Most donated organs in the United States come from brain dead donors, while a small percentage come from patients who die in “controlled,” or expected, circumstances, typically after the family or surrogate makes a decision to withdraw life support. The number of organs available for transplant could be substantially if donations were permitted in “uncontrolled” circumstances–that is, from people who die unexpectedly, often outside the hospital. According to projections from the Institute of Medicine, establishing programs permitting “uncontrolled donation after circulatory (...) determination of death,” or uDCDD, throughout the United States has the potential to provide 22,000 more donation opportunities annually. In contrast, U.S. controlled donation after circulatory determination of death, or cDCDD, cases have increased progressively over the past decade from 87 to 848 donors, but currently account for only 10.6 percent of all deceased donors. Following the IOM recommendations, several projects exploring the feasibility of uDCDD were funded by the federal government, including a grant from the Health Resources and Services Administration that supported a pilot project in New York City in which the authors of this article participated.A key feature of our protocol, and indeed of many uDCDD protocols, is the initiation of preservation methods such as chest compressions and extracorporeal membrane oxygenation shortly after death in order to perfuse and preserve the donor's organs. Critics of uDCDD argue that the means of determining death deviates from generally ascribed principles. They assert that reinstituting circulation in order to preserve organs has the effect of “undoing” the prior determination of death. The result is that cDCDD is widely accepted and practiced routinely even though it only marginally increases the number of organs available for transplantation, and uDCDD is widely considered unacceptable despite being ethically embraced and proven to significantly increase organ donation opportunities in other countries. This article explores the evolution of this counterintuitive state of affairs and calls for a policy that, in line with the IOM report, allows for both cDCDD and uDCDD protocols. (shrink)

This paper argues that professionals who make boundary-related decisions should be guided by relationship-based practice. In our roles as service users and professionals, drawing from our lived experiences of professional relationships, we argue we need to move away from distance-based practice. This includes understanding the boundary stories and narratives that exist for all of us – including the people we support, other professionals, as well as the organisations and systems within which we work. When we are dealing with professional boundary (...) issues, we should centre relationship-building skills that are central to many other aspects of our work. Skills that foster relationships at all levels – between professionals, service users, and services – need to be revalued. Our final recommendation is to create, develop, and foster safer spaces within and outside of organisations, as well as inter-professionally, for the discussion and exploration of boundary-related issues and practice. We are interested in hearing from those with experiences of being marginalised by boundaries so that they can inform a reshaping of our collective ideas around boundary related practices. To foster relationship-based practices in organisations, we have outlined several recommendations here; however, we recognize that these do not go far enough, and that collective action is needed to inform systemic change. The work of the French Lithuanian Jewish philosopher, Emmanuel Levinas, describes a perceptive rethinking of the possibility of concrete acts of goodness in the world, a rethinking never more necessary than now, in the wake of the cruel realities of the twentieth century—ten million dead in the First World War, forty million dead in the Second World War, Hiroshima, Nagasaki, the Soviet gulags, the grand slaughter of Mao’s “Great Leap Forward,” the pointless and gory Vietnam War, the Cambodian self-genocide and (...) many an attempted extermination from the face of the earth of entire nations of indigenous peoples. The list of horrors goes on and on—daily holocausts that do not distinguish between the civilian and the soldier, the guilty and the innocent, those deserving of life and those less deserving. Southeast Asia has numerous religions and diverse forms of state-governance, so the populations largely have the freedom to express themselves within the context of their society. Expressing oneself can occur within the context of their religion, using the language they have been cultured within, if they remain in their cultural-context. This paper explores the context of Buddhist nations using militarized-language, seen as problematic by Dr. Matthew Kosuta, who professes in his masters-thesis that it is a contradiction. A portion of my (...) article refutes his position, on the grounds that Buddhism developed within the Hindu-Brahmanism context and caste was normalized in regions influenced by such culture. Equally, it could be noted, the Buddha also used a language that was suitable for agriculturalists, merchants, Brahmins, so a militarized-language was just one way to speak to some people within their own social context, which was understandable not just to them, but to anyone interested in attaining Nibbāna. We present a cross-cultural analysis showing that the presence of an active or moral High God in societies varies generally along a continuum from lesser to greater technological complexity and subsistence productivity. Foragers are least likely to have High Gods. Horticulturalists and agriculturalists are more likely. Pastoralists are most likely, though they are less easily positioned along the productivity continuum. We suggest that belief in moral High Gods was fostered by emerging leaders in societies dependent on resources that were difficult (...) to manage and defend without group cooperation. These leaders used the concept of a supernatural moral enforcer to manipulate others into cooperating, which resulted in greater productivity. Reproductive success would accrue most to such leaders, but the average reproductive success of all individuals in the society would also increase with greater productivity. Supernatural enforcement of moral codes maintained social cohesion and allowed for further population growth, giving one society an advantage in competition with others. (shrink)

William Hasker thinks that his emergent dualism provides a plausible account of the mind’s survival of bodily death, giving it a crucial advantage over physicalism. I do not share this appraisal. Emergentism by its very nature works against the (immediate) survival of death. The analogies that Hasker employs to overcome this initial implausibility fail due to factual errors, and his position ends up in no less a difficult position than the physicalism that Hasker rejects. Hasker’s attempt to escape this difficulty (...) results in what is effectively an abandonment of emergentism, and the adoption of a more traditional dualism. (shrink)

According to the epistemological objection to divine command ethics, if morality is grounded in God’s commands, then those who do not believe in God cannot have moral knowledge. This objection has been raised—and answered before. However, the objection persists, and I argue here that it has not been substantially improved upon and does not deserve a second hearing. Whether or not God’s commands provide the basis of moral facts does not imply that unbelievers cannot have moral knowledge, since the ability (...) to know that something is true does not depend on our ability to know what makes it true. It is my contention that what is generally construed as the Euthyphro Dilemma as a reason to deny that moral facts are based on theological facts is one of the worst arguments proposed in philosophy of religion or ethical theory, and that Socrates, the character of the dialogue who poses the dilemma, was both morally bankrupt in his challenge to Euthyphro, but more importantly here, ought to have lost the argument hands down. But in any dialogue, the author controls what (...) people say. Plato was able to easily give Socrates the victory by writing the ending of the story himself, where Euthyphro, believing that piety is what the gods approve of, loses the argument abysmally. The version of events presented here is different. This time, Euthyphro is permitted to offer a reasonable defence of his position, and he has the benefit of having been able to read all that has been said on the Euthyphro dilemma over the last couple of millennia, and especially the last fifty years. Under such circumstances, Socrates does not stand a chance. We arrive at our scene, the steps of the Dunedin High Court, in Dunedin, New Zealand, on a cool mid-winter July morning. (shrink)

Rare copy number variants have a prominent role in the aetiology of schizophrenia and other neuropsychiatric disorders. Substantial risk for schizophrenia is conferred by large CNVs at several loci, including microdeletions at 1q21.1, 3q29, 15q13.3 and 22q11.2 and microduplication at 16p11.2. However, these CNVs collectively account for a small fraction of cases, and the relevant genes and neurobiological mechanisms are not well understood. Here we performed a large two-stage genome-wide scan of rare CNVs and report the significant association of copy (...) number gains at chromosome 7q36.3 with schizophrenia. Microduplications with variable breakpoints occurred within a 362-kilobase region and were detected in 29 of 8,290 patients versus 2 of 7,431 controls in the combined sample. All duplications overlapped or were located within 89 kilobases upstream of the vasoactive intestinal peptide receptor gene VIPR2. VIPR2 transcription and cyclic-AMP signalling were significantly increased in cultured lymphocytes from patients with microduplications of 7q36.3. These findings implicate altered vasoactive intestinal peptide signalling in the pathogenesis of schizophrenia and indicate the VPAC2 receptor as a potential target for the development of new antipsychotic drugs. © 2011 Macmillan Publishers Limited. The Chronicle and the Letter have been newly translated, and both they and the authoritative Colgrave translation of the Ecclesiastical History are supported by a detailed introduction and notes.

The phrase "the meaning of life" for many seems a quaint notion fit for satirical mauling by Monty Python or Douglas Adams. But in this spirited, stimulating, and quirky enquiry, famed critic Terry Eagleton takes a serious if often amusing look at the question and offers his own surprising answer. Eagleton first examines how centuries of thinkers and writers--from Marx and Schopenhauer to Shakespeare, Sartre, and Beckett--have responded to the ultimate question of meaning. He suggests, however, that it is only (...) in modern times that the question has become problematic. But instead of tackling it head-on, many of us cope with the feelings of meaninglessness in our lives by filling them with everything from football to sex, Kabbala, Scientology, "New Age softheadedness," or fundamentalism. On the other hand, Eagleton notes, many educated people believe that life is an evolutionary accident that has no intrinsic meaning. If our lives have meaning, it is something with which we manage to invest them, not something with which they come ready made. Eagleton probes this view of meaning as a kind of private enterprise, and concludes that it fails to holds up. He argues instead that the meaning of life is not a solution to a problem, but a matter of living in a certain way. It is not metaphysical but ethical. It is not something separate from life, but what makes it worth living--that is, a certain quality, depth, abundance and intensity of life. Here then is a brilliant discussion of the problem of meaning by a leading thinker, who writes with a light and often irreverent touch, but with a very serious end in mind. "If you were to ask what provides some meaning in life nowadays for a great many people, especially men, you could do worse than reply 'football.' Not many of them perhaps would be willing to admit as much; but sport stands in for all those noble causes--religious faith, national sovereignty, personal honor, ethnic identity--for which, over the centuries, people have been prepared to go to their deaths. The rethinking of the events of the Soviet past largely concerns the sphere of church-state relations. The significant losses of cultural and architectural heritage during the Soviet period make it expedient to study the mechanisms of liquidation of church buildings in the second half of the 1960s. The purpose and objectives of the article. To determine the principles of state policy in the field of religion and methods of its implementation by party-state structures concerning the church network of the Russian (...) Orthodox Church in the Ukrainian SSR. The research methodology. The research methodology is based on a set of philosophical, general scientific, and historical methods and principles. To solve the specific tasks of the study, the principles of objectivity, historicism, systematicity, and comprehensiveness were also used. Research results. The period of the second half of the 1960s was marked by the liberalization of official views on the religious sphere, the abandonment of repressive methods of fighting religion and the Church, the beginning of the latent destruction of sacred buildings, and the dominance of the Soviet state’s nihilistic attitude toward cultural heritage. The Russian Orthodox Church remained entirely subordinate to the Soviet state, which disregarded its legitimate rights and interests. Discussion. The repressions of the “Thaw“ period and the failure of building a communist society led to a loss of public confidence in the possibility of a just welfare state. It became clear that by practicing persecution, restricting the rights of priests, and banning them from studying in theological schools, the Soviet government was undermining its authority. Therefore, in the 1970s, ideologues of the CPSU Central Committee proposed a new model of churchstate relations — the adaptation to communism. There were no fundamental changes in party policy, as it was based on the Marxist doctrine of religion as the opium of the people. The anti-church orientation of state policy remained, only its forms and methods were transformed. The period of massive closure of Orthodox churches was replaced by a new long-term period, called the period of “streamlining the religious network”. Conclusions. One of the directions of the state’s policy towards religion in the mid-1960s was the targeted liquidation of religious buildings of the Russian Orthodox Church. The main reasons for it were the ideological and political priorities of the state leadership. The closure and destruction of religious buildings were systematic actions of the party-state structures in the process of implementing their policy in the field of religion in the Ukrainian SSR. Since the Opium War, the traditional Chinese society has been increasingly subject to the crushing impact of the modern Western civilization. Confucianism, as the normative discourse that had prevailed over the Chinese society for more than two thousands years, had inevitably to face the Western challenge head-on. From the end of the nineteenth century onward, the influence of Confucianism as regards the actual life of the Chinese people continues to diminish, and its normative place has almost been completely taken up (...) by the modern social theories imported from the West. Some Chinese scholars, therefore, has declared that Confucianism has become a "wandering ghost". But at the same time, there are also many contemporary Confucians who try to find a new body for the "ghost" of Confucianism. At the academic domain, this kind of attempt is usually based upon some sort of "modern interpretation" of Confucianism. But paradoxically, this kind of modern interpretation usually borrows its interpretative frameworks from the West. It goes without saying that the respective backgrounds upon which Confucianism and those modern West theories arose are gravely different. Hence, these modern interpretations generally tend to abstract the discourse of Confucianism from its social and historical background and treat it as some sorts of purely abstract concepts. But even after the abstraction, this kind of modern interpretations could still hardly fit well into the discourse of Confucianism. The aim of the present treatise is to re-interpret the discourse of Confucius, by means of both careful scholarship and detailed philosophical arguments, based upon the words of Confucius himself and other informations concerning his social and historical surroundings, in order to show that the discourse of Confucius is intricately involved with its living background. The author is going to point out that the discourse of Confucius is actually not so much concerned with "metaphysics of morals" of whatsoever sort as with the order and disorder of the society. At the end of the treatise, the author will also try to re-evaluate, with reference to the recent results of scientific research, the significance of Confucianism in the modern world. (shrink)

I utilize the conceptual resources found within the work of liberation theologians Gustavo Gutiérrez, Enrique Dussel, and Jon Sobrino to argue that a belief in heaven is able to constitute a protest against oppressed persons’ present hell. To strengthen the connection between a believer’s heavenly hope and a commitment to worldly struggle, I examine (...) how the hope must be conceived as the completion and fulfillment of a process of temporal transformation. High-speed internet should be considered a basic right because it connects people to social media, the new public sphere. Critics worry about the politically polarizing effects of online social media, but its ability to unify, connect, and shape policy decisions should also be taken into account. Engaging with Jürgen Habermas’s early work on the public sphere, I argue that (...) the technical and cultural extension of access to social media can realize Kant’s vision of the public sphere as a bridge between morality and politics. In this book Omar Dahbour examines all of the arguments that have been given for national self-determination, whether by international lawyers, moral philosophers, democratic theorists, or political communitarians.

This book critically investigates the notion of democracy without demos by unravelling the link that modern history has established between the concepts of democracy and the sovereignty of the people. This task is imposed on us by globalization. The individualization of the subject of rights is the result of the destruction of regimes of special rights of ancient societies by the centralizing action of a territorial power. This individualization, because it implies equality, has created a new form of political subjectivity (...) that has been the driving force of the democratization of democracies during the 19th and 20th centuries. Democracy and subjective rights discusses how asserting itself as the only guarantor of rights, the modern state has also nationalized citizenship. However, the author argues, the legal and judicial monopoly of the nation-state is weakened today by the multiplication and heterogeneity of the powers on which the rights of individuals depend. This situation forces us to denationalize citizenship without sacrificing, however, the specific form of political subjectivity that the individualization of rights has made possible. (shrink)

For centuries it has been assumed that democracy must refer to the empowerment of the People's voice. In this pioneering book, Jeffrey Edward Green makes the case for considering the People as an ocular entity rather than a vocal one. Green argues that it is both possible and desirable to understand democracy in terms of what the People gets to see instead of the traditional focus on what it gets to say. The Eyes of the People examines democracy from the (...) perspective of everyday citizens in their everyday lives. While it is customary to understand the citizen as a decision-maker, in fact most citizens rarely engage in decision-making and do not even have clear views on most political issues. The ordinary citizen is not a decision-maker but a spectator who watches and listens to the select few empowered to decide. Grounded on this everyday phenomenon of spectatorship, The Eyes of the People constructs a democratic theory applicable to the way democracy is actually experienced by most people most of the time. In approaching democracy from the perspective of the People's eyes, Green rediscovers and rehabilitates a forgotten "plebiscitarian" alternative within the history of democratic thought. Building off the contributions of a wide range of thinkers--including Aristotle, Shakespeare, Benjamin Constant, Max Weber, Joseph Schumpeter, and many others--Green outlines a novel democratic paradigm centered on empowering the People's gaze through forcing politicians to appear in public under conditions they do not fully control. The Eyes of the People is at once a sweeping overview of the state of democratic theory and a call to rethink the meaning of democracy within the sociological and technological conditions of the twenty-first century. This is the first complete translation into English of the Chronicle of Theophanes Confessor, which covers the period AD 284-813 and is one of the most important sources of Byzantine history, that of the Arabs under the Umayyad and Abbasid dynasties and of other neighbouring peoples. The Chronicle is a compilation of earlier sources, many of them now lost: in order to use it critically the historian needs to know what texts Theophanes had in front of him and how he (...) handled them. These have consequently been indicated in the text in so far as they can be indentified. Full annotation has also been provided on points of historical and topographical interest. In political theory it goes without saying that the constitution of government raises a claim for legitimacy. With the constitution of the people, however, it is different. It is often dismissed as a historical question. The conviction is that since the people cannot decide on its own composition the boundaries of democracy must be determined by other factors, such as the contingent forces of history. This article critically assesses this view. It argues that like the constitution of government, the constitution (...) of the people raises a claim for legitimacy. The failure to see this is what makes many theorists run into the arms of history. They submit the legitimacy of the people to the arbitrary and asymmetrical forces of the present. (shrink)

The fall of Communism continued an ongoing weakening of Marxist ideology, which had been hegemonic among the European Left since the Great War. While the decline of Marxist thought can be justly seen negatively as the historical correlative of globalization, this decline has also produced cultural space for a re-evaluation of non-Marxist critiques of capitalist civilization. One example of a powerful non-Marxist critique of capitalist civilization is Georg Simmel's sociology of money culture. Before turning to Simmel's radical critique, this essay (...) explains how Simmel came to be viewed as a conservative. Simmel's presumed conservatism is challenged via a re-examination of the civic implications of his Nietzschean sociology. A central question is whether Simmel's ethical individualism is conservative. This question is finally answered through an analysis of Ibsen's Enemy of the People. The choice of Ibsen is not random, for it amplifies the close ethical and historical affinity of Naturalist drama and Simmel's philosophy. (shrink)
