The term 'primary health care' (PHC) has come into widespread use by policy-makers, managers and health professionals in the past two decades. There is a variety of definitions and an even wider variety of uses of the term. The purpose of this paper is to examine critically existing definitions and uses, with a particular focus on their usefulness in health policy, clinical practice and research relating to health care systems. The paper has three parts. First, we examine the principal types (...) of definition and identify core features of primary care common to most definitions. Second, we discuss the application of these criteria to a range of services in the UK National Health Service (NHS). Lastly, we examine the ways in which policymakers, health professionals, researchers and lay people use the term, also drawing primarily on UK experience. We conclude by reflecting on the likelihood of achieving a single unambiguous definition and the usefulness of the term for different purposes. (shrink)

This paper discusses ethical dilemmas related to social work practice with young adults with autism. It does so via the use of a case study taken from real life practice. The different viewpoints and ethical frameworks of the young person, the young person's parents and the Local Authority (or the Local Authority social worker) are considered and discussed. The competing rights of the 2006 United Nations Convention on the Rights of Persons with Disabilities (and the Optional Protocol) are also considered.

This article considers some of the ethical implications of social workers undertaking more direct work with carers and children in the field of child protection. Following the UK government's near-complete acceptance of the recommendations of the Munro report into child protection in England and Wales, it seems inevitable that direct work will become more and more a feature of practice for child protection social workers. Whilst this development is almost universally welcomed, this should not disguise the fact that direct work (...) can be fraught with ethical difficulties and challenges. This article explores in general terms three of the main potential areas of difficulty?the use of video-recording, informed consent and interpreting the meaning of direct work?before considering some specific responses to these in the context of a particular skills-based training intervention to improve the ability of child protection social workers to engage in direct work. That is the central doctrine of hylomorphism (sometimes rendered “hylemorphism”—hyle, matter; morphe, form), and the view has become a live topic of inquiry today. Contemporary proponents of the doctrine include Jeffrey Brower, Kit Fine, David Hershenov, Mark Johnston, Kathrin Koslicki, Anna Marmodoro, Michael Rea, and Patrick Toner, among others. In the wake of these contemporary hylomorphic theories the doctrine has seen application to various topics within mainstream analytic metaphysics. (...) Several different evaluation issuesare perceived as important by people involved withinnovative projects intended to improve local food andnutrition systems; particularly the establishment oflocal food policy coalitions. Several such coalitionshave been formed in North America, Europe, andAustralia with the goal of improving community foodsecurity and promoting sustainable local food systems.Pioneer coalitions have served as models, yet therehas been little systematic evaluation of thesemodels. A qualitative study was conducted to identifyfactors that may hinder evaluation efforts. In grouptelephone interviews, we sought the views (...) ofacademics, project organizers, and funders, a total of24 key informants. Pressures to evaluate were assessed differently bythe three groups of key informants. Academics felt thefocus of evaluation should be on the effectiveness ofthe process used to discuss issues and formulatepolicies and plans. Project organizers and fundersperceived a need to assess project impact andoutcomes. A lack of suitable evaluation models andmethods was viewed as a formidable barrier. The use ofinappropriate methods and premature impact evaluationwere noted as potential threats to projectsustainability. External constraints and resourcelimitations were also said to inhibit evaluationefforts. It appears that several other factors may also beimpeding progress in conducting more (and more useful)evaluations including: (1) the apparent negativeconnotation of evaluation and the limited benefitsexpected from evaluation by stakeholders, (2) a lackof consensus about important evaluation questions,(3) insufficient evaluation expertise among projectorganizers, and (4) inadequate appreciation ofincreasing accountability pressures. DBS Think Tank IX was held on August 25–27, 2021 in Orlando FL with US based participants largely in person and overseas participants joining by video conferencing technology. The DBS Think Tank was founded in 2012 and provides an open platform where clinicians, engineers and researchers can freely discuss current and emerging deep brain stimulation technologies as well as the logistical and ethical issues facing the field. The consensus among the DBS Think Tank IX speakers was that DBS expanded in (...) its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. After collectively sharing our experiences, it was estimated that globally more than 230,000 DBS devices have been implanted for neurological and neuropsychiatric disorders. As such, this year’s meeting was focused on advances in the following areas: neuromodulation in Europe, Asia and Australia; cutting-edge technologies, neuroethics, interventional psychiatry, adaptive DBS, neuromodulation for pain, network neuromodulation for epilepsy and neuromodulation for traumatic brain injury. In the paper we discuss criticisms against David Armstrong’s general theory of truthmaking by Gonzalo Rodriguez-Pereyra, Peter Schulte and Benjamin Schnieder, and conclude that Armstrong’s theory survives these criticisms. Special attention is given to the problems concerning Entailment Principle, Conjunction Thesis, Disjunction Thesis and to the notion of explanation.

David Hull's (1988c) model of science as a selection process suffers from a two-fold inability: (a) to ascertain when a lineage of theories has been established; i.e., when theories are descendants of older theories or are novelties, and what counts as a distinct lineage; and (b) to specify what the scientific analogue is of genotype and phenotype. This paper seeks to clarify these issues and to propose an abstract model of theories analogous to particulate genetic structure, in order to (...) reconstruct relationships of descent and identity. In 1988, David Hull presented an evolutionary account of science. This was a direct analogy to evolutionary accounts of biological adaptation, and part of a generalized view of Darwinian selection accounts that he based upon the Universal Darwinism of Richard Dawkins. Criticisms of this view were made by, among others, Kim Sterelny, which led to it gaining only limited acceptance. Some of these criticisms are, I will argue, no longer valid in the light of developments in the formal modeling (...) of evolution, in particular that of Sergey Gavrilets’ work on adaptive landscapes. If we can usefully recast the Hullian view of science as being driven by selection in terms of Gavrilets’ and Kaufmann’s view of there being “giant components” of high-fitness networks through any realistic adaptive landscape, we may now find it useful to ask what the adaptive pressures on science are, and to extend the metaphor into a full analogy. This is in effect to reconcile the Fisherianism of the Dawkins–Hull approach to selection and replicators, with a Wrightean drift account of social constructionist views of science, preserving, it is to be hoped, the valuable aspects of both. In this essay, I first evaluate the conceptual analysis of human rights by Wilfried Hinsch and Markus Stepanians. Next I criticize Allen Buchanan’s claim that Rawls did not address basic human interests/capabilities theories of human nature. I argue Buchanan is doubly mistaken when he claims that John Rawls sought to avoid such theories because they are comprehensive doctrines. Then I evaluate David Reidy’s defense of Rawls, while questioning his efforts to show how Rawls’s list of human rights could be (...) expanded. Finally, I accept James Nickel’s argument that Rawls has tied human rights too closely to intervention on their behalf. However, I reject his, and by implication Rawls’s, refusal to accept a two-tiered approach to human rights. (shrink)

Historians generally grumble at the liberties taken with letters and papers by editors and biographers in the past, while reviewers may complain at the professorial pomposities which interfere with the reader's interaction with the text. Certainly, reading is not a mere matter of information retrieval or of source-mining, but a meeting of minds, and any over-zealous editing which makes this more difficult will have failed. Editors, whether of journals or of documents, are midwives of ideas—self-effacingly bringing an author's meaning and (...) style into the world. What reviewers praise is the unobtrusive, and what they damn is ‘a manner at once slapdash and intrusive’, making allowances perhaps for an ‘introduction which is as admirable as his footnotes are useless’. When in the 1960s new technology brought us a flood of facsimile reprints of scientific works, some avoided these problems by appearing naked and unashamed: but for a text on phrenology, or for Goethe's Theory of Colours, a fig leaf or two of commentary is really necessary to help the innocent reader to interact with the book. Facsimiles of nineteenth-century editions of Wilkins' papers, of some Newton correspondence, or of Henry More's poetry are even more problematic; the reader should know that these editors' assumptions cannot be taken for granted, and that their introductions are themselves historical documents. The exact reproduction of misprints and misbindings is of dubious assistance to the modern reader. (shrink)

The orientation and leadership of the revolutionary “renewal of the German mind,” whose witnesses and participants we are, point in two directions. On, after seizing power, would like to talk the mind into helping out with internal development and promises it a golden age if it joins up; indeed it even offers it the prospect of a certain voice in decision making. The other direction, on the contrary, attests its mistrust of the intellect by declaring that the revolutionary process will (...) continue indefinitely, and has room for the mind in its task; or it might also assure the intellect that it is not needed at all because a new mind has already turned up, and that the old one might as well jump into the fire and either burn to ashes or purify itself into its elements. What has happened up to the moment these words are being written leaves no doubt that the second direction is on the march, the first its musical accompaniment. Nor can it be otherwise than that a Movement [National Socialism] that has manifested itself so powerfully demands above all that the intellect complete assimilate and subordinate itself to the Movement. But then again, it is possible that the intellect cannot do this without renouncing itself. Surely there must be some sort of boundary here, since nothing happens that is not contingent; so it is a good test for the intellect that today it has everywhere been saddled with a kind of kangaroo-court mentality that judges it not according to its own laws, but according to the law of the Movement. In this paper I explore the pedagogical and political shift marked by the meaning and practice of diversity offered through New Labour education policy texts, specifically, the policy and practice of personalized learning (or personalization). The aim of this paper is to map the ways in which diversity relays and mobilizes a set of neoliberal positions and relationships in the field of education and seeks to govern education institutions and education users through politically circulating norms and values. These norms and (...) values, I want to argue, echo and redeem the kinds of frameworks, applications and rationalities typically aligned with modes of neoliberal or advanced liberal governance, e.g. marketization, monetarization, atomization and deregulation. I conclude the paper by considering how diversity in education renders problematic conventional antinomies of the citizen and consumer, public and private, state and civil society, etc., and forces us to confront the rhizomatic character of contemporary governance and education in the era of neoliberalism. I argue that, because there are considerations within physics and metaphysical inquiry which corroborate modal relationalist claims (...) concerning the possible geometric structures of spacetime, and dispositional properties are actual possible entities, the condition of being grounded in the concrete is consistent with the Barcan formula; and thus -- in the geometric setting -- merits adoption by the Necessitist. Part of the appeal of the biological approach to personal identity is that it does not have to countenance spatially coincident entities. But if the termination thesis is correct and the organism ceases to exist at death, then it appears that the corpse is a dead body that earlier was a living body and distinct from but spatially coincident with the organism. If the organism is identified with the body, then the unwelcome spatial coincidence could perhaps be avoided. It is (...) argued that such an identification would be a mistake. A living organism has a different part/whole relationship and persistence conditions than the alleged body. A case will be made that the concept ‘human body’ is a conceptual mess, vague in an unprincipled manner, and that an eliminativist stance towards dead bodies is the appropriate response. This book is a defense of modal realism; the thesis that our world is but one of a plurality of worlds, and that the individuals that inhabit our world are only a few out of all the inhabitants of all the worlds. Lewis argues that the philosophical utility of modal realism is a good reason for believing that it is true.

As a continuation of the work of the third author in [5], we make further observations on the features of Galois cohomology in the general model theoretic context. We make explicit the connection between forms of definable groups and first cohomology sets with coefficients in a suitable automorphism group. We then use a method of twisting cohomology (inspired by Serre’s algebraic twisting) to describe arbitrary fibres in cohomology sequences—yielding a useful “finiteness” result on cohomology sets. Applied to the special case (...) of differential fields and Kolchin’s constrained cohomology, we complete results from [3] by proving that the first constrained cohomology set of a differential algebraic group over a bounded, differentially large, field is countable.
