Background Older patients in emergency care often have complex needs and may have limited ability to make their voices heard. Hence, there are ethical challenges for healthcare professionals in establishing a trustful relationship to determine the patient’s preferences and then decide and act based on these preferences. With this comes further challenges regarding how the patient’s autonomy can be protected and promoted. Aim To describe nurses’ experiences of dealing with older patients’ autonomy when cared for in emergency departments (EDs). Research (...) design This study adopted reflective lifeworld theory and a phenomenological design. Participants and research context A total of 13 open-ended interviews were performed with nurses working at two EDs in Sweden. Ethical considerations The study was reviewed by the Ethical Advisory Board in South East Sweden and conducted according to the Declaration of Helsinki. All participants gave consent. Findings Nurses’ experiences of dealing with older patients’ autonomy in EDs are characterized by moving in a conflicting uphill struggle, indicating obscure thoughts on how patient autonomy can be protected in an ethically challenging context. The phenomenon is further described with its meaning constituents: ‘Being hampered by prioritization under stress’, ‘Balancing paternalism and patient autonomy’, ‘Making decisions without consent in the patient’s best interests’ and ‘Being trapped by notions of legitimate care needs’. Conclusion Stressful work conditions and lacking organizational strategies in EDs contribute to nurses maintaining unjustified paternalistic care, regardless of the patient’s ability and medical condition, and questioning who has legitimacy for participating in decisions about care. The nurses’ protection and promotion of older patients’ autonomy is dependent on the opportunity, ability and willingness to create a patient relationship where the patient’s voice and preferences are valued as important. Consequently, strategies are needed to improve patient autonomy in EDs based on the idea of ‘relational autonomy’. (shrink)

Misleading information and unfair commercial practices have to be viewed against the background of what consumers otherwise do, i.e., what their purchase decisions look like when no misleading information or no unfair commercial practices are in place. This article provides some of this background by studying how consumers sample information when making an in-store purchase decision. This was done by an eye-tracking study which reveals to what extent consumers succeed in purchasing the products that best meet their purchase intentions when (...) only a representative amount of misleading information is present. The study shows that decisions were suboptimal in relation to what the consumers claimed they wanted to purchase. Only in one product category did consumers in this study actually look at products that were slightly better than average, and as a result, they mainly selected products that were just as often poor as good. If the proportion of bad purchase decisions based on misleading information is small enough, perhaps it might be better to direct the authors’ attention to other ways of improving the decision environments that consumers encounter. In addition, the eye-tracking study provides some insight into how consumers sample information when making an in-store purchase decision. The present data show that consumers invested on average of less than 1 s to look at products. (shrink)

The influence of conscience on nurses in terms of guilt has frequently been described but its impact on care has received less attention. The aim of this study was to describe nurses' conceptions of the influence of conscience on the provision of inpatient care. The study employed a phenomenographic approach and analysis method. Fifteen nurses from three hospitals in western Sweden were interviewed. The results showed that these nurses considered conscience to be an important factor in the exercise of their (...) profession, as revealed by the descriptive categories: conscience as a driving force; conscience as a restricting factor; and conscience as a source of sensitivity. They perceived that conscience played a role in nursing actions involving patients and next of kin, and was an asset that guided them in their efforts to provide high quality care. (shrink)

In this article we refine the design ofcontext shifting experiments, which play a central role in contextualist debates, and we subject a large number of scenarios involving different types of expressions of interest to contextualists, including ‘know’ and color adjectives like ‘green’, to experimental investigation. Our experiment (i) reveals an effect of changing contexts on the evaluation of uses of the sentences that we examine, thereby overturning the absence of results reported in previous experimental studies (so‐callednull results), (ii) uncovers evidence (...) for a ‘truth bias' in favor of positive over negative sentences, and (iii) reveals previously unnoticed distinctions between the strength of the contextual effects displayed by scenarios involving knowledge ascriptions and scenarios concerning color and other miscellaneous scenarios. During the past decades, research collaboration between researchers from different disciplines has become more frequent. However, there is a need to look into the generic modalities and challenges. The article explores a series of potential obstructions to cross-disciplinary collaboration of methodological and epistemological nature. Furthermore, a number of contextual, inhibiting factors are outlined. As means of overcoming the obstacles, the importance of mutual knowledge, allocation of adequate time and conducive research management is emphasised. New teams may benefit from tutoring by (...) facilitators, who can help to make problem areas explicit and negotiate solutions. Owing to the training background of the author, most of the examples are drawn from the interface between biomedicine/natural science and applied medical anthropology. However, the issues raised basically apply to all sorts of cross-disciplinary research collaboration in various combinations. (shrink)

The aim of this essay is to reflect on the place and importance of the question of address and to show how it comes to the fore in Søren Kierkegaard’s writings. What shall be attempted, with regard to Kierkegaard’s already widely recognized renown as an existential thinker, is to catch a glimpse of issues that make up the larger background in which the question of address is embedded. In doing so, the essay explores several features of Kierkegaard’s inquiry into the (...) question of existence in the Concluding Unscientific Postscript and connect them with the notion of the reader. This latter notion provides a way of understanding the singularity of voice, while offering an account of the testimony to existence. After a brief recapitulation, the essay identifies in conclusion unresolved issues relating to speech, silence, confidentiality, and listening. (shrink)

In Harry Potter, Hagrid is written as the “animal lover” who appreciates all creatures. Analyzing Hagrid’s interactions with animals is a unique approach to theorizing animal ethics at Hogwarts. This article problematizes Hagrid’s characterization within the “animal lover” trope. Many of Hagrid’s actions are imperialist toward animals as collectible, exploitable, and only valuable insofar as they provide something for humans, which contradicts the definition of moral status. The potential for “animal lovers” to relate to Hagrid suggests a need to more (...) closely examine this trope in other works of fiction to avoid perpetuating unethical standards of animal care. This article examines the historically contentious relationship between the financial market and the public as discussed in academic literature, financial journalism and prescriptive how-to invest handbooks during the late 19th and early 20th centuries. Although financial markets thrive off active public participation, speculating at stock and commodity exchanges has been a sanctioned ritual reserved for a privileged minority. We argue that the financial establishment’s intent to control market access through financial entry-barriers (such as exchange membership fees and margin requirements) has (...) been part of a bigger story we need to understand: a history of delegitimating uninitiated ‘lay speculators’ through the construction of exclusionary narratives about unfit amateur investors and morally corrupt publics. We conceptualize this process as an ongoing and delicate boundary-making exercise contributing to a market participation discourse that has been characterized by a set of reductive binaries, such as those of insider–outsider, professional–amateur and speculator–gambler. We show, however, that attempts to delineate popular participation in financial markets through these binaries have been complicated by the idea that besides being a force of market instability and collective irrationality, the public was a largely untapped source of liquidity. We argue that today’s discourse on public participation in financial markets resuscitates these simplified narratives and propose a more nuanced view of non-professional market participants being both destabilizers and liquidity-providers. Gilles de la Tourette syndrome (TS) is a childhood neuropsychiatric disorder characterised by the presence of motor and vocal tics. Patients with malignant TS experience severe disease sequelae; risking morbidity and mortality due to tics, self-harm, psychiatric comorbidities and suicide. By definition, those cases termed ‘malignant’ are refractory to all conventional psychiatric and pharmacological regimens. In these instances, deep brain stimulation (DBS) may be efficacious. Current 2015 guidelines recommend a 6-month period absent of suicidal ideation before DBS is offered to (...) patients with TS. We therefore wondered whether it may be ethically justifiable to offer DBS to a minor with malignant TS. We begin with a discussion of non-maleficence and beneficence. New evidence suggests that suicide risk in young patients with TS has been underestimated. In turn, DBS may represent an invaluable opportunity for children with malignant TS to secure future safety, independence and fulfilment. Postponing treatment is associated with additional risks. Ultimately, we assert this unique risk-benefit calculus justifies offering DBS to paediatric patients with malignant TS. A multidisciplinary team of clinicians must determine whether DBS is in the best interest of their individual patients. We conclude with a suggestion for future TS-DBS guidelines regarding suicidal ideation. The importance of informed consent and assent is underscored. (shrink)

Over recent decades, public participation in technology assessment has spread internationally as an attempt to overcome or prevent societal conflicts over controversial technologies. One outcome of this new surge in public consultation initiatives has been the increased use of participatory consensus conferences in a number of countries. Existing evaluations of consensus conferences tend to focus on the modes of organization, as well as the outcomes, both procedural and substantial, of the conferences they examine. Such evaluations seem to rest on the (...) assumption that this type of procedure has universally agreed goals and meanings, and that therefore consensus conferences can readily be interpreted and applied across national boundaries. This article challenges this approach to consensus conferences. The core of the article is a study of national differences in ideas about what constitutes legitimate goals for participatory arrangements. The study looks at three consensus conferences on GMOs, which took place in France, Norway, and Denmark. Drawing on this study, the article discusses the ways in which interpretations of the concept of participation; the value attributed to lay knowledge vs. technical expertise; as well as ideas about the role of the layperson, are all questions that prompt entirely different answers from country to country. Further, the article analyses these national differences within a theoretical framework of notions of democratic legitimacy. (shrink)

Research has shown that specific individual values, such as green and environmental values, are important in motivating the decision to start a sustainable business. Beyond this finding, there is limited knowledge about why, how, and when such values become important and what this means for sustainable entrepreneurship engagement. We address this question abductively and conduct a multi-case study of 18 sustainable entrepreneurs and their fashion companies. Drawing on the self-activation and the impressionable years hypotheses, we identified three ways in which (...) sustainability-oriented values become activated and more important to individuals: (1) through informational stimuli, (2) through formative and life-changing experiences, and (3) through socialization. Further, we show that the entrepreneurs engaged in reflexive learning due to the value-activating experience, whereby they critically questioned their assumptions and actions. Together, one or more value activations and the involved reflexive learning contributed significantly to the decision to become a sustainable entrepreneur. With this novel explanation for why and how values become engaged in the first place, we contribute to the theory of (sustainable) entrepreneurship. Further, our research helps devise value-activating strategies for practitioners who want to help (prospective) entrepreneurs act more in line with their sustainability-oriented values and start a sustainable business. In recent decades there has been increasing demand for and considerable efforts to conduct cross-disciplinary research. However, assessment of research quality in such endeavours still is often based on mono-disciplinary criteria and not seldom carried out by reviewers without strong cross-disciplinary experience. The authors suggest a two-pronged approach to cross-disciplinary research evaluation. One part should comprise an individual review of all the disciplines involved based on their mono-disciplinary sets of criteria. The other part should be a separate evaluation of the (...) cross-disciplinary aspects based on the review of “problem formulation”, “integration and scope of the disciplines”, “parts and the whole”, “practical managerial aspects” and “the applied aspects”. The pros and cons of implementing this approach in a stepwise manner or simultaneously is discussed. It is suggested that funding agencies develop more fair sets of review procedures for cross-disciplinary research and show willingness to allocate extra funds and time to such forms of research that sometimes are regarded as relatively more “risky” than conventional mono-disciplinary types. Growing evidence suggests that natural environments – whether in outdoor or indoor settings – foster psychological health and physiological relaxation, indicated by increased wellbeing, reduced stress levels, and increased parasympathetic activity. Greater insight into differential psychological aspects modulating psychophysiological responses to nature-based relaxation videos could help understand modes of action and develop personalized relaxation interventions. We investigated heart rate variability as an indicator of autonomic regulation, specifically parasympathetic activity, in response to a 10-min video intervention in two consecutive studies as (...) well as heart rate. We hypothesized that a nature-based relaxation video elicits HRV increase and HR decrease, with response magnitude being affected by aspects of early life adversity and trait mindfulness. In Study 1, N = 60 participants watched a relaxation video intervention depicting different natural scenery. We analyzed changes in HR and respiratory sinus arrhythmia as a standard HRV measure, both based on 3-min segments from the experimental session, in multiple growth curve models. We found a decrease in HR and increase of RSA during the video intervention. Higher paternal care and lower trait mindfulness observing skills were associated with higher RSA values before but not during video exposure. In Study 2, N = 90 participants were assigned to three video conditions: natural scenery from Study 1, meditation video, or short clip from “The Lord of the Rings.” Again, HR decreased, and RSA increased during video segments, yet without expected group differences across different video types. We found higher parental care and lower parental overprotection to predict higher RSA at different times during the experiment. Interestingly, lower paternal overprotection predicted overall higher RSA. These results suggest a generic relaxation effect of video interventions on autonomic regulation that we discuss in light of different theories mapping restorative effects of natural environments. Further, psychological characteristics like aspects of early life adversity and trait mindfulness could contribute to individual differences in autonomic regulation. This study contributes to a better understanding of autonomic and psychological responses to relaxation videos. (shrink)

Throughout the animal kingdom, p53 genes function to restrain mobile elements and recent observations indicate that transposons become derepressed in human cancers. Together, these emerging lines of evidence suggest that cancers driven by p53 mutations could represent “transpospoathies,” i.e. disease states linked to eruptions of mobile elements. The transposopathy hypothesis predicts that p53 acts through conserved mechanisms to contain transposon movement, and in this way, prevents tumor formation. How transposon eruptions provoke neoplasias is not well understood but, from a broader (...) perspective, this hypothesis also provides an attractive framework to explore unrestrained mobile elements as inciters of late‐onset idiopathic disease.Also see the video abstract here. A proof of the consistency of Heyting arithmetic formulated in natural deduction is given. The proof is a reduction procedure for derivations of falsity and a vector assignment, such that each reduction reduces the vector. By an interpretation of the expressions of the vectors as ordinals each derivation of falsity is assigned an ordinal less than ε 0, thus proving termination of the procedure.

This article analyses problem situations in the context of anaesthesia care. It considers what it means for nurse anaesthetists to be in problematic situations in the anaesthesia care of older patients. Benner’s interpretive phenomenological approach proved useful for this purpose. Paradigm cases are used to aid the analysis of individual nurses’ experiences. Thirty narrated problematic anaesthesia care situations derived from seven interviews were studied. These show that experienced nurse anaesthetists perceive anaesthesia care as problematic and highly demanding when involving older (...) patients. To be in problematic anaesthesia care situations means becoming morally distressed, which arises from the experience or from being prevented from acting according to one’s legal and moral duty of care. An important issue that emerged from this study was the need for an ethical forum to discuss and articulate moral issues, so that moral stress of the kind experienced by these nurse anaesthetists can be dealt with and hopefully reduced. The inner brow raiser is a muscle movement that increases the size of the orbital cavity, leading to the appearance of so-called ‘puppy dog eyes’. In domestic dogs, this expression was suggested to be enhanced by artificial selection and to play an important role in the dog-human relationship. Production of the inner brow raiser has been shown to be sensitive to the attentive stance of a human, suggesting a possible communicative function. However, it has not yet been examined whether it (...) is sensitive to human presence. In the current study, we aimed to test whether the inner brow raiser differs depending on the presence or absence of an observer. We used two versions of a paradigm in an equivalent experimental setting in which dogs were trained to expect a reward; however, the presence/absence of a person in the test apparatus was varied. In the social context, a human facing the dog delivered the reward; in the non-social context, reward delivery was automatized. If the inner brow raiser has a communicative function and dogs adjust its expression to an audience, we expect it to be shown more frequently in the social context than in the non-social context. The frequency of the inner brow raiser differed between the two contexts, but contrary to the prediction, it was shown more frequently in the non-social context. We further demonstrate that the inner brow raiser is strongly associated with eye movements and occurs independently in only 6% of cases. This result challenges the hypothesis that the inner brow raiser has a communicative function in dog-human interactions and suggests a lower-level explanation for its production, namely an association with eye movements. Gödel’s ontological proof is by now well known based on the 1970 version, written in Gödel’s own hand, and Scott’s version of the proof. In this article new manuscript sources found in Gödel’s Nachlass are presented. Three versions of Gödel’s ontological proof have been transcribed, and completed from context as true to Gödel’s notes as possible. The discussion in this article is based on these new sources and reveals Gödel’s early intentions of a liberal comprehension principle for the higher order (...) modal logic, an explicit use of second-order Barcan schemas, as well as seemingly defining a rigidity condition for the system. None of these aspects occurs explicitly in the later 1970 version, and therefore they have long been in focus of the debate on Gödel’s ontological proof. (shrink)

In this study, we investigate the three-dot sign as a discourse marker (DM) with textual, subjective and intersubjective discourse functions. As a graphical marker that is used across languages, the three-dot sign is especially suitable for comparative studies and dynamics in language contact. Our corpus study targeting instant messages of different languages (English, German, Greek, Russian, Turkish) and speaker groups (monolinguals and bilingual heritage speakers) suggests that graphical DMs are prone to cross-linguistic influence. This depends on the specific contact situation (...) and does not seem to be a general effect of bilingualism. The societal status of a language might further influence the use of such markers in digital informal writing. Language-specific developments that relate to emerging functions indicate that functional versatility promotes frequent use of (graphical) DMs. (shrink)

We propose that leaders play a more important role in voters’ party sympathy in proportional representation systems than previous research has suggested. Voters, from the 2018 Swedish General Election, were in an experiment asked to describe leaders and parties with three indicative keywords. Statistical models were conducted on these text data to predict their vote choice. The results show that despite that the voters vote for a party, the descriptions of leaders predicted vote choice to a similar extent as descriptions (...) of parties. However, the order of the questions mattered, so that the first questions were more predictive than the second question. These analyses indicate that voters tend to conflate characteristics of leaders with their parties during election campaigns, and that leaders are a more important aspect of voting under PR than previous literature has suggested. Overall, this suggests that statistical analysis of words sheds new light of underlying sympathies related to voting. (shrink)

This paper gives a Gentzen-style proof of the consistency of Heyting arithmetic in an intuitionistic sequent calculus with explicit rules of weakening, contraction and cut. The reductions of the proof, which transform derivations of a contradiction into less complex derivations, are based on a method for direct cut-elimination without the use of multicut. This method treats contractions by tracing up from contracted cut formulas to the places in the derivation where each occurrence was first introduced. Thereby, Gentzen’s heightline argument, which (...) introduces additional cuts on contracted compound cut formulas, is avoided. To show termination of the reduction procedure an ordinal assignment based on techniques of Howard for Gödel’s T is used. (shrink)

This article examines the potential implications of the implementation of artificial intelligence in health care for both its delivery and the medical profession. To this end, the first section explores the basic features of AI and the yet theoretical concept of autonomous AI followed by an overview of current and developing AI applications. Against this background, the second section discusses the transforming roles of physicians and changes in the patient–physician relationship that could be a consequence of gradual expansion of AI (...) in health care. Subsequently, an examination of the responsibilities physicians should assume in this process is explored. The third section describes conceivable practical and ethical challenges that implementation of a single all-encompassing AI healthcare system would pose. The fourth section presents arguments for regulation of AI in health care to ensure that these applications do not violate basic ethical principles and that human control of AI will be preserved in the future. In the final section, fundamental components of a moral framework from which such regulation may be derived are brought forward, and some possible strategies for building a moral framework are discussed. It proposes that cosmopolitanism provides fertile ground for the study of WH, in particular if combined with sensitivity to distinct indigenous ethical and political claims. Underpinning my article is the question of whether the politics of WH, despite its peaceful and universalist intensions, obscures local disputes and subaltern voices. The empirical emphasis is placed on the WH site of Laponia in the North of Sweden – a location (...) of Sami indigenous communities and commercial mining interests. I provide a narrative analysis of the inconsistencies between the Swedish ‘good state’ narrative defined by support for human and indigenous rights globally and the protection ambition of WH and its generally favourable attitude towards mining, despite its potentially damaging effects on the WH site of Laponia and the cultural heritage and land rights of Sami people living there. My study also provides an analysis of Sami narratives on Laponia – storylines that are rarely included in the Swedish ‘good state’ narrative. The study seeks to contribute to scholarly understandings of indigenous peoples’ ability to lay claims to the cosmopolitan protection logic that prevails in WH. (shrink)

I argue that the absence of some of the ‘greatest hits’ of Western philosophy in Classical China can be explained by a Wittgensteinian take on the role of language in philosophy. One is the ‘Idea Theory’ of meaning which anchors Western Mind-Body dualism. Its attraction is removed when the writing reminds us that a picture does not by itself ‘give life to’ our language even while it plays a role of cross-linguistic communication. Another is the centrality of a law-command theory (...) of normativity which combines with mind-body dualism to give a natural push toward monotheistic supernaturalism. Western attempts to make the ‘God’ impulse logical (e.g., the Ontological Argument) fail because of differences in Chinese syntax. The upshot is we need not deny Chinese thinkers the status of philosophers for their failure to share our philosophical presuppositions and resultant agenda. (shrink)

Possivelmente como decorrência dos efeitos ameaçadores e destrutivos que as duas grandes guerras mundiais trouxeram para o mundo, na segunda metade do século XX tornou-se mais aguda a preocupação com os problemas planetários. E, nesta esteira, um dos fenômenos emergentes foi a proliferação de organizações supranacionais e instituições voltadas para uma preocupação com as questões internacionais, especialmente aquelas cujos impactos adquiriam âmbito global. A guerra fria, a competição pela hegemonia de nações imperialistas como EUA e União Soviética, impulsionou uma corrida (...) pelo protagonismo nas áreas militar, tecnológica e industrial, alavancando de maneira inédita e desmedida a exploração dos recursos ambientais. A busca dos resultados a qualquer custo, no intuito de otimizar os lucros, tornou-se a finalidade do capitalismo neoliberal ancorado em corporações multinacionais, que hoje se tornou preponderante em termos globais. E isso inclui o esgotamento dos recursos naturais, se isso for condição para lucros maiores. Em face deste cenário, apesar dos governos e instituições supranacionais se articularem no sentido de promover o desenvolvimento sustentável, de se organizarem eventos periódicos para a defesa do meio ambiente, as iniciativas implementadas mais parecem paliativos ante a voracidade dos mercados. Como, pois, enfrentar efetivamente este paradoxo? Existem perspectivas efetivas de sua ultrapassagem? A reflexão aqui trazida se propõe a abordar estas questões e as implicações a elas inerentes, partindo de uma base teórica discursiva de inspiração habermasiana para analisar o fenômeno da sustentabilidade em nível planetário, sob uma ótica democrática e cidadã (cosmopolita e ativa). The quality of the animal-human relationship and, consequently, the welfare of animals can be improved by gentle interactions such as stroking and talking. The perception of different stimuli during these interactions likely plays a key role in their emotional experience, but studies are scarce. During experiments, the standardization of verbal stimuli could be increased by using a recording. However, the use of a playback might influence the perception differently than ‘live’ talking, which is closer to on-farm practice. Thus, we compared (...) heifers’ (n = 28) reactions to stroking while an experimenter was talking soothingly (‘live’) or while a recording of the experimenter talking soothingly was played (‘playback’). Each animal was tested three times per condition and each trial comprised three phases: pre-stimulus, stimulus (stroking and talking) and post-stimulus. In both conditions, similar phrases with positive content were spoken calmly, using long low-pitched vowels. All tests were video recorded and analyzed for behaviors associated with different affective states. Effects on the heifers’ cardiac parameters were assessed using analysis of heart rate variability. Independently of the auditory stimuli, longer durations of neck stretching occurred during stroking, supporting our hypothesis of a positive perception of stroking. Observation of ear positions revealed longer durations of the back up position and less ear flicking and changes of ear positions during stroking. The predicted decrease in HR during stroking was not confirmed; instead we found a slightly increased mean HR during stroking with a subsequent decrease in HR, which was stronger after stroking with live talking. In combination with differences in HRV parameters, our findings suggest that live talking might have been more pleasurable to the animals and had a stronger relaxing effect than ‘playback’. The results regarding the effects of the degree of standardization of the stimulus on the variability of the data were inconclusive. We thus conclude that the use of recorded auditory stimuli to promote positive affective states during human-animal interactions in experimental settings is possible, but not necessarily preferable.
