Nursing on paper: therapeutic letters in nursing practice This paper offers a selected piece of interpretive research extracted from the context of a larger research study. The hermeneutic research inquiry described in this paper involved the examination of the nursing and family therapy intervention of therapeutic letters. It incorporated the textual interpretation of 11 therapeutic letters, clinical sessions with three families, clinical team discussions, and research interviews with four family members and three nurse clinicians (...) who participated in the writing of the letters. This particular paper extracts segments of the research related to the letters received by two participants, as well as some general findings, with a focus on the possibilities and influences of therapeutic letters in nursing practice. The findings of this research offer suggestions, not as a template, but as an inspiration and evocation to write therapeutic letters that address the obligation of meeting people experiencing illness at the point of their suffering, with words and questions that invite relationship, reflection, and are large enough to sustain a meeting. Therapeutic letter writing has been viewed by psychologists as a powerful form of creative writing in health care settings. I explore the cultural contexts that have aided its popularization to shed fresh light on debates about its psychological function and efficacy. I draw on the sociologist Frank Furedi’s analysis of ‘therapy culture’ to argue that contemporary ideologies of the vulnerable self have stimulated this practice, particularly in the form of letters written not-to-be-sent. I conclude by considering models of (...) developmental letter writing that attempt to challenge these ideologies, including narrative therapists’ provocative method of corresponding with clients. The few people familiar with the pseudo-Platonic dialogue Axiochus generally have a low opinion of it. It's easy to see why: the dialogue is a mish-mash of Platonic, Epicurean and Cynic arguments against the fear of death, seemingly tossed together with no regard whatsoever for their consistency. As Furley notes, the Axiochus appears to be horribly confused. Whereas in the Apology Socrates argues that death is either annihilation or a relocation of the soul, and is a blessing either way, "the (...) Socrates of the Axiochus wants to have it both ways": death is both annihilation and a release of the soul from the body into a better realm. This may be used to construct a valid argument for the conclusion that death is not evil, but at the expense of having a contradiction as one of its premises. But D. S. Hutchinson has recently proposed that these inconsistencies shouldn't surprise us if we view the Axiochus as "an unconventional version of a very conventional genre--the consolation letter." In this paper I expand on Hutchinson's brief suggestion and argue that the Axiochus can be rehabilitated by paying attention to its genre. Although the Axiochus does display many similarities to the consolation letter, the shift from letter to dialogue does--pace Hutchinson--significantly affect what's going on. Within the dialogue, Socrates behaves toward Axiochus in a way similar to the way the author of a consolation letter behaves towards the letter's reader: he is willing to use inconsistent arguments, borrowed from any source, in order to soothe the patient. However, in depicting this type of consolatory relationship between Socrates and Axiochus, the dialogue itself is not aiming at consoling its readers. Instead, it should be seen as displaying for the reader's consideration a certain type of consolatory argumentative practice. -/- Socrates notes that Axiochus is "very much in need of consolation" (365a), and he uses any means necessary to accomplish this task. Socrates exhibits many ways in which he is willing to sacrifice argumentative hygiene for the sake of therapeutic effectiveness. These include: -/- * Use of arguments with inconsistent premises, presented in propria persona. * Appeals to emotion * Tailoring arguments to the audience. * Presenting invalid arguments so as to induce unjustified but comforting beliefs. * Evasion. In these respects, I think that Socrates' argumentative practice is best compared to PH III 280-1, where Sextus Empiricus says that the skeptic will deliberately use logically weak arguments as long as they work. Dorothy Tarrant claims that what links the Socrates of the Axiochus to Socrates as he appears elsewhere in the Platonic corpus is his evident care for the welfare of his interlocutor's psyche. But this concern takes a quite different form in the Axiochus than it usually does. As with Sextus, psychic therapy in the Axiochus involves relief from pain. The primary difference between them is that Socrates, unlike Sextus, is not aiming at producing epochê in his patient. (shrink)

This paper analyses the problem of shame in the thinking of Lucius Annaeus Seneca. The authors examine this problem primarily in two contexts. The first, social meaning, understands shame as an emotion that appears during a conflict between a person’s “self” and social norms. Seneca mainly tackles this question concerning providing “benefits” (beneficia) in his On Benefits and eighty-first letter of Moral Epistles. The second therapeutic and didactic meaning utilises shame as an instrument to manage some illnesses of the (...) mind (for example, anger). Moreover, to educate a “Stoic disciple” (proficiens), deliberate exposure to shame in public marks one of the important techniques to near the perfect ideal of “the wise man” (sapiens) and to prepare oneself for service in public office. Especially in Moral Epistles, we find a detailed description of this didactic approach and the limiting factors to using this technique. (shrink)

Language-related potentials are increasingly used to objectify adaptive neuroplasticity in stroke-related aphasia recovery. Using preattentive [mismatch negativity ] and attentive phonologically related paradigms, neuroplasticity in sensory memory and cognitive functioning underlying phonological processing can be investigated. In aphasic patients, MMN amplitudes are generally reduced for speech sounds with a topographic source distribution in the right hemisphere. For P300 amplitudes and latencies, both normal and abnormal results have been reported. The current study investigates the preattentive and attentive phonological discrimination ability in (...) 17 aphasic patients at two timepoints during aphasia recovery. Between the two timepoints, a significant improvement of behavioral language performance in both languages is observed in all patients with the MMN latency at timepoint 1 as a predictive factor for aphasia recovery. In contrast to monolinguals, bilingual aphasic patients have a higher probability to improve their processing speed during rehabilitation, resulting in a shortening of the MMN latency over time, which sometimes progresses toward the normative values. The interval between two notes may be expressed as the rate of vibration of the higher note divided by the rate of vibration of the lower note. This ratio (...) is very roughly the same as the length of the air column for the lower note divided by the length of the air column for the higher note. This gross over-simplification has been used for all calculations in this article. The width of the air column, the size of the holes, and the reed itself affect the pitch considerably. AimsThis preliminary study aimed to investigate therapy-induced electrophysiological changes in persons with primary progressive aphasia. The investigated event-related potential components associated with language processing were the mismatch negativity, P300, N400, and P600.MethodsA linguistic ERP test battery and standardized language assessment were administered in four patients with PPA of which two received speech-language therapy and two did not receive therapy. The battery was administered twice with approximately 6 months in between in each patient. The results of the follow-up assessments were compared (...) to the results of the initial assessments.ResultsAlthough the results of the behavioral language assessment remained relatively stable between the initial and follow-up assessments, changes in the mean amplitudes, onset latencies, and duration of the ERP components were found in the four patients. In the two patients that did not receive SLT, an increased delay in 50% and a decreased mean amplitude in 25% of the measured ERP components were found. The electrophysiological changes found in the patients that received SLT were variable. Interestingly, the mismatch negativity and the N400 effect elicited by the categorical priming paradigm were less delayed and had an increased mean amplitude at the follow-up assessment in the patient with the non-fluent variant who received SLT. In this patient, the P600 component was absent at the initial assessment but present at the follow-up assessment.ConclusionAlthough no clear patterns in electrophysiological changes between patients who received SLT and patients who did not receive SLT were found by our preliminary study, it seems like the SLT induced improvements or compensation mechanisms in some specific language comprehension processes in the patient with the NFV. The results of this study are still preliminary because only four heterogeneous patients were included. Future studies should include larger patient groups of the three clinical variants because the therapy-induced electrophysiological changes might differ depending on the clinical variant and the underlying pathology. Holdsworth (bio)On 18 September 2008, the U.S. Food and Drug Administration (FDA) issued a draft set of guidelines for those involved in developing genetically engineered animals with heritable recombinant DNA (rDNA) constructs and is requesting comment from industry and the public about their content. The document does not impose new regulations but details (...) how existing laws and regulations apply to genetically engineered (GE) animals and offers advice to developers on how to negotiate the laws in the most efficient and productive manner. The draft guidance document is not legally enforceable at this juncture, but the guidelines are recommendations reflecting the FDA's "current thinking on a topic."The modification of GE animals being developed for commercialization, which are the focus of the document, fall into six categories: modifications intended to (1) enhance food quality—e.g., faster growing fish and meats with better nutritional profiles; (2) improve animal health via increased resistance to disease—e.g., mastitis in dairy cattle and Bovine Spongiform Encephalopathy (BSE); (3) create so-called "biopharm" animals capable of producing therapeutic products such as insulin, clotting factors, and tissues for human transplantation; (4) reduce the barriers to human animal interaction—e.g., hypoallergenic pets; (5) develop valid animal models of human disease—e.g., Parkinson's disease, Lesh-Nyhan's syndrome, and cancer; and (6) improve production of industrial or consumer products such as fiber. There are some data to suggest that although the concerns of scientists fall primarily into the category of possible biohazards, those of the public also reflect great concern about the intrinsic moral issues, such as whether there is something fundamentally wrong with this line of work, making it a form of forbidden knowledge (see, e.g., Gaskell 1997). When news of Paul Berg's attempts in 1970 to graft the primate tumor virus (SV 40) onto the intestinal tract bacteria Escherichia coli for the purpose of studying how normal cells may be transformed into cancerous cells was shared with colleagues, he was strongly encouraged to reflect on the potential public health risks if somehow the altered bacteria broke containment and entered the environment. Thanks to Berg's leadership, these concerns soon resulted in conferences held in Asilomar, California, and New Hampton, New Hampshire, where the ethical responsibilities of microbiologists involved in this research were discussed. These initial discussions resulted in what has come to be known as "The Moratorium Letter" (Shattuck 1996) published in the journal Science in 1974. This letter, signed by 11 leaders in the field, actually recommended restrictions on several types of experiments that were considered to be particularly risky and stated "that adherence to our major recommendations will entail postponement or possibly abandonment of certain types of scientifically worthwhile experiments" (Berg 1974). We do not, for obvious reasons, have anything of Freud's on Sartre, but we do have quite a lot of Sartre on Freud, and great quantities of Sartre on Sartre. It has sometimes seemed to me that reading through everything that Sartre wrote—not just the autobiographical material but everything, including (...) the carnets and the cahiers and the letters—might be a bit like having him in analysis. The speed and apparent openness with which he produced his texts, page after page in that quick yet legible script that French writers seem to turn out so effortlessly, mimic some of the conditions of free association, and an analytically sensitive eye, like the analyst's ear in therapeutic sessions, could no doubt piece together a plausible account of the Sartrean unconscious. When the University of Toronto withdrew a contract it held with me in December 2000, it initiated a sequence of events that led to a public letter to the University from senior figures in the world psychopharmacology community protesting against the infringement of academic freedom involved and a first ever legal action, undertaked by this author, seeking redress for a violation of academic freedom. The issues of academic freedom surrounding this case have been intertwined with a debate about the possibility (...) that the selective serotonin reuptake inhibitor (SSRI) group of antidepressants have the potential to trigger suicidality in a subgroup of patients. Whether the SSRIs do trigger suicidality or not, exploration of this issue has given rise to a number of worrying sets of observations. First, in my view, there is evidence that pharmaceutical companies have miscoded raw data on suicidal acts and suicidal ideation. Second, this author also maintains that there is a growing body of examples of ghostwriting of articles in the therapeutics domain. Many of the tensions evident in this case, therefore, can be linked to company abilities to keep clinical trial data out of the public domain — this is the point at which the pharmaceutical python gets a grip on academia. Volume 17 of Progress in Self Psychology, _The Narcissistic Patient Revisited_, begins with the next installment of Strozier's "From the Kohut Archives": first publication of a fragment by Kohut on social class and self-formation and of four letters from his final decade. Taken together, Hazel Ipp's richly textured "Case of Gayle" and the commentaries that it elicits amount to a searching reexamination of narcissistic pathology and the therapeutic process. This illuminating reprise on the clinical phenomenology Kohut associated with (...) "narcissistic personality disorder" accounts for the volume title. The ability of modern self psychology to integrate central concepts from other theories gains expression in Teicholz's proposal for a two-tiered theory of intersubjectivity, in Brownlow's examination of the fear of intimacy, and in Garfield's model for the treatment of psychosis. The social relevance of self psychology comes to the fore in an examination of the experience of adopted children and an inquiry into the roots of mystical experience, both of which concern the ubiquity of the human longing for an idealized parent imago. Among contributions that bring self-psychological ideas to bear on the arts, Frank Lachmann's provocative "Words and Music," which links the history of music to the history of psychoanalytic thought in the quest for universal substrata of psychological experience, deserves special mention. Annette Lachmann's consideration of empathic failure among the characters in Shakespeare's _Othello_ and Silverstein's reflections on Schubert's self-states and selfobject needs in relation to the specific poems set to music in his Lieder round out a collection as richly broad based as the field of self psychology itself. The Philadelphia Association is always linked with the name of R. D. Laing, one of its founders, but very little is known about its unorthodox contribution to the development of psychoanalysis. Founded in 1965, it took as its aim the relief of mental illnesss of all desciptions, in particular schizophrenia. At its inception it was a focus for people, with a diversity of backgrounds and interests, concerned with 'mental illness' and how society defines it. Its members - who included psychoanalysis, (...) psychiatrists and artists - developed a philosophical critique of psychoanalysis grounded in phenomenology, and an 'anti-psychiatry' critique of the institutional treatment of the mentally ill. They favoured the setting up of therapeutic community households of which the best known was Kingsley Hall. The PA was then very much part of the counter culture - it staged, for example, the famous Dialectics of Liberation Congress at London's Roundhouse in 1967. The psychotherapists who currently belong to the Philadelphia Association continue to work with community households and to offer a psychotherapy training. Here the authors share their innovative and distinctive approach on a number of interrelated themes: the herapeutic community and differend ways it can be understood; work with deeply regresed patients; the concept of identity in a social context; the debate between Lacan and Derrida on Poe's 'The Purloined Letter'; the phenomenology of knowledge and transference and authority. Together, they offer a cogent critique of many psychiatric and psychonalaytic assumptions. The contributors are Robin Cooper, Joseph friedman, Steven Gans, John Heaton, Chris Oakley, Haya Oakley and Paul Zeal. They are all psychotherapists and members of the Philadelphia Association. Psychoanalytic work with children is popular, but the sophisticated language used in psychoanalytic discourse can be at odds with how children communicate, and how best to communicate with them. _Dialogues with Children and Adolescents: A Psychoanalytic Guide _shows how these aims can be achieved for the most effective clinical outcome with children from infancy up to late adolescence. _Björn Salomonsson_ and _Majlis Winberg Salomonsson_ draw on extensive case material which reveals the essence of communication between child and therapist. These terms and theories help to explain a child’s behaviour, the analyst’s technique and the background to the disorder. This is new creative development in child therapy and analysis which is written in a very accessible style. _Dialogues with Children and Adolescents_ will be essential reading for beginners in psychoanalytic work with children and will cast a fresh light on such work for more experienced clinicians. An abundance of data unequivocally shows that exercise can be an effective tool in the fight against obesity and its associated co-morbidities. Indeed, physical activity can be more effective than widely-used pharmaceutical interventions. Whilst metformin reduces the incidence of diabetes by 31% (as compared with a placebo) in both men and women across different racial and ethnic groups, lifestyle intervention (including exercise) reduces the incidence by 58%. In this context, it is notable that a group of prominent medics and exercise (...) scientists recently sent a well-publicised letter to the General Medical Council and Medical Schools Council calling for the introduction of evidence-based lifestyle education into all medical curricula. The letter warns that there is a lack of understanding of the impact that exercise and nutrition can have on physical health amongst doctors. In the absence of an educational overhaul, the signatories warn that the government is likely to fail to reach its goal of preventing tens of thousands of premature deaths from heart disease and cancer by 2020. Whilst we agree with the need to address this apparent lack of understanding, we argue that the ethical justification of doing so is not limited to this broadly beneficence-based justification. There is also a justification grounded in the duty of non-maleficence, that is, the duty to avoid unreasonably harming patients. This duty requires that doctors refrain from (i) preventing patients from undergoing beneficial treatment without good reason, (ii) exposing patients to unreasonable risks and (iii) reducing the therapeutic effect of an effective medical intervention. This requires an understanding of the physical impact of exercise. (shrink)

In contemporary poetry, devices serving the purpose of creative destruction are increasingly coming into use. Usually not considered as proper to art making, these tools create by dismantling what is already there rather than generating something entirely new from nothing. Such methods of text deletion are at stake in so-called »erasure poetry«. Erasure poetry designates a mode of poetic production, in which an existing text is edited with a black felt marker or painted over with other suitable office supplies according (...) to a set of given parameters. This essay explores what happens when texts are no longer written but crossed out. Examples from a variety of contexts of use illustrate both the wide spectrum of the phenomenon and different functions of erasure. As a collective practice the technique may be therapeutic or simply helpful in coping with life. However, as an artistic strategy, erasure is currently deployed in many poetry collections and artists’ books that question common ways of engaging with literature as well as certain practices of power. Here, erasure emerges as an aesthetic strategy that can in fact reveal contents that have been concealed, negated, or threatened with oblivion. Both in terms of production and of reception, the decisive aesthetic feature in such cases is a break with conventional modes of representation. This break manifests itself in the ways the remaining words or letters are arranged on the page and put into relation to the empty space that surrounds them: Word placement and white space management are the two central operations of post-avant-garde literature and its page design. The term “patent” is an abbreviation of “letters patent”, the open form of document historically issued by the Crown for the purpose of conferring a right or privilege or otherwise communicating the royal will. In contemporary law it denotes the species of intellectual property that is granted as an inducement for the creation and disclosure of novel, inventive and industrially applicable inventions. In the UK that property is conferred under the Patents Act 1977, or with similar effect the European (...) Patent Convention (EPC), and comprises the exclusive right to use and sell a protected invention for up to 20 years. A protected invention may be any subject matter having technical character; a definition derived from the Act’s express exclusion from patentability of (among other things) discoveries, scientific theories, business methods and computer programs “as such”. Also excluded are surgical, therapeutic and diagnostic methods (for lack of industrial applicability), and inventions the commercial exploitation of which would be contrary to public policy or morality. In these and other respects the Patents Act follows the EPC, which it was the explicit purpose of that Act to implement. Patents are territorial, and are thus only valid and infringed in their country of issue. For a UK inventor to obtain protection outside the UK s/he must therefore obtain a patent from that country, either directly by applying under its laws, or if it is a party to the EPC or other intergovernmental agreement, indirectly by applying under that agreement. Changes in the nature and conception of philosophy reflect changes in the modes in which philosophy was pursued during the first thousand years of the formation and influence of Aristotle’s philosophy. In the Hellenic period, philosophy consisted of inquiry and discussion, oral or written, in expositions or dialogues. Recording other positions, past or present, that is, history, was part of both modes of philosophizing. In the Hellenistic period, philosophy moved into schools and libraries and became scholastic and scholarly. The schools (...) interpreted doctrines and methods, and the libraries edited books and classified the branches of knowledge and letters. The history of philosophy became records of the opinions of philosophers and the successions of schools, or histories of sciences and arts. Aristotelianism was formed among the Hellenistic schools of philosophy, and members of the Peripatetic school wrote the first histories of science. Aristotle’s works were not needed in either enterprise. His books were preserved, lost, forgotten, and emended, and editors determined their authenticity, contents, structure, and order. In the period of the Roman Empire, philosophy was rejoined to rhetoric, dialectic, and sophistic. It was the period of the Second Sophistic, during which political and forensic rhetoric declined, the Platonic dialectic of dialogues and Ideas was transformed into the Neoplatonic dialectic of hierarchies and the One, and Sophistic assumed therapeutic functions in conjunction with medicine, and theatrical and mass-educational functions as epideictic rhetoric. The works of Aristotle were epitomized and interpreted, and Aristotelian words and ideas entered into the formation of a new organization of the liberal arts, a new transcendental theology, and new forms of literature and literary criticism. From his stance within Buddhist tradition, Shinran identifies the crucial problem as the human tendency toward the reification of both sides (...) of this dichotomy-resulting in an autonomous self and substantial things-with the consequent attachments fueling the afflictions ("blind passions") of habitual pain and conflict for oneself and others. The focus of Shinran's treatment of this basic Buddhist concern is the point of engagement with dharma or true reality. In this, he draws into the arena of religious faith the probing scrutiny of lingering attachments to self, even in the dedicated practitioner, that is fundamental to the critical attitude of Mahayana Buddhist tradition.My aim in taking up the issue of truth in my article "Shinran and Heidegger on Truth" in The Boundaries of Knowledge in Buddhism, Christianity, and Science 1 is to suggest a way of reading Shinran that both avoids a common pitfall that Shinran himself cautions against-an engagement with Pure Land teachings that adheres to the subject-object dichotomy of ordinary awareness-and opens up his vision to resonances with contemporary western thought, particularly continental philosophy. This latter concern is not to claim that Shinran adumbrates contemporary thought or is affirmed by resemblances, but rather seeks to cast light on compelling aspects of his thought not often treated in traditional Shin scholastics. There are, therefore, both therapeutic and constructive sides in my attempt to engage Martin Heidegger's (1889-1976) thought from a perspective rooted in Shinran's Buddhism.The pitfall in reading Shinran indicated in my article is crystallized in the assumption, prevalent in Western scholarship but also widely seen in Japan, that his Shin Buddhist path centers on a simple faith in the Pure Land teachings, with such faith understood as the subjective acceptance of the truth of doctrinal propositions. In [End Page 189] other words, it is commonly presupposed that Shin Buddhist tradition (what he terms jōdo shinshū, "the true essence of the Pure Land way") implicitly affirms the subject-object dichotomy. This difficulty is frequently compounded by formulations of the supposed tenets of belief that are heavily molded and colored by Protestant Christian notions of self, sin, eschatology, and atonement. It is impossible to take up such comparative theological issues here. 2 In a sense, I have sought in my article to circumvent a hasty assessment of conceptual equivalence by pointing out the possibility of an encompassing or prior dissonance.Since Shinran's concept of "attaining shinjin" (true entrusting, which is also the Buddha-mind given to the practitioner) is built on the endeavor to deconstruct the reification of this dichotomy in engagement with the Pure Land Buddhist path, the unconsciousness reinstatement of it in reading his writings can lead one to a thorough but easily accomplished misconstrual of his thought. My suggestion in my article is that consideration of the analogues in Heidegger's thought can help us see all this. Such comparison also appears to present a relatively untested avenue for Buddhist-Christian conversation, particularly in view of Heidegger's early indebtedness to Luther's "theology of the cross" and the notion of the "alien work of God" as the critical stripping away of attachments to human capacities (destructio in the "Heidelberg Disputation"), and also his approach to Paul's letters to the Thessalonians in The Phenomenology of Religious Life. 3 The keystone in such interreligious conversation remains, however, not simply human fallibility as subject or culpability as agent, but the persistent challenge to the subject-object dichotomy, which may form a locus of refractory difference between the religious traditions.The constructive side of the comparison concerns two general dimensions of Shinran's thought that are highlighted in an approach to his writings that might be called phenomenological. In this paper I argue that Seneca’s philosophy is a form of therapy and that one of its main concerns is the transformation of one’s life through time control. Aristotelian tradition lies in the idea that philosophy is, in its highest aspect, an abstract form of knowledge. Seneca, on the other hand, is an inheritor of a long tradition that takes philosophy as mind or soul therapy and bases its structure in a practical approach. Epicurus, for instance, goes as far (...) as to declare that “empty is the word of the philosopher by whom no human suffering is treated”. Even if Seneca doesn’t despise the theoretical principles that support the construction of a philosophical system, he builds, in his Letters to Lucilius, a strong therapeutic praxis whose purpose is to heal one’s mind and life. The upmost and fundamental step is providing the pupil with different ways of earning back time deprivation. Time seems to be, in a certain sense, the essential matter of Seneca’s philosophy. Time control, for him, is a kind of self-control that would make it possible to match a simple existence and a meaningful life. An article by Mark Repenshek and a letter by Edward Delaquil published recently in The National Catholic Bioethics Quarterly underscore the urgent need for further moral and magisterial clarification regarding a number of highly complex and difficult bioethical issues. These involve ex utero therapeutic genomic interventions, the practice of in vitro fertilization and embryo transfer, and the ongoing debate over the morality of embryo adoption to help resolve the “absurd” fate of countless, cryopreserved human embryos.
