The informed consent is an ethical and legal requirement for potential participants to enroll in a study. There is ample of evidence that understanding consent information and enrollment is challenging for participants in clinical trials. On the other hand, the reasoning process behind decision-making in HIV clinical trials remains mostly unexplored. This study aims to examine the decision-making process of people living with HIV currently participating in antiretroviral clinical trials and their understanding of informed consent. We conducted a qualitative socio-constructivist (...) study using semi-structured interviews. Eleven participants were selected by purposive sampling in Argentina until data saturation was reached. A content analysis was performed. The findings highlight the fact that some participants decided to enroll on the spot, while others made the decision a few days later. In all cases, the decision was based on different aspects of trust (in doctors, in the clinical research site, in the clinical trials system) but also on emotions associated with HIV and/or treatment. Moreover, while people living with HIV felt truly informed after the consent dialogue with a researcher, consent forms were unintelligible and unfriendly. The immediacy of patient decision-making has rarely been described before. Enrollment in an HIV clinical trial is mainly a trust-based decision but this does not contradict the ethical values of autonomy, voluntariness, non-manipulation, and non‐exploitation. Thus, trust is a key issue to be included in reshaping professional practices to ensure the integrity of the informed consent process. (shrink)

ABSTRACT. In his Letters on the motion impressed by a moving mover, Gassendi offers a theory of the motion of composite bodies that closely follows Galileo’s. Elsewhere, he describes the motion of individual atoms in very different terms: individual atoms are always in motion, even when the body that contains them is at rest; atomic motion is discontinuous although the motion of composite bodies is at least apparently continuous; and atomic motion is grounded in an intrinsic vis motrix, motive power, (...) while composite bodies simply persist in their state of motion or rest in the absence of outside intereference. Gassendi does not make much effort to explain how his accounts of atomic and composite motion fit together, and it’s difficult to see how they could possible be integrated. My goal is to explain, given this difficulty, why he accepted both the Galilean theory of the motion of composite bodies and the Epicurean theory of atomic motion. (shrink)

Injury to the central or peripheral nervous system is often associated with persistent pain. After ischemic injury to the spinal cord, rats develop severe mechanical allodynia-like symptoms, expressed as a pain-like response to innocuous stimuli. In its short-lasting phase the allodynia can be relieved with the [gamma]-aminobutyric acid (GABA)-B receptor agonist baclofen, which also reverses the hyperexcitability of dorsal horn interneurons to mechanical stimuli. Furthermore, there is a reduction in GABA immunoreactivity in the dorsal horn of allodynic rats. Clinical neuropathic (...) pain of peripheral and central origin often cannot be relieved by opiates at doses that do not cause side effects. The loss of sensitivity to opiates may be associated with the up-regulation of endogenous antiopioid substances, such as the neuropeptide cholecystokinin (CCK). CCK and its receptor (CCK-R) protein is normally not detectable in rat dorsal root ganglion cells. After peripheral nerve section, both CCK and CCK-R are up-regulated in the dorsal root ganglia. Furthermore, CI 988, an antagonist of the CCK-B receptor, chronically coadministered with morphine, reduces autotomy, a behavior that may be a sign of neuropathic pain following peripheral nerve section. Thus, opiate insensitivity may be due to the release of CCK from injured primary afferents. Similarly, in the chronic phase of the spinal ischemic model of central pain, the allodynia-like symptom is not relieved by systemic morphine, but is significantly reversed by the CCK-B antagonist. Consequently, up-regulation of CCK and CCK-R in the CNS may also underlie opiate drug insensitivity following CNS injury. Thus, dysfunction of central inhibition involving GABA and endogenous opioids may be a factor underlying the development of sensory abnormalities and/or pain following injury to neural tissue. The theme of my target article was dysfunction of inhibition in the spinal cord as an important factor in the development of chronic pain states. Some commentaries focused on the role of more central mechanisms and the limited usefulness of animal models for understanding mechanisms of human pain. More specific comments concerned the roles of GABA and cholecystokinin in pain control.

Brazilian educator, Paulo Freire, best known for his work Pedagogy of the Oppressed, challenged education plans that contributed to the marginalization of minorities and the poor. Freire believed that education should be used for liberation by helping learners reflect on their experiences historically, giving immediate reality to issues of racism, sexism, and the exploitation of workers. Known as one of the most influential theoretical innovators of the twentieth century, his views have left a significant mark on progressive thinkers about education (...) and liberation. Reinventing Paulo Freire is an homage to him by protégé Antonia Darder. Here, she explores the legacy of Freire, interviews eight former students who studied him-- now teachers themselves, and reflects on the teaching practice as demonstrated by Freire himself. Basic actions are those intentional actions performed not by doing any other kind of thing intentionally. Complex actions involve doing one kind of thing intentionally by doing another kind of thing intentionally. There are both basic and complex mental actions. Some complex mental actions have a striking feature that has not been previously discussed: they have several distinct contents at once. This chapter introduces and explains this feature, here called “content plurality.” This chapter also argues for the philosophical significance of (...) this feature. The existence of content plurality offers a new explanation of transparent self-knowledge and suggests a new theory of inference. It also opens up a new conception of the relation between decisions to act and judgments about what ought to be done. (shrink)

In this chapter, Fourie identifies and systematizes the impairments associated with having privilege and evaluates their implications for theories of relational equality and distributive justice. Having certain social privileges, for example, being a man in a patriarchal society, can also be damaging; in other words, there are “impairments of privilege.” Fourie delineates six kinds of impairments—epistemic, evaluative, emotional, health-related, affiliative, and moral. She then goes on to assess the implications of the impairments of privilege for two theories in political philosophy. (...) Relational egalitarianism, she argues, has the theoretical resources to identify and address the problems associated with these impairments, whereas distributive egalitarianism does not. Furthermore, she argues that assessing the impairments of privilege through relational egalitarianism helps to characterize a society of equals: we must address the causes of functional impairments, express respect for the worse off by not normalizing the experiences of the privileged, and minimize competitive positionality. Background Informed consent is often cited as the “cornerstone” of research ethics. Its intent is that participants enter research voluntarily, with an understanding of what their participation entails. Despite agreement on the necessity to obtain informed consent in research, opinions vary on the threshold of disclosure necessary and the best method to obtain consent. We aimed to investigate Australian researchers’ views on, and their experiences with, obtaining informed consent. Methods Semi-structured interviews were conducted with 23 researchers from NSW institutions, working (...) in various fields of research. Interviews were analysed and coded to identify themes. Results Researchers reported that consent involved information disclosure, understanding and a voluntary decision. They emphasised the variability of consent interactions, which were dependent on potential participants’ abilities and interests, study complexity and context. All researchers reported providing written information to potential participants, yet questioned the readability and utility of this information. The majority reported using signed consent forms to ‘operationalise’ consent and reported little awareness of, and lack of support in implementing more dynamic informed consent procedures, such as verbal informed consent, that was fit for the purposes of their studies. Views on Human Research Ethics Committees varied. Some reported inconsistent, arduous inputs on the information form and consent process. Others expressed reliance on HRECs for guidance, viewing them as institutional safeguards. Conclusions This study highlights the importance of transparent relationships, both between researchers and participants, and between researchers and HRECs. Where the relationship with study participants was reported as more robust, researchers felt that they were better able to ensure participants made better, more informed decisions. Where the relationship with HRECs was reported as more robust, researchers were more likely to view them as institutional safeguards, rather than as bureaucratic hindrances. Conscientious and mindful researchers are paramount to ensuring the procedure accommodates individual requirements. This study advocates that when designing ethical informed consent practices, researchers should be integrated as autonomous players with a positive input on the process, rather than, in the worst case, predatory recruiters to be curtailed by information forms and oversight. What kind of equality should we value and why? Current debate centres around whether distributive equality is valuable. However, it is not the only (potentially) morally significant form of equality. David Miller and T. M. Scanlon have emphasised the importance of social equality—a strongly egalitarian notion distinct from distributive equality, and which cannot be reduced to a concern for overall welfare or the welfare of the worst-off. However, as debate tends to focus on distribution, social equality has been neglected and (...) we do not have a clear understanding of what it is and why it might be valuable. Much research is currently being conducted on health care practitioners' experiences of moral distress, especially the experience of nurses. What moral distress is, however, is not always clearly delineated and there is some debate as to how it should be defined. This article aims to help to clarify moral distress. My methodology consists primarily of a conceptual analysis, with especial focus on Andrew Jameton's influential description of moral distress. I will identify and aim to resolve two sources of confusion about (...) moral distress: the compound nature of a narrow definition of distress which stipulates a particular cause, i.e. moral constraint, and the distinction drawn between moral dilemma and moral distress, which implies that the two are mutually exclusive. In light of these concerns, I argue that the definition of moral distress should be revised so that moral constraint should not be a necessary condition of moral distress, and that moral conflict should be included as a potential cause of distress. Ultimately, I claim that moral distress should be understood as a specific psychological response to morally challenging situations such as those of moral constraint or moral conflict, or both. Just as bodily actions are things you do with your body, mental actions are things you do with your mind. Both are different from things that merely happen to you. Where does the idea of mental action come from? What are mental actions? And why do they matter in philosophy? These are the three main questions answered in this paper. Section 1 introduces mental action through a brief history of the topic in philosophy. Section 2 explains what it is to (...) be a mental action in terms of intentional action. Section 3 argues that the fact that we can perform mental actions has some striking consequences concerning mental content. Section 4 shows how mental action helps to explain some important facts about self-knowledge and the normativity of thought. In this chapter, I propose that conceptions of justice in bioethics must be feminist, meaning they must be able to capture how the domains of health, healthcare and medicine exacerbate the subordination of those perceived to be women and girls and how injustice impacts their health. After providing context in the first section, I identify three problems with conceptions of justice in the bioethics literature that interfere with their potential to be feminist. They tend to adopt the ahistoricism and distributivism (...) characteristic of theories of justice that have been dominant in political philosophy, and they often rely on gendered conceptions of health that result in troubling comparisons of men and women’s health. My argument is primarily “negative”: it is a critique of generalist conceptions of justice in bioethics. However, in the final section I present characteristics of what a feminist conception of justice needs to look like and indicate how feminist bioethicists are using such a conception in assessing issues of injustice in bioethics. I emphasize the centrality of actual structural injustices, relational egalitarianism, and the assessment of values in conceptions of health and the health sciences. (shrink)

Aesthetic value is not as peculiar as we might think. We do not walk into museums expecting to find there a good so refined that it bears no continuity with the.

Word co‐occurrence patterns in language corpora contain a surprising amount of conceptual knowledge. Large language models (LLMs), trained to predict words in context, leverage these patterns to achieve impressive performance on diverse semantic tasks requiring world knowledge. An important but understudied question about LLMs’ semantic abilities is whether they acquire generalized knowledge of common events. Here, we test whether five pretrained LLMs (from 2018's BERT to 2023's MPT) assign a higher likelihood to plausible descriptions of agent−patient interactions than to minimally (...) different implausible versions of the same event. Using three curated sets of minimal sentence pairs (total n = 1215), we found that pretrained LLMs possess substantial event knowledge, outperforming other distributional language models. In particular, they almost always assign a higher likelihood to possible versus impossible events (The teacher bought the laptop vs. The laptop bought the teacher). However, LLMs show less consistent preferences for likely versus unlikely events (The nanny tutored the boy vs. The boy tutored the nanny). In follow‐up analyses, we show that (i) LLM scores are driven by both plausibility and surface‐level sentence features, (ii) LLM scores generalize well across syntactic variants (active vs. passive constructions) but less well across semantic variants (synonymous sentences), (iii) some LLM errors mirror human judgment ambiguity, and (iv) sentence plausibility serves as an organizing dimension in internal LLM representations. Overall, our results show that important aspects of event knowledge naturally emerge from distributional linguistic patterns, but also highlight a gap between representations of possible/impossible and likely/unlikely events. The origin and evolutionary dynamics of the spatial heterogeneity in genomic base composition have been debated since its discovery in the 1970s. With the recent availability of numerous genome sequences from a wide range of species it has been possible to address this question from a comparative perspective, and similarities and differences in base composition between groups of organisms are becoming evident. Ample evidence suggests that the contrasting dynamics of base composition are driven by GC‐biased gene conversion (gBGC), a process (...) that is associated with meiotic recombination. In line with this hypothesis, base composition is associated with the rate of recombination and the evolutionary dynamics of the recombination landscape, therefore, governs base composition. In addition, and at first sight perhaps surprisingly, the relationship between demography and genomic base composition is in agreement with the gBGC hypothesis: organisms with larger populations have higher GC content than those with smaller populations. The notion of sovereignty plays an important part in various areas of law, such as constitutional law and international public law. Though the concept of sovereignty as applied in constitutional law differs from that used in international public law, there is no true consensus on the meaning of “sovereignty” within these respective fields, either. Is sovereignty about factual power, or only about legal equality? Do only democracies have sovereignty, because they have legitimacy, or is there no connection between democracy, legitimacy (...) and sovereignty? Has the European Union encroached upon the sovereignty of the Member States, or is transferring competences to the European Union an expression and exercise of the very sovereignty some claim is under attack? Is it about states, or is it about peoples having a right to self-determination, and if the latter, does this represent popular sovereignty or something else? In order to answer these and related questions, we need a clear grasp of what “sovereignty” means. This book provides an analytical and conceptual framework for “sovereignty” in the context of law. The book does not seek to describe how the term “sovereignty” is used in the different contexts and discourses in which it is employed, but rather distinguishes between two possible meanings of sovereignty that allow the reader to use the term with specificity and clarity. In this way, this book hopes to offer valuable analytical tools for politicians, constitutional and international lawyers and legal theorists that help them be clear about what they mean when they speak of “sovereignty.”. This article explores safety and politics of space in two ways. First, it reviews research on women’s fear and calls for safer cities, identifying four contradictions in the geography of fear discourse. Second, it elaborates on how including various forms of fear may repoliticize the contemporary depoliticized and co-opted safety discussion by focusing on sexist and racist threats rather than exclusively on the white middle classes. Here, threats to veiled Muslim women and their experiences in public spaces are, in particular, (...) emphasized as exemplifying fears that are neglected in the safety debate. The article concludes that, rather than the whole safety issue being dismissed as ‘neoliberal’, there is an urgent need to strengthen the analysis of power and illuminate experiences of pain and fear in sexist and racist violence. Aesthetic value empiricism claims that the aesthetic value of an object is grounded in the value of a certain kind of experience of it. The most popular version of value empiricism, and a dominant view in contemporary philosophical aesthetics more generally, is aesthetic hedonism. Hedonism restricts the grounds of aesthetic value to the pleasure enjoyed in the right kind of experience. But hedonism does not enjoy any clear advantage over a more permissive alternative version of value empiricism. This alternative is (...) aesthetic liberalism. On this view, an object’s aesthetic value is fully grounded in any value—not just the hedonic value—of a correct and complete experience of its sensory features. To demonstrate the advantages of liberalism over hedonism, I apply both views to analyze the aesthetic value of the Spring Temple Buddha and Anselm Kiefer’s Seraphim. I detail four advantages of liberalism over hedonism, and I conclude by defending liberalism from two kinds of objection. (shrink)

The effects of the COVID-19 pandemic have rippled across the United States’ (US) agri-food system, illuminating considerable issues. US seed systems, which form the foundation of food production, were particularly marked by panic-buying and heightened safety precautions in seed fulfillment facilities which precipitated a commercial seed sector overwhelmed and unprepared to meet consumer demand for seed, especially for non-commercial growers. In response, prominent scholars have emphasized the need to support both formal (commercial) and informal (farmer- and gardener-managed) seed systems to (...) holistically aid growers across various contexts. However, limited attention to non-commercial seed systems in the US, coupled with a lack of consensus surrounding what exactly a resilient seed system looks like, first warrants an exploration into the strengths and vulnerabilities of existing seed systems. This paper seeks to examine how growers navigated challenges in seed sourcing and how this may reflect the resilience of the seed systems to which they belong. Using a mixed-methods approach which includes data from online surveys (n = 158) and semi-structured interviews (n = 31) with farmers and gardeners in Vermont, findings suggest that growers were able to adapt – albeit through different mechanisms depending on their positionality (commercial or non-commercial) within the agri-food system. However, systemic challenges emerged including a lack of access to diverse, locally adapted, and organic seeds. Insights from this study illuminate the importance of creating linkages between formal and informal seed systems in the US to help growers respond to manifold challenges, as well as promote a robust and sustainable stock of planting material. Background Euthanasia is a controversial practice in many countries. Since Spain’s Euthanasia Law came into effect on March 24, 2021, healthcare providers have faced a new challenge since they must inform patients, provide care, accompany them, and implement the law. It also represents a new stumbling block at universities, which must adapt to regulatory changes and educate future professionals accordingly. Little is known about the attitude of nursing students in Spain toward euthanasia since this law was implemented. Objective This study (...) aims to answer the following research questions: What is the attitude of nursing students toward euthanasia? What factors influence this attitude? Research design A cross-sectional study was conducted using an online questionnaire. Participants and research context The study population comprised all nursing students at a public university in Barcelona ( n = 444), Spain, during the 2022–2023 academic year. The validated Spanish version of the Euthanasia Attitude Scale was employed. A bivariate analysis was performed. Ethical considerations The university Ethics Committee (CEEAH 6247) approved this study. All participating students signed an informed consent form. Participation was voluntary, and data anonymity and confidentiality were guaranteed. Results Two hundred and forty-four nursing students responded to the questionnaire. The mean total score was 79.64. Participants with religious beliefs presented lower scores, indicating a more negative attitude toward euthanasia. Participants in their second, third, or fourth year of the nursing degree scored higher, demonstrating a more positive attitude. Conclusions The attitude of nursing students toward euthanasia was remarkably positive. Working on ethical content during the degree course and clinical practice are factors that help to develop a more positive attitude. In addition, nursing education should encourage professional aspects to prevail over religious beliefs in euthanasia situations. (shrink)

The scene takes place in O'Higgins Park, in Santiago, Chile, on 1 October 1995. Some women have just taken their place on the stage and the enthusiastic audience is applauding, the women have started singing accompanied by a guitar, but they cannot be heard, for the audience is still applauding. One of the women gets up. Like the others, she is wearing a white blouse and a long black skirt, she is old and her hair is grey. She moves to (...) the front of the stage, chooses her place and stops, her eyes search the horizon but do not wander aimlessly, her eyes are fixed somewhere between the here and the hereafter. When she finds what she is looking for, she raises her right hand to wave a white handkerchief, and the dance begins. The audience, suddenly attentive, watches. And what do they see? Not the woman alone on the stage, but the empty space which she traces. The empty space disturbs us, as Chilean spectators, because the cueca, the national folklore dance, is danced by two people, or it is not danced at all. The invisible other is invoked by this woman's every gesture and we do not smile when faced with the absence of her man because we have a premonition that she is not pretending, we have a premonition that she can see him. The image of women dancing alone has become one of the symbols of the struggle of the families of the disappeared under the dictatorship of General Pinochet. The families are commemorating, on 1 October 1995, the twentieth anniversary of their organization into an association in 1975. The term “the disappeared” has progressively become accepted to indicate the victims of a novel, coercive practice in Chile, since the coup d’état on 11 September 1973. The forced disappearance of people differs from political assassination, as Amnesty International never ceases to indicate in the various reports which it commissions on them. In the Chilean case, this speciality is in part due to the security system which has taken on the responsibility of destroying all organized opposition to the military dictatorship: the DINA, the intelligence service created by the military junta, which is only answerable for its actions to the junta. More fundamentally, the special character of disappearance is due to the invisibility of the victim's body and to the doubt that this invisibility arouses within the victim's family circle: the disappeared is not identified as dead because his death is never certified, on the other hand his sudden and unexplained absence indicates that he is a person in danger and that this danger extends to all those who notice his absence. The adaptation-by-binding account and the arousal-biased competition model suggest that emotional arousal increases binding effects for transient links between stimuli and responses. Two highly-powered, pre-registered experiments tested whether transient stimulus-response bindings are stronger for high versus low arousing stimuli. Emotional words were presented in a sequential prime-probe design in which stimulus relation, response relation, and stimulus arousal were orthogonally manipulated. In Experiment 1 (N = 101), words with high and low arousal levels were presented individually in prime and probe displays. (...) In Experiment 2 (N = 170), a high arousing affective word was presented simultaneously with a neutral word during the prime display; in the subsequent probe display, either the arousing or the neutral word repeated or a different high versus low arousal word was shown. Data from both experiments did not demonstrate a modulation of SRBR effects by stimulus arousal and SRBR effects were of equal magnitude for word stimuli of high and low arousal levels. These null results are not in line with binding accounts that hypothesise a modulatory influence of emotional arousal on perception-action binding. (shrink)

This article conveys some of the findings from a hermeneutic-phenomenological study on lived experiences of school failure. The informants were students in Swedish senior high schools and teenagers in Swedish juvenile institutions. Contrary to the common belief that school failure is related to low grades or failing exams, the students’ descriptions of lived experiences of failure had little to do with intellectual shortcomings. The students’ interpretation of my research question did not encompass cognitive deficiencies. They rarely spoke of failure to (...) understand, or failure to meet scholastic demands. Instead, the students offered stories about failure to behave according to expectations and the way in which they experienced their teachers’ reactions to this ‘deviant’ behaviour. Thus, the question of school failure did not revolve around the students’ cognitive knowledge and proficiency – or lack of thereof – but around the hidden curriculum. The feelings the students lived through while experiencing failure included lack of trust, confidence, belief, joyfulness, patience, hope, and serenity. To this day no consensus has been reached concerning the authorship of the 3rd book of Philoponus’ on the Soul. I will begin this article by surveying the discussion of the authorship. In this first section I go into some detail concerning the arguments of scholars who attribute the DAC 3 to Philoponus, and those who do not believe that it is by Philoponus, including those who attribute it to Stephanus of Alexandria. The second section of the paper consists in (...) an analysis and evaluation of the arguments involved in the debate. Finally, I conclude by presenting my own position, including new arguments in favor of the attribution of the DAC 3 to John Philoponus. (shrink)

The notion of spirit was a central notion of two ancient traditions: the medical and the philosophical tradition of ancient Greece and the neoplatonic philosophical one. In this paper we will examine the way in which a Byzantine philosopher of the 6th century, John Philoponus, handled this notion from a neoplatonic point of view, and how he assimilated the Aristotelian, the medical and the neoplatonic “πνεῦμα” theory in a single conception. Philoponus’ career can be divided into two periods: the one (...) during which he was studying under Ammonius and the one during which he was writing in his own name, expressing his Christian convictions as a Byzantine philosopher, and criticizing neoplatonic doctrines opposed to the Christian dogma. We shall, thus, also present the refutation he gives to the neoplatonic theory of “πνεῦμα”, during the second period of his career. (shrink)

El presente trabajo conceptualiza la categoría de sufrimiento o dolor social desde un enfoque relacional y constructivista sobre la vida social y escolar. Particularmente, focalizamos en los desarrollos del psicoanálisis de Silvia Bleichmar, la sociología figuracional de Norbert Elias y el interaccionismo simbólico de David Le Breton. Sus perspectivas poseen una serie de puntos de confluencia: a) interpretan al sufrimiento como una emoción que imbrica procesos psico y sociogenéticos; b) conciben que el dolor remite a la relación entre cuerpo y (...) subjetividad; c) sostienen que el debilitamiento o la ausencia de lazos sociales significativos se encuentra en la base de las experiencias de padecimiento.
