On August 29, 1995, the United States District Court for the Northern District of Ohio ruled that certain practices of Blue Cross and Blue Shield of Ohio relating to the calculation of copayments on insurance claims violated provisions of ERISA, and thus BCBSO could be liable for unpaid benefits and breach of fiduciary duty ). According to BCBSO's Explanation of Benefits and Schedule of Benefits, beneficiaries were responsible for a 20 percent copayment for hospital charges, and the remaining 80 percent (...) of the Provider's Reasonable Charges would be paid by BCBSO. BCBSO negotiated with the hospitals to create discount agreements whereby the actual charge to BCBSO was less than 80 percent of the total amount of the hospital's charges. By this arrangement, BCBSO paid less than 80 percent of what patients were billed; concurrently, patients paid more than 20 percent of the amount ultimately paid to hospitals for their services. Mikhail Bulgakov's novel The Master and Margarita was written between 1929 and 1940. Although delayed for a quarter of a century, it quickly found a stable place in our life as soon as it was published [for the first publication of the novel see: Moskva, 1966, no. 11; 1967, no. 1]. It is usually classified as a satirical philosophical novel. The satirical element puts it in the same family as such well-known works of the end of the '20s as the (...) novels of I. Il'f and E. Petrov, Twelve Chairs [Dvenadtsat' stul'ev] and The Golden Calf [Zolotoi telenok], but its emphatically philosophical orientation makes it all but a unique phenomenon in the history of Soviet literature. The novel's philosophical aspects have already been examined in a number of essays. Thus, for example, N. P. Utekhin analyzes the reflection of certain general philosophical categories in the novel. G. Chernikova and I. L. Galinskaia have endeavored to determine the concrete literary sources of the novel's philosophy [Chernikova 1971, pp. 213-219; Galinskaia 1986]. (shrink)

Medical confidentiality is a core concept of professionalism and should be an integral part of pregraduate and postgraduate medical education. The aim of our study was to define the factors influencing attitudes towards patient confidentiality in everyday situations in order to define the need for offering further education to various subgroups of physicians. All internists and general practitioners who were registered members of the association of physicians in Geneva or who were working in the department of internal medicine or in (...) the medical polyclinic of the University Hospital of Geneva in 2004 received a standardised questionnaire. Physicians were asked to indicate for seven vignettes whether a violation of confidentiality had occurred and whether the violation was not important, important or serious (scores 1–3; no violation = 0). 508 completed questionnaires were returned (participation rate 55%). Physicians who had worked in the hospital for more than 20 years identified violations of confidentiality more often than physicians with less hospital experience. Binary logistic regression showed that ethics education, total years of professional experience, being an internist, having a private practice, the length of working in private practice and gender were factors associated with correct identification of violations and their severity. However, each factor played a specific role only for single cases or a small number of situations (Cronbach α <0.6). Postgraduate education programs on confidentiality should be offered to a wide range of physicians and should address specific hypothetical situations in which there is a risk of avoidable breaches of confidentiality. The present work posits that social motives, particularly status seeking in the form of moral grandstanding, are likely at least partially to blame for elevated levels of affective polarization and ideological extremism in the U.S. In Study 1, results from both undergraduates (N = 981; Mean age = 19.4; SD = 2.1; 69.7% women) and a cross-section of U.S. adults matched to 2010 census norms (N = 1,063; Mean age = 48.20, SD = 16.38; 49.8% women) indicated that prestige-motived grandstanding (...) was consistently and robustly related to more extreme ideological views on a variety of issues. In Study 2, results from a weighted, nationally-representative cross-section of U.S. adults (N = 2,519; Mean age = 47.5, SD = 17.8; 51.4% women) found that prestige motivated grandstanding was reliably related to both ideological extremism and affective polarization. This research explores the contextual approach within Christian-Jewish religious education, addressing a notable gap in existing literature and offering fresh insights into the application of the Contextual Teaching and Learning (CTL) model within Christian contexts. Through a qualitative literature study employing a three-step methodology, including an in-depth analysis of Deuteronomy 11:19–20, this study reveals that this biblical text provides both educational guidance and theological significance, serving as a foundational support for the CTL model in Christian-Jewish religious education. The integration of (...) digital media into biblical sources holds promise for future research, while the central challenge lies in harmonising life experiences and empirical phenomena with biblical reflections to seamlessly integrate inquiry processes and pedagogical phases, with potential avenues for further exploration in scripture models endorsing CTL. This research provides valuable insights for educators, parents and policymakers in Indonesia and beyond, advancing educational research in contextualised teaching models within religious education.Contribution: This article demonstrates that the CTL model in the digital era has the potential to revolutionise religious education and profoundly impact learners, making it a significant scientific contribution in the context of Christian-Jewish religious education. Many authors suggest the need to define ‘sustainable development’in operational terms. When ethics committees are consulted about patients who have or need court-appointed guardians, they lack empirical evidence about several common issues, including the relationship between guardianship and prolonged, potentially medically unnecessary hospitalizations for patients. To provide information about this issue, we conducted quantitative and qualitative analyses using a retrospective cohort from Veterans Healthcare Administration. To examine the relationship between guardianship appointment and hospital length of stay, we first compared 116 persons hospitalized prior to guardianship appointment to a comparison group (n (...) = 348) 3:1 matched for age, diagnosis, date of admission, and comorbidity. We then compared 91 persons hospitalized in the year following guardianship appointment to a second matched comparison group (n = 273). Inappropriate authorship is a common problem in biomedical research and may be becoming one in bioethics, due to the increase in multiple authorship. This paper investigates the authorship policies of bioethics journals to determine whether they provide adequate guidance for researchers who submit articles for publication, which can help deter inappropriate authorship. It was found that 63.3% of bioethics journals provide no guidance on authorship; 36.7% provide guidance on which contributions merit authorship, 23.3% provide guidance on which contributions do not (...) merit authorship, 23.3% require authors to take responsibility for their contributions or for the article as a whole, 20% provide guidance on which contributions merit an acknowledgement but not authorship, 6.7% require authors to describe their contributions, and only 3.3% distinguish between authorship in empirical and conceptual research. To provide authors with effective guidance and promote integrity in bioethics research, bioethics journals should adopt authorship policies that address several important topics, such as the qualifications for authorship, describing authorship contributions, taking responsibility for the research and the difference between authorship in empirical and conceptual research. A superconducting transition temperature as high as 100 K was recently discovered in one monolayer FeSe grown on SrTiO3. The discovery ignited efforts to identify the mechanism for the markedly enhanced Tc from its bulk value of 8 K. There are two main views about the origin of the Tc enhancement: interfacial effects and/or excess electrons with strong electron correlation. Here, we report the observation of superconductivity below 20 K in surface electron-doped bulk FeSe. The doped surface layer possesses all (...) the key spectroscopic aspects of the monolayer FeSe on SrTiO3. Without interfacial effects, the surface layer state has a moderate Tc of 20 K with a smaller gap opening of 4.2 meV. Our results show that excess electrons with strong correlation cannot induce the maximum Tc, which in turn reveals the need for interfacial effects to achieve the highest Tc in one monolayer FeSe on SrTiO3. With a population of nearly 20 million inhabitants, Cameroon is a very diverse country, geographically, culturally, and linguistically.

The study of Borel equivalence relations under Borel reducibility has developed into an important area of descriptive set theory. The dichotomies of Silver [20] and Harrington, Kechris and Louveau [6] show that with respect to Borel reducibility, any Borel equivalence relation strictly above equality on ω is above equality on , the power set of ω, and any Borel equivalence relation strictly above equality on the reals is above equality modulo finite on . In this article we examine the effective (...) content of these and related results by studying effectively Borel equivalence relations under effectively Borel reducibility. The resulting structure is complex, even for equivalence relations with finitely many equivalence classes. However use of Kleene’s O as a parameter is sufficient to restore the picture from the noneffective setting. Nisbet and Kraggerud make good cases for taking the ninth Epode as a dramatic recreation of the Actium campaign. Horace begins in fearful anticipation; then the crisis comes, first on land and then on sea; Antony turns to flight; and — even though some danger remains, and there is metus as well as joy at the end of the poem — the celebrations can finally begin. On this reading there remains the familiar problem of vv. 17–20: at huc frementes uerterunt (...) bis mille equos Galli canentes Caesarem, hostiliumque nauium portu latent puppes sinistrorsum citae. The first couplet clearly relates to the defection of Amyntas' Galatians, the decisive moment in the fighting on land; the second must describe the crucial battle on sea. There is no problem in portu latent. The fleet has withdrawn, and is skulking in harbour instead of fighting. But what of nauium…puppes sinistrorsum citae? The difficulty is notorious: the secondary sources do not clearly describe any movement ‘toward the left’, and it is hard to see why Horace chooses so enigmatic a phrase to capture the fighting. His audience would not make much of the topographical detail in any case: unless they had been at Actium themselves , their reaction to the words would centre on other associations — the contrast between these magnificent puppes and their undignified sideways movement; the suggestions of ill omen in sinistrorsum. Global human population expansion is rooted in a remarkably successful evolutionary innovation. The neolithic transformation of the natural world gave rise to a symbiosis between humans and their domesticated plant and animal partners that will expand from a current 20 per cent to 60 percent of terrestrial biomass by the middle of the coming century. Such an increase must necessarily be accompanied by a concomitant decrease in wildlife biomass. We suggest that current trends in population growth are unlikely to abate (...) for three reasons: first, there are intrinsic biological pressures to reproduce regardless of social engineering; second, the character of the domestic alliance makes it a formidable competitor to wildlife; and third, the timeframe before population doubling is, from a biological perspective, virtually instantaneous. This paper draws from a wide body of research in the biological and social sciences. We neither condone nor endorse this picture of inexorable population increase. Rather, we appeal for a change in the nature of the discussion of population among environmentalists, to focus on the question of how best to manage what wildlife will be left on the margins of a domesticated world. Epidemics that were probably influenza have been reported throughout recorded history. There were 13 fairly severe epidemics during the 18th century and 12 during the 19th century. Probably 8 of these 25 were influenza pandemics. In the 20th century there have been 4 pandemics (1918/19, 1957/58, 1968/69 and 1977) due to the emergence of new subtypes of influenza A virus. The great pandemic of 1918/19 caused an estimated 20 million deaths. Between pandemics usually there have been epidemics of varying severity (...) at intervals of one to three years and a trickle of sporadic cases every winter. The morbidity and mortality rates have varied greatly from epidemic to epidemic and from place to place during the same epidemic. Generally the morbidity has been lowest in people over 60 years of age, but, except for 1918/19, the mortality has been predominantly in old people. The epidemic behaviour of influenza has been so erratic and difficult to understand that there are still a few scientists who consider that extraterrestrial influences operate. These views are not taken seriously by most virologists but there are puzzling aspects of influenza that are not yet understood. (shrink)
