Copyright © 2015 by the Society of Criti. Objective: Global lung stress varies considerably with low tidal volume ventilation for acute respiratory distress syndrome. High stress despite low tidal volumes may worsen lung injury and increase risk of death. No widely available parameter exists to assess global lung stress. We aimed to determine whether the volume delivered during a recruitment maneuver is inversely associated with lung stress and mortality in acute respiratory distress syndrome. Design: Substudy of an acute respiratory distress (...) syndrome clinical trial on esophageal pressure-guided positive end-expiratory pressure titration. Setting: U.S. academic medical center. Patients: Forty-two patients with acute respiratory distress syndrome in whom airflow, airway pressure, and esophageal pressure were recorded during the recruitment maneuver. Interventions: A single recruitment maneuver was performed before initiating protocol-directed ventilator management. Recruitment maneuvers consisted of a 30-second breath hold at 40 cm H2O airway pressure under heavy sedation or paralysis. VRM was calculated by integrating the flow-time waveform during the maneuver. End-inspiratory stress was defined as the transpulmonary pressure during end-inspiratory pause of a tidal breath and tidal stress as the transpulmonary pressure difference between end-inspiratory and end-expiratory pauses. Measurements and Main Results: VRM ranged between 7.4 and 34.7 mL/kg predicted body weight. Lower VRM predicted high end-inspiratory and tidal lung stress. After adjusting for Pao2/Fio2 and either driving pressure, tidal volume, or plateau pressure and positive end-expiratory pressure, VRM remained independently associated with both end-inspiratory and tidal stress. In unadjusted analysis, low VRM predicted increased risk of death. VRM remained significantly associated with mortality after adjusting for study arm. Conclusions: Low VRM independently predicts high lung stress and may predict risk of death in patients with acute respiratory distress syndrome. (shrink)

The study of metaphor is now firmly established as a central topic within cognitive science and the humanities. We marvel at the creative dexterity of gifted speakers and writers for their special talents in both thinking about certain ideas in new ways, and communicating these thoughts in vivid, poetic forms. Yet metaphors may not only be special communicative devices, but a fundamental part of everyday cognition in the form of 'conceptual metaphors'. An enormous body of empirical evidence from cognitive linguistics (...) and related disciplines has emerged detailing how conceptual metaphors underlie significant aspects of language, thought, cultural and expressive action. Despite its influence and popularity, there have been major criticisms of conceptual metaphor. This book offers an evaluation of the arguments and empirical evidence for and against conceptual metaphors, much of which scholars on both sides of the wars fail to properly acknowledge. (shrink)

According to the colored-brain thesis, sense data or qualia are real physical-chemical qualities, located inside the brain, possibly at a specific locus. Our hypothesis is that the seats of phenomenal consciousness have a structure and a materiality. According to the proposed view, a chromatic quale emerges when a certain pixel of the visual sensorium is fed with a certain pattern Σ of spikes; a change in this pattern quickly changes the color that is subjectively generated. How could one manage to (...) measure chromatic qualia? In principle, with nanoscopical techniques, one could capture all the patterns that fall on the sensorium, and transmit the information to other media. But this does not capture the qualia. However, if the patterns are made to fall on a tissue of the same kind, typically inside another person’s brain, this other person will have roughly the same subjective experience as the first person. The model is used to explore two different situations involving qualia inversion. The paper also explores Cartesian materialism, and the claim that phenomenal time and space are identical to a region of physical time and space. (shrink)

Traditional accounts stress certain features of theoretical objects such as their alleged imperceptibility, that are taken to raise epistemological difficulties. But these accounts do not show how theoretical objects, rightly understood, either differ in kind from more ordinary sorts of objects or make science possible. I sketch a new account that focuses on the underdetermination and similarity of theoretical objects, features closely connected to the explanatory roles they play, and construes them on an algebraic model.

The disagreement between John Duns Scotus and William of Ockham on whether the exterior act has intrinsic moral worth is a turning point for a new understanding of the relationship between the interior and the exterior act. Is someone who successfully commits murder as guilty as someone who fails in her attempt? Does the martyr merit more than someone who merely wills to undergo martyrdom but is denied the opportunity? In these cases, the completion of the act is the exterior (...) act, and the interior act is that part of the act which leads up to and includes the agent’s choice or command. Scotus synthesizes and develops an earlier tradition in which the exterior act has its own moral goodness or malice which is independent from that of the interior act. Ockham criticizes Scotus and this tradition, thus apparently becoming one of the first thinkers since Abelard to deny that the exterior act has any intrinsic moral value. Ockham redefines the exterior act as a purely natural act without intrinsic moral value and consequently traces the goodness or malice of an act entirely to an intrinsically moral act of the will. The question of which is the logic that underlies quantum physics does not have an absolute answer, but only in relation to a conventional choice of interpretation . Most of the interpretations that have been offered work within the framework of classical logic. In contrast to these, we examine the corpuscular interpretation which is assumed in the application of non-distributive logic . The experiment in which single photons pass through a Mach-Zehnder interferometer is examined, indicating the difficulty of employing a (...) realist corpuscular interpretation in this context. One way to save this interpretation would be to use non-distributive logic to analyze the experiment, but this is not satisfactory . However, the use of an alethic modal logic solves the problem, blocking the argument that put the aforementioned corpuscular interpretation into difficulty. In the discussion of the conceptual problems involved, we suggest that a stochastic corpuscular interpretation is well adapted to this logical description. The project of extending this modal logical approach to other experiments in quantum physics, and of providing a rigorous logical treatment, is left open. (shrink)

The distinctions between the different sense of "perfect" and "imperfect" virtue are essential for understanding Thomas’ view of the development of and connection between the virtues. In this article I set out a fairly traditional schema of the states of virtue and shown how they are found in Thomas’ own texts. An understanding of the distinction between imperfect and perfect acquired virtue is necessary in order to grasp the issue at stake in my previous article on the Augustianism of Thomas (...) Aquinas's Moral Theory: “The question is whether without grace someone can be good by the fact that he has acquired virtues that are perfect – that is, connected with the other virtues through prudence.”. In 1966 the Brazilian physicist Klaus Tausk (b. 1927) circulated a preprint from the International Centre for Theoretical Physics in Trieste, Italy, criticizing Adriana Daneri, Angelo Loinger, and Giovanni Maria Prosperi`s theory of 1962 on the measurement problem in quantum mechanics. A heated controversy ensued between two opposing camps within the orthodox interpretation of quantum theory, represented by Leon Rosenfeld and Eugene P. Wigner. The controversy went well beyond the strictly scientific issues, however, reflecting philosophical and political commitments within the (...) context of the Cold War, the relationship between science in developed and Third World countries, the importance of social skills, and personal idiosyncrasies. There are two competing approaches to sustainability in agriculture. One stresses a strict economic approach in which market forces should guide the activities of agricultural producers. The other advocates the need to balance economic with environmental and social objectives, even to the point of reducing profitability. The writings of the eighteenth century moral philosopher Adam Smith could bridge the debate. Smith certainly promoted profit-seeking, private property, and free market exchange consistent with the strict economic perspective. However, his writings are also (...) consistent with many aspects of sustainable agriculture. For example, Smith argued that people ought to exercise restraint in their pursuit of self-interest, and he believed in balancing economic with environmental and social considerations. If both sides of the debate more fully regard the work of Adam Smith, then proponents of the strict economic perspective might be more appreciative of the concerns raised within the sustainable agriculture community, while advocates of sustainability might be more effective in achieving the objective of a sustainable agriculture. An experimental realization of quantum computers is composed of two or more calcium ions trapped in a magnetic quadripole. Information is transferred to and read from the ions by means of structured lasers that interact with the ions’ vibration pattern, causing changes of energy distribution in their electronic structure. Departing from an initial state when the ions are cooled, the use of lasers modifies the internal state of one ion that is entangled with the others, then changing the collective states. (...) In such quantum computers, some of the physically possible electronic states are avoided or not taken into consideration, to force the system to work as a binary device. In this essay, we discuss the dynamics that the ions could spontaneously display and its possible implications for the principles of computational logics. (shrink)

This Element provides an account of Thomas Aquinas's moral philosophy that emphasizes the intrinsic connection between happiness and the human good, human virtue, and the precepts of practical reason. Human beings by nature have an end to which they are directed and concerning which they do not deliberate, namely happiness. Humans achieve this end by performing good human acts, which are produced by the intellect and the will, and perfected by the relevant virtues.
