Agouti expression during the middle portion of the mouse hair growth cycle induces melanocytes to synthesize yellow instead of black pigment, generating black hairs with a yellow band. Dominant agouti alleles increase the amount of yellow pigment in the coat and are associated with pleiotropic effects including obesity, diabetes and increased tumor susceptibility. Four dominant agouti alleles (Aiapy, Aiy, Asy and Avy) were recently shown to result from insertions that cause ubiquitous expression of chimeric transcripts encoding a wild‐type agouti protein(1,2). SummaryIntense, sustained nursing lengthens inter-birth intervals and is causally linked with low natural fertility. However, in traditional settings, the effects of such nursing on fertility are difficult to disentangle from those of nutrition. Results from an prospective, direct observational study of reproductive function in well-nourished Amele women who nurse intensively and persistently but who also have high fertility are here presented. Endocrine measures show that ovarian activity resumes by median 11·0 months postpartum. Median duration of postpartum amenorrhoea is 11·3 months, (...) time to next conception is 19·0 months, and the inter-birth interval is 28·0 months. Average life time fertility is 6·8. High fertility in Amele women is due both to refractoriness of reproductive function to suckling stimuli, and to maintenance of equivalent age-specific fertility rates across the reproductive life span. Context: The need to reinvigorate medical confidentiality protections is recognised as an important objective in building patient trust necessary for successful health outcomes. Little is known about patient understanding and expectations from medical confidentiality.Objective: To identify and describe patient views of medical confidentiality and to assess provisionally the range of these views.Design: Qualitative study using indepth, open ended face-to-face interviews.Setting: Southeastern Pennsylvania and southern New Jersey, USA.Participants: A total of 85 women interviewed at two clinical sites and three community/research centres.Main (...) outcome measures: Subjects’ understanding of medical confidentiality, beliefs about the handling of confidential information and concerns influencing disclosure of information to doctors.Results: The subjects defined medical confidentiality as the expectation that something done or said would be kept “private” but differed on what information was confidential and the basis and methods for protecting information. Some considered all medical information as confidential and thought confidentiality protections functioned to limit its circulation to medical uses and reimbursement needs. Others defined only sensitive or potentially stigmatising information as confidential. Many of these also defined medical confidentiality as a strict limit prohibiting information release, although some noted that specific permission or urgent need could override this limit.Conclusions: Patients share a basic understanding of confidentiality as protection of information, but some might have expectations that are likely not met by current practice nor anticipated by doctors. Doctors should recognise that patients might have their own medical confidentiality models. They should address divergences from current practice and provide support to those who face emotional or practical obstacles to self-revelation. (shrink)

Certified nursing assistants (CNAs) provide the majority of direct care to nursing home residents in the United States and, therefore, are keys to ensuring optimal health outcomes for this frail older adult population. These diverse direct care workers, however, are often not recognized for their important contributions to older adult care and are subjected to poor working conditions. It is probable that social‐based discrimination lies at the core of poor treatment toward CNAs. This review uses perspectives from critical social theory (...) to explore the phenomenon of social‐based discrimination toward CNAs that may originate from social order, power, and culture. Understanding manifestations of social‐based discrimination in nursing homes is critical to creating solutions for severe disparity problems among perceived lower‐class workers and subsequently improving resident care delivery. This paper opens with first‐hand accounts of critical care medical interventions in which detainees, in the custody of U.S. Immigration and Customs Enforcement (ICE), are brought to the emergency department for treatment. This case dramatizes the extent to which the provision of ethical and acceptable nursing care is jeopardized by federal law enforcement paradigms. Drawing on the scholarship of Michel Foucault and Giorgio Agamben, this paper offers a theoretical account of the power dynamics that inform the health care of patients (...) who find themselves caught in the custodial scaffolding of a vast immigration and detention apparatus. It offers an analysis of the display of sovereign and biopolitical power over the lives (and deaths) of detainees (Foucault), as well as the ways these individuals are reduced to “bare life” under the political pretext of an emergency or “state of exception” (Agamben). Our purpose here is both theoretical and practical: to better understand the often hidden agency or impersonal “will” exercised by the immigrant detention system, but also to equip clinicians in these and cognate facilities (e.g., prisons) with the critical tools by which they might better navigate incommensurable paradigms (i.e., care vs. custody) in order to deliver the best care while upholding their ethical duties as a care provider. This is all the more pressing because hospitals are not sanctuaries and given the incursion of federal law enforcement agents, nurses may find themselves conscripted as de facto agents of the state. In its considered treatment of energy options and policy responses, [End Page 198] Climate Justice works to make applied Christian ethics competent to (...) address climate change. By interpreting political possibilities with Christian commitments to justice, the book makes a distinctively practical contribution to the literature on Christianity and climate; it lifts an important theological voice in the growing subfield of climate ethics.The book begins by elaborating the general notion of ecological justice into workable criteria. Martin-Schramm describes four basic norms of ecojustice (sustainability, sufficiency, participation, and solidarity), from which he develops twelve energy policy guidelines (e.g., equity, adequacy). He then specifies those guidelines into fourteen criteria by which to assess the justice of climate action (e.g., historical responsibility, emissions verifiability).Subsequent chapters use those criteria to evaluate policy options. A chief difficulty in applying them lies in the tension between intergenerational and intragenerational claims. Global climate negotiations founder on disagreements about these undeveloped, plural dimensions of justice. Martin-Schramm therefore deploys criteria with a balancing objective: to develop obligations to the future while recognizing claims of the present poor and vulnerable, and while keeping the result within the bounds of North American political viability.For example, Martin-Schramm recognizes that the criterion of historical responsibility indicates that the United States should bear about one-third of the costs of climate transition. But he finds that the resulting figure ($331 billion) so outstrips realistic acceptance that he rejects financial transfer in favor of a “national obligation wedge” in which the nation compensates by pledging to reduce future emissions. Since those emission reductions are unlikely apart from a price on carbon, Martin-Schramm argues that justice mandates a fossil fuel tax. With an eye to political viability, he proposes beginning with a modest $5/ton (for comparison, the Stern Report estimates the social costs of carbon at $85/ton).Controversy in the details matters to the whole project because the point of Martin-Schramm’s approach is that a meaningful climate ethic should guide contextual action. Yet other accounts of climate justice will find a scandal of modesty on those contextual points. Some authors sustain that historical research is an effect of a specific historiographical context (Jenkins, 1991; González de Oleaga, 2009). An approach to the historiographical debate between constructivism and recontructivism is presented in this paper. Two theses are here defended. The first one affirms that the above mentioned debate is deeply related to epistemological questions (study of mental representations, different conceptions about historical reasoning functions, historical reasoning, cognitive bias, and informal falacies). The second thesis affirms that each historiographical conception (...) can be understood as the effect of assuming a specific perspective about these epistemic questions. As an evidence of this, some connections between historiography and epistemology will be analysed through the analogy between the reconstructivism vs. constructivism debate, and the epistemological debate detectivism vs. constitutivism (Finkelstein, 2003). In a recent article Duncan Purves, Ryan Jenkins, and B. J. Strawser argue that in order for a decision in war to be just, or indeed the decision to resort to war to be just, it must be the case that the decision is made for the right reasons. Furthermore, they argue that this requirement holds regardless of how much good is produced by said action. In this essay I argue that their argument is flawed, in that it mistakes (...) what makes an agent morally good for what makes an act morally good. I argue that the main thrust of Purves et al.’s argument in fact undermines the conclusion they wish to draw, and that the reasons for one’s action do not make an in principle difference to the morality of actions in war. I further argue that this position undermines the traditional ad bellum just war constraint of right intention, and that the morality of actions in war is, at core, only concerned with outcomes. I conclude by clarifying that one’s reasons for action do in fact matter when deciding to enter war or kill in war, but only because one’s reasons significantly impact the way in which one acts. The purpose of this paper is to clear the theoretical space by showing why intentions/reasons do not in principle matter when assessing the morality of war (or killing), but this should not be taken as an argument that we should ignore intentions/reasons altogether. I argue for three claims. (1) The phenomenology of visual experience is exhausted by awareness of appearance properties (i.e., certain constantly changing characteristics of external objects that are relational and viewpoint‐dependent). (2) Cognition differs from perception in that it has a purely discursive or linguistic dimension, whereas perception is pervasively analog and iconic; but this does not determine a border between the two domains, for cognition also has a massive iconic dimension. And (3) certain raging debates in teleosemantics can be (...) resolved by acknowledging that perceptual representations in more primitive organisms tend to have dual contents (e.g., both small, moving black object, and food). Recently Carrie S. Jenkins formulated an epistemology of mathematics, or rather arithmetic, respecting apriorism, empiricism, and realism. Central is an idea of concept grounding. The adequacy of this idea has been questioned e.g. concerning the grounding of the mathematically central concept of set (or class), and of composite concepts. In this paper we present a view of concept formation in mathematics, based on ideas from Carnap, leading to modifications of Jenkins’s epistemology that may solve some problematic issues with (...) her ideas. Despite its place in the humanities, the career prospects and numbers of women in philosophy much more closely resemble those found in the sciences and engineering. This book collects a series of critical essays by female philosophers pursuing the question of why philosophy continues to be inhospitable to women and what can be done to change it. By examining the social and institutional conditions of contemporary academic philosophy in the Anglophone world as well as its methods, culture, and characteristic commitments, (...) the volume provides a case study in interpretation of one academic discipline in which women's progress seems to have stalled since initial gains made in the 1980s. Some contributors make use of concepts developed in other contexts to explain women's under-representation, including the effects of unconscious biases, stereotype threat, and micro-inequities. Other chapters draw on the resources of feminist philosophy to challenge everyday understandings of time, communication, authority and merit, as these shape effective but often unrecognized forms of discrimination and exclusion. Often it is assumed that women need to change to fit existing institutions. This book instead offers concrete reflections on the way in which philosophy needs to change, in order to accommodate and benefit from the important contribution women's full participation makes to the discipline. Our gender identity is our sense of ourselves as a woman, a man, as genderqueer, or as another gender. Our gender is the property we have of being a woman, being a man, being non-binary, or being another gender. What is the relationship between our gender identity and our gender? Recently, much work has been done on ameliorative accounts of the gender concepts that we should accept and on the metaphysics of gender properties. From this work 4 views of the (...) relationship between having gender identity G (e.g. having a female gender identity) and being a gender G (e.g. being a woman) have emerged. A first, the no connection view, says that gender identity and gender are entirely distinct; Sally Haslanger’s ameliorative account of gender, as well as Theodore Bach’s account of gender properties, are instances of this view. A second, the gender-identity-first view, says that all there is to our gender is our gender identity; this view is at least similar to views that Talia Bettcher and R.A. Briggs and B.R. George have argued for. A third, contextualist view, says that in some contexts and relative to certain standards, our gender identity determines our gender, and that in others it does not; Ásta and Robin Dembroff have proposed and defended views along these lines. A fourth view, the two gender properties view, says that there are two properties that can make it the case that one is gender G (in any context), one’s having gender identity G and one’s being socially classed as a G; Katharine Jenkins and Elizabeth Barnes have proposed views along these lines. This article explains these 4 different views and spells out their prospects, and the problems they face, as both ameliorative and as metaphysical views. (shrink)

The image of the hospital is presented to the public as a place of healing. Though the oft‐criticized total institutions of the past have been notably dismantled, the totalizing practices therein are now operationalized in the health care system. Through the lens of Erving Goffman, this article offers ways in which health care institutions operationalize totalizing practices, contributing to the mortification of patients and nurses alike in service to the bureaucratic machine. In Boghossian's 1997 paper, 'Analyticity' he presented an account of a prioriknowledge of basic logical principles as available by inference from knowledge of their role in determining the meaning of the logical constants by implicit definitiontogether with knowledge of the meanings so-determined that we possess through ourprivileged access to meaning. Some commentators (e.g. BonJour (1998), Glüer (2003),Jenkins (2008)) have objected that if the thesis of implicit definition on which he relieswere true, knowledge of the meaning of the constants would (...) presuppose knowledge of the very logical principles knowledge of which the account purports to explain. Aconsequence would seem to be that implicit definition is incompatible with privilegedaccess. I argue that whilst it is possible for Boghossian to defend against theseobjections the form of argument he proposes does exhibit a subtle form of questionbegging such that it exhibits a transmission of warrant-failure. To investigate the metaphysics of gender categories—categories like “woman,” “genderqueer,” and “man”—is to ask questions about what gender categories are and how they exist. This chapter offers a pluralist account of the metaphysics of gender categories, according to which there are several different varieties of gender categories. I begin by giving a brief overview of some feminist accounts of the metaphysics of gender categories and illustrating how certain moral and political considerations have been in play in these discussions as constraints (...) on acceptable accounts of gender categories. I then present my pluralist account of gender categories and highlight some of its virtues. Finally, I assess how my pluralist account fares relative to the moral and political considerations identified earlier in the chapter. I argue that although the account does not fare well, we should in fact revise our understanding of what those considerations are and how they should constrain our accounts of gender categories. I then show that, on this revised understanding, my pluralist account emerges as a viable position. (shrink)

Williamson’s “anti-luminosity” argument aims to establish that there are no significant luminous conditions. “Far from forming a cognitive home”, luminous conditions are mere “curiosities”. Even supposing Williamson’s argument succeeds in showing that there are no significant luminous states his conclusion has not thereby been established. When it comes to determining what is luminous, mental events and processes are among the best candidates. It is events and processes, after all, which constitute the stream of consciousness. Judgment, for instance, is plausibly self-conscious. (...) If I am judging then plausibly I must know that I am. Similarly, deliberation is plausibly self-conscious. To be deliberating about some matter I plausibly must know that I am. That one is judging and that one is deliberating are thus plausibly luminous conditions. Furthermore, I argue, Williamson’s anti-luminosity argument fails to speak against this suggestion. This reveals that Williamson’s argument is more limited in scope than has been thought—something likely missed due to a misplaced focus on mental states in the luminosity debate as well as in epistemology and the philosophy of mind more generally. For all Williamson shows, there may be luminous events and processes of a sort apt to constitute a cognitive home. I conclude by considering how the anti-luminosity argument might be reinstated in the judgment case and argue that the natural way to do so fails. (shrink)

This chapter attempts to put structure on some of the different philosophical uses of ‘intuition’. It argues that ‘intuition’-hood is associated with four bundles of symptoms: a commonsensicality bundle; an a prioricity and immediacy bundle, and a metaphilosophical bundle. Tentatively suggesting that the word ‘intuition’ as used by philosophers is best regarded as ambiguous, the chapter offers a much simpler view concerning the meaning of ‘intuition’ in philosophy. With some of the attacks on ‘intuition’ as an epistemic source explored, the (...) chapter concludes that one significant kind of philosophical ‘intuition’, related to a prioricity and conceptual truth, can be defended against a range of typical epistemological challenges. The article examines the change of theoretical framework in analytic philosophy of mind. It is well known fact that nowadays philosophical problems of mind are frequently seen as incredibly difficult. It is noteworthy that the first programs of analytical philosophy of mind were skeptical about difficulty of that realm of problems. One of the most notable features of both those programs was the strong antimetaphysical stance, those programs considered philosophy of mind unproblematic in its nature. However, the consequent evolution of (...) philosophy of mind shows evaporating of that stance and gradual recovery of the more sympathetic view toward the mind problematic. Thus, there were two main frameworks in analytical philosophy of mind: 1) the framework of logical positivism and ordinary language philosophy dominated in the 1930s and the 1940s; 2) the framework that dominated since the 1950s and was featured by the critique of the first framework. Thus, the history of analytical philosophy of mind moves between two highly opposite understandings of the mind problematic. The article aims to found the causes of that move in the ideas of C. Hempel and G. Ryle, who were the most notable philosophers of mind in the 1930s and the 1940s. (shrink)

This thesis brings together two concerns. The first is the nature of inference—what it is to infer—where inference is understood as a distinctive kind of conscious and self-conscious occurrence. The second concern is the possibility of doxastic agency. To be capable of doxastic agency is to be such that one is capable of directly exercising agency over one’s beliefs. It is to be capable of exercising agency over one’s beliefs in a way which does not amount to mere self-manipulation. Subjects (...) who can exercise doxastic agency can settle questions for themselves. A challenge to the possibility of doxastic agency stems from the fact that we cannot believe or come to believe “at will”, where this in turn seems to be so because belief “aims at truth”. It must be explained how we are capable of doxastic agency despite that we cannot believe or come to believe at will. On the orthodox ‘causalist’ conception of inference for an inference to occur is for one act of acceptance to cause another in some specifiable “right way”. This conception of inference prevents its advocates from adequately seeing how reasoning could be a means to exercise doxastic agency, as it is natural to think it is. Suppose, for instance, that one reasons and concludes by inferring where one’s inference yields belief in what one infers. Such an inference cannot be performed at will. We cannot infer at will when inference yields belief any more than we can believe or come to believe at will. When it comes to understanding the extent to which one could be exercising agency in such a case the causalist conception of inference suggests that we must look to the causal history of one’s concluding act of acceptance, the nature of the act’s being determined by the way in which it is caused. What results is a picture on which such reasoning as a whole cannot be action. We are at best capable of actions of a kind which lead causally to belief fixation through “mental ballistics”. The causalist account of inference, I argue, is in fact either inadequate or unmotivated. It either fails to accommodate the self-consciousness of inference or is not best placed to play the very explanatory role which it is put forward to play. On the alternative I develop when one infers one’s inference is the conscious event which is one’s act of accepting that which one is inferring. The act’s being an inference is determined, not by the way it is caused, but by the self-knowledge which it constitutively involves. This corrected understanding of inference renders the move from the challenge to the possibility of doxastic agency to the above ballistics picture no longer tempting. It also yields an account of how we are capable of exercising doxastic agency by reasoning despite being unable to believe or come to believe at will. In order to see how such reasoning could amount to the exercise of doxastic agency it needs to be conceived of appropriately. I suggest that paradigm reasoning which potentially amounts the exercise of doxastic agency ought to be conceived of as primarily epistemic agency—agency the aim of which is knowledge. With inference conceived as suggested, I argue, it can be seen how to engage in such reasoning can just be to successfully exercise such agency. (shrink)
