve (...) class='Hi'>rural and under-served communities which included displaced forest-based tribes. In the initial years, with the naivety of the inexperienced, we believed that by correcting the dehydration of the doe-eyed six-year-old Mare and giving her a free course of antibiotics, we had made health care accessible to her. Much to our dismay, within a month, Mare was back in the outpatient clinic, with diarrhea all over again and looking thinner than ever. We realized that it was a losing battle to keep her healthy as long as she continued to drink water from the same contaminated stream, live in unhygienic surroundings, and eat only the paltry meals that her family could barely afford. We gradually began to connect these living examples to what our preventive medicine text books had stressed all along—there are many social determinants of health which, if left unaddressed, do not permit realization of the vision of health for all.Such instances continually pulled us out of the hospital building and into the villages and tribal hamlets. We discovered that our textbook of Preventive and Social Medicine (Park, 1986) (P&SM as we used to call it), was our most valuable possession. [End Page 106] We devoured the descriptions of national health programs, sanitation procedures, water treatment protocols, and maintenance of cold chain for vaccines with a completely new perspective. We discovered the Manual of Basic Techniques for a Health Laboratory, (World Health Organization, 1980) detailing how to set up a laboratory with low cost, reliable and simple methods. We learned to drive a jeep to haul supplies and patients. To step up the level of clinical care, some of us pursued post-graduate programs and equipped ourselves to provide more specialized care too. It has been over two decades since we embarked on this journey. We have encountered a variety of perplexing dilemmas with no clear solutions. We have sometimes been compelled to adopt health practices that were not really evidence-based, seemingly irrational, inequitable and even downright unethical. This narration describes only a few of our thousands of cases, and hopefully conveys our periods of self-doubt, despair and hope, as well as, the challenges we face to reconcile with the difficult choices we are forced to make every day.Unforeseen Challenges in Saving Mothers and ChildrenHigh maternal and child mortality rates have been one of the most disturbing aspects of the health care scenario in rural India. To ensure immunization of all children, health workers with vaccine carriers go from village to village and hamlet to hamlet to reach those infants that would have missed visits to the immunization clinics. Our grass-root workers facilitate these visits by identifying households with such children. Occasionally, they encounter families who spirit away the infant into the adjoining woods as soon as the health worker is seen approaching, and blandly proclaim that the child is not at home. Despite reassurance and counseling about the safety and necessity of the vaccine, their apprehensions and misconceptions are insurmountable. What is the extent of the responsibility of the health worker? Is it ethical to hunt down the child and force the administration of the vaccine in view of the public health gains, besides the child's welfare?A few months ago, a pre-term neonate was admitted to our hospital, but worsened and needed referral to a higher centre in the city. The family refused to go despite repeated persuasion and detailed explanation about the technical limitations of care at our centre. From the family's perspective, the variety of social, cultural, and monetary challenges they would face in the city were themselves limitations for care. Farming computer games enable the ‘desk chair countryside’—millions of people actively engaged in performing farming and rural activities on-line—to co-produce their desired representations of rural life, in line with the parameters set by game creators. In this paper, I critique the narratives and images of farming life expressed in the popular computer game ‘Stardew Valley’. Stardew is based on a scenario whereby players leave a [meaningless] urban desk job to revitalize the family farm. Player are given a (...) choice to invest in the Community Center or to support ‘JojaMart’, a ‘big-box’ development. The farming narrative demonstrates the hallmarks of classical American agrarianism: farming as the basic profession on which other occupations depend, the virtue of hard work, the ‘natural’ and moral nature of agricultural life, and the economic independence of the farmer. More recent discourses of critical agrarianism are noticeably absent, particularly in relation to environmental protection. Conflict is centred on urban-based big business, whereas the farm is represented as a ‘bolt-hole’ or sanctuary from urban life. I argue that embedding issues of big-box development in gameplay enrols players in active reflection and debate on desirable responses, whereas the emphasis on reproducing classical agrarian tropes risks desensitizing game players to contemporary agrarian social and environmental justice issues. However, Stardew Valley gameplay implicitly reinforces the ideal that low input farming is the way that agriculture should be practiced. The success of the game in eliciting on-line debates, and the requirement for active performance and decision-making, demonstrates the specific potential of computer games as mediums for influencing and intervening in ongoing reworking of farming imaginaries, and enabling more critically engagement of the ‘desk chair countryside’ in important global debates. Given UBI’s performance in poor and rural areas of India and Namibia and its transformative effects on livelihoods, one can foresee a potential for UBI supporting refugees and Internally Displaced Persons rebuild their lives in their country of origin. Furthermore, given UBI’s egalitarian rationale stemming from the idea of a more just society with a minimum level of economic security to all, UBI can be considered a key element of a state’s welfare system, the relevance of which cannot be (...) overstated for countries emerging from conflict. Additionally, the field of international aid has as of recently, for efficiency considerations, started questioning its policies focused on in-kind aid, vouchers, prescribed spending in favor of cash transfers. To test UBI’s potential in post-conflict scenarios, we propose to examine its applicability in Syria, source of today’s largest refugee population. (shrink)

A large section of the population (70%) of Uttarakhand largely depends upon agricultural based activities for their livelihood. Rural community of the mountains has developed several indigenous and traditional methods of farming to conserve the crop diversity and rejoice agrodiversity with religious and cultural vehemence. Traditional food items are prepared during occasion, festivals, weddings, and other religious rituals from diversified agrodiversity are a mean to maintain agrodiversity in the agriculture system. Agrodiversity is an insurance against disease and extreme climatic (...) fluctuations, as a coping mechanism in times of scarcity, as a means to enhance overall productivity of farms, as a source of critical nutrition and medicine in the Himalayan region. The different traditional system of agriculture and indigenous method of maintaining soil fertility, socio-cultural and religious rituals has saved many crops that are under threatened category. But all these system and practices are ignored nauseatingly in hill agriculture policy, where more emphasis was given for plain areas. Less emphasis is being put on local systems that rely on existing natural, human, and social assets such as biodiversity, traditional knowledge, and social capital underpinning collective action to ensure food security. Of late, development planners have realized the importance of appropriate technologies and therefore have stressed the need for on-site training, and capacity building of user groups in rural areas of the region. Rural technology demonstration and training center have been supposed as a means disseminating technologies enabling improvement in the yield potential of farms, income generation from off-farm activities, and conservation and efficient use of natural resources. There is a strong need to bring desirable changes in the agricultural policy, research, and development in reference to mountainous regions. The present paper describe present scenario of agriculture, traditional, and socio-cultural practices of retaining soil fertility and agrodiversity, policy dimensions, and strategies for management of the Himalayan agroecosystems. Pioneering researchers claim that telepsychiatry presents the possibility of improving both the quality and quantity of patient care for populations in general as well as for those in rural and remote locations. The prevalence of, and literature on telepsychiatry has increased dramatically in the last decade, covering all aspects of research endeavors. However, little can be found on the topic of ethics in telepsychiatry. Using various clinical scenarios we may provide insight into the moral challenge in telepsychiatry—the lack of (...) in-person contact. The difficulty is to articulate what the significance of in-person contact is and further, its meaning in the therapeutic relationship between the patient and the physician. Using the personalist perspective and related philosophical approaches we may sketch an idea of the patient as person, existentially considered as a relational and bodily human being. By applying Brennan’s model for health technology assessment we may evaluate the morally troubling aspect of telepsychiatry—a lack of in-person contact—on this philosophical sketch of the person. BackgroundMedical professionalism has been developing in the Peoples’ Republic of China as one way to better address perennial and new challenges in healthcare in an ever-changing society. Among many recent developments in this area is promotion by the national Chinese Medical Doctor Association of the principles and values contained in the international document, “Medical Professionalism in the New Millennium: A Physician Charter.”ObjectiveTo discover Chinese physicians’ attitudes toward and understanding of medical professionalism.MethodologyThe authors distributed a self-reporting questionnaire that included 34 statements (...) and four case scenarios concerning the general principles of medical professionalism: the primacy of patients’ welfare, respect for patients’ autonomy, promotion of social justice, and professional self-regulation. The questionnaire included controversial issues such as the role of the family in decision making and reporting medical errors. A total of 2,966 practicing physicians, randomly selected from the Chinese Medical Association database, were surveyed, and 1,198 valid questionnaires were returned. Our sample covered 23 provinces and 51 cities throughout the Peoples’ Republic of China.ResultsMore than 80 percent of the physicians who responded agreed that the physician-patient relationship should be a relationship of trust founded on professional altruism, and that informed consent is necessary. More than 95 percent agreed that physicians should promote professional self-regulation as well as social justice. More than half agreed with the principle of the primacy of patients’ welfare (62.8 percent), and that physicians have a responsibility to report medical errors and incompetent colleagues (51.0 percent). In certain cases, a great majority of Chinese physicians favored familism and paternalism.LimitationsThe study does not include data on how Chinese physicians practice medical professionalism, or the perspectives of physicians working in smaller cities and in rural areas.ConclusionsBased on responses to the survey, Chinese physicians strongly support the majority of the fundamental principles and responsibilities of medical professionalism, including dedication, altruism, social justice, self-regulation, and informed consent. However, their support for the primacy of patients’ welfare as a general principle, and the physician’s responsibility to report medical errors and incompetent colleagues, is relatively low. To help advance medical professionalism in the People’s Republic of China, professional development programs and medical ethics education should not only emphasize the general principles involved, but also formulate guidelines on how these principles can be carried out in practice. An exodus of health care workers and other constraints have made the transfer of critically ill patients very difficult. In this disquieting "new-normal" of our work life, we routinely must (...) call bigger cities throughout our state to find beds for complex patients.So, it was not surprising when a young patient, an IV drug user in sepsis from a heart infection, was admitted to me in the middle of the night. In less disheartening times, before the pandemic, this patient would have been transferred promptly to the big-city hospital. Now, though, the possibility of finding an open bed is next to impossible. Resigned, I placed admission orders, continued antibiotics and fluids, and crossed my fingers. Then the unexpected happened: a bed opened up, and we could send her at once!The patient had a harsh heart murmur. It was likely that a "vegetation" was clinging to the valve, between the right upper and lower chambers of her heart. A vegetation is a clump of debris, such as platelets, clotting proteins, and bacteria that is a menace to the body. Her chest imaging looked like her infection had already spread to her lungs.A definitive diagnosis for endocarditis requires a special ultrasound with a probe that goes down through the esophagus, a procedure performed by cardiologists. It's a serious infection, one that requires weeks of IV antibiotics and is best monitored by infectious disease specialists. Sometimes, cardiothoracic surgeons are called upon to strip away the infection at the site, depending on how badly the valve has been slayed or the size of the vegetation. My only recommendation for a young, otherwise healthy patient in this scenario, is transfer.All of these reasons for transfer were lolling around in my tired head when the night-time [End Page 14] nursing supervisor approached me and said, "We have a problem." When the patient was updated about the transfer, she balked: "She says she is tired and refusing to go," the supervisor began. "She doesn't feel like getting into an ambulance in the middle of the night and she doesn't care if her condition is life-threatening. She knows we can't force her to go and she will only talk about it again in the morning."The cartoon version of myself at that moment had bugged-out eyes, a dropped jaw, fists tight and clenched at my sides, and steam coming out of my head. I can look past a patient's self-neglect, poor medication adherence, abysmal communication, alcohol and drug addiction, and other crimes. But saying no to a bed at a tertiary care facility? Unacceptable!I marched down the quiet hallway and into her room. She was lying on her side, facing the door, one IV in a thin arm. I was confident I could reason with her once I explained the scarcity of beds and the medical necessity of transfer. But she was having none of it. I was calm until I could no longer maintain my cool: "You can't give up this bed!""Maybe in the morning," she insisted, yawning. "But not now. I am tired and want to rest!""But there might not be a bed in the morning," I countered. "And your infection can be life-threatening." (This is true, but we both knew she wasn't dying at this very moment.)"I am not going anywhere!" She raised her voice and then closed her eyes.And she was right. I had no choice but to keep her. I could not kick her out of the hospital because she was making a poor choice, and I was frustrated for having to manage the fallout of that decision. Agricultural commercialization as a mechanism to alleviate rural poverty raises concerns about small land-holders, non-adopters, and inequity in the distribution of benefits within transforming economies. Farm gross margins were calculated to assess the economic status and impact of cash cropping on the economic well-being of agrarian households in the Mid-hills of Nepal. On an individual crop basis, tomatoes and potatoes were the most profitable. On a per farm basis, 50 of the households with positive farm gross margins grew at (...) least one vegetable crop, while only 25 of households with negative farm gross margins included vegetable crops in their rotation. Farmers have been hesitant to produce primarily for the market given the rudimentary infrastructure and high variability in prices. Farmers reported selling more crops, but when corrected for inflation, gross revenues declined over time. The costs and benefits of developing markets have been unevenly distributed with small holders unable to capitalize on market opportunities, and wealthier farmers engaging in input intensive cash cropping. Farms growing vegetables had an average gross margin of US$137 per year compared to US$12 per year for farms growing only staple crops. However, the area under production is small, and while vegetable production is likely to continue increasing, sensitivity analysis and scenarios suggest high variability and limited short-term impact on poverty alleviation. This study uses the Locke and Okali gender analysis framework to explore gender relations surrounding grain storage management and marketing in Binga District of Zimbabwe. The study was conducted during one grain storage season and involved multiple visits to selected households, which were used as case studies. The main question that the study sought to address was: “What bargaining goes on between men and women in the area of stored grain management and marketing?” Data were collected from four households, fitting (...) into the following categories: simple monogamous, complex monogamous (two scenarios), and polygamous. Participatory rural appraisal tools and techniques were extensively used and formed the backdrop of all the data collection. The study established that much bargaining and strategizing occurs within the household in order for women to exercise control over the use of stored grain. The bargaining process was found to be a complex one of give-and-take without an immediately recognizable winner. There is evidence that women use this bargaining power to exert influence on their relative position in the household in terms of independent income generation and management, seniority, and overall household food security policies. While bargaining between and within gender remains shrouded in subtleness, individuals in a household consciously use their skills to manipulate the situation to their best advantage. This article is expected to initiate broader debate in the area of gender roles and bargaining in grain post-harvest management, an area often kept private by smallholder farmers in sub-Saharan Africa. Social and behavioral scientists — that is, students of human nature — nowadays hardly ever use the term ‘human nature’. This reticence reflects both a becoming modesty about the aims of their disciplines and a healthy skepticism about whether there is any one thing really worthy of the label ‘human nature’. For some feature of humankind to be identified as accounting for our ‘nature’, it would have to reflect some property both distinctive of our species and systematically influential enough to (...) explain some very important aspect of our behavior. Compare: molecular structure gives the essence or the nature of water just because it explains most of its salient properties. Few students of the human sciences currently hold that there is just one or a small number of such features that can explain our actions and/or our institutions. And even among those who do, there is reluctance to label their theories as claims about ‘human nature’. Among anthropologists and sociologists, the label seems too universal and indiscriminant to be useful. The idea that there is a single underlying character that might explain similarities threatens the differences among people and cultures that these social scientists seek to uncover. Even economists, who have explicitly attempted to parlay rational choice theory into an account of all human behavior, do not claim that the maximization of transitive preferences is ‘human nature’. I think part of the reason that social scientists are reluctant to use ‘human nature’ is that the term has traditionally labeled a theory with normative implications as well as descriptive ones. This paper presents a speculative model of the cognitive mechanisms underlying the transition from episodic to mimetic (or memetic) culture with the arrival of Homo erectus, which Donald [1991] claims paved the way for the unique features of human culture. The model draws on Kauffman's [1993] theory of how an information-evolving system emerges through the formation of an autocatalytic network. Though originally formulated to explain the origin of life, this theory also provides a plausible account of how discrete episodic memories (...) become woven into an internal model of the world, or worldview, that both structures, and is structured by, self-triggered streams of thought. Social interaction plays a role in (and may be critical to) this process. Implications for cognitive development are explored. (shrink)

Despite emerging study of business initiatives that attempt to support local peace and development, we still have significant knowledge gaps on their effectiveness and efficiency. This article builds theory on business engagements for peace through exploration of the Footprints for Peace (FOP) peacebuilding project by the Federación Nacional de Cafeteros de Colombia (FNC). FOP was a business-peace initiative that attempted to improve the lives of vulnerable populations in conflict-affected regions. Through 70 stakeholder interviews, we show how FOP operationalized local peace (...) and development in four conflict-affected departments of Colombia, and examine FNC’s motivations for and effectiveness of its peacebuilding activities. Our main finding is that FOP’s success supported several existing theories on business engagement in peace both in terms of peacebuilding by business and for local economic and societal development, providing evidence in support of development–business collaborations and local peacebuilding by business under certain targeted circumstances. We relate these findings to existing literature, highlighting where existing business-peace theory is supported, where FOP challenged assumptions, and where it illuminated new research gaps. These findings serve to take business-peace theory forward and improve our understandings of what can constitute success for business-peace initiatives in Colombia and possibly other conflict-affected regions. In this short paper, we discuss a dialectic methodology surrounding the interpretation of knowledge transfer, and the conditional elements which can be seen to support the concept of a unity of knowledge. We discuss a differing standpoint to knowledge and knowledge value, based on the knowledge transfer practitioner’s perspective, but still in a business context. We ask why, if knowledge is vital for business success and competitive advantage, the transfer of knowledge is rarely a simple unproblematic event. Further, that the (...) creation of knowledge before transfer is recognised as a significant factor in determining a starting point for analogous scrutiny, and often under a premise of doxastic attitude. This discussion therefore aims to synthesise current literature and research into an elemental epistemic principal of FIGURATION DYNAMICS, and in doing so, may help focus congruent knowledge transfer theories. Digital business has marked an era of transformation, but also an unprecedented growth of cyber threats. While digital explosion witnessed by the banking sector since the COVID-19 pandemic has been significant, the level and frequency of cybercrimes have gone up as well. Cybercrime officials attribute it to remote working—people using home computers or laptops with vulnerable online security than office systems; malicious actors relentlessly developing their tactics to find new ways to break into enterprise networks and grasping defence evasion; persons (...) unemployed during the pandemic getting into hacking; cloud and data corruption; digital fatigue causing negligence; etc. This study adopts a case-based approach to explore the importance of business ethics, information sharing and transparency to build an information-driven society by scouting the case of Punjab and Maharashtra Co-operative (PMC) Bank, India. PMC defaulted on payments to its depositors and was placed under Reserve Bank of India’s directions due to financial irregularities and a massive fraud perpetrated by bank officials by orchestrating the bank’s IT systems. The crisis worsened when panic-stricken investors advanced their narrative through fake news peddled via social media channels, resulting in alarm that caused deaths of numerous depositors. It exposed several loopholes in information management in India’s deposit insurance system and steered the policy makers to restructure the same, thus driving the country consistent with its emerging market peers. The study further identifies best practices for aligning employees towards ethical behaviour in a virtual workplace and the pedagogical approaches for information management in the new normal. Francis Hutcheson is generally accepted as producing the first systematic study of aesthetics, in the first treatise of An Inquiry into the Original of Our Ideas of Beauty and Virtue, initially published in 1725. His theory reflected the eighteenth century concern with beauty rather than art, and has drawn accusations of vagueness since the first critical response, by Charles Louis DeVillete in 1750. The most serious critique concerns the idea of beauty itself: whether it was simple or complex, and the (...) idea of a primary or secondary quality. It is the latter question I shall answer, attempting to clarify the problematic passage that appears at the end of the first section of Hutcheson’s first treatise. (shrink)

The study zeroed in on the perception of college students who are exposed to sights of poverty in their immediate environment. The student-participants were asked to provide their perception, understanding, and behaviour towards poverty using the photographs that they took on their own. In qualitative research practice, this methodology is called photo elicitation. It was revealed, among others, that the participants have shown negative perceptions about poverty. They strongly felt bad about each photograph that they took and what these images (...) portrayed. The participants visualized their community and thereby weaved analyses and opinions that encroached on various philosophical and sociological theories. Raising awareness of the environmental impact of energy generation and consumption has been a recent concern of contemporary society worldwide. Underlying the awareness of energy consumption is an intricate network of perception and social interaction that can be mediated by technology. In this paper we argue that issues regarding energy, environment and technology are very much situated and involve tensions of sociocultural nature. This exploratory investigation addresses the subject by introducing the design of a Socially-inspired Energy Eco-Feedback Technology, which is (...) composed of an interactive system to trigger and mediate collective savings and a tangible device as a public feedback. Results of an evaluation situated in the context of a school in a socially disadvantaged area in Brazil are discussed, shedding light on the sociocultural aspects related to the subject. The role of the SEET to motivate energy awareness collectively among the social group is assessed, as well as the design characteristics that contributed to that. Outcomes bring to light social aspects and dynamics that would hardly have been predicted, evidencing critical factors related to a socially inspired design approach in the energy awareness domain. Bedbugs are tiny, wingless insects which feed on mammal blood and leave behind painful, itchy sores. Although they can live in other settings, they are most commonly found in warm, dark places inhabited by humans, like beds. After being absent in the United States for over 60 years, thanks to powerful pesticides, bed bugs, have returned in force and are present in every state and nearly every city. For reasons not entirely understood, bed bugs have developed resistance to traditional pesticides (...) such as Permethrin and are therefore difficult to control. Although commonly believed to be associated with dirty housekeeping and associated with substandard housing, bed bugs are equally likely to be present in five-star hotels as they are in homeless shelters. Boehm reasons that human morality began when several heads of households formed a coalition to limit the despotic bullying of an alpha male. The logic is clear and the argument is persuasive. The premises require that: dominant individuals behave like chimpanzees, bullying their subordinates, early humans somehow developed one-male units from a chimpanzee like society and, the power of a despot is limited by group consensus and political activities. Not all alpha males behave like chimpanzees; most primate societies show little (...) evidence of bullying. One-male units are formed by some monkeys but there is little evidence of such tendencies in chimpanzees and bonobos. Hamadryas males do not compete with familiar males for females or possessions regardless of their dominance relationships, but this inhibition is not based on group consensus or coalitions repressing alpha males. Boehm's argument is, nonetheless, possible and plausible but cannot be empirically tested. (shrink)

Considering the sensory dimension of clinical decision-making is a challenge for the training of health professionals. Therefore, how can we help them stimulate a form of sensory acuity? We conducted a study in partnership with the Simulation Laboratory of a Canadian hospital to understand the sensory work accomplished by nurses during a clinical decision-making. To do this, we analyzed the interactions occurring during focus groups to reveal the sensory work performed by nurses during the clinical evaluation of patients (personas).
