The aim of this study was to identify and analyse ethical decision making in the home care of persons with long-term mental illness. A focus was placed on how health care workers interpret and deal with the principle of autonomy in actual situations. Three focus groups involving mental health nurses who were experienced in the home care of persons with chronic mental illness were conducted in order to stimulate an interactive dialogue on this topic. A constant comparative analysis of the (...) transcribed audiotaped sessions identified a central theme that concerned the moral symbolic meaning of ‘home’. Panentheism has become a familiar term in contemporary Christian systematic theology and philosophy, for it is widely believed to be an appropriate way to overcome the alleged dualism found in the classical God-world relationship. But what is meant by the term panentheism, and how does it work so as to avoid becoming still another form of pantheism or cosmic monism? This paper provides a partial solution to the completeness problem for Joyal's axiomatization of open and etale maps, under the additional assumption that a collection axiom holds.

The basic (negative and positive) methodological maxims of three currents of philosophy of science (logical empiricism, falsificationism, and postpositivism) are formulated. Many of these maxims (stratagems) are controversial, e.g., the stance about the nonsense of metaphysics, and that of its indispensability. The restricted validity of these maxims allows for their unification. Within the framework of most of them there may be a relationship of (synchronic, or diachronic) subordination of the contradicting desiderata. In this vein ten stratagems are formulated.

Context: The theory of radical constructivism offers a tool for the evaluation of knowledge in general: especially with regard to its epistemic and ontological character. This applies in particular to knowledge that is non-cognitive, such as, e.g., ethical convictions. Problem: What impact can radical constructivism have on the topic of ethics? Specifically, how can ethical issues be resolved within a radical-constructivist epistemic approach? Method: I extend the theory of radical constructivism to include also items of non-cognitive knowledge. This makes it (...) possible to discuss ethical issues, which are non-cognitive, in a constructivist epistemic and ontological perspective. Some arguments against this conception of “strictly personal ethics” are discussed. Results: Radical constructivism is neutral on ethical issues, and thus cannot be invoked to endorse any particular ethical position. However, this causes no problem for the theory: the individual knower will construct her own ethical values and convictions, as part of her store of non-cognitive knowledge, in interaction with her environment (including other individuals. Hence ethical values cannot be argued in cognitive terms; and this elevates the knower into a position of personal responsibility with respect to ethical issues. Constructivist content: I focus on the role played by radical constructivism in the approach to cognitive vs. non-cognitive knowledge. The construction of knowledge (of any kind) emerges as a strictly personal enterprise. For instance, in the context addressed here, constructed non-cognitive knowledge forms a basis for the individual knower’s ethical position. (shrink)

Beebe-Center: "Mr. Allen's book develops in detail the view that pleasure and unpleasure are essentially manifestations of the progression and thwarting of impulses. Part one is a brief summary of the principal theories of feeling. Part two is devoted to "sensory" or "bodily" pleasure and unpleasure. These forms of feeling, it is argued, 'depend on an analogue of conation existing in the organism, a nisus to maintain, or to carry out to the (...) full extent, the functioning proper to the bodily system.' Part three -- the piece de resistance (about half the book) -- deals with the relation of pleasure and unpleasure to the main instincts. Patient competence, defined as the ability to reason, appreciate, understand, and express a choice is rarely discussed in patients with obsessive compulsive disorder, and coercive measures are seldom used. Nevertheless, a psychiatrist of psychologist may doubt whether OCD patients who refuse treatment understand their disease and the consequences of not being treated, which could result in tension between respecting the patient’s autonomy and beneficence. The purpose of this article is to develop a notion of competence that is grounded in clinical (...) practice and corresponds with the experiences of patients with obsessions and/or compulsions. We present a naturalistic case study giving both the patient’s and the therapist’s perspective based on in-depth interviews and a narrative analysis. The case study shows that competence is not merely an assessment by a therapist, but also a co-constructed reality shaped by the experiences and stories of patient and therapist. The patient, a medical student, initially told her story in a restitution narrative, focusing on cognitive rationality. Reconstructing the history of her disease, her story changed into a quest narrative where there was room for emotions, values and moral learning. This fitted well with the therapist’s approach, who used motivational interventions with a view to appealing to the patient’s responsibility to deal with her condition. We conclude that in practice both the patient and therapist used a quest narrative, approaching competence as the potential for practical reasoning to incorporate values and emotions. (shrink)
