Controversy persists over the ethics of compensating research participants and providing posttrial benefits to communities in developing countries. Little is known about residents' views on these subjects. In this study, interviews about compensation and posttrial benefits from a hypothetical HIV vaccine trial were conducted in Uganda’s Rakai District. Most respondents said researchers owed the community posttrial benefits and research compensation, but opinions differed as to what these should be. The understanding of appropriate ethical protections for participants of biomedical research has not been static. It has evolved over time, with the evolution of biomedical research as well as social values. Since World War II, there have been four major paradigms of research and research oversight operative in the United States. These paradigms incorporate different values and provide different approaches to research oversight and the protection of research participants.

We argue that charging people to participate in research is likely to undermine the fundamental ethical bases of clinical research, especially the principles of social value, scientific validity, and fair subject selection.

SeitzThe Tenth Conference of the European Network of Buddhist-Christian Studies (ENBCS) brought together between sixty and seventy people at the Oude Abdij, Drongen, Belgium, between 27 June and 1 July 2013, to examine the theme “History as a Challenge to Buddhism and Christianity.” It was (...) a collaborative effort between the ENBCS and the Katholieke Universiteit Leuven. The 2011 ENBCS conference had focused on whether hope was a relevant concept for Buddhists and Christians. The choice of history as a theme complemented this. It also resonated with a research concern of both KU Leuven and the Network.Five key themes were chosen for examination: the traditional Christian and Buddhist conceptions of history; Jesus and the Buddha: fact and fiction; Buddhist and Christian historiography; dangerous memory in Christianity and Buddhism; and permitting historical consciousness in Buddhism and Christianity. Each was approached from a Buddhist and a Christian perspective. An opening lecture addressed the theme “Modern Historical Consciousness and the Challenge to Religion.” In addition, eighteen research papers on Buddhist-Christian relations were given in parallel sessions on Friday evening and Saturday morning. These covered topics as various as the writings of Edmund Pezet (Pierre Gillet), Paul Knitter’s understanding of Buddhist-Christian double belonging (Achim Riggert), Keiji Nishitani and the death of God (Jonatan Navarro Estrada), the encounter of Zen Buddhism and Christianity in the works of D. T. Suzuki (Mariusz Rucki), Buddhist-Christian encounters by Han Yong Woon (Seung Chul Kim), and “Liberating the Conflict from ‘History’ in Sri Lanka” (Jude Lal Fernando).Perry Schmidt-Leukel (Münster, Germany), in the opening lecture, discussed [End Page 189] seven challenges that modern historical consciousness was posing to religion: the challenge of falsification (when historical evidence, for example, showed some doctrines cherished by believers were not taught by the founder), the challenge of uncertainty or methodological doubt, the challenge of methodological naturalism (should a natural explanation be considered more probable than a supernatural one?), the challenge of the distance between past and present, the challenge of change (religions are not static), the challenge of demystification, and the challenge of relativity (the writing of history is perspectival, conditioned by context). He stressed that it would be a blessing to religion if these challenges were faced and, throughout the conference, encouraged other speakers to face them.The first theme was tackled by Mark Blum (Berkeley, USA) from a Buddhist perspective and Jan-Olav Henriksen (Norwegian School of Theology) from a Christian perspective. In a magisterial historical and geographical survey of the Buddhist tradition, Blum first noted that no one had unearthed a traditional notion of Buddhist history in the early texts. The theme of decline and reformation had been taken from Indian cosmology, but with a Buddhist twist and the conviction that it was pointless to talk about beginnings. And in the growth of the Mahāyāna tradition, a dehistoricization of the Buddha had occurred in the belief that the Buddha’s dharma-body was his only “real” body.Henriksen argued that Christian salvation history had its origin in the narrative of the Fall, an event for which there was no historical evidence, and developed into a history of God’s call (election) to all people that they should be brought together in community. It was a history that was linked with the divine logos, a force that Christian theology has seen as deeply interwoven into creation from the beginning of time and that was made human in Jesus.Terrence Merrigan (KU Leuven, Belgium), giving a Christian perspective on the second theme, distinguished three quests or attempts to reach the historical Jesus. The first, from 1774 to 1906, sought to uncover the Jesus that had been obscured by dogma. It imploded with the realization that separating “core” from “myth” was impossible, and conditioned a reactive “no-quest” in the first half of the twentieth century. Psychiatry is rapidly adopting digital phenotyping and artificial intelligence/machine learning tools to study mental illness based on tracking participants’ locations, online activity, phone and text message usage, heart rate, sleep, physical activity, and more. Existing ethical frameworks for return of individual research results (IRRs) are inadequate to guide researchers for when, if, and how to return this unprecedented number of potentially sensitive results about each participant’s real-world behavior. To address this gap, we convened an interdisciplinary expert working group, supported by (...) a National Institute of Mental Health grant. Building on established guidelines and the emerging norm of returning results in participant-centered research, we present a novel framework specific to the ethical, legal, and social implications of returning IRRs in digital phenotyping research. Our framework offers researchers, clinicians, and Institutional Review Boards (IRBs) urgently needed guidance, and the principles developed here in the context of psychiatry will be readily adaptable to other therapeutic areas. Importance Informed consent is essential to ethical, rigorous research and is important to recruitment and retention in cancer trials.Objective To examine cancer clinical trial (CCT) participants’ perceptions of informed consent processes and variations in perceptions by cancer type.Design and Setting and Participants Cross-sectional survey from mixed-methods study at National Cancer Institute–designated Northeast comprehensive cancer center. Open-ended and forced-choice items addressed: (1) enrollment and informed consent experiences and (2) decision-making processes, including risk-benefit assessment. Eligibility: CCT participant with gastro-intestinal or genitourinary, hematologic-lymphatic (...) malignancies, lung cancer, and breast or gynecological cancer (N = 334).Main Outcome Measures Percentages satisfied with consent process and information provided; and assessing participation’s perceptions of risks/benefits. Multivariable logistic or ordinal regression examined differences by cancer type.Results Most patient-participants felt well informed by the consent process (more than 90% overall and by cancer type) and. most (87.4%) reported that the consent form provided all the information they wanted, although nearly half (44.8%) reported that they read the form somewhat carefully or less. More than half (57.9%) said that talking to research staff (i.e., the consent process) had a greater impact on participation decisions than reading the consent form (2.1%). A third (31.1%) were very sure of joining in research studies before the informed consent process (almost half of lung cancer patients did—47.1%). Most patients personally assessed the risks and benefits before consenting. However, trust in physicians played an important role in the decision to enroll in CCT.Conclusions and Relevance Cancer patients rely less on written features of the informed consent process than on information obtained from the research staff and their own physicians. Research should focus on information and communication strategies that support informed consent from referring physicians, researchers, and others to improve patient risk-benefit assessment and decision-making. Objectives: To determine the effects of risk and payment on subjects’ willingness to participate, and to examine how payment influences subjects’ potential behaviours and risk evaluations.Methods: A 3 × 3 , between subjects, completely randomised factorial design was used. Students enrolled at one of five US pharmacy schools read a recruitment notice and informed consent form for a hypothetical study, and completed a questionnaire. Risk level was manipulated using recruitment notices and informed consent documents from hypothetical biomedical research projects. Payment (...) levels were determined using the payment models evaluated by Dickert and Grady as a guide. Five dependent variables were assessed in the questionnaire: willingness to participate, willingness to participate with no payment, propensity to neglect to tell about restricted activities, propensity to neglect to tell about negative effects, and risk rating.Results: Monetary payment had positive effects on respondents’ willingness to participate in research, regardless of the level of risk. However, higher monetary payments did not appear to blind respondents to the risks of a study. Payment had some influence on respondents’ potential behaviours regarding concealing information about restricted activities. However, payment did not appear to have a significant effect on respondents’ propensity to neglect to tell researchers about negative effects.Conclusions: Monetary payments appear to do what they are intended to do: make subjects more willing to participate in research. Concerns about payments blinding subjects to risks could not be substantiated in the present study. However, the findings do raise other concerns—notably the potential for payments to diminish the integrity of a study’s findings. Future research is critical to make sound decisions about the payment of research subjects. (shrink)
