Chronic obstructive pulmonary disease (COPD) is one of the most deadly and costly chronic diseases in the world characterized by many breathing problems. The management of COPD and the prevention of exacerbations are a priority goals to improve the quality of life in patients affected by this illness. In addition, it is also crucial to improve the patients’ adherence to care which, in turn, depends on their knowledge and understanding of some factors such as the prescribed medical treatment, changes in (...) dailylife, and the process of breathing. In turn, the adherence to care leads to greater autonomy for the patient who is thus able to better manage his illness. Here we presented the application of the Model IARA in patients affected by COPD in order to achieve their autonomy in illness management which, in turn, leads to a better quality of life. IARA is an intervention program which improve the awareness and knowledge of patients with respect to both the disease and symptoms through health education. Moreover, through IARA the patients are encouraged to become more actively involved in COPD care process, also regarding drug therapy adherence. Using St. George's Respiratory Questionnaire combined with qualitative analysis, we demonstrated that IARA could be considered a useful approach in COPD management. (shrink)

During the latter half of the thirteenth century there arose around Tuscany a strange and unprecedented poetry, erudite, abstract, and arrogantly intellectual. It sang beyond courtly conventions about the wonders of the rational universe whose complex secrets the new speculative sciences were eagerly systematizing. Appropriating the language of natural philosophy, Aristotelian psychology, and even theology, love poetry developed a new theoretical understanding of its enterprise which allowed it to redefine love as spiritualized search for knowledge. This intellectualization of erotic desire (...) culminates in the Florentine sitlnovisti, a handful of learned poets who turned love poetry into an eclectic philosophical affair. Guido Cavalcanti’s famous canzone “Donna me prega” was universally considered to be not only the technically most perfect canzone ever written but also a rigorous philosophical treatise. As much as in our own day, exegeses of the poem were forced into the arcana of Scholastic Aristotelianism in order to make sense of its abstract, psychologistic definition of love’s essence. While Cavalcanti lyricized an Averroistic logic of the unified intellect, his younger friend Dante was preparing to put all of medieval philosophy, theology, and science into terza rima. It was in this terza rima that medieval Paris found perhaps its most felicitous expression, for the Divine Comedy represents, among other things, a creative transfiguration of the critical discourses Paris was diffusing throughout Europe.What recalls that situation today is the way Paris again marks the center of critical thought, while in Italiy a new generation of poets has emerged that translates the lessons of contemporary philosophy into poetry. In this essay I plan to discuss some of the most radical or, by analogy, “stilnovistic” of these lyricists. For purposes of convenience I will refer to them as the “favorite malice” poets. The phrase comes from the title of an anthology of select contemporary Italian poetry, recently published in a bilingual English edition: The Favorite Malice: Ontology and Reference in Contemporary Italian Poetry.2 The title alludes to a passage of Friedrich Nietzsche: “It is my favorite malice and art that my silence has learned not to betray itself through silence.” These words from Thus Spoke Zarathrustra serves as the anthology’s epigraphs and signal the peculiar poetics that brings the poets together in one volume. They are not brought together as a “school” but as a loose convergence of individual practices. The most illustrious name among the group is that of Andrea Zanzotto , who belongs to an older generation but whom the other poets call their “youngest traveling companion.” The “older” companions include Nanni Cagnone, Luigi Ballerini, Raffaele Perrotta, and Angelo Lumelli. Robert P. Harrison is assistant professor of Italian at Stanford University. If affirmative action and other ethnicity-based social programs are justified, then J. Angelo Corlett believes it is important to come to an adequate understanding of the nature of ethnicity in general and ethnic group membership in particular. In Race, Racism, and Reparations, Corlett reconceptualizes traditional ideas of race in terms of ethnicity. As he makes clear, the answers to the questions "What is a Native American?" or "What is a Latino?" have important implications for public policy, especially for those (...) programs designed to address historic injustices and economic and social imbalances among different groups in our society. Having supplanted "race" with a well-defined concept of ethnicity, the author then analyzes the nature and function of racism. Corlett argues for a notion of racism that must encompass not only racist beliefs but also racist actions, omissions, and attempted actions. His aim is to craft a definition of racism that will prove useful in legal and public policy contexts. Corlett places special emphasis on the broad questions of whether reparations for ethnic groups are desirable and what forms those reparations should take: land, money, social programs? He addresses the need for differential affirmative action programs and reparations policies—the experiences of different ethnic groups vary greatly. Arguments for reparations to Native and African Americans are considered in light of a variety of objections that are or might be raised against them. We compute the effect of collapse models on neutrino oscillations. The effect of the collapse is to modify the evolution of the spatial part of the wave function and we will show that this indirectly amounts to a change on the flavor components. For the analysis we use the mass proportional CSL model, and perform the calculation to second order perturbation theory. As we will show, the CSL effect is very small—mainly due to the very small mass of neutrinos—and practically (...) undetectable. (shrink)

Collapse models predict the spontaneous collapse of the wave function, in order to avoid the emergence of macroscopic superpositions. In their mass-dependent formulation, they claim that the collapse of any system’s wave function depends on its mass. Neutral K, D, B mesons are oscillating systems that are given by Nature as superposition of two distinct mass eigenstates. Thus they are unique laboratory for testing collapse models that are sensitive to the mass. In this paper we derive—for the single mesons and (...) bipartite entangled mesons—the effect of the mass-proportional CSL (Continuous Spontaneous Localization) collapse model on the dynamics on neutral mesons. Chat Generative Pre-Trained Transformer (ChatGPT) has been a growing point of interest in medical education yet has not been assessed in the field of bioethics. This study evaluated the accuracy of ChatGPT-3.5 (April 2023 version) in answering text-based, multiple choice bioethics questions at the level of US third-year and fourth-year medical students. A total of 114 bioethical questions were identified from the widely utilised question banks UWorld and AMBOSS. Accuracy, bioethical categories, difficulty levels, specialty data, error analysis and character count (...) were analysed. We found that ChatGPT had an accuracy of 59.6%, with greater accuracy in topics surrounding death and patient–physician relationships and performed poorly on questions pertaining to informed consent. Of all the specialties, it performed best in paediatrics. Yet, certain specialties and bioethical categories were under-represented. Among the errors made, it tended towards content errors and application errors. There were no significant associations between character count and accuracy. Nevertheless, this investigation contributes to the ongoing dialogue on artificial intelligence’s (AI) role in healthcare and medical education, advocating for further research to fully understand AI systems’ capabilities and constraints in the nuanced field of medical bioethics. The COVID-19 pandemic has caused a record global crisis, particularly and extremely, for the service sectors. Due to extensive security measures, many service sector employees have to work remotely to maintain services. Drawing upon the conservation of resources theory, this research investigates the impact of servant leadership on the task performance of employees in virtual working environments during the COVID-19 crisis. Our theoretical model was tested using data collected from 335 individual employees in the education sector of Pakistan. SPSS version (...) 26.0 was applied to find the hypothesized relationship between the study variables. To find the indirect mediating effect, we applied Model 4; for moderation, we applied Model 1; and for the moderation and mediation effect, we applied Model 7 of the Process Macro model of Hayes. The results of the study revealed that servant leadership is positively related to task performance in a virtual environment during crises. Furthermore, psychological empowerment partially mediates the relationship between servant leadership and task performance. Perceived supervisor support positively moderates the relationship between servant leadership and task performance. Moreover, the indirect effect of servant leadership on task performance via psychological empowerment is moderated by perceived supervisor support. The results provided guidance to the educational sector on how to lead effectively in times of crisis when service sector employees work predominantly in virtual environments. The theoretical and practical implications of these findings are discussed. (shrink)

Lethal Autonomous Weapons (LAWs) are robotic weapons systems, primarily of value to the military, that could engage in offensive or defensive actions without human intervention. This paper assesses and engages the current arguments for and against the use of LAWs through the lens of achieving more ethical warfare. Specific interest is given particularly to ethical LAWs, which are artificially intelligent weapons systems that make decisions within the bounds of their ethics-based code. To ensure that a wide, but not exhaustive, survey (...) of the implications of employing such ethical devices to replace humans in warfare is taken into account, this paper will engage on matters related to current scholarship on the rejection or acceptance of LAWs—including contemporary technological shortcomings of LAWs to differentiate between targets and the behavioral and psychological volatility of humans—and current and proposed regulatory infrastructures for developing and using such devices. After careful consideration of these factors, this paper will conclude that only ethical LAWs should be used to replace human involvement in war, and, by extension of their consistent abilities, should remove humans from war until a more formidable discovery is made in conducting ethical warfare. Background:The understanding of factors that shape risk perception is crucial to modulate the perceived threat and, in turn, to promote optimal engagement in preventive actions.Methods:An on-line, cross-sectional, survey was conducted in Italy between May and July 2020 to investigate risk perception for COVID-19 and the adoption of preventive measures. A total of 964 volunteers participated in the study. Possible predictors of risk perception were identified through a hierarchical multiple linear regression analysis, including sociodemographic, epidemiological and, most of all, psychological factors. A path analysis was adopted to probe the possible mediating role of risk perception on the relationship between the independent variables considered and the adoption of preventive measures.Results:Focusing on the psychological predictors of risk perception, high levels of anxiety, an anxious attachment, and an external locus of control predicted higher perceived risk. Conversely, high levels of openness personality and of avoidant attachment predicted a lower perception of risk. In turn, the higher was the perceived risk the higher was the adoption of precautionary measures. Furthermore, psychological factors influenced the adoption of preventive behaviors both directly and indirectly through their effect on risk perception.Conclusions:Our findings might be taken into high consideration by stakeholders, who are responsible for promoting a truthful perception of risk and proper compliance with precautionary measures. Recent controversies about the level of replicability of behavioral research analyzed using statistical inference have cast interest in developing more efficient techniques for analyzing the results of psychological experiments. Here we claim that complementing the analytical workflow of psychological experiments with Machine Learning-based analysis will both maximize accuracy and minimize replicability issues. As compared to statistical inference, ML analysis of experimental data is model agnostic and primarily focused on prediction rather than inference. We also highlight some potential pitfalls resulting from (...) adoption of Machine Learning based experiment analysis. If not properly used it can lead to over-optimistic accuracy estimates similarly observed using statistical inference. Remedies to such pitfalls are also presented such and building model based on cross validation and the use of ensemble models. ML models are typically regarded as black boxes and we will discuss strategies aimed at rendering more transparent the predictions. (shrink)

About 35 years ago Wheeler introduced the motto “law without law” to highlight the possibility that Physics may be understood only following regularity principles and few relevant facts, rather than relying on a treatment in terms of fundamental theories. Such a proposal can be seen as part of a more general attempt summarized by the slogan “it from bit”, which privileges the information as the basic ingredient. Apparently it seems that it is possible to obtain, without the use of physical (...) laws, some important results in an easy way, for instance, the probability distribution of the canonical ensemble. In this paper we will present a general discussion on those ideas of Wheeler’s that originated the motto “law without law”. In particular we will show how the claimed simplicity is only apparent and it is rather easy to produce wrong results. We will show that it is possible to obtain some of the results treated by Wheeler in the realm of the statistical mechanics, using precise assumptions and nontrivial results of probability theory, mainly concerning ergodicity and limit theorems. Modus ponens (from A and “if A then C” infer C) is one of the most basic inference rules. The probabilistic modus ponens allows for managing uncertainty by transmitting assigned uncertainties from the premises to the conclusion (i.e., from P(A) and P(C|A) infer P(C)). In this paper, we generalize the probabilistic modus ponens by replacing A by the conditional event A|H. The resulting inference rule involves iterated conditionals (formalized by conditional random quantities) and propagates previsions from the premises to the (...) conclusion. Interestingly, the propagation rules for the lower and the upper bounds on the conclusion of the generalized probabilistic modus ponens coincide with the respective bounds on the conclusion for the (non-nested) probabilistic modus ponens. We discuss generalized pobabilistic models for which states not necessarily obey Kolmogorov's axioms of probability. We study the relationship between properties and probabilistic measures in this setting, and explore some possible interpretations of these measures.

Social interaction, especially for older people living alone is a challenge currently facing human-robot interaction (HRI). There has been little research on user preference towards HRI interfaces. In this paper, we took both objective observations and participants’ opinions into account in studying older users with a robot partner. The developed dual-modal robot interface offered older users options of speech or touch screen to perform tasks. Fifteen people aged from 70 to 89 years old, participated. We analyzed the spontaneous actions of (...) the participants, including their attentional activities and conversational activities, the temporal characteristics of these social behaviours, as well as questionnaires. It has been revealed that social engagement with the robot demonstrated by older people was no different from what might be expected towards a human partner. This study is an early attempt to reveal the social connections between human beings and a personal robot in real life. (shrink)

We critically analyse the point of view for which laws of nature are just a mean to compress data. Discussing some basic notions of dynamical systems and information theory, we show that the idea that the analysis of large amount of data by means of an algorithm of compression is equivalent to the knowledge one can have from scientific laws, is rather naive. In particular we discuss the subtle conceptual topic of the initial conditions of phenomena which are generally incompressible. (...) Starting from this point, we argue that laws of nature represent more than a pure compression of data, and that the availability of large amount of data, in general, is not particularly useful to understand the behaviour of complex phenomena. In this paper, we generalize the set-theoretic translation method for poly-modal logic introduced in [11] to extended modal logics. Instead of devising an ad-hoc translation for each logic, we develop a general framework within which a number of extended modal logics can be dealt with. We first extend the basic set-theoretic translation method to weak monadic second-order logic through a suitable change in the underlying set theory that connects up in interesting ways with constructibility; then, we show how to tailor (...) such a translation to work with specific cases of extended modal logics. Health technologies such as apps for digital contract tracing [DCT] played a crucial role in containing and combating infections during the COVID-19 pandemic. Their primary function was to prevent the spread of SARS-CoV-2 by consistently generating and disseminating information related to various events such as encounters, vaccinations or infections. While the functionality of DCT has been well researched, the necessity of transparency in the use of DCT and the consent to share sensitive information such as users’ health, vaccination and location (...) status remains unclear. On one hand, DCT enabled the continuous monitoring of various risk factors, including data-based calculations of infection probabilities. On the other hand, digital monitoring of health risks was closely associated with various uncertainties, such as the ambiguous storage of personal data and its potential future misuse, e.g., by tech companies or health authorities. Our contribution aims to retrospectively analyze the COVID-19 pandemic from a post-pandemic perspective and utilize it as a case study for the implementation of new technological measures. We argue that under the condition of voluntary use of DCT, transparency plays a key role in convincing individuals to install health technologies on their mobile devices, keep them activated and consent to the sharing of sensitive data. We support our argument with qualitative data from an expert survey conducted between 2020 and 2021 and analyzed according to the principles of Grounded Theory. (shrink)

_Background:_ Literature has documented the role of family in the outcome of chronic schizophrenia. In the light of this, family interventions (FIs) are becoming an integral component of treatment for psychosis. The First Episode of Psychosis (FEP) is the period when most of the changes in family atmosphere are observed; unfortunately, few studies on the relatives are available. _Objective:_ To explore burden of care and emotional distress at baseline and at 9-month follow-up and the levels of service satisfaction at follow-up (...) in the two groups of relatives (experimental treatment EXP vs. treatment as usual TAU) recruited in the cluster-randomized controlled GET UP PIANO trial. _Methods:_ The experimental treatment was provided by routine public Community Mental Health Centers (Italian National Health Service) and consisted of Treatment as Usual plus evidence-based additional treatment (Cognitive Behavioral Therapy for psychosis for patients, Family Intervention for psychosis, and Case Management). TAU consisted of personalized outpatient psychopharmacological treatment, combined with non-specific supportive clinical management and informal support/educational sessions for families. The outcomes on relatives were assessed by the Involvement Evaluation Questionnaire (IEQ-EU), the General Health Questionnaire (GHQ-12), and the Verona Service Satisfaction Scale (VSSS-EU). Differences within and between groups were evaluated. _Results:_ At baseline, 75 TAU and 185 EXP caregivers were assessed. In the experimental group 92% of relatives participated in at least 1 family session. At follow-up both groups experienced improvement in all IEQ and GHQ items, but caregivers belonging to the EXP arm experienced a significantly greater change in 10 IEQ items (mainly pertaining to the “Tension” dimension) and in GHQ items. Due to the low sample size, a significant effectiveness was only observed for 2 IEQ items and 1 GHQ-12 item. With respect to VSSS data at follow-up, caregivers in the EXP arm experienced significantly greater satisfaction in 8 items, almost all pertaining to the dimensions “Relatives' Involvement” and “Professionals' Skills and Behavior.” _Conclusions:_ The Family intervention for psychosis delivered in the GET UP PIANO trial reduced family burden of illness and improved emotional distress and satisfaction with services. These results should encourage to promote FIs on caregivers of first-episode psychosis patients. (shrink)

Research involving transgender and gender diverse people (TGD) increased in the last years, mostly concerning healthcare associated to this population. Few studies dedicated their analysis to the impact of parental support on transgender people, even though this is an important aspect in creating a safe environment on which these individuals can build their identity. In addition, the link between family support, TGD identity and homelessness is not completely established. Thus, due to the specificities of the family context of TGD individuals, (...) the aim of this study is to investigate the association between family support and TGD in different moments of the process of gender affirmation. In addition, this study also aims to explore the relationship between a lack of social support and low self-esteem, home abandonment, and dwelling in the street. The survey was designed based on the TransPULSE project and was made available in electronic format. The sample was constituted of 423 TGD residents in two Brazilian states. A Structural Equation Model analysis suggested that the impact of gender affirmation status on homelessness was mediated by parental support, through self-esteem, and the need to move from home. The association between the status of the gender affirmation procedures, family support and self-esteem was significant and indicated that the further TGD individuals advanced in gender affirmation, the more self-esteem and family support they would have. The association between family support and self-esteem indicated that family support was associated with higher self-esteem. Low family support was associated with the willingness to move from home due to one’s TGD status and there was also a significant correlation between low self-esteem and the willingness to move from home due to one’s TGD status. Finally, homelessness was associated with the willingness to move with a large effect size. Limitations include the sample that was constituted by individuals with Internet access and who had more contact with TGD communities. The findings indicate directions for interventions involving TGD people and their families, considering the parental relationship as a critical variable to improve TGD quality of life in the process of gender affirmation. Neuromodulatory effects of non-invasive brain stimulation (NIBS) have been extensively studied in chronic pain. A hypothetic mechanism of action would be to prevent or revert the ongoing maladaptive plasticity within the pain matrix. In this review, the authors discuss the mechanisms underlying the development of an abnormal plasticity in patients with chronic pain and the putative mechanisms of NIBS in modulating synaptic plasticity in neuropathic pain conditions.

Review of Lee Braver's 2012 book, Groundless Grounds: A Study of Wittgenstein and Heidegger.

It is well-established that toddlers can correctly select a novel referent from an ambiguous array in response to a novel label. There is also a growing consensus that robust word learning requires repeated label-object encounters. However, the effect of the context in which a novel object is encountered is less well-understood. We present two embodied neural network replications of recent empirical tasks, which demonstrated that the context in which a target object is encountered is fundamental to referent selection and word (...) learning. Our model offers an explicit account of the bottom-up associative and embodied mechanisms which could support children’s early word learning and emphasises the importance of viewing behaviour as the interaction of learning at multiple timescales.
