Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making.Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments.DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system.Trial RegistrationISRCTN89993391. Children learn about the world from the testimony of other people, often coming to accept what they are told about a variety of unobservable and indeed counter-intuitive phenomena. However, research on children’s learning from testimony has paid limited attention to the foundations of that capacity. We ask whether those foundations can be observed in infancy. We review evidence from two areas of research: infants’ sensitivity to the emotional expressions of other people; and their capacity to understand the exchange of information (...) through non-verbal gestures and vocalization. We conclude that a grasp of the bi-directional exchange of information is present early in the second year. We discuss the implications for future research, especially across different cultural settings. (shrink)

This chapter reviews the historical evidence concerning the development of slavery in eastern Africa, the various forms found in societies on the coast and in the interior, the social and cultural consequences of enslavement, and its ultimate abolition. It then looks at the known and potential archaeological traces of the trajectories of these different systems of slavery, with particular reference to the area along the middle and lower Pangani River, Tanzania. The chapter concludes with a consideration of whether or not (...) it would be possible to discern slavery from the surviving archaeological remains alone, and the implications of this answer for future archaeological investigations of slavery elsewhere in the region. Leading archaeologists and historians provide new studies of slavery, slave resistance and the economic, environmental and political consequences of slave trading in Africa, from the first millennium AD through to the nineteenth century.

This chapter presents a synopsis of the historical evidence concerning the expansion of slavery and the trade in ivory during the Turco-Egyptian era in the Sudan between 1820 and 1881, and a description of the results of recent and very preliminary archaeological investigations at three sites associated with this trade around the town of Rumbek in Lakes State, South Sudan. The chapter begins with a brief review of the establishment of Ottoman rule in Egypt, before moving on to consider the (...) broader geopolitical forces that gave rise to the decision by the Egyptian Khedive, Mehmed Ali, to invade Sinnar, Kordofan and adjacent areas of northern Sudan. It then discusses the economic consequences and legal changes following the establishment of Turco-Egyptian rule that helped create the conditions for the expansion of slaving expeditions into southern Sudan and the establishment of a series of fortified camps or zaribas in these areas. I spell out a problem with the doctrine of creatio ex nihilo: that, contra the doctrine, it is not possible to efficiently cause something from nothing. This is because an efficient cause requires a material cause in order to have an effect. The material cause supplies the potency that the efficient cause actualises. Because nothingness has no potencies, there is nothing for an efficient cause to actualise. I show that this objection presupposes that the theory of noneism (the proposition that (...) some things do not exist) is false. I postulate that the universe (i.e. the created order) is a non-existent item and so there is no problem with the claim that it was efficiently caused to come from nothing – the universe has no being anyway. After rehearsing the rather strong reasons in favour of the truth of noneism, I deal with two objections that are peculiar to my claim that the universe lacks reality: that creation possesses characteristics that are sufficient to render it existent and that a non-existent object has its properties independent of divine fiat. I show that there are sensible replies to both objections. With regard to the first I show that the possession of such characteristics at most shows that the universe has an ontological status that is equivalent to some reference point. With regard to the second I argue that the Characterisation Principle (i.e. in some world – not necessarily the actual world – an object has the properties that it is characterised as having) entails that non-existent objects possess their properties in virtue of some existent entity and that the only plausible candidate for such an entity is a divine mind of some sort. William Lane Craig is Research Professor of Philosophy at Talbot School of Theology. Quentin Smith considers him “one of the leading philosophers of time,” and his impressive, exhaustive three volume series on God, time, relativity, and eternity clearly demonstrates this. As the first two volumes are devoted to the problem of time—examining whether a dynamic/tensed or static/tenseless view of time is correct—I review them back-to-back.

Philosopher William Lane Craig of the Talbot School of Theology has published three other Kluwer books on time and eternity and God’s relationship to them. In this book, Craig draws some important strands together regarding the concept of God and relativity theory. He notes the striking “paucity of integrative literature” in this regard: “I am convinced that this lack is largely due to the fact that theologians and philosophers of religion do not understand Einstein’s theories and so are reduced (...) to merely parroting what they read in scientific popularizations,” resulting in “a superficial and uncritical interaction”. This is a review of *The Kalām Cosmological Argument* (edited by Paul Copan and William Lane Craig). In this review, I focus primarily on the papers in the first volume by Waters, Loke, and Oderberg. (I have also written an independent review of the second volume.).

I try to clear up a couple of misunderstandings in William Craig’s review. The first has to do with the difference between what I call “Historical Biblical Criticism” and historical scholarship. I claim there is conflict between the first and Christian belief; I don’t for a moment think there is conflict between historical scholarship and Christian belief. The second has to do with Platonism, theism and causality. I point out that theism has the resources to see abstract objects as like (...) divine thoughts, in which case they are not causally isolated; this offers a reply to Paul Benacerraf’s suggestion that if, as on Platonism, abstract objects are causally isolated from everything, then there is no way in which we could come to know them or anything about them. This self-contained treatise, originally published in 1947 by Holt, Rinehart and Winston, examines fundamental features of nature in order to lay the groundwork for providing a solution to the major problems of ethics.

Since Freud, clinicians have understood that disturbing memories contribute to psychopathology and that new emotional experiences contribute to therapeutic change. Yet, controversy remains about what is truly essential to bring about psychotherapeutic change. Mounting evidence from empirical studies suggests that emotional arousal is a key ingredient in therapeutic change in many modalities. In addition, memory seems to play an important role but there is a lack of consensus on the role of understanding what happened in the past in bringing about (...) therapeutic change. The core idea of this paper is that therapeutic change in a variety of modalities, including behavioral therapy, cognitive-behavioral therapy, emotion-focused therapy, and psychodynamic psychotherapy, results from the updating of prior emotional memories through a process of reconsolidation that incorporates new emotional experiences. We present an integrated memory model with three interactive components – autobiographical (event) memories, semantic structures, and emotional responses – supported by emerging evidence from cognitive neuroscience on implicit and explicit emotion, implicit and explicit memory, emotion-memory interactions, memory reconsolidation, and the relationship between autobiographical and semantic memory. We propose that the essential ingredients of therapeutic change include: (1) reactivating old memories; (2) engaging in new emotional experiences that are incorporated into these reactivated memories via the process of reconsolidation; and (3) reinforcing the integrated memory structure by practicing a new way of behaving and experiencing the world in a variety of contexts. The implications of this new, neurobiologically grounded synthesis for research, clinical practice, and teaching are discussed. (shrink)

The options in a decision problem generally have outcomes with common features. Putting aside the common features simplifies deliberations, but the simplification requires a philosophical justification that this book provides.
