Mechanisms of attention play a crucial role in filtering sensory inputs from the external world, allowing information to be prioritised for goal directed behaviour. To what extent might these same capacity-limited processes influence grapheme-colour synaesthesia, in which letters, numbers or words evoke concurrent experiences of colour? Asking synaesthetes themselves whether attention seems important in their experiences has provided a range of answers. On the one hand, for some synaesthetes, diverting attention can diminish the quality of their synaesthetic colours. On the (...) other, there are suggestions that synaesthetic experiences can themselves alter the manner in which attention is allocated to sensory stimuli in the environment. Here, we review a range of empirical investigations that have examined the role of attention in grapheme-colour synaesthesia. A particular focus of these studies has been the extent to which an inducing stimulus - such as an achromatic letter or digit - must be attended or consciously perceived to trigger a concurrent synaesthetic experience. In most cases, limiting attention or masking inducers tends to reduce or eliminate behavioural evidence of synaesthetic experiences. We also discuss how synaesthesia might improve performance in visual search tasks through post-attentive processes such as grouping, or by facilitating decision-making processes. (shrink)

Mechanisms of selective attention exert a powerful influence on visual perception. We examined whether attentional selection is necessary for generation of the vivid colours experienced by individuals with grapheme-colour synaesthesia. Twelve synaesthetes and matched controls viewed rapid serial displays of nonsense characters within which were embedded an oriented grating (T1) and a letter-prime (T2), forming a modified attentional blink (AB) task. At the end of the stream a coloured probe appeared that was either congruent or incongruent with the synaesthetic colour (...) elicited by the letter-prime. When the prime was attended, synaesthetes showed a reliable effect of prime-probe congruency. In contrast, when the prime appeared at 350 ms following T1 (during the AB), the congruency effect was eliminated. The requisite modification however, Removes whatever "metaphysical teeth" the unmodified version might have had. Backround Education in ethics and professionalism should reflect the realities medical students encounter in the hospital and clinic. Method We performed content analyses on Case Observation and Assessments (COAs) written by third-year medical students about ethical and professional issues encountered during their internal medicine and paediatrics clinical clerkships. Results A cohort of 141 third-year medical students wrote 272 COAs. Content analyses identified 35 subcategories of ethical and professional issues within 7 major domains: decisions regarding treatment (31.4%), communication (21.4%), professional duties (...) (18.4%), justice (9.8%), student-specific issues (5.4%), quality of care (3.8%), and miscellaneous (9.8%). Conclusions Students encountered a wide variety of ethical and professional issues that can be used to guide pre-clinical and clinical education. Comparison of our findings with results from similar studies suggests that the wording of an assignment (specifying “ethical” issues, “professional” issues, or both) may influence the kinds of issues students identify in their experience-based clinical narratives. This paper details a combined graduate/undergraduate course on the Holocaust. This course was designed to cover the legal, social, political, and moral dimensions of the Holocaust, as well as to familiarize students with its significant historical details and persons. Special attention was devoted to the question of why the perpetrators of the Holocaust should be brought to justice, making connections to contemporary forms of prejudice and discrimination and emphasizing that such efforts at justice are not an issue between Jews and (...) Holocaust perpetrators but a matter of upholding the rule of law. The specifics of the legal cases brought against various Nazis in order to try them as war criminals were examined through both modern and contemporary theoretical frameworks (e.g. early legal positivism vs. decisionism), and other key philosophical themes were introduced throughout the term in order to provide frameworks for discussion and critique. The author reviews course requirements and discusses the implicit anti-Semitism expressed by students, concluding with a brief consideration of the value of such a course beyond the classroom. Actuarial underwriting, or discrimination based on an individual's health status, is a business feature of the voluntary private insurance market. The term “discrimination” in this paper is not intended to convey the concept of unfair treatment, but rather how the insurance industry differentiates among individuals in designing and administering health insurance and employee health benefit products. Discrimination can occur at the point of enrollment, coverage design, or decisions regarding scope of coverage. Several major federal laws aimed at regulating insurance discrimination (...) based on health status focus at the point of enrollment. However, because of multiple exceptions and loopholes, these laws offer relatively limited protections. This paper provides a brief overview of discrimination practices, the federal law, and federal reform options to manage discriminatory practices in the insurance and employee health benefit markets. This article reviews the relationship between managed care and public health. Managed care, with its seemingly infinite structural and organizational variation, dominates the modern American health-care system for the non-elderly U.S. population. Through its emphasis on standarhzed practice norms and performance measurement, coupled with industrial purchasing techniques, prepayment, risk downstreaming, and incentives-based compensation, managed care has the potential to exert considerable influence over the manner in which the health-care system is organized and functions. Given the degree to which the attainment (...) of the basic public health goal of protecting the public against population health threats for which there are known and effective medical interventions depends on the successful interaction between public health policy and the medical care system, the importance of a viable working relationship between public health and managed care is difficult to overstate.The potential for conflict between public health and medical care is nothing new; indeed, delineating the boundaries of public health to shape and influence medical practice has occupied the energies of policymakers and the medical industry for well over a century. The nascent for-profit psychedelic industry has begun to engage in corporate practices like funding scientific research and research programs. There is substantial evidence that such practices from other industries like tobacco, alcohol, pharmaceuticals and food create conflicts of interest and can negatively influence population health. However, in a context of funding pressures, low publicly funded success rates and precarious academic labor, there is limited ethics guidance for researchers working at the intersection of clinical practice and population health as to how (...) they should approach potential financial sponsorship from for-profit entities, such as the psychedelic industry. This article reports on a reflective exercise among a group of clinician scientists working in psychedelic science, where we applied Adams’ (2016) PERIL (Purpose, Extent, Relevant harm, Identifiers, Link) ethical decision-making framework to a fictionalized case of corporate psychedelic financial sponsorship. Our analysis suggests financial relationships with the corporate psychedelic sector may create varying degrees of risk to a research program’s purpose, autonomy and integrity. We argue that the commercial determinants of health provide a useful framework for understanding the ethics of industry-healthcare entanglements and can provide an important population health ethics lens to examine nascent industries such as psychedelics, and work toward potential solutions.
