BackgroundInformed consent is a key element of ethical clinical research. Addicted population may be at risk for impaired consent capacity. However, very little research has focused on their comprehension of consent forms. The aim of this study is to assess the capacity of addicted individuals to provide consent to research.Methods53 subjects with DSM-5 diagnoses of a Substance Use Disorder and 50 non psychiatric comparison subjects participated in the survey from December 2014 to March 2015. This cross-sectional study was carried out (...) at a community-based Outpatient Treatment Center and at an urban-located Health Centre in Spain. A binary judgment of capacity/incapacity was made guided by the MacArthur Competence Assessment Tool for Clinical Research and a clinical interview. Demographics and clinical characteristics were assessed by cases notes and the Mini-Mental State Examination, the Global Assessment Functional Scale and the Clinical Global Impression Scale.ResultsNPCs performed the best on the MacCAT–CR, and patients with SUD had the worst performance, particularly on the Understanding and Appreciation subscales. 32.7 % SUD people lacked research-related decisional capacity. There were no statistically significant differences between the groups in terms of capacity to consent to research.ConclusionsThe findings of our study provide evidence that a large proportion of individuals with SUD had decisional capacity for consent to research. It is therefore inappropriate to draw conclusions about capacity to make research decisions on the basis of a SUD diagnosis. In the absence of advanced cognitive impairment, acute withdrawal or intoxication, we should assume that addicted persons possess decision-making capacity. Thus, the view that people with SUD would ipso facto lose decision-making power for research consent is flawed and stigmatizing. (shrink)

In humans, the intestine is the major reservoir of microbes. Although the intestinal microbial community exists in a state of homeostasis called eubiosis, environmental and genetics factors can lead to microbial perturbation or dysbiosis, a state associated with various pathologies including inflammatory bowel diseases (IBD) and colorectal cancer (CRC). Dysbiotic microbiota is thought to contribute to the initiation and progression of CRC. At the opposite end of the spectrum, two recently published studies inSciencereveal that the microbiota is essential for chemotherapeutic (...) drug efficacy, suggesting a beneficial microbial function in cancer management. The dichotomy between the beneficial and detrimental roles of the microbiota during cancer initiation, progression, and treatment emphasize the interwoven relationship between bacteria and cancer. Moreover, these findings suggest that the microbiota could be considered as a therapeutic target, not only at the level of cancer prevention, but also during management, i.e. by enhancing the efficacy of chemotherapeutics. This paper presents a human-robot closely collaborative solution to cooperatively perform surface treatment tasks such as polishing, grinding, finishing, deburring, etc. The proposed scheme is based on task priority and non-conventional sliding mode control. Furthermore, the proposal includes two force sensors attached to the manipulator end-effector and tool: one sensor is used to properly accomplish the surface treatment task, while the second one is used by the operator to guide the robot tool. The applicability and feasibility of the proposed collaborative (...) solution for robotic surface treatment are substantiated by experimental results using a redundant 7R manipulator: the Sawyer collaborative robot. El presente artículo propone la lectura del Informe sobre Ciegos como una forma de acceder, desde una perspectiva vivencial, a la complejidad del pensamiento existencial kierkegaardiano.

Neste artigo procuramos apresentar os elementos que possibilitam a constituição das identidades coletivas a partir de quatro conceitos fundamentais: a demanda, o significante vazio, a identificação e o gozo. Primeiro, revisitamos Freud trazendo a noção de identificação pensada em sua determinação política. Em seguida, buscamos justificar a especificidade da noção lacaniana de significante vazio, repensado por Ernesto Laclau enquanto um conceito político fundamental implicado na constituição e formação das identidades coletivas. Avançamos na lógica do significante e das demandas insatisfeitas (...) que se organizam basilarmente na composição das identidades coletivas a partir de casos específicos e formalizações. Com este trabalho, temos como objetivo contribuir com a reflexão e debate psicanalítico, filosófico e político acerca das identidades políticas, sociais, de gênero, de diversidade sexual, étnicas e culturais em suas mais variadas formas. En el presente trabajo se valora la impronta de la perspectiva deconstructiva del mal en/de/por la soberanía, en la obra de Jaques Derrida, para una crítica anti-idolátrica de la democracia. Parte del estudio crítico de dos tradiciones de fundamentación contemporánea de la democracia representativa. El examen de la propuesta de democracia deliberativa de Habermas y de la concepción de radicalización de la democracia de Ernesto Laclau, las cuales permiten evidenciar la reproducción del proceso de formalización de la democracia. Ambas (...) critican al paradigma del conflicto material de clases de origen marxista y responden con una comprensión comunicativa que parte de la centralidad del lenguaje y el discurso como práctica subjetiva consensual. A partir de la reducción del proceso subjetivo a la anterioridad preminente del discurso, la democracia deja de comprender las condiciones materiales de reproducción del sujeto corporal. Marco ante el cual valoramos el papel de la crítica al malestar de la soberanía presente en la obra de Jaques Derrida. El pensamiento derridiano en torno al Malestar de Soberanía critica la reducción de la diferencia desde una constelación categorial y metódica que implica el reconocimiento del conflicto irreductible que representa la emergencia contingencial del otro. La espectralidad como intuición comprensiva permite la afirmación de la responsabilidad ética, como base de toda posición gnoseológica, que pretenda dar condición de posibilidad a un discurso verdadero. Posición que resguarda la posibilidad transformadora del ejercicio de la soberanía a partir de develar sus procesos de represión subjetiva. Esta constelación categorial es sustantiva a una crítica de la reducción de la democracia en tanto ejercicio soberano. (shrink)

Since Socrates, and through Descartes to the present day, the problems of self-knowledge have been central to philosophy's understanding of itself. Today the idea of ''first-person authority''--the claim of a distinctive relation each person has toward his or her own mental life--has been challenged from a number of directions, to the point where many doubt the person bears any distinctive relation to his or her own mental life, let alone a privileged one. In Authority and Estrangement, Richard Moran argues for (...) a reconception of the first-person and its claims. Indeed, he writes, a more thorough repudiation of the idea of privileged inner observation leads to a deeper appreciation of the systematic differences between self-knowledge and the knowledge of others, differences that are both irreducible and constitutive of the very concept and life of the person. Masterfully blending philosophy of mind and moral psychology, Moran develops a view of self-knowledge that concentrates on the self as agent rather than spectator. He argues that while each person does speak for his own thought and feeling with a distinctive authority, that very authority is tied just as much to the disprivileging of the first-person, to its specific possibilities of alienation. Drawing on certain themes from Wittgenstein, Sartre, and others, the book explores the extent to which what we say about ourselves is a matter of discovery or of creation, the difficulties and limitations in being ''objective'' toward ourselves, and the conflicting demands of realism about oneself and responsibility for oneself. Los argumentos tradicionales contrarios a la disponibilidad de la propia vida conservan alguna fuerza: en algunas sociedades, el intento de suicidio se considera todavía un delito y la colaboración —o complicidad— sigue siendo igualmente una conducta delictiva. No obstante, en los últimos años se ha llevado a cabo un creciente reconocimiento del llamado “derecho a una muerte digna”. De todos modos, ese derecho soporta ciertas condiciones que lo convierten, a mi entender, más en un eslogan pegadizo y engañoso que en (...) un derecho claramente consolidado y reconocido. En el presente artículo intentaré explicar diversos argumentos a favor y en contra de la libre disponibilidad de la propia vida formulados a lo largo de la historia del pensamiento occidental. Posteriormente, trataré de demostrar que la libre disposición de la propia vida encuentra demasiadas restricciones en España y que en algunas de ellas subyacen principios extraseculares impropios de una sociedad democrática, pluricultural y secular como la nuestra. This paper is concerned with the paradox of decrease. Its aim is to defend the answer to this puzzle that was propounded by its originator, namely, the Stoic philosopher Chrysippus. The main trouble with this answer to the paradox is that it has the seemingly problematic implication that a material thing could perish due merely to extrinsic change. It follows that in order to defend Chrysippus’ answer to the paradox, one has to explain how it could be that Theon is (...) destroyed by the amputation without changing intrinsically. In this paper, I shall answer this challenge by appealing to the broadly Aristotelian idea that at least some of the proper parts of a material substance are ontologically dependent on that substance. I will also appeal to this idea in order to offer a new solution to the structurally similar paradox of increase. In this way, we will end up with a unified solution to two structurally similar paradoxes. (shrink)

Requiring family authorization for apnea testing subtracts health professionals control over death determination, a procedure that has traditionally been considered a matter of clinical expertise alone. In this commentary, we first provide evidence showing that health professionals’ (HPs) disposition to act on death determination without family’s prior consent could be much lower than that referred to by Berkowitz and Garrett (2020). We hypothesize that HPs may have reservations about their own expertise as regards death, and may thus hesitate to impose (...) their views on patients’ families. We then address the theoretical question of clinical expertise in death determination by distinguishing judgments about facts (e.g., the presence or absence of spontaneous breathing) from interpretations given of these facts (i.e., their meaning for the vital status of an individual). We argue that, while clinicians may claim some expert authority on the former, they hold no particular authority on the latter. (shrink)

We do lots of things together in a shared manner. From the phenomenological point of view, does joint or shared agency need a conscious sense of shared agency? Yet there are many processes where we seem to just go along with the group without conscious intent. Building on the classic phenomenological accounts of Edmund Husserl, Alfred Schutz, Martin Heidegger (and the synthetic account of Berger & Luckmann), I want to emphasize the thick horizon of the life-world as a fundamental condition (...) for intentional shared agency. Joint agency has divergent forms with their own peculiar intentionality, attentivity, anticipations and expectations, and embeddedness in a pre-predicative tacit knowledge in the overall live-world. Phenomenology recognizes that even ego-centered activities that appear to be fully ‘agential’ can be carried out in an anonymous un-owned manner, in the manner which Heidegger calls ‘das Man’, or ‘the one’. This suggests that tacit belonging to the collective ‘we’ undergirds individual agency. Husserl, Heidegger, and Schutz all have accounts of this ‘anonymous’, pre-predicative kind of group participation. Phenomenology has rich accounts of anonymous, voluntary, shared, social participation that demand a new concept of agency, one neglected in the current literature in philosophy of action. (shrink)

Accountability is the bedrock upon which all lasting success is built. It is the necessary virtue for both individuals, and organizations, to realize their full potential. Accountability enables learning and growth, improves well-being, reduces stress, and drives results. But what if nearly everyone is wrong about the true nature of accountability? What if we have substituted something else it it's place, something that works to improve short-term results, but limits long term organizational health and success? What if the widespread management (...) approach of "holding others accountable" instead of creating accountability, encourages blame, limits results and growth, inhibits risk-taking, generates fear, fosters workplace conflict, and lowers over-all health and well-being? They show individuals how to take personal ownership, and thereby change their circumstances. One way in which the characteristic gestures of philosophy and criticism differ from each other lies in their involvements with disillusionment, with the undoing of our naivete, especially regarding what we take ourselves to know about the meaning of what we say. Philosophy will often find less than we thought was there, perhaps nothing at all, in what we say about the “external” world, or in our judgments of value, or in our ordinary psychological talk. The work of criticism, on (...) the other hand, frequently disillusions by finding disturbingly more in what is said than we precritically thought was there. In our relation to the meaningfulness of what we say, there is a disillusionment of plentitude as well as of emptiness. And no doubt what is “less” for one discipline may be “more” of what someone else is looking for.In recent years, metaphor has attracted more than its share of both philosophical and critical attention, including philosophical denials of the obvious, as well as critical challenges to the obviousness of the ways we talk about metaphor. In this paper I discuss a problem of each sort and suggest a complex of relations between them. The particular denial of the obvious that I’m interested in is the claim recently made by Donald Davidson that “a metaphor doesn’t say anything beyond its literal meaning ,” nor is it even correct to speak of metaphor as a form of communication.1 There’s disillusionment with a vengeance; and even if not strictly believable, it is still not without its therapeutic value, as we shall see. 1. A short critical response to Hannah Ginsborg’s book, The Normativity of Nature, in which I raise some questions about how to understand the idea that calling something beautiful is a form of praise of that thing.

The ordinary human concerns with the past and the future can be seen both as forms of suffering and as illusory because they involve the failure to appreciate the primary reality of the present. In this lecture I argue that while there are certainly ways of being occupied with past or future times that we have reason to criticize, such criticism cannot base itself on any metaphysical claim to the singular or exclusive reality of the present. The task of developing (...) useful forms of describing and assessing the different ways we can go wrong in temporalizing our lives is hindered rather than helped by the suggestion that our concerns with the past and with the future are as such forms of attachment to the Unreal. Its antithesis, the anti-hero, has more virtue that defects, no evil but he does not care on the means to achieve his goals. In the eyes of everyone involved in sports competition, these characters captivate and at the same time, create conflicts of ethics and aesthetics. (...) The purpose of this paper is to perform an ethical reflection linked to principles that contribute for the human growth and accomplishment, as well as the aesthetic on the perception of the sensitive, reverberated by sensations and feelings emerging from athletes.
