BackgroundAlthough patient advocates have developed templates for standard consent forms, evaluating patient preferences for first in human (FIH) and window of opportunity (Window) trial consent forms is critical due to their unique risks. FIH trials are the initial use of a novel compound in study participants. In contrast, Window trials give an investigational agent over a fixed duration to treatment naïve patients in the time between diagnosis and standard of care (SOC) surgery. Our goal was to determine the patient-preferred presentation (...) of important information in consent forms for these trials.MethodsThe study consisted of two phases: (1) analyses of oncology FIH and Window consents; (2) interviews of trial participants. FIH consent forms were analyzed for the location(s) of information stating that the study drug has not been tested in humans (FIH information); Window consents were analyzed for the location(s) of information stating the trial may delay SOC surgery (delay information). Participants were asked about their preferred placement of the information in their own trial’s consent form. The location of information in the consent forms was compared to the participants’ suggestions for placement. Results34 [17 FIH; 17 Window] of 42(81%) cancer patients approached participated. 25 consents [20 FIH; 5 Window] were analyzed. 19/20 FIH consent forms included FIH information, and 4/5 Window consent forms included delay information. 19/20(95%) FIH consent forms contained FIH information in the risks section 12/17(71%) patients preferred the same. Fourteen (82%) patients wanted FIH information in the purpose, but only 5(25%) consents mentioned it there. 9/17(53%) Window patients preferred delay information to be located early in the consent, before the “Risks” section. 3/5(60%) consents did this.ConclusionsDesigning consents that reflect patient preferences more accurately is essential for ethical informed consent; however, a one-size fits all approach will not accurately capture patient preferences. We found that preferences differed for FIH and Window trial consents, though for both, patients preferred key risk information early in the consent. Next steps include determining if FIH and Window consent templates improve understanding. (shrink)

Physical Relativity explores the nature of the distinction at the heart of Einstein's 1905 formulation of his special theory of relativity: that between kinematics and dynamics. Einstein himself became increasingly uncomfortable with this distinction, and with the limitations of what he called the 'principle theory' approach inspired by the logic of thermodynamics. A handful of physicists and philosophers have over the last century likewise expressed doubts about Einstein's treatment of the relativistic behaviour of rigid bodies and clocks in motion in (...) the kinematical part of his great paper, and suggested that the dynamical understanding of length contraction and time dilation intimated by the immediate precursors of Einstein is more fundamental. Harvey Brown both examines and extends these arguments, after giving a careful analysis of key features of the pre-history of relativity theory. He argues furthermore that the geometrization of the theory by Minkowski in 1908 brought illumination, but not a causal explanation of relativistic effects. Finally, Brown tries to show that the dynamical interpretation of special relativity defended in the book is consistent with the role this theory must play as a limiting case of Einstein's 1915 theory of gravity: the general theory of relativity.Appearing in the centennial year of Einstein's celebrated paper on special relativity, Physical Relativity is an unusual, critical examination of the way Einstein formulated his theory. It also examines in detail certain specific historical and conceptual issues that have long given rise to debate in both special and general relativity theory, such as the conventionality of simultaneity, the principle of general covariance, and the consistency or otherwise of the special theory with quantum mechanics. In many research studies, tumor biopsies are an unavoidable requirement for achieving key scientific aims. Yet some commentators view mandatory research biopsies as coercive and suggest they should be optional, or at least optional until further data are obtained regarding their scientific usefulness. Further complicating the ethical picture is the fact that some research biopsies offer a potential for clinical benefit to trial participants. We interviewed and surveyed a convenience sample of participants in phase I clinical trials at a single (...) institution. Our primary aim was to describe phase I participants’ understanding of whether a research biopsy offered them the prospect of medical benefit. We also endeavored to describe participants’ views about biopsies—specifically, the benefits of biopsies, if any, and whether biopsies were acceptable, risky, or discouraged trial participation. Finally, we collected data on demographics and attitudes to see if any strong correlations with misunderstanding, acceptability, or riskiness existed. Overall, the respondents tended to view research biopsies as acceptable, though they did not succeed in identifying the lack of benefit of a research biopsy. These findings call for renewed efforts in consent conversations and documents to carefully describe the benefits, or lack thereof, of research biopsies. (shrink)

The influence of historiography on aspects of political thought in France, Italy and Germany. In recent years the overlap between political thought and historiography has changed the boundaries of intellectual history. Donald Kelley, the longtime editor of The Journal of the History of Ideas has played a leading part in this process. These essays by his friends and former students follow in his footsteps. We discuss what we take to be three possible misconceptions in the foundations of general relativity, relating to: the interpretation of the weak equivalence principle and the relationship between gravity and inertia; the connection between gravitational redshift results and spacetime curvature; and the Einstein equivalence principle and the ability to ``transform away" gravity in local inertial coordinate systems.

We review the dynamical approach to spacetime theories---in particular, its origins in the development of special relativity, its opposition to the contemporary `geometrical' approach, and the manner in which it plays out in general relativity. In addition, we demonstrate that the approach is compatible with the `angle bracket school'.

It is argued that Minkowski space-time cannot serve as the deep structure within a ``constructive'' version of the special theory of relativity, contrary to widespread opinion in the philosophical community.

It is argued that awareness of the distinction between dynamical and variational symmetries is crucial to understanding the significance of Noether's 1918 work. Specific attention is paid, by way of a number of striking examples, to Noether's first theorem, which establishes a correlation between dynamical symmetries and conservation principles.

The purpose of this paper is to evaluate the `Lorentzian Pedagogy' defended by J.S. Bell in his essay ``How to teach special relativity'', and to explore its consistency with Einstein's thinking from 1905 to 1952. Some remarks are also made in this context on Weyl's philosophy of relativity and his 1918 gauge theory. Finally, it is argued that the Lorentzian pedagogy---which stresses the important connection between kinematics and dynamics---clarifies the role of rods and clocks in general relativity.

This paper investigates what the source of time-asymmetry is in thermodynamics, and comments on the question whether a time-symmetric formulation of the Second Law is possible.

It has recently been suggested by Richard R. La Croix that there is a logical incompatibility between the doctrine of divine omniprescience — the notion that God knows what will happen in the future — and the commonly held belief that literary authors are creative with respect to the compositions they produce. This suggestion is, I take it, part of the overall claim that God's omniscience rules out human free choices, since if what one does is known before one does (...) it, then one can hardly be said to have chosen it. I will not, however, be concerned with this general controversy in the present paper, but rather with the specific one regarding literary creativity and divine omniprescience. La Croix defends this claim by defining literary creativity as follows: literary authorship is creative in the sense that by means of his composing activity an author is an agent who brings about the existence of some thing which did not exist prior to the composing activity of that agent and which would not exist without the composing activity of that agent or some similar agent. This study reports on the development of scale items derived from the pluralistic moral philosophy literature. In addition, the manner in which individuals combine aspects of the different philosophies in making ethical evaluations was explored.

In a recent article in Religious Studies , Gregory S. Kavka argues that John Hick was wrong when he said that the statement ‘God exists’ is verifiable but not falsifiable. Kavka constructs an imaginary `resurrection world' ruled by Satan and inhabited by such resurrected evildoers as Hitler and Stalin. In such a world, those who had been virtuous in earthly life in the hopes of a Christ-dominated resurrection world discover that virtue is inversely rewarded, with the ‘living’ intolerable for them (...) and worst of all for those among them who had lived especially virtuous lives, i.e. the saints. Further, they are told by presumably knowledgeable inhabitants of this world that: …the historical Christ was an agent sent by Satan to raise in good people false hopes of eternal salvation. This story is confirmed by Satan himself who demonstrates through a huge telescope-like device how he continues to send false saviours to other planets inhabited by intelligent beings in a cruel plan to make such beings miserable in the long run. Quantum field theory, one of the most rapidly developing areas of contemporary physics, is full of problems of great theoretical and philosophical interest. This collection of essays is the first systematic exploration of the nature and implications of quantum field theory. The contributors discuss quantum field theory from a wide variety of standpoints, exploring in detail its mathematical structure and metaphysical and methodological implications.

It is argued that an unheralded moment marking the beginnings of relativity theory occurred in 1889, when G. F. FitzGerald, no doubt with the puzzling 1887 Michelson-Morley experiment fresh in mind, wrote to Heaviside about the possible effects of motion on inter-molecular forces in bodies. Emphasis is placed on the difference between FitzGerald's and Lorentz's independent justifications of the shape distortion effect involved. Finally, the importance of the their `constructive' approach to kinematics---stripped of any commitment to the physicality of the (...) ether--- will be defended, in the spirit of Pauli, Swann and Bell. The conceptual model presented in this article argues that corporations exhibit specific behaviors that signal their true level of moral development. Accordingly, the authors identify five levels of moral development and discuss the dynamics that move corporations from one level to another. Examples of corporate behavior which are indicative of specific stages of moral development are offered.

The Cambridge Companion to Socrates is a collection of essays providing a comprehensive guide to Socrates, the most famous Greek philosopher. Because Socrates himself wrote nothing, our evidence comes from the writings of his friends (above all Plato), his enemies, and later writers. Socrates is thus a literary figure as well as a historical person. Both aspects of Socrates' legacy are covered in this volume. Socrates' character is full of paradox, and so are his philosophical views. These paradoxes have led (...) to deep differences in scholar's interpretation of Socrates and his thought. Mirroring this wide range of thought about Socrates, this volume's contributors are unusually diverse in their background and perspective. Modern insolubility proofs of the measurement problem in quantum mechanics not only differ in their complexity and degree of generality, but also reveal a lack of agreement concerning the fundamental question of what constitutes such a proof. A systematic reworking of the (incomplete) 1970 Fine theorem is presented, which is intended to go some way toward clarifying the issue.

Abstract The historical evolution of the principle of relativity from Galileo to Einstein is briefly traced, and purported difficulties with Einstein's formulation of the principle are examined and dismissed. This formulation is then compared to a precise version formulated recently in the geometrical language of spacetime theories. We claim that the recent version is both logically puzzling and fails to capture a crucial physical insight contained in the earlier formulations. The existence of a definite tangent space structure (metric with Lorentzian signature) in the general theory of relativity is the consequence of a fundamental assumption concerning the local validity of special relativity. There is then at the heart of Einstein's theory of gravity an absolute element which depends essentially on a common feature of all the non-gravitational interactions in the world, and which has nothing to do with space-time curvature. Tentative implications of this point for the significance of the vacuum (...) solutions in general relativity, and for the issue of quantising gravity, are briefly examined. (shrink)

The purpose of the paper is to explore different aspects of the covariance of non-relativistic quantum mechanics. First, doubts are expressed concerning the claim that gauge fields can be 'generated' by way of imposition of gauge covariance of the single-particle wave equation. Then a brief review is given of Galilean covariance in the general case of external fields, and the connection between Galilean boosts and gauge transformations. Under time-dependent translations the geometric phase associated with Schrödinger evolution is non-invariant, and the (...) significance of this result is briefly analysed. The covariance properties of Schrödinger dynamics are then brought to bear on certain versions of the modal interpretation of quantum mechanics. The conclusion that it is only relational properties that can be considered coordinate- or gauge-independent elements of reality is reinforced by appeal to the theory of quantum reference frames due to Aharonov and Kauffher. , Cambridge University Press ; pp. 45-70.). Clinical experience suggests that adult survivors of childhood trauma arrive at their memories in a number of ways, with varying degrees of associated distress and uncertainty and, in some cases, after memory lapses of varying duration and extent. Among those patients who enter psychotherapy as a result of early abuse, three general patterns of traumatic recall are identified: relatively continuous and complete recall of childhood abuse experiences coupled with changing interpretations of these experiences, partial amnesia for abuse events, accompanied by (...) a mixture of delayed recall and delayed understanding, and delayed recall following a period of profound and pervasive amnesia. These patterns are represented by three composite clinical vignettes. Variations among them suggest that the phenomena underlying traumatic recall are continuous not dichotomous. Future research into the nature of traumatic memory should be informed by clinical observation. (shrink)

Between 1964 and 1990, the notion of nonlocality in Bell's papers underwent a profound change as his nonlocality theorem gradually became detached from quantum mechanics, and referred to wider probabilistic theories involving correlations between separated beables. The proposition that standard quantum mechanics is itself nonlocal became divorced from the Bell theorem per se from 1976 on, although this important point is widely overlooked in the literature. In 1990, the year of his death, Bell would express serious misgivings about the mathematical (...) form of the local causality condition, and leave ill-defined the issue of the consistency between special relativity and violation of the Bell-type inequality. In our view, the significance of the Bell theorem, both in its deterministic and stochastic forms, can only be fully understood by taking into account the fact that a fully Lorentz-covariant version of quantum theory, free of action-at-a-distance, can be articulated in the Everett interpretation. In a comparison of the principles of special relativity and of quantum mechanics, the former theory is marked by its relative economy and apparent explanatory simplicity. A number of theorists have thus been led to search for a small number of postulates - essentially information theoretic in nature - that would play the role in quantum mechanics that the relativity principle and the light postulate jointly play in Einstein's 1905 special relativity theory. The purpose of the present paper is to (...) resist this idea, at least in so far as it is supposed to reveal the fundamental form of the theory. It is argued that the methodology of Einstein's 1905 theory represents a victory of pragmatism over explanatory depth; and that its adoption only made sense in the context of the chaotic state state of physics at the start of the 20th century - as Einstein well knew. The view that the initiation of branching into two sympatric species may not require natural selection emerged in Victorian times. In the 1980s paleontologist Stephen Jay Gould gave a theoretical underpinning of this nongenic “chromosomal” view, thus reinstating Richard Goldschmidt’s “heresy” of the 1930s. From modeling studies with computer-generated “biomorphs,” zoologist Richard Dawkins also affirmed Goldschmidt, proclaiming the “evolution of evolvability.” However, in the 1990s, while Gould and Dawkins were recanting, bioinformatic, biochemical, and cytological studies were providing a deeper underpinning. (...) In 2001 this came under attack from leaders in the field who favored Dawkins’ genic emphasis. Now, with growing evidence for the uncoupling of speciation from adaptation, we can reinstate again Goldschmidt’s view and clarify its 19th-century roots. (shrink)

One of the widespread confusions concerning the history of the 1887 Michelson-Morley experiment has to do with the initial explanation of this celebrated null result due independently to FitzGerald and Lorentz. In neither case was a strict, longitudinal length contraction hypothesis invoked, as is commonly supposed. Lorentz postulated, particularly in 1895, any one of a certain family of possible deformation effects for rigid bodies in motion, including purely transverse alteration, and expansion as well as contraction; FitzGerald may well have had (...) the same family in mind. A careful analysis of the Michelson-Morley experiment (which reveals a number of serious inadequacies in many text-book treatments) indeed shows that strict contraction is not required. (shrink)

Heisenberg'sgendanken experiments in quantum mechanics have given rise to a widespread belief that the indeterminacy relations holding for the variables of a quantal system can be explained quasiclassically in terms of a disturbance suffered by the system in interaction with a quantal measurement, or state preparation, agent. There are a number of criticisms of this doctrine in the literature, which are critically examined in this article and found to be ininconclusive, the chief error being the conflation of this disturbance with (...) the projection postulate. We present a critique of the disturbance theory based on the fact that the required disturbance will in general depend on the interaction time of the system and state-preparer. This point is exploited in the construction of a spin-interaction model which acts as a counterexample to the disturbance doctrine, while remaining faithful to the spirit of Heisenberg'sgedanken experiments. Several consequences of this result are discussed. (shrink)

From its first significant appearance in physics, the notion of probability has been linked in the minds of physicists with the notion of uncertainty. But the link may prove to be tenuous, if quantum mechanics, construed in terms of the Everett interpretation, is anything to go by.
