When analysing and evaluating discourse, the discourse itself, the speech event and the activity type it represents, forces the analyst to search for a theoretical and methodological framework which is suitable for analysing the activity exposed in the data. Interactive political argumentation demands both a theory of argumentation and a theory of spoken language to fully grasp what is going on in the discourse. The pragma-dialectical argumentation theory offers both analytical and evaluative tools, but rests upon a reconstruction of the (...) argumentative discourse which is hierarchical and static. If applied to interactive argumentation this kind of reconstruction will not reveal interactive aspects of the parties' argumentation. On the other hand, conversation analysis, which concentrates on interactive aspects of the discourse, does not offer a tool for the evaluation of the quality of each party's argumentation. (shrink)

This research explored the ethical issues that nurses reported in the process of elaboration and further disclosure after an initial diagnosis of a terminal illness had been given. One hundred and six hospice nurses in Norway and Denmark completed a questionnaire containing 45 items of forced-choice and open-ended questions. This questionnaire was tested and used in three countries prior to this study; for this research it was tested on Danish and Norwegian nurses. All respondents supported the ethics of ongoing disclosure (...) to terminally ill patients based on ethical principles embedded in their country’s Patients’ Rights Acts. Truth, as an intrinsic value, proved foundational to patient autonomy, the most frequent ethical principle these nurses reported to justify their ethical position on information disclosure to terminally ill people. Telling the truth about a diagnosis was not the end of ethics in hospice care, but rather the beginning because what occurs ethically in dealing with prognosis issues became central to these hospice nurses, the patients and their families. Coupled with truth-telling, compassionate interaction and care become extensions of patients’ rights. (shrink)

By classic results in social choice theory, any reasonable preferential voting method sometimes gives individuals an incentive to report an insincere preference. The extent to which different voting methods are more or less resistant to such strategic manipulation has become a key consideration for comparing voting methods. Here we measure resistance to manipulation by whether neural networks of varying sizes can learn to profitably manipulate a given voting method in expectation, given different types of limited information about how other voters (...) will vote. We trained over 70,000 neural networks of 26 sizes to manipulate against 8 different voting methods, under 6 types of limited information, in committee-sized elections with 5-21 voters and 3-6 candidates. We find that some voting methods, such as Borda, are highly manipulable by networks with limited information, while others, such as Instant Runoff, are not, despite being quite profitably manipulated by an ideal manipulator with full information. For the two probability models for elections that we use, the overall least manipulable of the 8 methods we study are Condorcet methods, namely Minimax and Split Cycle. (shrink)

Geprägt durch Diogenes von Sinope und Krates von Theben, seinen markantesten Vertretern, war der Kynismus zu einem wichtigen Bestandteil der griechischrömanischen Popularphilosophie geworden. The aim of this study was to gain a deeper understanding of the experience of time when living with severe incurable disease. A phenomenological and philosophical approach of description and deciphering were used. In our modern health care system there is an on-going focus on utilizing and recording the use of time, but less focus on the patient’s experience of time, which highlights the need to explore the patients’ experiences, particularly when life is vulnerable and time is limited. The empirical (...) data consisted of 26 open-ended interviews with 23 participants receiving palliative care at home, in hospital or in a nursing home in Norway. The theoretical frameworks used are mainly based upon K. Martinsens philosophy of care, K. E. Løgstrup phenomenological philosophy, in addition to C. Saunders’ hospice philosophy, L. Feigenberg’s thanatology and U. Qvarnström’s research exploring patient’s reactions to impending death. Experience of time is described as being a movement that moves the individual towards death in the field of opposites, and deciphered to be a universal, but a typical and unique experience emerging through three integrated levels: Sense of time; where time is described as a movement that is proceeding at varying speeds. Relate totime; where the awareness of limited life changes the understanding of time to be more existential. Being in time; where limited time seems to clarify the basic living conditions and phenomena of life. The existence of life when the prospect of death is present is characterized by emotional swings that move within polarizing dimensions which is reflected in the experience of time illustrated as the moves of the pendulum in a grandfather clock. The diversity of the experience of time is oscillating between going fast or slow, being busy or calm, being unpredictable but predictable, safe or unsafe and between being good or bad, depending on the embodied situation of the individual. BackgroundUpper limb impairment is common after stroke, and many will not regain full upper limb function. Different technologies based on surface electromyography (sEMG) have been used in stroke rehabilitation, but there is no collated evidence on the different sEMG-driven interventions and their effect on upper limb function in people with stroke.AimSynthesize existing evidence and perform a meta-analysis on the effect of different types of sEMG-driven interventions on upper limb function in people with stroke.MethodsPubMed, SCOPUS, and PEDro databases were systematically searched (...) for eligible randomized clinical trials that utilize sEMG-driven interventions to improve upper limb function assessed by Fugl-Meyer Assessment (FMA-UE) in stroke. The PEDro scale was used to evaluate the methodological quality and the risk of bias of the included studies. In addition, a meta-analysis utilizing a random effect model was performed for studies comparing sEMG interventions to non-sEMG interventions and for studies comparing different sEMG interventions protocols.ResultsTwenty-four studies comprising 808 participants were included in this review. The methodological quality was good to fair. The meta-analysis showed no differences in the total effect, assessed by total FMA-UE score, comparing sEMG interventions to non-sEMG interventions (14 studies, 509 participants, SMD 0.14, P 0.37, 95% CI –0.18 to 0.46, I255%). Similarly, no difference in the overall effect was found for the meta-analysis comparing different types of sEMG interventions (7 studies, 213 participants, SMD 0.42, P 0.23, 95% CI –0.34 to 1.18, I273%). Twenty out of the twenty-four studies, including participants with varying impairment levels at all stages of stroke recovery, reported statistically significant improvements in upper limb function at post-sEMG intervention compared to baseline.ConclusionThis review and meta-analysis could not discern the effect of sEMG in comparison to a non-sEMG intervention or the most effective type of sEMG intervention for improving upper limb function in stroke populations. Current evidence suggests that sEMG is a promising tool to further improve functional recovery, but randomized clinical trials with larger sample sizes are needed to verify whether the effect on upper extremity function of a specific sEMG intervention is superior compared to other non-sEMG or other type of sEMG interventions. The word ‘empowerment’ has become a popular term, widely used as an important claim, also within the health services. In this paper the concept's philosophical roots are traced from Freire and his ‘Pedagogy of the Oppressed’ to the philosophical thoughts of Hegel, Habermas, and Sartre. An understanding of the concept, as a way to facilitate coping and well‐being in patients through reflection and dialogue, emerges. Within an empowerment strategy the important claim on the nurse and the patient will be to (...) reveal the patient's own resources and limitations in times with sickness and reduced functionality to promote the patient's choice to act and cope. From this point of view an education‐programme for the frail elderly is outlined. If the nurse wants to empower the elderly patient she has to be willing to be educated through the dialogue with the patient, and to look for the patient's own meaning of being frail and elderly. The coping and self‐care solutions for the patient may then even be different from the preferences of the nurse, and this does not mean that the empowerment strategy is a failure or that the patient then has to continue without the assistance from the nurse. Within an empowerment strategy, in the Freirerian sense, the important thing is that both the patient and the nurse together critically reflect on the meanings of the sickness so that the patient can be able to make his own conscious choices. Dabei wird sichtbar, dass es neben der Intuition, dem Gedächtnis und dem *élan vital* vor allem die Dauer ist, die begrifflich im Mittelpunkt von Bergsons Philosophie steht. In the spirit of ancient Aristotle and Plato, Continental pedagogues and philosophers have for centuries explored the meaning of pedagogical practice/praxis and of the pedagogical good, the quality of both being regarded not as a means to an educational end, but (...) as the end itself. But what, indeed, is the pedagogical good, and what is the significance of the pedagogical good for students? Somehow we know the good, and yet we know it not. We recognize the good experientially, but the real meaning of what we intuit eludes our grasp. So how do we explore this elusive pedagogical quality - and is it possible to explore it? Based on phenomenological interviews with both young students and adults recalling episodes from school, as well as artistic narratives, this paper aims to illuminate experiential aspects of the pedagogical good and to reflect on the significance of the good in terms of pedagogical relational practice. It is suggested that the pedagogical good is not a quality that we as teachers can possess, or do, or practise, but rather a relational force beyond our pedagogical practice that opens up the world to children and preconditions the pedagogical relation. Brillant geschriebenes und nobelpreisgekröntes Hauptwerk Bergsons, das weit über die Lebensphilosophie hinaus auf die Literatur und Ästhetik der Folgezeit wirkte und auf Autoren wie Proust, Gide, T. S. Eliot und Musil großen Einfluß ausübte. Mit seinem epochemachenden Hauptwerk "L'évolution créatrice", für das er 1927 den Nobelpreis für Literatur erhielt, greift Bergson unmittelbar in die Diskussion über zeitgenössische Evolutionstheorien ein, die die Biologie um die Jahrhundertwende beherrschten. Gegenüber mechanistischen Konzepten wie dem Neo-Darwinismus, die er - mit großer Sachkenntnis im Detail - (...) als unzureichend für das Verständnis der Komplexität evolutionärer Prozesse kritisiert, versucht Bergson für die Philosophie die Deutungshoheit über den Lebensbegriff zurückzugewinnen. Die Grundfrage des Werks lautet: Gibt es etwas, das alle Lebewesen in ihrer Entwicklung gemeinsam haben und das insofern ein bestimmendes Merkmal des Lebens selbst ist? Bergson findet die Antwort im titelgebenden Begriff der "schöpferischen Evolution". Nicht nur ist die Evolution als schöpferisch zu denken, sondern zugleich der schöpferische Akt als "evolutiv". Als treibende Kraft und Ursache aller Veränderungen macht Bergson dabei den ursprünglichen "élan vital" (Lebensimpuls) aus, der zu einem Zentralbegriff seiner Philosophie wurde. Die Kritik am Szientismus seiner Zeit führt Bergson darüber hinaus zu einem neuen Zugang zur Lebenswirklichkeit, den er in der (methodisch präzise bestimmten) "Intuition" entdeckt. So wird die Auseinandersetzung mit der Evolutionstheorie zugleich zu einer großangelegten Neubestimmung der philosophischen Methode. L'évolution créatrice wurde 1912 von Gertrud Kantorowicz zum ersten Mal ins Deutsche übertragen. Ihre in Sprache und Terminologie stark vom George-Kreis geprägte Übersetzung wurde für diese Neuausgabe vollständig überarbeitet und enthält jetzt auch die in der ersten deutschen Ausgabe fehlenden Anmerkungen und Nachweise Bergsons. In »Materie und Gedächtnis« , seinem Hauptwerk, erörtert der französische Philosoph und Nobelpreisträger Henri Bergson das Zusammenwirken von Körper und Geist in der freien Handlung. Auf höchstem gedanklichen Niveau, aber in bestechend einfacher Sprache geschrieben, zählt das Werk zu den wenigen herausragenden Grundtexten der Gegenwartsphilosophie, vergleichbar »in ihrer Zeit mit Berkeleys Principles oder Kants Kritik« .Nachdem Bergson im Essai sur les données immédiates de la conscience das Verhältnis von Freiheit und Determinismus auf eine neue Basis zu stellen versucht hat, wendet (...) er sich hier, in Auseinandersetzung mit der Psychologie und Hirnphysiologie seiner Zeit, dem alten philosophischen Problem des Verhältnisses von Geist und Materie zu. Dabei reduziert er ›Materie‹ weder – idealistisch – auf reine Vorstellungen noch nimmt sie – materialistisch – als deren dinglichen Grund an, sondern sieht sie als eine Gesamtheit von ›Bildern‹, wobei er den Bildbegriff auf halbem Wege zwischen Ding und Vorstellung ansetzt und damit vor die Trennung von Existenz und Erscheinung zurückgeht. Eine herausragende Bedeutung kommt dabei dem Gedächtnis zu: hier verstanden nicht als ›mechanisches‹ Registrieren aufgenommener Inhalte, sondern als ›reines‹, lebendiges Gedächtnis, das sich einer geistigen, in der Vergangenheit entstandenen Bilderwelt bedient und der Dauer korrespondiert. Das auf Dauer ausgerichtete Gedächtnis verweist auf eine Selbstständigkeit des Geistes, die allerdings qua Handlung wieder auf den Körper zurückwirkt. »Der Geist entnimmt der Materie die Wahrnehmungen, aus denen er seine Nahrung zieht, und gibt sie ihr zurück in Form von Bewegung, der er seine Freiheit aufgeprägt hat«. Esse estudo objetivou analisar a violência infantil no contexto brasileiro, correlacionando-a com fatores socioeconômicos, socioculturais, geracionais e de gênero, investigando o impacto dos mesmos na saúde das crianças. Realizou-se um estudo epidemiológico descritivo, realizado a partir de dados de artigos científicos e documentos oficiais sobre e violência infantil, além do banco de dados do Sistema de Informação de Agravos de Notificação (SINAN) e da plataforma SciELO e Google Acadêmico. Encontrou-se uma maior prevalência de violência física e negligência, maior índice de (...) violência sexual contra o sexo feminino e negligência no sexo masculino. Os principais agressores identificados foram o pai e a mãe. Acerca da etnia, crianças pardas foram as mais agredidas, bem como as de menor escolaridade. As consequências da violência infantil se estendem para além do momento da agressão, ao perturbar funções cognitivas e mesmo influenciar na gênese de transtornos psiquiátricos. (shrink)

Nanomaterials have attracted much interest in the medical field and related applications as their distinct properties in the nanorange enable new and improved diagnosis and therapies. Owing to these properties and their potential interactions with the human body and the environment, the impact of nanomaterials on humans and their potential toxicity have been regarded a very significant issue. Consequently, nanomaterials are the subject of a wide range of cutting-edge research efforts in the medical and related fields to thoroughly probe their (...) potential beneficial utilizations and their more negative effects. We posit that the lack of standardization in the field is a serious shortcoming as it has led to the establishment of methods and results that do not ensure sufficient consistency and thus in our view can possibly result in research outputs that are not as robust as they should be. The main aim of this article is to present how NanoDiaRA, a large FP7 European multidisciplinary project that seeks to investigate and develop nanotechnology-based diagnostic systems, has developed and implemented robust, standardized methods to support research practices involving the engineering and manipulation of nanomaterials. First, to contextualize this research, an overview of the measures defined by different regulatory bodies concerning nanosafety is presented. Although these authorities have been very active in the past several years, many questions remain unanswered in our view. Second, a number of national and international projects that attempted to ensure more reliable exchanges of methods and results are discussed. However, the frequent lack of publication of procedures and protocols in research can often be a hindrance for sharing those good practices. Subsequently, the efforts made through NanoDiaRA to introduce standardized methods and techniques to support the development and utilization of nanomaterials are discussed in depth. A series of semi-structured interviews were conducted with the partners of this project, and the interviews were analyzed thematically to highlight the determined efforts of the researchers to standardize their methods. Finally, some recommendations are made toward the setting up of well-defined methods to support the high-quality work of collaborative nanoparticle-based research and development projects and to enhance standardization processes.
