Responsible Research and Innovation (‘RRI’) is a cross-cutting priority for scientific research in the European Union and beyond. This paper considers whether the way such research is organised and delivered lends itself to the aims of RRI. We focus particularly on international consortia, which have emerged as a common model to organise large-scale, multi-disciplinary research in contemporary biomedical science. Typically, these consortia operate through fixed-term contracts, and employ governance frameworks consisting of reasonably standard, modular components such as management committees, advisory (...) boards, and data access committees, to co-ordinate the activities of partner institutions and align them with funding agency priorities. These have advantages for organisation and management of the research, but can actively inhibit researchers seeking to implement RRI activities. Conventional consortia governance structures pose specific problems for meaningful public and participant involvement, data sharing, transparency, and ‘legacy’ planning to deal with societal commitments that persist beyond the duration of the original project. In particular, the ‘upstream’ negotiation of contractual terms between funders and the institutions employing researchers can undermine the ability for those researchers to subsequently make decisions about data, or participant remuneration, or indeed what happens to consortia outputs after the project is finished, and can inhibit attempts to make project activities and goals responsive to input from ongoing dialogue with various stakeholders. Having explored these challenges, we make some recommendations for alternative consortia governance structures to better support RRI in future. (shrink)

While cadaver dissection remains an unmatched learning tool for structural anatomy, recent shifts in medical culture and pedagogy indicate that developing humanistic practices and fostering empathic responses are crucial components of early medical education. The Donor Letter Project was designed to accompany a traditional dissection curriculum, and the pilot, described here, tested its quality and efficacy. In 2017, family members of recently deceased donors to the Colorado State Anatomical Board were invited to submit letters about their loved ones, and forty-seven (...) first-year medical students at the University of Colorado School of Medicine volunteered to read the letters after their human anatomy course. The students then completed a survey about their experience. Because student and donor family responses to the DLP were overwhelmingly positive, the DLP will be repeated with incoming medical school classes, and an addendum to the State Anatomical Board donation application will invite donors to submit letters along with their enrollment materials that may be read by students at the time of donation. Clinicians, as well as other health-care professionals in genetics clinics, may find themselves in the position where they must consider whether it would be appropriate to offer a diagnostic genetic test to an adolescent. While a clinician's decision to offer a diagnostic genetic test may be straightforward in clinical terms, the dynamics of family interaction and circumstances may make the decision-making process more complicated. Disagreement between parent and child place clinicians in a difficult position and they must be clear about (...) the scope of their professional responsibility and obligations, to both parents and the adolescent. The purpose of this paper is to discuss the Gillick principles and statutory requirements regarding the genetic testing of adolescents. While I will discuss the clinician's obligations, these legal requirements also have applicability to other health-care professionals, such as genetic counsellors, working in genetics clinics. Objective Futile treatment, which by definition cannot benefit a patient, is undesirable. This research investigated why doctors believe that treatment that they consider to be futile is sometimes provided at the end of a patient9s life. Design Semistructured in-depth interviews. Setting Three large tertiary public hospitals in Brisbane, Australia. Participants 96 doctors from emergency, intensive care, palliative care, oncology, renal medicine, internal medicine, respiratory medicine, surgery, cardiology, geriatric medicine and medical administration departments. Participants were recruited using purposive maximum variation sampling. (...) Results Doctors attributed the provision of futile treatment to a wide range of inter-related factors. One was the characteristics of treating doctors, including their orientation towards curative treatment, discomfort or inexperience with death and dying, concerns about legal risk and poor communication skills. Second, the attributes of the patient and family, including their requests or demands for further treatment, prognostic uncertainty and lack of information about patient wishes. Third, there were hospital factors including a high degree of specialisation, the availability of routine tests and interventions, and organisational barriers to diverting a patient from a curative to a palliative pathway. Doctors nominated family or patient request and doctors being locked into a curative role as the _main_ reasons for futile care. Conclusions Doctors believe that a range of factors contribute to the provision of futile treatment. A combination of strategies is necessary to reduce futile treatment, including better training for doctors who treat patients at the end of life, educating the community about the limits of medicine and the need to plan for death and dying, and structural reform at the hospital level. The language of “participant-driven research,” “crowdsourcing” and “citizen science” is increasingly being used to encourage the public to become involved in research ventures as both subjects and scientists. Originally, these labels were invoked by volunteer research efforts propelled by amateurs outside of traditional research institutions and aimed at appealing to those looking for more “democratic,” “patient-centric,” or “lay” alternatives to the professional science establishment. As mainstream translational biomedical research requires increasingly larger participant pools, however, corporate, academic and governmental research programs (...) are embracing this populist rhetoric to encourage wider public participation. We examine the ethical and social implications of this recruitment strategy. We begin by surveying examples of “citizen science” outside of biomedicine, as paradigmatic of the aspirations this democratizing rhetoric was originally meant to embody. Next, we discuss the ways these aspirations become articulated in the biomedical context, with a view to drawing out the multiple and potentially conflicting meanings of “public engagement” when citizens are also the subjects of the science. We then illustrate two uses of public engagement rhetoric to gain public support for national biomedical research efforts: its post-hoc use in the “care.data” project of the National Health Service in England, and its proactive uses in the “Precision Medicine Initiative” of the United States White House. These examples will serve as the basis for a normative analysis, discussing the potential ethical and social ramifications of this rhetoric. We pay particular attention to the implications of government strategies that cultivate the idea that members of the public have a civic duty to participate in government-sponsored research initiatives. We argue that such initiatives should draw from policy frameworks that support normative analysis of the role of citizenry. And, we conclude it is imperative to make visible and clear the full spectrum of meanings of “citizen science,” the contexts in which it is used, and its demands with respect to participation, engagement, and governance. BackgroundWhereas many adolescents and young people with HIV require the transfer of care from paediatric/adolescent clinics to adult ART clinics, this transition is beset with a multitude of factors that have the potential to hinder or facilitate the process, thereby raising ethical challenges of the transition process. Decisions made regarding therapy, such as when and how to transition to adult HIV care, should consider ethical benefits and risks. Understanding and addressing ethical challenges in the healthcare transition could ensure a smooth (...) and successful transition. The purpose of this study was to analyze the ethical challenges of transitioning HIV care for adolescents into adult HIV clinics.MethodsData presented were derived from 191 adolescents attending nine different health facilities in Uganda, who constituted 18 focus group discussions. In the discussions, facilitators and barriers regarding adolescents transitioning to adult HIV clinics were explored. Guided by the Silences Framework for data interpretation, thematic data analysis was used to analyze the data. The principles of bioethics and the four-boxes ethics framework for clinical care (patient autonomy, medical indications, the context of care, and quality of life) were used to analyze the ethical issues surrounding the transition from adolescent to adult HIV care.ResultsThe key emerging ethical issues were: reduced patient autonomy; increased risk of harm from stigma and loss of privacy and confidentiality; unfriendly adult clinics induce disengagement and disruption of the care continuum; patient preference to transition as a cohort, and contextual factors are critical to a successful transition.ConclusionThe priority outcomes of the healthcare transition for adolescents should address ethical challenges of the healthcare transition such as loss of autonomy, stigma, loss of privacy, and discontinuity of care to ensure retention in HIV care, facilitate long-term self-care, offer ongoing all-inclusive healthcare, promote adolescent health and wellbeing and foster trust in the healthcare system. Identifying and addressing the ethical issues related to what hinders or facilitates successful transitions with targeted interventions for the transition process may ensure adolescents and young people with HIV infection remain healthy across the healthcare transition. The DC model enables individuals to know and to decide how personal research information is being used and provides a way in which to exercise legal rights provided in privacy and data protection law. The DC tool is flexible and responsive, enabling legal and ethical requirements in research data sharing to be met and for online health (...) information to be maintained. DC has been used in rare diseases and genomics, to enable people to control and express their preferences regarding their own data. However, DC has never been explored in relationship to historical collections of bioscientific and genetic heritage or to contexts involving Aboriginal and Torres Strait Islander people.In response to the growing interest by First Peoples throughout Australia in genetic and genomic research, and the increasing number of invitations from researchers to participate in community health and wellbeing projects, this article examines the legal and ethical attributes and challenges of DC in these contexts. It also explores opportunities for including First Peoples' cultural perspectives, governance, and leadership as a method for defining DC on cultural terms that engage best practice research and data analysis as well as respect for meaningful and longitudinal individual and family participation. BackgroundInnovations in technology have contributed to rapid changes in the way that modern biomedical research is carried out. Researchers are increasingly required to endorse adaptive and flexible approaches to accommodate these innovations and comply with ethical, legal and regulatory requirements. This paper explores how Dynamic Consent may provide solutions to address challenges encountered when researchers invite individuals to participate in research and follow them up over time in a continuously changing environment.MethodsAn interdisciplinary workshop jointly organised by the University of Oxford (...) and the COST Action CHIP ME gathered clinicians, researchers, ethicists, lawyers, research participants and patient representatives to discuss experiences of using Dynamic Consent, and how such use may facilitate the conduct of specific research tasks. The data collected during the workshop were analysed using a content analysis approach.ResultsDynamic Consent can provide practical, sustainable and future-proof solutions to challenges related to participant recruitment, the attainment of informed consent, participant retention and consent management, and may bring economic efficiencies.ConclusionsDynamic Consent offers opportunities for ongoing communication between researchers and research participants that can positively impact research. Dynamic Consent supports inter-sector, cross-border approaches and large scale data-sharing. Whilst it is relatively easy to set up and maintain, its implementation will require that researchers re-consider their relationship with research participants and adopt new procedures. (shrink)

Fodor and others who think that scientific, computational psychology will vindicate commonsense belief-desire psychology have maintained that belief can be identified with the explicit storage of a token with appropriate content. I review and develop problems for the explicit storage view and show that a more plausible account identifies belief with the disposition to use a token with appropriate content in explicit reasoning and planning processes and as a basis for action. I argue that this type of inner disposition account (...) will also apply to most other common sense attitudes. The result is a realism about commonsense belief-desire psychology that is more modest than Fodor's: While such inner dispositions probably do exist, these states will probably not be the main focus of scientific psychological theories. In an earlier paper the first author initiated the study of generic cuts of a model of Peano arithmetic relative to a notion of an indicator in the model. This paper extends that work. We generalise the idea of an indicator to a related neighbourhood system; this allows the theory to be extended to one that includes the case of elementary cuts. Most results transfer to this more general context, and in particular we obtain the idea of a generic cut (...) relative to a neighbourhood system, which is studied in more detail. The main new result on generic cuts presented here is a description of truth in the structure , where I is a generic cut of a model M of Peano arithmetic. The special case of elementary generic cuts provides a partial answer to a question of Kossak [R. Kossak, Four problems concerning recursively saturated models of arithmetic, Notre Dame Journal of Formal Logic 36 519–530]. (shrink)

Reading fiction for pleasurable is robustly correlated with improved cognitive attainment and other benefits. It is also in decline among young people in developed nations, in part because of competition from moving image fiction. We review existing research on the differences between reading/hearing verbal fiction and watching moving image fiction, as well as looking more broadly at research on image/text interactions and visual versus verbal processing. We conclude that verbal narrative generates more diverse responses than moving image narrative., We note (...) that reading and viewing narrative are different tasks, with different cognitive loads. Viewing moving image narrative mostly involves visual processing with some working memory engagement, whereas reading narrative involves verbal processing, visual imagery and personal memory (Xu et al 2005). but Attempts to compare the two by creating suggest that existing research is flawed by attempts to create equivalent stimuli and task demands face a number of challenges, and we discuss these difficulties in comparative approaches. We then investigate the possibility of identifying lower level processing mechanisms that might distinguish cognition of the two media, and propose a focus on internal scene construction and on working memory as foci for future research. Although many of the sources we draw on are focussed on English speaking participants in European or North American settings, we also cover material relating to speakers of Dutch, German, Hebrew and Japanese in their respective countries, and studies of a remote Turkish mountain community. El origen de la escritura china sigue siendo a día de hoy una cuestión que no deja de despertar gran interés. Con el presente trabajo pretendemos exponer ciertos aspectos de dicha escritura, centrando la investigación en sus orígenes y prestando una especial atención a las explicaciones mitológicas que se han dado al respecto. Se exponen cuatro personajes destacados de la mitología china en torno a los cuales se han centrado los mitos sobre el inicio de esta escritura milenaria.

Dentro de la dilatada herencia intelectual que nos ha legado Ratzinger / Benedicto XVI es destacable su aportación a la Teología Fundamental. Su amor y su vasto conocimiento tanto de filosofía como de teología permiten al lector descubrir valiosas herramientas para afrontar problemas actuales. En este caso, proporciona recursos utilísimos para, en primer lugar, analizar la crisis que vive la relación fe-filosofía —la centralidad del guion que la une desde su origen— y, en segundo lugar, muestra posibles caminos para recuperar (...) dicha relación tan esencial para la reflexión teológica y, también, para la vida de la Iglesia. Through transatlantic contact and subsequent debates, the “humanity” of Amerindians was first established for Europeans according to the dictates of philosophical anthropology and theology. This hierarchical and colonial anthropology is problematic precisely because it normalizes a singular, indigenous way of “being human” as the only correct and universal formulation of the “human being,” i.e., Man. Consequently, people that live outside this constructed definition are exposed to dispossession, dehumanization, and genocide because they are deemed outside the bounds of Mankind. Through a (...) three-part analysis, this work will examine Bartolomé de Las Casas’ categorization of Amerindians though his formulation of “barbarianism,” compare this taxonomy with Orson Scott Card’s “Hierarchy of Foreignness,” and finally argue that Indigenous traditions of thought—as seen in “grounded normativity” and “place-thought”—allow not only for the dissolution of colonial anthropologies but also permit for a teleological suspension of Man. Os estudos da linguagem no século XX e agora no atual século são marcados pela hegemonia de três correntes de pesquisa: o Estruturalismo; o Inatismo e o Interacionismo. No Estruturalismo, o usuário da língua é submetido a um código soberano, contra o qual ele não tem escolha a não ser cumprir as normas do uso correto. Desviar do padrão significa erro. No Inatismo, prevalece o logicismo. Na mente do usuário reside tudo de que ele precisa para ser bem sucedido em (...) sua linguagem. Se o usuário pensa corretamente, também correta será sua expressão linguística. Se o uso não condiz com a lógica de base é sinal de que o usuário não pensa corretamente. No Interacionismo, a linguagem é fruto da interação entre usuários ideologicamente e contextualmente delimitados. Os usos da língua são compartilhados de acordo com o conhecimento do usuário do quadro semântico e pragmático que reside em cada contexto. Na contra hegemonia dessas correntes, este texto rechaça essas três correntes e constrói a sua crítica a partir das ideias de Michel Foucault. Nesse sentido, vai sendo delineado o rumo conceitual dado pelo filósofo francês, uma nova proposta, denominada aqui de linguística do enunciado, sob a forma de uma arqueologia da linguagem, para dar conta dos discursos que dão sentido aos acontecimentos. Ainda, este texto relaciona essa arqueologia a um novo modelo de ensino da língua na escola, dando a essa prática escolar um sentido diferente do que tem imperado em diversas escolas desse país. This paper consists of an initial investigation about the meaning of a good childhood following the ethical ideal of authenticity. In this introduction to a philosophy of childhood and authenticity, the central theme is to investigate how the authenticity ideal is already presupposed in the contemporary discourse on what constitutes a good childhood. In the emerging field of philosophy of childhood, the capacities of children for agency, autonomy, and committing and the fundamental role of parents in guaranteeing possibilities to exercise (...) them are being increasingly highlighted, together with a discourse that there are some intrinsic goods of childhood. These developments parallel contemporary reconstructions of authenticity as an ethical ideal. Current debates emphasize the importance of a person finding, creating and constructing their originality, and how to realize it. At the same time, this search must recognize demands emanating from something more than human desires: from one’s culture and community. The parallel dynamics between these two discourses - children-parent and individual-society - point to a direction that applying the concept of authenticity to the construction of novel interpretations and practices of a good childhood can bring fruitful results. After examining such parallels, some of these practices that emerge from the analysis of good childhoods as authentic childhood are pointed out, such as the importance of cultivating children’s moments of caring and committing, and the development of personal projects.
