BackgroundCommentators believe that the ethical decision-making climate is instrumental in enhancing interprofessional collaboration in intensive care units. Our aim was twofold: to determine the perception of the ethical climate, levels of moral distress, and intention to leave one's job among nurses and physicians, and between the different ICU types and determine the association between the ethical climate, moral distress, and intention to leave.MethodsWe performed a cross-sectional questionnaire study between May 2021 and August 2021 involving 206 nurses and physicians in a (...) large urban academic hospital. We used the validated Ethical Decision-Making Climate Questionnaire and the Measure of Moral Distress for Healthcare Professionals tools and asked respondents their intention to leave their jobs. We also made comparisons between the different ICU types. We used Pearson's correlation coefficient to identify statistically significant associations between the Ethical Climate, Moral Distress, and Intention to Leave.ResultsNurses perceived the ethical climate for decision-making as less favorable than physicians. They also had significantly greater levels of moral distress and higher intention to leave their job rates than physicians. Regarding the ICU types, the Neonatal/pediatric unit had a significantly higher overall ethical climate score than the Medical and Surgical units and also demonstrated lower moral distress scores and lower “intention to leave” scores compared with both the Medical and Surgical units. The ethical climate and moral distress scores were negatively correlated ; moral distress and "intention to leave" was positively correlated ; and ethical climate and “intention to leave” were negatively correlated.ConclusionsSignificant differences exist in the perception of the ethical climate, levels of moral distress, and intention to leave between nurses and physicians and between the different ICU types. Inspecting the individual factors of the ethical climate and moral distress tools can help hospital leadership target organizational factors that improve interprofessional collaboration, lessening moral distress, decreasing turnover, and improved patient care. In the face of limited resources during the COVID-19 pandemic response, public health experts and ethicists have sought to apply guiding principles in determining how those resources, including vaccines, should be allocated.

We give a general method for producing various effective Null and Positivstellensätze, and getting new Positivstellensätze in algebraically closed valued fields and ordered groups. These various effective Nullstellensätze produce algebraic identities certifying that some geometric conditions cannot be simultaneously satisfied. We produce also constructive versions of abstract classical results of algebra based on Zorn's lemma in several cases where such constructive version did not exist. For example, the fact that a real field can be totally ordered, or the fact that (...) a field can be embedded in an algebraically closed field. Our results are based on the concepts we develop of dynamical proofs and simultaneous collapse. (shrink)

Certified nursing assistants (CNAs) provide up to 80% of the direct care to older adults in long‐term care facilities.CNAs are perceived as being at the bottom of the hierarchy among healthcare professionals often negatively affecting their job satisfaction. However, manyCNAs persevere in providing quality care and even reporting high levels of job satisfaction. The aim of the present investigation was to identify primary themes that may helpCNAs make meaning of their chosen career; thus potentially partially explaining increases in job satisfaction (...) among this group. Focus groups were conducted withCNAs at three long‐term care facilities. Four themes emerged from the data:CNAwork is good or special;CNAas relationship builder;CNAas expert;CNAas team member. These themes reflect the perceptions that theseCNAs held in regard to themselves and their relationships to others in the work environment and, when present, can contribute to intrinsic job satisfaction. Our meaning‐making themes support the premise thatCNAs do not passively accept the evaluations of others but instead actively frame identities that validate their importance to residents and the institution. Cet entretien réalisé le 30 novembre 1999 à Paris présente l’évolution de la philosophie de Michel Henry, depuis L’essence de la manifestation jusqu’à l’Incarnation. Il s’agit de présenter le projet, la place et la centralité de L’essence de la manifestation dans l’œuvre de M. Henry, puisque c’est dans cet ouvrage qu’il a mis au jour les présuppositions de sa phénoménologie de la vie et de l’intériorité subjective, présuppositions qu’il fera travailler dans tous les ouvrages qui lui succéderont, ceci jusqu’à (...) L’incarnation. Elle s’oppose à une phénoménologie du monde, de l’extériorité et de l’objectivité. Son concept central, celui de chair, pense la matière affective du Soi, c’est-à-dire du corps subjectif. Il est inséparable d’une thèse : à savoir que la corporéité (rapport au corps) est antérieure à l’intentionnalité (rapport au monde). Une phénoménologie du corps subjectif est donc apte à saisir la vie dans son immanence. Elle conduit à une archéologie de la chair dans l’Incarnation, analyse de la matière constituante de la vie qui précède et habite toute intentionnalité. (shrink)

Cardiovascular disease (CVD) and sustained high blood glucose as prediabetes are an established comorbidity. People's experience in reconciling these long‐term conditions requires deeper appreciation if nurses are to more effectively support person‐centred care for people who have them. Our analysis explores the initial experience of people admitted to hospital with CVD who then find they also have sustained high blood glucose. Our methodology is informed by the philosophy of Gadamer and applies interpretive description to develop an interpretation of participant experiences. (...) The major theme emerging from participant interviews was the ‘invisible disequilibrium’ characterised by three subthemes: ‘losing equilibrium’, ‘becoming embattled’ and ‘evolving illness’. This study examines CVD and prediabetes in conjunction with the Gadamerian notion of the ‘whole’, as being in a social and emotional world in which illness is also a component part. We explore how participants lived within an ‘invisible disequilibrium’, with prediabetes frequently remaining unnoticed, while CVD was manifest. To identify multiple conditions and support effective intervention to manage them as part of person‐centric care, nursing practice should explore the ‘whole’ of the person's experience, value people’s knowledge as potential indicators of complex illness, thereby reducing the risk of accelerating complex illness. L'œuvre de Michel Henry aura été de « recueillir » et développer le côté affectif de la phénoménologie, que Husserl, dans un passage clé de ses « Idées directrices », a délaissé au profit du côté intentionnel de la subjectivité - tout comme trois siècles plus tôt Descartes avait « recueilli » la subjectivité écartée par Galitée. Le texte qui suit compare le concept de soubassement, ou âme, chez Husserl avec celui de chair chez Michel Henry. On verra (...) que les deux philosophes parlent d'une couche sensible, qu'on peut d'une certaine manière situer entre corps et esprit, de deux points de vue diamétralement opposés. Au-delà de l'opposition entre les deux philosophies, cette recherche mène à une interprétation topologique du rapport ente corps, âme et esprit, tel que suggéré par le terme husserlien de soubassement, et propose un éclairage différent de la notion henryenne d'auto-affection. Comment comprendre que nous puissions avoir conscience de notre vie affective? Si l’on prend au sérieux les analyses de Michel Henry, cette expérience commune devrait être impossible. Si c’est la vie et l’expérience qui doivent nous servir de guides, alors il faut intégrer les acquis incontournables de la phénoménologie matérielle et la dépasser.

Human genomics is a translational field spanning research, clinical care, public health, and direct-to-consumer testing. However, law differs across these domains on issues including liability, consent, promoting quality of analysis and interpretation, and safeguarding privacy. Genomic activities crossing domains can thus encounter confusion and conflicts among these approaches. This paper suggests how to resolve these conflicts while protecting the rights and interests of individuals sequenced. Translational genomics requires this more translational approach to law.

Henri Bouillard est initié par Kant à un regard critique. Sa rencontre avec saint Thomas le solidarise avec la «nouvelle théologie», et l'ouvre à une démarche herméneutique. Sa rencontre avec Maurice Blondel l'amène à opérer le tournant anthropologique en théologie. Sa rencontre avec Karl Barth le conduit à se rallier avec réserve à la position de celui-ci. Sa rencontre avec Éric Weil l'entraîne à approfondir le tournant anthropologique, mais à récuser les théologies politiques, et à repousser le recours aux sciences (...) humaines en théologie. Bref, ces quatre rencontres l'engagent à passer d'une démarche dogmatique à une approche herméneutique, mais à écarter une méthode généalogique entée sur les sciences humaines. (shrink)

Sur la base d’une réflexion sur la moralité du sens commun, Henry Sidgwick dégage les différentes méthodes éthiques auxquelles celle-ci adhère inconsciemment. Trois méthodes principales construisent des systèmes d’évaluation et de décision de l’action humaine : l’hédonisme égoïste, l’intuitionnisme et l’utilitarisme. S’il est largement possible de montrer les fondements intuitifs de l’utilitarisme, il est par contre impossible d’apporter la preuve rationnelle à quiconque fait de son bonheur privé le souverain bien qu’il doit travailler à promouvoir le plus grand bonheur du (...) plus grand nombre, pour la raison essentielle que le calcul utilitariste peut exiger, en certaines circonstances, le sacrifice de l’intérêt propre, de sorte qu’ultimement ce qui se révèle, au théoricien de l’éthique, c’est le dualisme de la raison pratique et le chaos de la moralité. A series of studies investigated the capacity of children between the ages of 7 and 12 to give free and informed consent to participation in psychological research. Children were reasonably accurate in describing the purpose of studies, but many did not understand the possible benefits or especially the possible risks of participating. In several studies children's consent was not affected by the knowledge that their parents had given their permission or by the parents saying that they would not be upset (...) if the children refused. In contrast, other studies found that children were much more likely to stop their participation if the experimenter said explicitly that she would not be upset if they stopped. We suggest that experimenters should pay more attention to describing the possible risks and benefits of participation in research, and that they should also make it clearer to children that they are free to stop once they have begun. Cela permet au théologien français d'établir que la relation de l'homme à Dieu est à la fois oeuvre de Dieu et oeuvre de l'homme, et en particulier que la connaissance de Dieu est une connaissance révélée et en même temps une connaissance naturelle, que l'analogie de la (...) foi ne va pas sans l'analogie de l'être, et que la morale et la politique sont hétéronomes dans leur inspiration et autonomes dans leur détermination.
