Background Paternalism/overprotection limits communication between healthcare professionals and patients and does not promote shared therapeutic decision-making. In the global north, communication patterns have been regulated to promote autonomy, whereas in the global south, they reflect the physician’s personal choices. The goal of this study was to contribute to knowledge on the communication patterns used in clinical practice in Mexico and to identify the determinants that favour a doctor–patient relationship characterized by low paternalism/autonomy. Methods A self-report study on communication patterns in (...) a sample of 761 mental healthcare professionals in Central and Western Mexico was conducted. Multiple ordinal logistic regression models were used to analyse paternalism and associated factors. Results A high prevalence of paternalism was observed among mental health professionals in Mexico. The main determinants of low paternalism/autonomy were medical specialty and gender, with female physicians being more likely to explicitly share diagnoses and therapeutic strategies with patients and their families. A pattern of highly explicit communication was strongly associated with low paternalism/autonomy. Finally, a modifying effect of age strata on the association between communication pattern or specialty and low paternalism/autonomy was observed. Conclusions Among mental health professionals in Mexico, high paternalism prevailed. Gender, specialty, and a pattern of open communication were closely associated with low paternalism/autonomy. Strengthening health professionals’ competencies and promoting explicit communication could contribute to the transition towards more autonomist communication in clinical practice in Mexico. The ethical implications will need to be resolved in the near future. (shrink)

Como habitantes e investigadores de las luchas que emergen desde situaciones de contaminación en la zona sur de la ciudad de Córdoba, nuestro interés es analizar los modos de construcción de las identidades políticas en estos contextos discursivos situados. Metodológicamente, apelamos a los desarrollos de Laclau para el análisis de las prácticas articulatorias, tomando como herramientas para el análisis del discurso las figuras de la retórica clásica (sinécdoque, catacresis, metáfora y metonimia); como unidad de análisis, cuatro experiencias de luchas desde (...) situaciones de contaminación; y como corpus, el material recogido en entrevistas grupales en profundidad a fin de identificar los significantes privilegiados por los participantes para dar cuenta del interior/exterior del grupo y las operaciones de construcción de cadenas de equivalencias/diferencias, observando las dimensiones catacrésicas, sinecdóquicas, metafóricas y metonímicas en el uso de los significantes. Luego, nos centramos en el análisis de las distinciones vecinos/no vecinos, contaminación/ambiente, barrio/sector sur, afectado/no afectado. Como resultados, señalamos dos modos de articulación diferentes en la construcción de identidades desde situaciones de injusticia ambiental: la de los afectados directos, basada en vínculos metonímicos, en contigüidades con la fuente de contaminación; y otra, la de quienes no se reconocen como afectados directos, basada en vínculos metafóricos. This article deals with a modern disease of academic science that consists of an enormous increase in the number of scientific publications without a corresponding advance of knowledge. Findings are sliced as thin as salami and submitted to different journals to produce more papers. If we consider academic papers as a kind of scientific ‘currency’ that is backed by gold bullion in the central bank of ‘true’ science, then we are witnessing an article-inflation phenomenon, a scientometric bubble that is most (...) harmful for science and promotes an unethical and antiscientific culture among researchers. The main problem behind the scenes is that the impact factor is used as a proxy for quality. Therefore, not only for convenience, but also based on ethical principles of scientific research, we adhere to the San Francisco Declaration on Research Assessment when it emphasizes “the need to eliminate the use of journal-based metrics in funding, appointment and promotion considerations; and the need to assess research on its own merits rather on the journal in which the research is published”. Our message is mainly addressed to the funding agencies and universities that award tenures or grants and manage research programmes, especially in developing countries. The message is also addressed to well-established scientists who have the power to change things when they participate in committees for grants and jobs. (shrink)

There are many ways in which a speaker can confuse their audience. In this paper, I will focus on one such way, namely, a way of talking that seems to manifest a cross-level kind of cognitive dissonance on the part of the speaker. The goal of the paper is to explain why such ways of talking sound so twisted. The explanation is two-pronged, since their twisted nature may come either from the very mental states that the speaker thereby makes manifest, (...) or from how the speaker chooses to express themselves (even if there is nothing wrong with their mental states). So-called ‘Moore-paradoxical’ utterances are but one example of the phenomenon, and the explanation of what is wrong about them is subsumed under a more general explanation here—one that captures also the twisted-ness of utterances whereby questions are raised or intentions expressed. Este texto, aprovechando la reciente publicación del último Premio Espasa de Ensayo, Lenguas en guerra, de Irene Lozano, hace un recorrido por las ideas más destacables del libro, tanto sobre el origen filogenético del lenguaje como sobre la problemática relación que este origen supone, según la autora, para las justificaciones al uso sobre los nacionalismos fundamentados en la singularidad de la lengua propia, y sitúa estas ideas en el contexto de los conocimientos que se tienen hoy sobre evolución, genética e (...) historia del pensamiento. El texto también hace una crítica a algunos de los razonamientos de Lozano y presta atención al contraste entre su perspectiva idealista sobre la funcionalidad y el uso recto del lenguaje y su visión más pragmática y posibilista sobre el nacionalismo y el conservacionismo lingüístico. Diferentes investigaciones han abordado el fenómeno de cómo el jugador de ajedrez alcanza un nivel alto de experticia; sin embargo, la gran mayoría se han llevado a cabo desde una perspectiva en tercera persona, es decir, sin tomar en cuenta la experiencia subjetiva del ajedrecista. El presente trabajo tiene como objetivos: 1) analizar la experticia de ajedrecistas de nivel proficiente desde una perspectiva fenomenológica, esto es, desde la experiencia subjetiva del jugador; 2) señalar los estados internos que parecen ser cognitivamente (...) relevantes en las acciones de los ajedrecistas; 3) teorizar sobre el eventual papel que juega la intuición en las acciones de los jugadores de ajedrez. Para ello se realizó un estudio exploratorio con cinco jugadores de ajedrez durante una competencia de corte nacional en México, utilizando como método la entrevista microfenomenológica propuesta por Claire Petitmengin. Los resultados indican que hay regularidades en los estados internos de los jugadores a nivel intra e interpersonal asociadas implícitamente a momentos críticos de las partidas. Lo anterior permite ofrecer una definición operacional de la intuición y afirmar que esta, en efecto, forma parte de los procesos cognitivamente relevantes de los ajedrecistas de nivel proficiente, aunque las acciones de estos aún dependen, en buena medida, de los cálculos explícitos que realizan durante los momentos críticos. To what extent does the loosening of the ties between a social democratic party and its sister unions indicate a shift in the party's traditional objectives or bases of support? In this article it is argued that the estrangement between these parties and their unions is not always an indicator or a proof of the abandonment of workers and solidaristic goals by social democratic parties. The evolution of the party-union relationship is not only shaped by the economic and social policies (...) of the party but is also constrained by the organizational context of the trade unions. And when a close relationship exists in a context of competitive unionism and a growing economy, it is highly likely that the pressure and threats that unions direct at their social democratic parties are largely inspired by their need to maintain and enhance their organizational position vis-à-vis their union rivals rather than by a change in the goals and policies of their sister parties themselves. In this paper I present reasons for us to accept the hypothesis that suspended judgment has correctness conditions, just like beliefs do. Roughly put, the idea is that suspended judgment about p is correct when both p and ¬p might be true in view of certain facts that characterize the subject’s situation. The reasons to accept that hypothesis are broadly theoretical ones: it adds unifying power to our epistemological theories, it delivers good and conservative consequences, and it allows us to (...) assess processes of reasoning involving attitudes of suspended judgment. Um alinhamento responsável à alguma versão do naturalismo filosófico requer a articulação explicita e cuidadosa de um argumento em sua defesa. Em quatro passos, o texto que segue abaixo expande e examina a validade de um argumento que é frequentemente rascunhado em favor do naturalismo. Como veremos, contudo, a versão do naturalismo que esse argumento nos permite é um pouco diferente dos naturalismos filosóficos mais populares.

What I call the Doxastic Puzzle, is the impression that while each of these claims seems true, at least one of them must be false: (a) Claims of the form ‘S ought to have doxastic attitude D towards p at t’ are sometimes true at t, (b) If Φ-ing at t is not within S’s effective control at t, then it is false, at t, that ‘S ought to Φ at t’, (c) For all S, p, and t, having doxastic (...) attitude D towards p at t is not within S’s effective control at t. All three natural replies to the puzzle have been pursued. Some have claimed that doxastic attitudes like believing that p are, in fact, within our effective control, or sufficiently so. Others have claimed that doxastic ought-claims, strictly speaking, are always false. And some have denied that effective control is required for the adequacy of doxastic ought-claims in general. I here pursue and examine a different strategy. In the first part of this paper, I argue that these claims are not only each true but actually not in tension with each other in the first place. Instead of attempting to dispel the puzzle, this solution proposes to evade it instead: to solve it by properly understanding, and by thereby accepting without contradiction, all of its constitutive claims. In the second part of the paper, I argue that the evasive strategy forces us to re-think our understanding of the place of normative reasons in epistemology. More exactly, it seems to come at the cost of one central way of thinking about our reasons for having doxastic attitudes, one where such reasons are good-standing exemplars of normative reasons in general. The evasive strategy, that is, threatens to lead us very quickly to a deflationary picture of epistemic normativity: it rescues normative talk, but sacrifices normative substance. I conclude by explaining why I think this is more consequential than some have made it out to be, and by suggesting that these consequences are welcome nonetheless. (shrink)

My starting point is what I call the Normative Authority Conception of justification, where S is justified in their belief that p at t (to some degree n) if and only if their believing that p at t is not ruled out by epistemic norms that have normative authority over S at t. With this in mind, this paper develops an account of doxastic justification by first developing an account of the normative authority of epistemic norms. Drawing from work in (...) political philosophy, I argue that (a) the cognitive and evaluative commitments and concerns behind our actual practices of holding each other and ourselves accountable for our beliefs reveal which epistemic norms we have consented to be under, and that (b) it is because we have consented to be under the authority of these norms – by actually holding ourselves and others accountable to them – that they in turn have normative authority over us. By connecting the authority of epistemic norms to the authority we have over ourselves in this way, the resulting account of doxastic justification (i) explains why it can be appropriate to criticize, resent, or sanction someone for having unjustified beliefs, (ii) avoids the phenomena of normative alienation and normative parochialism, and (iii) respects the social and collective nature of epistemic justification. “Literature and the Right to Death” is probably the most quoted of French thinker Maurice Blanchot’s texts. For decades it has been discussed by such important figures as Emmanuel Levinas, Jacques Derrida, or even the writer Paul Auster. Its importance is twofold: on the one hand, it is often considered an important gateway to Blanchot’s literary thought. On the other hand, it constitutes a substantial example of the new reception of Hegel in France after the Second World War. Although there (...) is no doubt about its Kojèvo-Hegelian inspiration and its center in what we could call “literary experience”, this article intends to explore another of its layers: its relation to the discussion around humanism and the Terror, as forgotten as it was central in France at the end of the 1940’s. We will thus try to lay the foundations for a new reading of this fundamental piece in Blanchot’s production. (shrink)

Since Freud and his co-author Breuer spoke of dissociation in 1895, a scientific paradigm was painstakingly established in the field of unconscious cognition. This is the dissociation paradigm. However, recent critical analysis of the many and various reported dissociations reveals their blurred, or unveridical, character. Moreover, we remain ignorant with respect to the ways cognitive phenomena transition from consciousness to an unconscious mode. This hinders us from filling in the puzzle of the unified mind. We conclude that we have reached (...) a Kuhnian crisis in the field of unconscious cognition, and we predict that new models, incorporating partly the relevant findings of the dissociation paradigm—but also of dynamic psychology—, will soon be established. We further predict that some of these models will be largely based on the pairs representation–process and analog–digital. (shrink)

Cosmopolitanism is said by many critics to be arrogant. In emphasizing universal principles and granting no fundamental moral significance to national or other group belonging, it wrongly treats those making non-universalist claims as not authorized to speak, while treating those in non-Western societies as not qualified. This book works to address such objections. It does so in part by engaging the work of B.R. Ambedkar, architect of India's 1950 Constitution and revered champion of the country's Dalits (formerly "untouchables"). Ambedkar cited (...) universal principles of equality and rights in confronting domestic exclusions and the "arrogance" of caste. He sought to advance forms of political humility, or the affirmation of equal standing within political institutions and openness to input and challenge within them. This book examines how an "institutional global citizenship" approach to cosmopolitanism could similarly advance political humility, in supporting the development of input and challenge mechanisms beyond the state. It employs a grounded normative theory method, taking insights for the model from field research among Dalit activists pressing for domestic reforms through the UN human rights regime, and from their critics in the Hindu nationalist Bharatiya Janata Party. Insights also are taken from Turkish protesters challenging a rising domestic authoritarianism, and from UK Independence Party members demanding "Brexit" from the European Union-in part because of possibilities that predominantly Muslim Turkey will join. Overall, it is shown, an appropriately configured institutional cosmopolitanism should orient fundamentally to political humility rather than arrogance, while holding significant potential for advancing global rights protections and more equitable rights specifications. Foundational ontologies, central constructs in ontological investigations and engineering alike, are based on ontological categories. Firstly proposed by Aristotle as the very ur- elements from which the whole of reality can be derived, they are not easy to identify, let alone partition and/or hierarchize; in particular, the question of their number poses serious challenges. The late medieval philosopher Dietrich of Freiberg wrote around 1286 a tutorial that can help us today with this exceedingly difficult task. In this paper, I discuss (...) ontological categories and their importance for foundational ontologies from both the contemporary perspective and the original Aristotelian viewpoint, I provide the translation from the Latin into English of Dietrich's De origine II with an introductory elaboration, and I extract a foundational ontology–that is in fact a single-category one–from this text rooted in Dietrich's specification of types of subjecthood and his conception of intentionality as causal operation. Ontologies are some of the most central constructs in today's large plethora of knowledge technologies, namely in the context of the semantic web. As their coinage indicates, they are direct heirs to the ontological investigations in the long Western philosophical tradition, but it is not easy to make bridges between them. Contemporary ontological commitments often take causality as a central aspect for the ur-segregation of entities, especially in scientific upper ontologies; theories of causality and philosophical ontological investigations often go hand-in-hand, (...) and were essentially inseparable in medieval thought. This constitutes the foundation for a bridge, and this article analyzes the causality-based ontology of the late medieval philosopher Dietrich of Freiberg from the viewpoint of today's upper-ontology engineering. In this bridging attempt, it offers a translation into English of the first part of Dietrich's De origine (abbreviated title) that is a compromise between traditional scholarly translations of medieval Latin philosophical texts and contemporary ontology.
