Background While it is generally acknowledged that self-prescribing among physicians poses some risk, research finds such behaviour to be common and in certain cases accepted by the medical community. Largely absent from the literature is knowledge about other activities doctors perform for their own medical care or for the informal treatment of family and friends. This study examined the variety, frequency and association of behaviours doctors report providing informally. Informal care included prescriptions, as well as any other type of personal (...) medical treatment (eg, monitoring chronic or serious conditions). Method A survey was sent to 2500 randomly-selected physicians in Colorado, 600 individuals returned questionnaires with usable data. The authors hypothesised: (1) physicians would prescribe the same types of treatment at home as they prescribed professionally; and (2) physicians who informally prescribed addictive medications would be more likely to engage in other types of informal medical care. Results Physicians who wrote prescriptions for antibiotics, psychotropics and opioids at work were more likely to prescribe these medications at home. Those prescribing addictive drugs outside of the office treated more serious illnesses in emergency situations, more chronic conditions and more major medical/surgical conditions informally than did those not routinely prescribing addictive medications. Physicians reported a variety of informal care behaviour and high frequency of informal care to family and friends. Discussion The frequency and variety of informal care reported in this study strongly argues for profession-wide discussion about ethical and guideline considerations for such behaviour. Sluggish cognitive tempo is a cluster of attentional symptoms characterized by slow information processing and behavior, distractibility, mental confusion, absent-mindedness, and hypoactivity. The present study aimed to compare early and late selective attention in the information processing speed of adults with SCT to those with attention-deficit/hyperactivity disorder and adults without any attentional problems. The participants were screened using Barkley Adult ADHD Rating Scale-IV and divided into the following groups: SCT, ADHD, and controls. All participants completed the irrelevant distractor task (...) measuring early and late selective attention under load condition and distractor condition. The inefficiency index was calculated by subtracting the reaction time of no-distractor condition of correct trials from the reaction time of distractor condition to control the impact of accuracy. Upon analysis, the SCT group showed a lower efficiency compared to the ADHD group under high load, while the ADHD group showed lower efficiency under low load than high load. This meant that the ADHD group had increased efficiency of selective attention with higher load, while the SCT group had low efficiency of selective attention even under high loads. These results suggest that the symptoms of “slow” or “distracted” in SCT could be attributed to the reduced speed and efficiency of selective attention in early information processing and the problem can be pronounced in situations with distractors. The results of the study imply that the attention-deficit-like symptoms shown in those with SCT and ADHD can be distinguished in specific stage of information processing. (shrink)

The discovery, made in Oxford, that crude penicillin could cure systemic and life‐threatening bacterial infections was followed by attempts to purify penicillin, to determine its structure and then to produce it by total chemical synthesis. The β‐lactam structure of the molecule, first proposed in October 1943, was a source of controversy until 1945. However, no useful chemical synthesis was achieved and fermentation became the commercial source of the antibiotic.In 1953, one of the products of a Cephalosporium sp. from Sardinia was (...) shown to be a new and hydrophilic penicillin (penicillin N). This was contaminated with a substance having the same side‐chain but a characteristic absorption spectrum. The latter, cephalosporin C, showed antibacterial activity but was not inactivated by a penicillinase. The determination of its β‐lactam structure and isolation of its nucleus enabled pharmaceutical companies to produce many semisynthetic cephalosporins.A new tripeptide was later found to be an intermediate in the biosynthesis of both penicillin N and cephalosporin C, and this was followed by the complete elucidation of the biosynthetic pathways leading to these compounds and to benzylpenicillin. Often translated simply as "logic," the Sanskrit word _nyāya_ means "rule of reasoning" or "method of reasoning." Texts from the school of classical Indian philosophy that bears this name are concerned with cognition, reasoning, and the norms that govern rational debate. This translation of selections from the early school of Nyāya focuses on its foundational text, the _Nyāya-sūtra_, with excerpts from the early commentaries. The improbability of a spontaneously generated self-assembling molecule has suggested that life began with a set of simpler, collectively replicating elements, such as an enclosed autocatalytic set of polymers (or autocell). Since replication occurs without a self-assembly code, acquired characteristics are inherited. Moreover, there is no strict distinction between alive and dead; one can only infer that an autocell was alive if it replicates. These features of early life render natural selection inapplicable to the description of its change-of-state (...) because they defy its underlying assumptions. Moreover, natural selection describes only randomly generated novelty; it cannot describe the emergence of form at the interface between organism and environment. Self-organization is also inadequate because it is restricted to interactions amongst parts; it too cannot account for context-driven change. A modified version of selection theory or self-organization would not work because the description of change-of-state through interaction with an incompletely specified context has a completely different mathematical structure, i.e. entails a non-Kolmogorovian probability model. It is proposed that the evolution of early life is appropriately described as lineage transformation through context-driven actualization of potential, with self-organized change-of-state being a special case of no contextual influence, and competitive exclusion of less fit individuals through a selection-like process possibly (but not necessarily) playing a secondary role. It is argued that natural selection played an important role in evolution only after genetically mediated replication was established. (shrink)

: In this article I discuss Athanasios Raftopoulos’ view on the epistemic role of attention and early vision, as outlined in his most recent book. I start by examining his view on attention, which he illustrates during his discussion of structured cognitive contents and their interactions with perceptual contents, as well as during his discussion of selection effects. According to Raftopoulos, attention not only operates pre-perceptual input selection, but also influences perceptual processing during late vision biasing the (...) sampling of the iconic image for perceptual hypotheses-testing. Afterwards, I critically assess Raftopoulos’ conclusions about the epistemic role of early vision, which are based on his view about the role of attention. From this assessment, I raise a potential concern for his proposal in the form of a new problem of selection: the interesting epistemic consequences of cognitive penetrability either follow or do not follow from selection effects, depending on how these selection effects are construed, but regardless of the stage of visual processing in which they take place. La mia discussione comincia da un esame delle sue idee riguardo all’attenzione, che illustra sia durante la sua discussione dei contenuti cognitivi strutturati e delle loro interazioni con i contenuti percettivi, sia durante la sua discussione degli effetti di selezione. Secondo Raftopoulos, l’attenzione non opera solamente una selezione pre-percettiva degli input, ma influenza anche i processi percettivi di late vision orientando il processo di valutazione delle ipotesi percettive sulla base dell’immagine iconica. Successivamente, valuto criticamente le conclusioni di Raftopoulos’ riguardo al ruolo epistemico di early vision, fondate sulla sua concezione del ruolo dell’attenzione. Da questo esame, evidenzio una potenziale criticità nella sua proposta costituita da un nuovo problema della selezione: le conseguenze epistemiche della penetrabilità cognitiva seguono o non seguono dai processi di selezione, a seconda di come tali effetti sono concepiti, ma indipendentemente dal livello nella serie dei processi visivi nel quale si verificano. Recently, much philosophical discussion has centered on the best way to characterize the concepts of random drift and natural selection, and, in particular, whether selection and drift can be conceptually distinguished (Beatty, 1984; Brandon, 2005; Hodge, 1983, 1987; Millstein, 2002, 2005; Pfeifer, 2005; Shanahan, 1992; Stephens, 2004). These authors all contend, to a greater or lesser degree, that their concepts make sense of biological practice. So it should be instructive to see how the concepts of drift and (...) class='Hi'>selection were distinguished by the disputants in a high-profile debate; debates such as these often force biologists to take a more philosophical turn, discussing the concepts at issue in greater detail than usual. Moreover, it is important to consider a debate where the disputants are actually trying to apply the models of population genetics to natural populations; only then can their proper interpretations become fully apparent. (Indeed, I contend that some of the philosophical confusion has arisen because authors have considered only the models themselves, and not the phenomena that the models are attempting to represent). A prime candidate for just such a case study is what Provine (1986) has termed “The Great Snail Debate,” that is, the debates over the highly polymorphic land snails Cepaea nemoralis and C. hortensis in the 1950s and early 1960s. These studies represent one of the best, if not the best, of the early attempts to demonstrate drift in natural populations. (shrink)

In this collection of essays, philosopher Paul Russell addresses major figures and central topics of the history of early modern philosophy. Most of these essays are studies on the philosophy of David Hume, one of the great figures in the history of philosophy. One central theme, connecting many of the essays, concerns Hume's fundamental irreligious intentions. Russell argues that a proper appreciation of the significance of Hume's irreligious concerns, which runs through his whole philosophy, serves to discredit the deeply (...) entrenched framework for understanding Hume - and much of early modern philosophy - in terms of the idea of "British Empiricism". The liver is the human body’s mandatory organ, but detecting liver disease at an early stage is very difficult due to the hiddenness of symptoms. Liver diseases may cause loss of energy or weakness when some irregularities in the working of the liver get visible. Cancer is one of the most common diseases of the liver and also the most fatal of all. Uncontrolled growth of harmful cells is developed inside the liver. If diagnosed late, it may cause death. (...) Treatment of liver diseases at an early stage is, therefore, an important issue as is designing a model to diagnose early disease. Firstly, an appropriate feature should be identified which plays a more significant part in the detection of liver cancer at an early stage. Therefore, it is essential to extract some essential features from thousands of unwanted features. So, these features will be mined using data mining and soft computing techniques. These techniques give optimized results that will be helpful in disease diagnosis at an early stage. In these techniques, we use feature selection methods to reduce the dataset’s feature, which include Filter, Wrapper, and Embedded methods. Different Regression algorithms are then applied to these methods individually to evaluate the result. Regression algorithms include Linear Regression, Ridge Regression, LASSO Regression, Support Vector Regression, Decision Tree Regression, Multilayer Perceptron Regression, and Random Forest Regression. Based on the accuracy and error rates generated by these Regression algorithms, we have evaluated our results. The result shows that Random Forest Regression with the Wrapper Method from all the deployed Regression techniques is the best and gives the highest R2-Score of 0.8923 and lowest MSE of 0.0618. I note with interest the Controversy regarding a baby born free of an inherited predisposition to early onset Alzheimer’s disease through the use of preimplantation genetic diagnosis .1,2 As the medical geneticist for the PGD programme for single gene disorders in Melbourne, Australia, I have seen many couples who have considered PGD for a wide range of genetic conditions. My observation is that many couples look to PGD for “milder” conditions and adult onset conditions for which they are not (...) comfortable to have traditional prenatal diagnosis and termination of pregnancy.An example of this is that in the last 11 years our unit has undertaken 13 prenatal diagnoses for Huntington’s disease from nine couples, whereas in the two years that we have been offering it we have had six requests for PGD for Huntington’s disease and three couples …. This work is a translation of selected sutras of the Nyāya-sūtra, together with relevant extracts from three commentaries: Nyāya-sūtra-bhāṣya of Vātsyāyana; Nyāya-vārttika of Uddyotakara; and Nyāya-vārttika-tātparya-ṭīkā of Vācaspatimiśra. The translators' introduction gives a general overview of the Nyāya school, its overall aims, and its place within classical Indian philosophy. Each of the nine chapters covers a particular topic in the Nyāya scheme: knowledge sources, philosophical method, the Nyāya defense of metaphysical realism, the self, substance and causation, God, theory of meaning, (...) value theory, and debate. Archer provides seemingly compelling evidence for his claim that sexual selection explains sex differences in human aggression better than social role theory. I challenge Archer's interpretation of some of this evidence. I argue that the same evidence could be used to support the claim that what has been selected for is the ability to curb aggression and discuss implications for Archer's theory.
