The 2015 Varsity Medical Ethics debate convened upon the motion: “This house believes nootropic drugs should be available under prescription”. This annual debate between students from the Universities of Oxford and Cambridge, now in its seventh year, provided the starting point for arguments on the subject. The present article brings together and extends many of the arguments put forward during the debate. We explore the current usage of nootropic drugs, their safety and whether it would be beneficial to individuals and (...) society as a whole for them to be available under prescription. The Varsity Medical Debate was first held in 2008 with the aim of allowing students to engage in discussion about ethics and policy within healthcare. The event is held annually and it is hoped that this will allow future leaders to voice a perspective on the arguments behind topics that will feature heavily in future healthcare and science policy. This year the Oxford University Medical Society at the Oxford Union hosted the debate. (shrink)

BackgroundInternationally, patient access to notes is increasing. This has been driven by respect for patient autonomy, often recognised as a primary tenet of medical ethics: patients should be able to access their records to be fully engaged with their care. While research has been conducted on the impact of patient access to outpatient and primary care records and to patient portals, there is no such review looking at access to hospital medical records in real time, nor an ethical analysis of (...) the issues involved in such a change in process.MethodsThis study employed a systematic review framework in two stems, to integrate literature identified from two searches: Medline, CINAHL and Scopus databases were conducted, (for (1) hospitalised patients, patient access to records and its effects on communication and trust within the doctor-patient relationship; and (2) patient access to medical records and the ethical implications identified). The qualitative and quantitative results of both searches were integrated and critically analysed.Results3954 empirical and 4929 ethical studies were identified; 18 papers representing 16 studies were identified for review (12 empirical and 6 ethical). The review reveals a consensus that our current approach to giving information to patients – almost exclusively verbally – is insufficient; that patient access to notes is a welcome next step for patient-centred care, but that simply allowing full access, without explanation or summary, is also insufficient. Several ethical implications need to be considered: increased information could improve patient trust and knowledge but might transfer an (unwelcome) sense of responsibility to patients; doctors and patients have conflicting views on how much information should be shared and when; sharing written information might increase the already significant disparity in access to health care, and have unforeseen opportunity costs. The impact on medical practice of sharing notes in real time will also need to be evaluated.ConclusionsThe review presents encouraging data to support patient access to medical notes. However, sharing information is a critical part of clinical practice; changing how it is done could have significant empirical and ethical impacts; any changes should be carefully evaluated. There is disagreement in the literature about the exact nature of the phenomenon of empathy. There are emotional, cognitive, and conditioning views, applying in varying degrees across species. An adequate description of the ultimate and proximate mechanism can integrate these views. Proximately, the perception of an object's state activates the subject's corresponding representations, which in turn activate somatic and autonomic responses. This mechanism supports basic behaviors that are crucial for the reproductive success of animals living in groups. The Perception-Action Model, (...) together with an understanding of how representations change with experience, can explain the major empirical effects in the literature. It can also predict a variety of empathy disorders. The interaction between the PAM and prefrontal functioning can also explain different levels of empathy across species and age groups. This view can advance our evolutionary understanding of empathy beyond inclusive fitness and reciprocal altruism and can explain different levels of empathy across individuals, species, stages of development, and situations. A surfeit of research confirms that people activate personal, affective, and conceptual representations when perceiving the states of others. However, researchers continue to debate the role of self–other overlap in empathy due to a failure to dissociate neural overlap, subjective resonance, and personal distress. A perception–action view posits that neural-level overlap is necessary during early processing for all social understanding, but need not be conscious or aversive. This neural overlap can subsequently produce a variety of states depending on the context (...) and degree of common experience and emotionality. We outline a framework for understanding the interrelationship between neural and subjective overlap, and among empathic states, through a dynamic-systems view of how information is processed in the brain and body. Only a broad theory that looks across levels of analysis can encompass the many perspectives on the phenomenon of empathy. We address the major points of our commentators by emphasizing that the basic perception-action process, while automatic, is subject to control and modulation, and is greatly affected by experience and context because of the role of representations. For at least the past 15 years, food safety stakeholders across all levels of government have recognized the critical role that state and local agencies play in our nation's food safety system. State and local agencies are the first responders to foodborne outbreaks and have primary responsibility for keeping their residents safe from foodborne disease through effective surveillance and rapid response to outbreaks. They also conduct the vast majority of food safety inspections across the nation's restaurants, grocery stores, and other (...) food service and food retail establishments.Recent efforts among industry and government officials alike have focused on creating a more integrated, prevention-oriented food safety system. The Food Safety Modernization Act, the first major overhaul of the nation's food safety laws since the 1930s, now mandates that most sectors of the food industry put preventive controls in place, changing the emphasis of what the FDA does from response to prevention. Perception–action approaches are sometimes criticized because empathy takes cognitive forms and people do not overtly imitate or feel all observed states. These complaints reflect a misunderstanding of the framework, which we tried to clarify through a review that bridged social and neuroscientific views. Far from “simple fixes,” these misunderstandings appear to reflect deeply rooted differences in the way that each discipline conceptualizes science and the mind. We address the important points made by the commentators and reiterate the need to incorporate (...) rich, phenomenological descriptions into academic works so that we may prevent such conceptual cross-talk in the future. The open exchange of ideas across fields is often difficult, but essential to an integrated, scientific view of empathy. (shrink)

The shared circuits model (SCM) relies on well-regarded theories of perception-action, mirror neurons, and forward models, but the functional/informational level of the model limits its ability to explain complex behavior such as true imitation. Data from our lab and others confirm the more general details of the model, accepted by most, but specify the neural mechanisms involved in perception-action processes.

This article demonstrates how researchers from both the sciences and the humanities can learn from Charles Darwin’s mixed methodology. We identify two basic challenges that face emotion research in the sciences, namely a mismatch between experiment design and the complexity of life that we aim to explain, and problematic efforts to bridge the gap, including invalid inferences from constrained study designs, and equivocal use of terms like “sympathy” and “empathy” that poorly reflect such methodological constraints. We argue that Darwin’s mixed (...) methodology is a model for addressing these challenges even in laboratory work on emotion, because it shows how close observation of emotional phenomena makes sense only within broader historical contexts. This is valid and important but overlooks the barriers inherent in the current US partisan context. Psychology can be applied to help people of mixed allyship join together, to effectively and quickly force institutions and corporations to accept structural change.

Bereby‐Meyer, Hayakawa, Shalvi, Corey, Costa and Keysar investigate lying for self‐serving reasons. Participants in their experiments had to report the outcome of rolling a die only known to them. They inflated their outcomes less, and thus lied less, when using a foreign language than when using their native language. The authors suggest that lying for self‐serving reasons is an automatic tendency that can be overcome by speaking in a foreign language. [71].

Barth's book Ad Limina Apostolorum: An Appraisal of Vatican II contains his questions and an account from Barth of his visit, along with some other important essays. It is still deeply challenging in its incisiveness, despite its brevity. I have always been struck by Barth's Marian criticisms that are so central to his viewpoint about divine agency and human cooperation. These give us an excellent insight into his ecclesiology. They represent the reasons for much of his questioning of various Roman Catholic positions on all sorts of matters. Eberhard Busch's diaries, while Barth's assistant during this period, form a fascinating complimentary account of the visit. These tell of the not-so-fruitful visit to Augustin Cardinal Bea and the great highpoint (for Barth) of his one-hour audience with Pope Paul VI. Barth was impressed by the Jesuits during his visit, but not so much by the Dominicans. He thought the latter had absorbed too many streams of liberal modernity. The two Dominicans participating in this book turn those tides. Indeed, all the authors could be located within the conserving wings of their ecclesial membership, which makes the volume all the stronger in raising serious unresolved disagreements and noting important common ground. It is testimony to Barth's enduring value that this volume seems so timely. The Barth Centre at Princeton and the Pontifical Faculty of the Dominican House in Washington were the sponsors of this collection.The volume is structured around five of the key documents commented on by Barth, with a Catholic and Reformed voice reflecting on both the particular document and Barth's input (with the exception of Francesca Aran Murphy, who deals with George Lindbeck rather than Barth), thereby simultaneously pushing forward the Catholic–Reformed engagement with the Council and with Barth. Lewis Ayres rather impishly takes what Barth saw as the main weakness of the text (its chapter 2: "Handing on Divine Revelation") and makes it the greatest strength. Ayers provides a deeply learned and original exploration of the concept of "tradition" as sacramental. In this duet, Barth comes out looking like he may have missed hearing the melodies while too closely studying the score.Then follow two profound essays on Lumen Gentium, the Dogmatic Constitution on the Church. Christoph Schwöbel builds up a strong Protestant case for the necessary visible ecclesial identity of the Church, thus showing a deep convergence without denying differences that remain. White, in a deeply insightful and probing essay, goes to the heart of the ecclesiological difference between Barth and Yves Congar (and some other key Catholics at the time): the relationship between uncreated divine agency and created human agency, especially within the context of the post-apostolic Church. White suggests that, without some stable ecclesial mediations, there can be no proper relationship envisaged between the transcendent causality of God operating in Christ and the created, active cooperative acts with this grace. Objectives The use of empathy during clinical practice is paramount to delivering quality patient care and is important for understanding patient concerns at both the cognitive and affective levels. This study sought to determine how and when physicians self-report the use of empathy when interacting with their patients. Methods A cross-sectional survey of 76 physicians working in a large urban hospital was conducted in August of 2017. Physicians were asked a series of questions with Likert scale responses as well as (...) asked to respond to open-ended questions. Results All physicians self-report that they always (69%) or usually (29.3%) use empathic statements when engaging with patients. 93.1% of physicians believe that their colleagues always (20.7%) or usually (69%) use empathic statements when communicating with patients. Nearly one-third of physicians (33%) indicated that using the words “I understand” denotes an empathic statement. Although 36% of physicians reported that they would like to receive more training or assistance about how and when to use empathy during clinical practice. Significance of Results Despite the self-reported prevalent use of empathic statements, one-third of physicians indicate a desire for more training in what empathy means and when it should be used in a clinical setting. Additionally, nearly one-third of physicians in this study reported using responses that patients may not perceive as being empathic, even when intended to be empathic. This suggests that many physicians feel uncertain about a clinical skill they believe should be used in most, if not all, encounters. (shrink)

This investigation: evaluates the correspondence of two well-utilized CoI databases: the Centers for Medicare and Medicaid Services Open Payments and ProPublica’s Dollars for Docs and quantifies CoIs among authors of a publicly available point of care clinical support website which is used to inform evidence-based medicine decisions. Two data (...) sources were used: the hundred most common drugs and the top fifty causes of death. These topics were entered into a freely available database. The authors were then input into CMSOP and PDD and compensation and number of payments were determined for 2013–2015. The subset of highly compensated authors that also reported “Nothing to disclose” were further examined. There was a high degree of similarity between CMSOP and PDD for compensation and payment number. The amount received was 1.4% higher in CMSOP than in PDD. The articles where the authors had received the greatest compensation were in neurology, oncology, and endocrinology. Two authors reporting “Nothing to disclose” received appreciable and potentially relevant compensation. CMSOP and PDD produced almost identical results. CoIs were common among authors but self-reporting may be an inadequate reporting mechanism. Recommendations are offered for improving the CoI transparency of pharmaceutical-author interactions in point-of-care electronic resources. (shrink)

We prove frame determination results for the family of many-valued modal logics introduced by M. Fitting in the early '90s. Each modal language of this family is based on a Heyting algebra, which serves as the space of truth values, and is interpreted on an interesting version of possible-worlds semantics: the modal frames are directed graphs whose edges are labelled with an element of the underlying Heyting algebra. We introduce interesting generalized forms of the classical axioms D, T, B, 4, (...) and 5 and prove that they are canonical for certain algebraic frame properties, which generalize seriality, reflexivity, symmetry, transitivity and euclideanness. Our results are quite general as they hold for any modal language built on a complete Heyting algebra. (shrink)

In this paper we provide frame definability results for weak versions of classical modal axioms that can be expressed in Fitting's many-valued modal languages. These languages were introduced by M. Fitting in the early '90s and are built on Heyting algebras which serve as the space of truth values. The possible-worlds frames interpreting these languages are directed graphs whose edges are labelled with an element of the underlying Heyting algebra, providing us a form of many-valued accessibility relation. Weak axioms of (...) the form we treat here have been examined from the completeness perspective and further explored for applications in non-monotonic reasoning. Here, we introduce more weak many-valued modal axioms and prove a frame correspondence result for all of them. The classes of corresponding labelled frames possess algebraic properties which are strongly reminiscent of many classical ones, such as the Church-Rosser property, reflexivity, transitivity, partial functionality, etc. (shrink)

We examine the impact of corporate culture on environmental performance using a sample of 7199 firm-year observations over the period of 2002–2018. We find that stronger corporate culture improves environmental performance, measured by the amount of toxic chemical release (TCR). Our result is both statistically and economically significant. We also show that cultural norms of innovation, quality and teamwork as well as a technology-oriented corporate culture have a greater impact on enhancing environmental performance. Further analyses show that managerial competence and (...) strong institutional ownership moderate the relationship between corporate culture and environmental performance. We introduce the decomposition of expected and unexpected components of TCR and document that firms with a strong corporate culture implement strategies to reduce the unexpected component of TCR in addition to the expected component of TCR. Finally, we document that strong corporate culture and environmental performance improve firms’ financial performance. Our results are robust to several sensitivity tests and procedures to mitigate endogeneity and self-selection problems. From a practical point of view, our findings suggest that a firm’s culture can determine its environmental sustainability and ethical practices. (shrink)

Defeasible conditionals are statements of the form ‘if A then normally B’. One plausible interpretation introduced in nonmonotonic reasoning dictates that ) is true iff B is true in ‘most’ A-worlds. In this paper, we investigate defeasible conditionals constructed upon a notion of ‘overwhelming majority’, defined as ‘truth in a cofinite subset of \’, the first infinite ordinal. One approach employs the modal logic of the frame \\), used in the temporal logic of discrete linear time. We introduce and investigate (...) conditionals, defined modally over \\); several modal definitions of the conditional connective are examined, with an emphasis on the nonmonotonic ones. An alternative interpretation of ‘majority’ as sets cofinal ) rather than cofinite ) is examined. For these modal approaches over \\), a decision procedure readily emerges, as the modal logic \ of this frame is well-known and a translation of the conditional sentences can be mechanically checked for validity; this allows also for a quick proof of \-completeness of the satisfiability problem for these logics. A second approach employs the conditional version of Scott-Montague semantics, in the form of \-many possible worlds, endowed with neighborhoods populated by collections of cofinite subsets of \. This approach gives rise to weak conditional logics, as expected. The relative strength of the conditionals introduced is compared to KLM logics and other conditional logics in the literature. Weak filters were introduced by K. Schlechta in the ’90s with the aim of interpreting defaults via a generalized ‘most’ quantifier in first-order logic. They arguably represent the largest class of structures that qualify as a ‘collection of large subsets’ of a given index set |$I$|⁠, in the sense that it is difficult to think of a weaker, but still plausible, definition of the concept. The notion of weak ultrafilter naturally emerges and has been used in epistemic logic and other (...) knowledge representation (KR) applications. We provide a comprehensive exposition of weak filters and ultrafilters, comparing them with their classical counterparts that have found very important applications in logic, set theory and topology. Weak (ultra)filters capture the ‘majorities’ of social choice theory (which coincide with the commonsense understanding of a ‘large subset’) and in that respect, they outperform classical (ultra)filters in their role as ‘collections of large subsets’. Yet, they lack some of the elegant properties of classical (ultra)filters that make them an almost perfect match for logical theories. We investigate the extent to which some important classical results carry through in this new setting, and we focus on genuinely weak filters and genuinely weak ultrafilters. For weak ultrafilters, we proceed to provide concrete examples, answer questions of existence and characterize their construction. The class of weak (ultra)filters represents a genuine contribution of KR to set theory that might be of some interest to set theorists too and we initiate this study by providing a glimpse on natural set-theoretic questions.
