This book takes stock of the wide range of practices of deliberative mini-publics. More concretely, it takes an informed look at preconditions, processes, and outcomes. Furthermore, it provides a critical assessment of the experience with mini-publics, in particular (the lack of) policy-impact. The book brings together leading scholars in the field, most notably James S. Fishkin and Mark E. Warren. It speaks to students and scholars with an interest in democracy and democratic innovations. Several studies show that healthcare professionals need to communicate inter-professionally in order to manage ethical difficulties. A model of clinical ethics support inspired by Habermas’ theory of discourse ethics has been developed by our research group. In this version of CES sessions healthcare professionals meet inter-professionally to communicate and reflect on ethical difficulties in a cooperative manner with the aim of reaching communicative agreement or reflective consensus. In order to understand the course of action during CES, the aim of this (...) study was to describe the communication of value conflicts during a series of inter-professional CES sessions. Ten audio- and video-recorded CES sessions were conducted over eight months and were analyzed by using the video analysis tool Transana and qualitative content analysis. The results showed that during the CES sessions the professionals as a group moved through the following five phases: a value conflict expressed as feelings of frustration, sharing disempowerment and helplessness, the revelation of the value conflict, enhancing realistic expectations, seeing opportunities to change the situation instead of obstacles. In the course of CES, the professionals moved from an individual interpretation of the situation to a common, new understanding and then to a change in approach. An open and permissive communication climate meant that the professionals dared to expose themselves, share their feelings, face their own emotions, and eventually arrive at a mutual shared reality. BackgroundThis study is part of a major study about difficulties in communicating ethical problems within and among professional groups working in hemodialysis care. Describing experiences of ethically difficult situations that induce a troubled conscience may raise consciousness about ethical problems and thereby open the way to further reflection.The aim of this study was to illuminate the meanings of being in ethically difficult situations that led to the burden of a troubled conscience, as narrated by physicians working in dialysis care.MethodA phenomenological (...) hermeneutic method was used to analyze the transcribed narrative interviews with five physicians who had varying lengths of experience in nephrology.ResultsThe analysis shows that physicians working in hemodialysis care suffered from a troubled conscience when they felt torn by conflicting demands and trapped in irresolution. They faced ethical dilemmas where they were forced to make crucial decisions about life or death, or to prioritize when squeezed between time restraints and professional and personal demands. In these ethical dilemmas the physicians avoided arousing conflicts, were afraid of using their authority, were burdened by moral responsibility and felt devalued and questioned about their way of handling the situation. The findings point to another way of encountering ethical dilemmas, being guided by their conscience. This mean sharing the agony of deciding how to act, being brave enough to bring up the crucial problem, feeling certain that better ways of acting have not been overlooked, being respected and confirmed regarding decisions made.ConclusionThe meanings of being in ethically difficult situations that led to the burden of a troubled conscience in those working in hemodialysis care, indicate the importance of increasing the level of communication within and among various professional groups - to transform being burdened by a troubled conscience into using conscience as a guide - in situations where no way of solving the problem seems to be good. In the last thirty years, a relatively large group of cognitive scientists have begun characterising the mind in terms of two distinct, relatively autonomous systems. To account for paradoxes in empirical results of studies mainly on reasoning, Dual Process Theories were developed. Such Dual Process Theories generally agree that System 1 is rapid, automatic, parallel, and heuristic-based and System 2 is slow, capacity-demanding, sequential, and related to consciousness. While System 2 can still be decently understood from a traditional cognitivist approach, (...) BackgroundMoral distress has been described as an emotionally draining condition caused by being prevented from providing care according to one’s convictions. Studies have described the impact of moral distress on healthcare professionals, their situations and experiences. The Measure of Moral Distress for Healthcare Professionals (MMD-HP) is a questionnaire that measures moral distress experienced by healthcare professionals at three levels: patient, system and team. The aim of this project was to translate and make a cultural adaption of the MMD -HP to (...) the Swedish context.MethodsThe questionnaire comprises 27 items, rated according to frequency and intensity on a five-point Likert scale (0–4). The procedure for translating MMD-HP followed WHO guidelines (2020). These entailed a forward translation from English to Swedish, a back translation, expert panel validation, pretesting and cognitive face-to-face interviews with 10 healthcare professionals from various professions and healthcare contexts.ResultsThe Swedish version of MMD-HP corresponds essentially to the concept of the original version. Parts of some items’ had to be adjusted or removed in order to make the item relevant and comprehensible in a Swedish context. Overall, the cognitive interviewees recognized the content of the items which generally seemed relevant and comprehensible.ConclusionThe Swedish version of MMD-HP could be a useful tool for measuring moral distress among healthcare professionals in a Swedish healthcare context. BackgroundMoral distress is a problem and negative experience among health-care professionals. Various instruments have been developed to measure the level and underlying reasons for experienced moral distress. The moral distress thermometer (MDT) is a single-tool instrument to capture the level of moral distress experienced in real-time.AimThe aim of this study was to translate the MDT and adapt it to the Swedish cultural context.Research designThe first part of this study concerns the translation of MDT to the Swedish context, and the second (...) part the psychometric testing of the Swedish version.Participants and research context89 healthcare professionals working at a hospital in northern Sweden participated. Convergent validity was tested between MDT and Measure of Moral Distress-Healthcare Professionals (MMD-HP), and construct validity was tested by comparing MDT scores among healthcare professionals. MDT was compared with responses to the final questions in MMD-HP. One-way ANOVA, Welch’s ANOVA, Games–Howell post-hoc test and Pearson’s correlation analysis were done.Ethical considerationsThe study was approved by the Swedish Ethics Review Authority (dnr 2020-04120) in accordance with Helsinki Declaration.ResultsThe translated Swedish version of MDT was described as relevant to capture the experience of moral distress. The mean value for MDT was 2.26, with a median of 2 and a mode value of 0. The result showed moderate correlations between the MDT and MMD-HP total scores. There was a significant difference when comparing MDT and healthcare professionals who had never considered leaving their present position with those who had left and those who had considered leaving but had not done so, with the latter assessing significantly higher moral distress.ConclusionThe MDT is an easily available instrument useful as an extension to MMD-HP to measure the real-time experience of moral distress among healthcare professionals in a Swedish context. Background Ethics communication has been described as a pedagogical form, promoting development of ethical competence among nursing students. The ‘one to five method’ was developed by this research group as a tool for facilitating ethical communication in groups among healthcare professionals but has not yet been evaluated. Aim To explore post-graduate nursing students’ experiences of practicing ethical communication in groups Research design The study design is qualitative. Participants and research context The study comprised 12 nursing students on a post-graduate course (...) for Registered Nurses focusing on palliative care. After education the students engaged as participants and facilitators in ethics communication in groups, with support from the ‘one to five method’. Data were derived from the students written reflections and subjected to thematic analysis. Ethical considerations The study was performed in accordance with the ethical standards in the 2013 Helsinki Declaration and approved by the Ethics Committee of the Medical Faculty at Umeå University Results An overall theme was identified, ‘being supported to achieve a democratic dialogue concerning ethical problems’ and five themes, related to each step of the ‘one to five method’. The themes concerned: putting the experience of an ethical dilemma into words; being confirmed by shared emotions; defining the value conflict using ethical principles; expressing virtues of empathy and raised awareness; jointly finding various action approaches. Discussion Regular ethics communication in groups encourages nursing students to actively seek understanding and clarification of ethically difficult situations. Conclusions The results indicate that ethics communication in groups could be a way of developing virtues during education. Interventions using the ‘one to five method’ need to be further studied among student groups from other healthcare contexts. (shrink)

This paper reviews the history and current status of electronic identities (eID) and eID management in Sweden, including an outlook for the future. The paper is based on official policy documents, technical documentation, presentations by key experts, and comments from government agencies and independent experts. The future perspective is based on the October 2009 public investigation (SOU 2009:86) by the E-delegation. It is concluded that the E-delegation proposal, while still pending political decision, is a major step forward in terms of (...) making eID more established as an infrastructural element in the government electronic service program. Voluntary participation is connected to cultural, political, religious and social contexts. Social and societal factors can provide opportunities, expectations and requirements for voluntary activity, as well as influence the values and norms promoting this. These contexts are especially central in the case of voluntary participation among students as they are often responding to the societal demands for building a career and qualifying for future assignments and/or government requirements for completing community service. This article questions how cultural values affect attitudes towards (...) volunteerism, using data from an empirical research project on student volunteering activity in 13 countries in North America, Europe, the Middle East, and the Asia Pacific region. The findings indicate that there are differences in motivation between countries which represent different cultural values. This article sets these findings in context by comparing structural and cultural factors which may influence volunteerism within each country. (shrink)

Background On a daily basis, healthcare professionals deal with various ethical issues and it can be difficult to determine how to act best. Clinical ethics support (CES) has been developed to provide support for healthcare professionals dealing with complex ethical issues. A long-term perspective of participating in inter-professional dialogue and reflective-based CES sessions is seemingly sparse in the literature. Research aim The aim was to describe experiences of impact of Inter-professional Ethics Communication in groups (IEC) based on Habermas’ theory of (...) communicative actions, after 6 months from the perspective of an inter-professional team. Research design A qualitative inductive approach was chosen, and individual interviews ( n = 13) were conducted. Interview data were analysed using qualitative content analysis. Participants The participants, 10 females and two males, represented assistant nurses, registered nurses, physicians, occupational therapists, physiotherapists, welfare officers and psychologists. Each had attended at least four IEC sessions. Ethical considerations The study was approved by the Regional Ethical Review Board in Umeå, Sweden, and it has been undertaken in accordance with the Helsinki Declaration. Findings Overall, the descriptions expressed a perceived achievement of a deepened and integrated ethical awareness that increased the participants’ awareness of ethically difficult situations as well as their own ethical thinking, actions and approaches in daily work. Perspectives were shared and the team become more welded. They carried the memories of the reflections within them, which was perceived as supportive when encountered new ethically situations. Discussion Putting words to unarticulated thoughts may stimulate repeated reflections, leading to new insights and alternative thoughts. Conclusion The outcome of IEC sessions 6 months following the last session can be described as an incorporated knowledge that enables actions in ethically difficult situations based on an ethical awareness both at a ‘We-level’ and an ‘I-level’. Sale's new book SDS feeds that fascination by treating historically the recent experience of many leftists who related to SDS. Rarely have revolutionaries been so flattered by having their experience sympathetically legitimized between hard covers so recently after the events happen. For that reason, the most read, and certainly the first read part of Sale's book is the (...) index where ex-SDS'ers will search for familiar names, places, and events. Those familiar areas will be covered with varying degrees of success depending on the reader's particular experience, yet for many this book will fail to serve that purpose adequately. (shrink)

The semantic of the terms “sustainable development” and “corporate social responsibility” have changed over time to a point where these concepts have become two interrelated processes for ensuring the far‐reaching development of society. Their convergence has given dimension to the environmental and corporate regulation mechanisms in strong economies. This article deals with the question of how the ethos of this convergence could be incorporated into the self‐regulation of businesses in weak economies where nonlegal drivers are either inadequate or inefficient. In late 2019, the COVID-19 pandemic began to spread over the world, causing millions of deaths. In the first few months of the pandemic, several countries prevented the spread of the pandemic successfully. By contrast, the pandemic in many other countries was not controlled well. For example, India encountered a second serious outbreak of COVID-19 from April 2021 due to the poor resistance measures implemented by the government. To figure out the effective countermeasures to the pandemic, this research proposes a (...) COVID-19 pandemic and its response system, which consists of the infection subsystem, the quarantine subsystem, and the medical subsystem. On this basis, an improved SEIR-SD model is established which is utilized to analyze the response measures to the pandemic quantitatively. This model successfully simulates the actual epidemic scenarios in Wuhan, which verifies its effectiveness. Afterward, the impact of hospital administration rate, quarantine rate, average contact number, and contact infection rate on the cumulative number of infections and deaths are analyzed by simulation. The results show that both the medical and administrative efforts, especially in the early stage of the epidemic, are significant in reducing the number of infections and shortening the epidemic period. In the medical aspect, the more stringent quarantine brings the earlier inflection point of the epidemic; more importantly, improving the treatment rate significantly reduces the scale of the epidemic. In the administrative aspect, enforcing individual protection and strict community closure can effectively cut off the transmission of the virus and curb the spread of the epidemic. Finally, this research proposes several practical suggestions in response to the COVID-19 pandemic. The main contribution of this research is that the effects of different response measures on the number of new infections daily and the cumulative number of deaths of a country or region in the COVID-19 pandemic are estimated quantitatively based on modeling and simulation. La niche en conglomérat SD 108 est proche de l’entrée du sanctuaire delphique d’Apollon. Depuis sa découverte, on a beaucoup écrit au sujet de son architecture, de sa datation et surtout de sa destination première, mais la connaissance des réalités du terrain égalait rarement celle des textes. Au bout de près de soixante-dix ans, G. Roux a démontré, contre l’opinion commune, qu’elle ne pouvait pas avoir contenu les statues des «Navarques » érigées en mémoire de la victoire lacédémonienne d’Aigos-Potamoi, mais (...) il s’est trompé sur le reste. Le dossier archéologique constitué à partir de 1966 a permis d’établir que SD 108 était d’époque classique et comportait une colonnade prostyle irrégulière de douze colonnes doriques dressées sur un podium. Cette forme insolite se doublait d’un aménagement intérieur extraordinaire. Enfin l’orientation, différente de celle des autres niches, semble avoir correspondu à celle du monument des «Navarques » et découlé d’un remaniement de tout le secteur Sud-Est. (shrink)

Notes on the stairway of the theater at Delphi (SD 541) The stairway, made of limestone, by which one reached the theater at Delphi since its discovery (end of the 19th c.) was dismantled in 1974. Below it were discovered two earlier ones, made of poros, a third of the width and seemingly not related to the theater. The datings have varied according to time and author. The limestone stairway has even been considered a paleochristian construction. It is (...) true that its first narrow flight of steps resulted from a transformation of that date. But a trace still visible on the string wall permits documenting the original profile of this part and reconstituting a width equal to the rest of the structure, thus restoring a monumental aspect to the ensemble.
