Reconstructive allografts using Vascularized Composite Allotransplantation are providing individuals living with upper limb loss and facial disfigurement with new opportunities for a sensate, esthetically acceptable, and functional alternative to current treatment strategies. Important research attention is being paid to how best to assess and screen candidates for VCA, measure optimal patient outcomes, and support patient adherence to lifelong behaviors and medical regimens. Far less attention, however, has been dedicated to the team science required for these complex VCA teams to form, (...) prepare, and provide the highest quality clinical and psychosocial care to those receiving VCA. VCA teams are unique in that they require specialized team members whose scope of practice may not otherwise overlap. The team also needs to constantly negotiate balancing patient safety with multiple risks throughout the transplant process. This study aimed to elucidate the team science needed for this highly innovative and complex area of medicine. Using in-depth qualitative interviews with 14 VCA team members and observations at team meetings, we found that careful consideration of team composition, team structure, and organizational commitment influences team performance and patient outcomes, but that to be efficient and truly effective, teams need to commit to developing processes that foster collaboration. These processes are action-oriented, strategic and interpersonal. Dedication and commitment to team science allows teams to manage conflict under stress and exercise ways to leverage strengths to provide optimal performance or patient psychosocial and clinical outcomes. This study can provide insight into quality improvement efforts for VCA teams and guidance for other transplant programs that wish to consider expansion into VCA. (shrink)

BackgroundOrgan donation has become one of the most effective ways to save lives and improve the quality of life for patients with end-stage organ failure. No previous studies have investigated the preferences for the different consenting options for organ donation in Egypt. This study aims to assess Egyptians’ preferences regarding consenting options for posthumous organ donation, and measure their awareness and acceptance of the Egyptian law articles regulating organ donation.MethodsA cross sectional study was conducted among 2743 participants over two years. (...) Each participant was required to rank eleven consenting options from 1 to 11, and to report his awareness and acceptance of the seven articles of the Egyptian law of organ donation.Results47% of the participants expressed willingness to donate their organs after death. This percentage increased to 78% when consenting options were explained to participants. “Informed consent by donor only” was the most preferred type of consent for one third of respondents. Awareness of the law articles regulating organ donation was relatively low ranging from 56% to 23%.ConclusionCurrently, around half of the Egyptian population agree to posthumous organ donation. This percentage could be increased significantly by raising the awareness about how the process of donation could be regulated and how the patient’s right of decision could be protected. (shrink)

A principal type-scheme of a -term is the most general type-scheme for the term. The converse principal type-scheme theorem (J.R. Hindley, The principal typescheme of an object in combinatory logic, Trans. Amer. Math. Soc. 146 (1969) 29–60) states that every type-scheme of a combinatory term is a principal type-scheme of some combinatory term.This paper shows a simple proof for the theorem in -calculus, by constructing an algorithm which transforms a type assignment to a -term into a principal type assignment (...) to another -term that has the type as its principal type-scheme. The clearness of the algorithm is due to the characterization theorem of principal type-assignment figures. The algorithm is applicable to BCIW--terms as well. Thus a uniform proof is presented for the converse principal type-scheme theorem for general -terms and BCIW--terms. (shrink)

Solovay’s random-real forcing [R.M. Solovay, Real-valued measurable cardinals, in: Axiomatic Set Theory , Amer. Math. Soc., Providence, R.I., 1971, pp. 397–428] is the standard way of producing real-valued measurable cardinals. Following questions of Fremlin, by giving a new construction, we show that there are combinatorial, measure-theoretic properties of Solovay’s model that do not follow from the existence of real-valued measurability.

In this article we characterize all those sequences of cardinals of length ω1 which are cardinal sequences of some compact scattered space . This extends the similar results from [R. La Grange, Concerning the cardinal sequence of a Boolean algebra, Algebra Universalis, 7 307–313] for such sequences of countable length. For ordinals between ω1 and ω2 we can only give a sufficient condition for a sequence of that length to be a cardinal sequence of a compact scattered space. This condition (...) is, arguably, the most one can expect in ZFC. In any case, ours is a significant extension of the sufficient conditions given in [J.C. Martinez, A consistency result on thin-tall superatomic Boolean algebras, Proc. Amer. Math. Soc. 115 473–477] and [J. Bagaria, Locally generic Boolean algebras and cardinal sequences, Algebra Universalis 47 283–302]. (shrink)

Let X be a separable Banach space and Q be a coanalytic subset of . We prove that the set of sequences in X which are weakly convergent to some eX and is a coanalytic subset of . The proof applies methods of effective descriptive set theory to Banach space theory. Using Silver’s Theorem [J. Silver, Every analytic set is Ramsey, J. Symbolic Logic 35 60–64], this result leads to the following dichotomy theorem: if X is a Banach space, is (...) a regular method of summability and is a bounded sequence in X, then there exists a subsequence iL such that either there exists eX such that every subsequence iH of iL is weakly summable w.r.t. to e and QiH,e); or for every subsequence iH of iL and every eX with QiH,e)the sequence iH is not weakly summable to e w.r.t. . This is a version for weak convergence of an Erdös–Magidor result, see [P. Erdös, M. Magidor, A note on Regular Methods of Summability, Proc. Amer. Math. Soc. 59 232–234]. This anthology consists of a collection of papers on the nature of dis positions and the role of disposition concepts in scientific theories. I have tried to make the collection as representative as possible, except that problems specifically connected with dispositions in various special sciences are relatively little discussed. Most of these articles have been previously published. The papers by Mackie, Essler and Trapp, Fetzer (in Section 11), Levi, and Tuomela appear here for the first time, and are simultaneously published (...) in Synthese 34, No. 4, which is a special issue on dispositions. Of the previously published material it should be emphasized that the papers by Hempel and Fisk have been extensively revised specially for this anthology. The papers are grouped in four sections, partlyon the basis of their content. However, due to the complexity of the issues involved, there is considerable overlap in content between the different sections, especially between Sections land 11. In The Value Gap, Toni Rønnow-Rasmussen addresses the distinction between what is finally good and what is finally good-for, two value notions that are central to ethics and practical deliberation. The first part of the book argues against views that claim that one of these notions is either faulty, or at best conceptually dependent on the other notion. Whereas these two views disagree on whether it is good or good-for that is the flawed or dependent concept, it is argued, as (...) against both approaches, that goodness and goodness-for are independent value notions that cannot be fully understood in terms of one another. The second part provides an analysis of good and good-for in terms of a fitting-attitude analysis. By elaborating a more nuanced understanding of the key elements of this analysis—reasons and pro-attitudes—Rønnow-Rasmussen challenges the widespread idea that there are no genuine practical and moral dilemmas. The result is that the gap between favouring for a reason what is good and favouring for a reason what is good for someone appears insurmountable. (shrink)

We discuss five kinds of representations of rationales and provide a formal account of how they can alter disputation. The formal model of disputation is derived from recent work in argument. The five kinds of rationales are compilation rationales, which can be represented without assuming domain-knowledge (such as utilities) beyond that normally required for argument. The principal thesis is that such rationales can be analyzed in a framework of argument not too different from what AI already has. The result is (...) a formal understanding of rationales, a partial taxonomy, and a foundation for computer programs that represent and reason with rationales.The five kinds of rationales are as follows: (c)ompression and (s)pecialization, which yield rules, and (d)isputation, which yields a decision. These are modeled as potentially changing the focus of the dispute. Then there are (f)it, a rationale for rules, and (r)esolution, a rationale for decisions. These cannot be modeled as simply; they force disputation to a meta-level, at least temporarily. (shrink)

The dynamics of populations of self-replicating, hierarchically structured individuals, exposedto accidents which destroy their sub-units, is analyzed mathematically, specifically with regardto the roles of redundancy and sexual repair. The following points emerge from this analysis:0 A population of individuals with redundant sub-structure has no intrinsic steady-statepoint; it tends to either zero or infinity depending on a critical accident rate α c . Increased redundancy renders populations less accident prone initially, but populationdecline is steeper if a is greater than a fixed (...) value α d . Periodic, sexual repair at system-specific intervals prevents continuous decline and stabilizesthe population insofar as it will now oscillate between two fixed population levels. The stabilizing sexual interval increases with increased complexity provided this is accom-panied by appropriate levels of redundancy. The model closely simulates the dynamics of heterosis effects. Repair fitness is a population fitness: the chance of an individual being repaired is a functionof the statistical make-up of the population as a whole at that particular period. Populationsliving at α > α c either engage in sexual repair at the appropriate time or they die out. The mathematical properties of the model illustrate mechanisms which possibly played arole in the evolution of a mortal soma in relation to sexual reproduction. (shrink)

This book is a timely new interpretation of the moral and political philosophy of Thomas Hobbes. Staying close to Hobbes's text and working from a careful examination of the actual substance of the account of natural law, R.E. Ewin argues that Hobbes well understood the importance of moral behavior to civilized society. This interpretation stands as a much-needed corrective to readings of Hobbes that emphasize the rationally calculated, self-interested nature of human behavior. It poses a significant challenge to currently fashionable (...) game theoretic reconstructions of Hobbesian logic. It is generally agreed that Hobbes applied what he took to be a geometrical method to political theory. But, as Ewin forcefully argues, modern readers have misconstrued Hobbes's geometric method, and this has led to a series of misunderstandings of Hobbes's view of the relationship between politics and morality. Important implications of Ewin's reading are that Hobbes never thought that "the war of each against all" was an empirical possibility for citizens; that his political theory actually presupposes moral agency; and that Hobbes's account of natural law forces us to the conclusion that Hobbes was a virtue theorist. This major contribution to Hobbes studies will be praised and criticized, welcomed and challenged, but it cannot be ignored. All philosophers, political theorists, and historians of ideas dealing with Hobbes will need to take account of it. (shrink)

The problem that confronts us when we try to compare the structure of discourse and explanation in different domains of knowledge is that no one is an insider in more than one field, and insider information is essential. An observer who is not immersed in the practice of a particular scholarship and who wants to understand it is at the mercy of the practitioners. Yet those practitioners are themselves mystified by a largely unexamined communal myth of how scholarship is carried (...) on. R. G. Collingwood, although primarily a philosopher, was immersed in the community of historians and understood how history is done, so that he has had an immense influence on our ideas about historiography. Every historian knows The Idea of History.1 He was also a metaphysician, yet his influence on scientists’ understanding of nature, and of science, has been nil, and it is a rare scientist indeed who has ever heard of Collingwood or read The Idea of Nature.2 Collingwood’s views of the structure of science had to be constructed in large part from the elaborate fictions created by scientists and by an earlier generation of philosophers and historians of science who participated in the Baconian myth of the hypothetical-deductive scheme. 1. Each value judgment expresses the belief that something is the case on condition that something else is the case. Professor R. J. Getty has drawn attention to a tenth- or early eleventh-century manuscript of Statius’ Thebaid, hitherto examined only in Book I, namely Turonensis . Dr. Klotz, in his Teubner edition of 1908, gave citations from Book I, and wrote , ‘dolendum est sane de hoc codice primum tantum librum innotuisse, sed cum Roffensis libri maxime affinis accuratiorem notitiam haberemus, collatione quamvis -aegre careri posse nobis visum est.’ I have collated both T and Roffensis in full, and find firstly (...) that the citations of r by Klotz are far from accurate, especially in the last six books, and secondly that while it is indeed true thatT and r are very closely related, T is a much better representative than r of their common source. Wittgenstein's Tractatus contains a wide range of profound insights into the nature of logic and language – insights which will survive the particular theories of the Tractatus and seem to me to mark definitive and unassailable landmarks in our understanding of some of the deepest questions of philosophy. And yet alongside these insights there is a theory of the nature of the relation between language and reality which appears both to be impossible to work out in detail in a way (...) which is completely satisfactory, and to be bizarre and incredible. I am referring to the so-called logical atomism of the Tractatus . The main outlines of this theory at least are clear and familiar: there are elementary propositions which gain their sense from being models of possible states of affairs; such propositions are configurations of names of simple objects, signifying that those simples are analogously configured; every proposition has its sense through being analysable as a truth-functional compound of elementary propositions, thus deriving its sense from the sense of the elementary propositions when this view is taken in conjunction with the idea that the sense of a proposition is completely specified by specifying its truth-conditions. In this way the Tractatus incorporates in its working out a philosophical system analogous to the classical philosophical systems of Leibniz or Spinoza which are regarded by many people, in a sense rightly, as the prehistoric monsters of philosophy which are not to be studied as living organisms, but studied as the curiosities of human thought. And we may here agree that in the end we must simply reject a philosophy which incorporates such features as its postulation of simple eternal objects, or of a possibility of an analysis of a proposition which was presented as a pre-condition for the propositions that we ordinarily utter to make sense, and yet the specific form of which we are unaware of, and so on. Of the unusual grammatical constructions which Statius employs for the sake of variety and novel effect, among the most remarkable is his use of the ablative case. There are striking instances at Th. 8. 157, Th. 10. 309, Ach. 1. 219, Ach. 2. 129; and W. C. Summers was led to say: ‘We see some traces in Valerius of the lax use of this case which became almost a disease with Statius, who employs it for almost any kind of idea.’ (...) There are three passages in Statius which have been misinterpreted or emended through the failure to recognize a use of the ablative to which he was especially prone—the local ablative where the poetic dative of ‘motion towards’ would seem more normal. I shall first list examples of this use in Statius where there is little or no doubt about interpretation or reading, and in the light of this accumulated evidence consider the three passages which are disputed.
