ObjectivePatient involvement is used to describe the inclusion of patients as active participants in healthcare decision-making and research. This study aimed to investigate incoming year 1 medical (MBChB) students’ attitudes and opinions regarding patient involvement in this context.MethodsWe established a staff–student partnership to formulate the design of an online research survey, which included Likert scale questions and three short vignette scenarios designed to probe student attitudes towards patient involvement linked to existing legal precedent. Incoming year 1 medical students (n=333) were (...) invited to participate in the survey before formal teaching commenced.ResultsSurvey data (49 participants) indicate that students were broadly familiar with, and supportive of, patient involvement in medical treatment. There was least support for patient involvement in conducting (23.9%), contributing to (37.0%) or communicating research (32.6%), whereas there was unanimous support for patients choosing treatment from a selection of options (100%).ConclusionIncoming members of the medical profession demonstrate awareness of the need to actively involve patients in healthcare decision-making but are unfamiliar with the utility and value of such involvement in research. Clinical next generation sequencing is a term that refers to a variety of technologies that permit rapid sequencing of large numbers of DNA segments, up to and including entire genomes. As an approach that is playing an increasingly important role in obtaining genetic information from patients, it may be viewed by public and private payers either positively, as an enabler of the promised benefits of personalized medicine, or as “the perfect storm” resulting from the confluence of high market demand, an (...) uproven technology, and an unprepared delivery system. A number of recent studies have noted that coverage and reimbursement will be critical for clinical integration of NGS, yet the evidentiary pathway for payer decision-making is unclear. Although there are multiple reasons for this uncertain reimbursement environment, the situation stems in large part from a long-standing lack of alignment between the information needs of regulators and post-regulatory decision-makers such as payers. Testimony is an invaluable source of knowledge. We rely on the reports of those around us for everything from the ingredients in our food and medicine to the identity of our family members. Recent years have seen an explosion of interest in the epistemology of testimony. Despite the multitude of views offered, a single thesis is nearly universally accepted: testimonial knowledge is acquired through the process of transmission from speaker to hearer. In this book, Jennifer Lackey shows that this (...) thesis is false and, hence, that the literature on testimony has been shaped at its core by a view that is fundamentally misguided. She then defends a detailed alternative to this conception of testimony: whereas the views currently dominant focus on the epistemic status of what speakers believe, Lackey advances a theory that instead centers on what speakers say. The upshot is that, strictly speaking, we do not learn from one another's beliefs - we learn from one another's words. Once this shift in focus is in place, Lackey goes on to argue that, though positive reasons are necessary for testimonial knowledge, testimony itself is an irreducible epistemic source. This leads to the development of a theory that gives proper credence to testimony's epistemologically dual nature: both the speaker and the hearer must make a positive epistemic contribution to testimonial knowledge. The resulting view not only reveals that testimony has the capacity to generate knowledge, but it also gives appropriate weight to our nature as both socially indebted and individually rational creatures. The approach found in this book will, then, represent a radical departure from the views currently dominating the epistemology of testimony, and thus is intended to reshape our understanding of the deep and ubiquitous reliance we have on the testimony of those around us. The last thirty years have seen a resurgence of interest in virtue among philosophers, psychologists, and educators. This co-authored book brings an interdisciplinary response to the study of virtue: it not only provides a framework for quantifying virtues, but also explores how we can understand virtue in a philosophically-informed way that is compatible with the best current thinking in personality psychology. The volume presents a major contribution to theemerging science of virtue and character measurement.

Skepticism about the epistemic value of intuition in theoretical and philosophical inquiry has recently been bolstered by empirical research suggesting that people’s concrete-case intuitions are vulnerable to irrational biases (e.g., the order effect). What is more, skeptics argue that we have no way to ‘‘calibrate” our intuitions against these biases and no way of anticipating intuitional instability. This paper challenges the skeptical position, introducing data from two studies that suggest not only that people’s concrete-case intuitions are often stable, but also (...) that people have introspective awareness of this stability, providing a promising means by which to assess the epistemic value of our intuitions. Jennifer McKitrick offers an opinionated guide to the philosophy of dispositions. In her view, when an object has a disposition, it is such that, if a certain type of circumstance were to occur, a certain kind of event would occur. Since this is very common for this to be the case, dispositions are an abundant and diverse feature of our world.

In this paper, I explore and defend ideology critique as a method that is descended from the project of the critique of reason. Specifically, I interpret ideology critique as operating through what critical theory calls the dialectics of immanence and transcendence. Turning to Hegel and Marx, I further argue that the dialectics of immanence and transcendence must be more concretely understood as the dialectics of life and self-consciousness. Understanding the relation between life and self-consciousness is crucial for ideology critique because (...) what ideologies distort is the relation between self-consciousness and life, a relation that is fundamental to the actualization of human freedom. I argue that ideologies are social pathologies, or wrong ways of living. I analyze two concepts that illuminate the method of ideology critique in particular: Hegel’s “Idea,” and Marx’s Gattungswesen (species-being). These two concepts provide the normative basis for reconsidering ideology critique in light of a non-reductive critical naturalism. (shrink)

Moral abolitionists claim that morality ought to be abolished. According to one of their most prominent arguments, this is because making moral judgments renders people significantly less tolerant toward anyone who holds divergent views. In this paper we investigate the hypothesis that morality’s tolerance-decreasing effect only occurs if people are realists about moral issues, i.e., they interpret these issues as objectively grounded. We found support for this hypothesis (Studies 1 and 2). Yet, it also turned out that the intolerance associated (...) with realism is mediated by moral conviction and perceived consensus. People tend to feel more strongly about those moral issues they ground objectively and, in doing so, are more prone to display the vice of moral smugness toward those who disagree with them. The remedy for this that has been recommended is humility which we found (Study 3) is indeed related to reduced intolerance, in part by predicting a reduction in realism, but also in part through a direct connection to intolerance. These results put pressure on abolitionists’ “argument from intolerance.”. (shrink)

Jennifer Lackey's Learning from Words: Testimony as a Source of Knowledge is a useful and intelligent guide, a well informed and appreciative but critical and provocative commentary on a large and growing body of literature.According to Lackey, most of (...) the literature assumes that testimony can spread but not create knowledge, much as memory can be a reminder of old but not a source of new truth. Lackey maintains that the assumption is mistaken and offers an account of testimony, according to which, testimony can give rise to new knowledge as well as transmit old truths from one person to another.In her introduction, Lackey characterizes what she calls the belief view of testimony and suggests that this view dominates today's literature. Background Few studies have been conducted to assess the quality of orientation practices for ethics advisory committees that conduct ethics consultation. This survey study focused on several Harvard teaching hospitals, exploring orientation quality and committee members’ self-evaluation in the American Society of Bioethics and Humanities ethics consultation competencies. Methods We conducted a survey study that involved 116 members and 16 chairs of ethics advisory committees, respectively. Predictor variables included professional demographics, duration on committees and level of training. Outcome variables included (...) familiarity with and preparedness in the ASBH competencies and satisfaction with orientations. We hypothesised that responses would be associated with both the aforementioned predictors and whether or not participants had encountered the ASBH competencies in training. Results A majority of respondents found their orientation curricula to be helpful, although a significant portion of respondents did not receive any orientation or were unsatisfied with their orientation. Familiarity with ASBH competencies was a statistically significant predictor of respondents’ self-evaluation in particular categories. Standard educational materials were reported as offered during orientation, such as readings and case studies ; different medium resources were less evidenced such as videos on ethics consultation. Conclusions Institutions should re-evaluate orientation practices for ethics committee members that perform ethics consultation. Integrating ASBH competencies and useful methods into a resourceful pedagogy will help improve both member satisfaction with orientation and preparation in consultation. (shrink)

BackgroundArtificial intelligence has been described as the “fourth industrial revolution” with transformative and global implications, including in healthcare, public health, and global health. AI approaches hold promise for improving health systems worldwide, as well as individual and population health outcomes. While AI may have potential for advancing health equity within and between countries, we must consider the ethical implications of its deployment in order to mitigate its potential harms, particularly for the most vulnerable. This scoping review addresses the following question: (...) What ethical issues have been identified in relation to AI in the field of health, including from a global health perspective?MethodsEight electronic databases were searched for peer reviewed and grey literature published before April 2018 using the concepts of health, ethics, and AI, and their related terms. Records were independently screened by two reviewers and were included if they reported on AI in relation to health and ethics and were written in the English language. Data was charted on a piloted data charting form, and a descriptive and thematic analysis was performed.ResultsUpon reviewing 12,722 articles, 103 met the predetermined inclusion criteria. The literature was primarily focused on the ethics of AI in health care, particularly on carer robots, diagnostics, and precision medicine, but was largely silent on ethics of AI in public and population health. The literature highlighted a number of common ethical concerns related to privacy, trust, accountability and responsibility, and bias. Largely missing from the literature was the ethics of AI in global health, particularly in the context of low- and middle-income countries.ConclusionsThe ethical issues surrounding AI in the field of health are both vast and complex. While AI holds the potential to improve health and health systems, our analysis suggests that its introduction should be approached with cautious optimism. The dearth of literature on the ethics of AI within LMICs, as well as in public health, also points to a critical need for further research into the ethical implications of AI within both global and public health, to ensure that its development and implementation is ethical for everyone, everywhere. Against the standard interpretation that Hegel's idealism, in particular speculative logic, should be understood as an extension of Kant's transcendental idealism, I argue that Hegel's Logic should be understood as a logic of actuality (Wirklichkeit). Rather than seeking to determine the necessary and merely formal conditions and categories for the knowledge of any possible object, speculative logic is the immanent and active process of determining the truth of actual objects and actuality itself. Through a discussion of the status of the (...) transition between the Phenomenology and the Logic, as well as a detailed reading of Hegel’s treatment of the modal categories in the Doctrine of Essence, I seek to show how speculative logic offers a way to think the unity of a thing and its conditions without reverting to pre-critical metaphysics. By breaking down the traditional distinctions between actuality, possibility, necessity, and contingency, as well as demonstrating the necessity of contingency in the activity of thinking, I suggest that Hegel provides us with the categories necessary for a new understanding of the relation between thought and reality beyond the Kantian frame. More specifically, this study is a pilot study of the impact of moral intensity on financial accountants' propensity to manage earnings. The instrument, once validated, will be used in a full-study of managers in the hotel industry. Different ethical scenarios were presented to respondents in the survey; each ethical scenario was designed in both high or (...) low moral intensity form, to reflect the importance of the moral dilemma at hand. The results were analysed by factor analysis. The findings of this study have positively validated the instrument, with three of the five moral intensity components identified as having appropriate eigenvalues. This indicates that they have a significant influence in the study. The first factor captures the social consensus dimension and one scenario of the proximity dimension. The second factor indicates an interaction between the temporal immediacy and the magnitude of consequences dimension. The third dimension is probability of effect and one scenario of the proximity dimension. In addition, t-tests indicated that the manipulation of high and low conditions within each scenario were also successful. One limitation of the study might be the use of undergraduate accounting students as manager proxies, although prior evidence suggests use of accounting students as proxies is a valid approach in this type of study. This is a highly novel project as most prior studies have focussed on moral intensity and the general ethical decision-making process. (shrink)

ObjectivesTo describe reporting of informed consent in pragmatic trials, justifications for waivers of consent and reporting of alternative approaches to standard written consent. To identify factors associated with (1) not reporting and (2) not obtaining consent.MethodsSurvey of primary trial reports, published 2014–2019, identified using an electronic search filter for pragmatic trials implemented in MEDLINE, and registered in ClinicalTrials.gov.ResultsAmong 1988 trials, 132 (6.6%) did not include a statement about participant consent, 1691 (85.0%) reported consent had been obtained, 139 (7.0%) reported a (...) waiver and 26 (1.3%) reported consent for one aspect (eg, data collection) but a waiver for another (eg, intervention). Of the 165 trials reporting a waiver, 76 (46.1%) provided a justification. Few (53, 2.9%) explicitly reported use of alternative approaches to consent. In multivariable logistic regression analyses, lower journal impact factor (p=0.001) and cluster randomisation (p<0.0001) were significantly associated with not reporting on consent, while trial recency, cluster randomisation, higher-income country settings, health services research and explicit labelling as pragmatic were significantly associated with not obtaining consent (all p<0.0001).DiscussionNot obtaining consent seems to be increasing and is associated with the use of cluster randomisation and pragmatic aims, but neither cluster randomisation nor pragmatism are currently accepted justifications for waivers of consent. Rather than considering either standard written informed consent or waivers of consent, researchers and research ethics committees could consider alternative consent approaches that may facilitate the conduct of pragmatic trials while preserving patient autonomy and the public’s trust in research. In this paper we show that there is a pair of superhigh r.e. degree that forms a minimal pair. An analysis of the proof shows that a critical ingredient is the growth rates of certain order functions. This leads us to investigate certain high r.e. degrees, which resemble ∅′ very closely in terms of ∅′-jump traceability. In particular, we will construct an ultrahigh degree which is cappable.

This article treats the public display of emotion as social performance. The concept of "emotive performance" is developed to highlight the overlooked quality of performativity in the social use of emotion. We argue that emotive performance is reflexive, cultural, and communicative. As an active social act, emotive performance draws from the cultural repertoire of interpretative frameworks and dominant narratives. We illustrate the utility of the concept by analyzing two episodes of unrehearsed emotive performances by two well-known politicians, Bill Clinton and (...) Jiang Zemin. The two cases demonstrate how emotion can be analyzed as a domain in which culturally specific narratives and rhetorics are used to advance the situational agenda of actors. The concept opens up a more expansive research agenda for sociology. It pushes sociologists to pay greater attention to people's experiences, interpretations, and deployments of emotions in social life. (shrink)

Recent observations associate naturally occurring trace levels of Lithium in ground water with significantly lower suicide rates. It has been suggested that adding trace Lithium to drinking water could be a safe and effective way to reduce suicide. This article discusses the many ethical implications of such population-wide Lithium medication. It compares this policy to more targeted solutions that introduce trace amounts of Lithium to groups at higher risk of suicide or lower risk of adverse effects. The question of mass (...) treatment with Lithium recalls other choices in public health between population-wide and more targeted interventions. The framework we propose could be relevant to some of these other dilemmas. (shrink)

After an initial period of feminist theorizing concerned with understanding patriarchy as a structure of male domination, many thinkers turned away from theorizing domination as such and focused instead on women's (constructed) subjectivity, identity, and agency. While this has fostered important insights into the formation of women's preferences, desires, and choices, this focus on subjectivity and subject formation has largely overshadowed deeper understandings of patriarchy as a structure of male domination while producing elisions between agency and freedom. In this article, (...) I move to show how domination as a structural concept can help us to reclaim the idea of ‘patriarchy’ as a source of women's systematic oppression while freedom as non-domination, derived from early republican conceptualizations of freedom, can help us to disambiguate freedom from agency by taking as central the relative positions of actors within social and political structures. Structural freedom as non-domination is thus useful for feminist thinkers in that it gives us critical purchase on the dynamics inherent in unequal social and political relationships and can be linked clearly to the institutions and ideologies that shape and justify interactions between more powerful and less powerful groups. Further, from this point of view intersecting structures of domination can be analysed rather than intersecting identity categories, allowing us to take intersectionality into account and avoiding the need to ground feminist action on a unitary ‘category woman’. Finally, this analysis points toward the radical democratic connexion between freedom and participation in the creation of the material and symbolic structures that frame our collective lives. In the first book on the development of John Dewey's ethical thought, Jennifer Welchman revises the prevalent interpretation of his ethics. Her clear and engaging account traces the history of Dewey's distinctive moral philosophy from its roots in idealism during the 1890s through the pragmatist approach of his 1922 work, Human Nature and Conduct. Central to the development of Dewey's ethics was his lifelong conviction that the realms of science and morals, facts and values were reconcilable. This conviction, Welchman (...) demonstrates, drove Dewey to reject the orthodox ethics of his day in favor of radical alternatives?first absolute idealism and later pragmatism. She reveals how Dewey came to adopt and subsequently to modify idealist ethics of self-realization. Welchman then explores the transformations in Dewey's conception of science that exploded the fragile truce between fact and value that he had negotiated as an idealist. A growing body of empirical research suggests that people's concrete‐case intuitions are unstable—i.e., vulnerable to biasing influences—with no way of anticipating the instability. This has led some to challenge the use of intuitions in philosophical practice. In this chapter, I consider responses to this challenge. One is that the empirical research cited has no bearing on the epistemic status of intuitions because it fails to actually test intuitions. While this is a worry worth taking seriously, there is another response available—namely, (...) that even if we grant that these studies successfully elicited intuitions, in the midst of the instability uncovered there was also a fair amount of stability. And an investigation into the features of unstable vs. stable judgments, and the contexts within which they are generated, suggests that the instability in question may not actually be intuitional. Launched by the United Nations in 2000, the Global Compact (GC) promotes private sector compliance with 10 basic principles covering human rights, labour standards, the environment, and anti-corruption. Its sponsors aim to establish a global corporate social responsibility (CSR) network based on a pledge to observe the 10 principles adopted by companies across the range of company size and regional origin, backed by a modest reporting system and collaborative programmes. The author analyzes the GC's progress toward building a global network (...) from its launch through 2006 and finds that, while the GC's nominal membership base of nearly 3000 companies makes it the largest system among collective action institutions (CAIs) for corporate responsibility, the GC has not reached 'critical mass'. Deficiencies in its nascent global network include limited market penetration among the largest corporations, a membership heavily weighted toward Western European companies, and major weaknesses in compliance with its reporting system. The author concludes that the GC must improve both penetration and compliance if it is to succeed in building a global standard for CSR. (shrink)
