Moral agency is an important constituent of the nursing role. We explored issues of ethical development in Greek nursing students during clinical practice at the beginning of their studies. Specifically, we aimed to explore students’ lived experience of ethics, and their perceptions and understanding of encountered ethical conflicts through phenomenological analysis of written narratives. The process of developing an awareness of personal values through empathizing with patients was identified as the core theme of the students’ experience. Six more common themes (...) were identified. Development of the students’ moral awareness was conceptualized as a set of stages, commencing with empathizing with patients and nurses, moving on to taking a moral stand and, finally, concluding by becoming aware of their personal values and showing evidence of an emerging professional moral personhood. The notions of empathy, caring and emotion were in evidence throughout the students’ experience. Implications for practice and nurse education are discussed. (shrink)

This paper considers several philosophers’ efforts to explain the metaphysical orientations of historical narratives, ones which expose the lack of common ground in modes of establishing truth and documenting change. Although philosophers have been writing about history since before Plato’s time, this brief inquiry is primarily restricted to Hegel, Maritain, R. G. Collingwood, and W. H. Walsh. Suicide is a major problem in the United States, with the number of suicides annually exceeding the number of homicides by 10,000. Many studies have examined the relationship between media coverage of suicides and the suicide rate. This article reviews literature on imitative suicide and discusses implications of suicide stories on people in crisis. In addition, it explores the options for suicide coverage and gives suggestions for more ethical coverage that could save people's lives, rather than reinforcing suicide as an (...) option. (shrink)

This new and important study of semantic change examines how new meanings arise through language use, especially the various ways in which speakers and writers experiment with uses of words and constructions in the flow of strategic interaction with addressees. In the last few decades there has been growing interest in exploring systemicities in semantic change from a number of perspectives including theories of metaphor, pragmatic inferencing, and grammaticalization. Like earlier studies, these have for the most part been based on (...) data taken out of context. This book is the first detailed examination of semantic change from the perspective of historical pragmatics and discourse analysis. Drawing on extensive corpus data from over a thousand years of English and Japanese textual history, Traugott and Dasher show that most changes in meaning originate in and are motivated by the associative flow of speech and conceptual metonymy. How does talk work, and can we engage the public in a dialogue about the scientific study of talk? This article presents a history, critical evaluation and empirical illustration of the public science of talk. We chart the public ethos of conversation analysis that treats talk as an inherently public phenomenon and its transcribed recordings as public data. We examine the inherent contradictions that conversation analysis is simultaneously obscure yet highly cited; it studies an object that people understand intuitively, yet (...) routinely produces counter-intuitive findings about talk. We describe a novel methodology for engaging the public in a science exhibition event and show how our ‘conversational rollercoaster’ used live recording, transcription and public-led analysis to address the challenge of demonstrating how talk can become an informative object of scientific research. We conclude by encouraging researchers not only to engage in a public dialogue but also to find ways to actively engage people in taking a scientific approach to talk as a pervasive, structural feature of their everyday lives. It is one thing to read about the methodology and methods of Heideggerian hermeneutic phenomenological research, the ontic description. It is quite another thing to be faced with an interview transcript. This article draws on a study that asked doctoral students about their experience of doing such research. How did they become “phenomenological/hermeneutic” in their thinking and writing? What helped them to find their way? We offer this article as a means of letting others learn from our own experiences. How do politicians engage with and offer to assist their constituents: the people who vote them into power? We address the question by analysing a corpus of 80 interactions recorded at the office of a Member of Parliament in the United Kingdom, and comprising telephone calls between constituents and the MP’s clerical ‘caseworkers’ as well as face-to-face encounters with MPs in their fortnightly ‘surgeries’. The data were transcribed, and then analysed using conversation analysis, focusing on the design and placement of (...) offers of assistance. We identified three types of offers within a longer ‘offering’ sequence: ‘proposal offers’, which typically appear first in any offering sequence, in which politicians and caseworkers make proposals to help their constituents using formats that request permission to do so, or check that the constituent does indeed want help ; ‘announcement offers’, which appear second, and indicate that something has been decided and confirm the intention to act and ‘request offers’, which appear third, and take for form ‘let me do X’. Request offers indicate that the offer is available but cannot be completed until the current conversation is closed; they also appear in environments in which the constituent reissues their problems and appears dissatisfied with the offers so far. The article contributes to what we know about making offers in institutional settings, as well as shedding the first empirical light on the workings of the constituency office: the site of engagement between everyday members of the public and their elected representatives. We can freely cross disciplinary boundaries, as well as the line between theory and practice, and allow practices to cast their light back on the theory and show us its deficiencies. In short, this approach reorients some much-discussed issues of professional, business, and military ethics and reveals them as variations on one deeply rooted theme. The author does not treat current institutions as final and unalterable. If these arrangements frustrate moral evaluation, she finds that an argument for change. To make (...) intelligent changes, however, we need a clear view of the reasoning that makes them seem natural and inevitable. That is what this book attempts to do. In the process, the author also reexamines the concept of "person." Not all cultures put so much stress on the idea as Western - and particularly American - cultures do. This book explores the tradition of the 'science of man' in French medicine of the era 1750-1850, focusing on controversies about the nature of the 'physical-moral' relation and their effects on the role of medicine in French society. Its chief purpose is to recover the history of a holistic tradition in French medicine that has been neglected because it lay outside the mainstream themes of modern medicine, which include experimental, reductionist, and localistic conceptions of health and disease. Professor Williams also (...) challenges existing historiography, which argues that the 'anthropological' approach to medicine was a short-term by-product of the leftist politics of the French Revolution. This work argues instead that the medical science of man long outlived the Revolution, that it spanned traditional ideological divisions, and that it reflected the shared aim of French physicians, whatever their politics, to claim broad cultural authority in French society. This article raises a problem for Cornell varieties of moral realism. According to Cornell moral realists, we can know about moral facts just as we do the empirical facts of the natural sciences. If this is so, it would remove any special mystery that is supposed to attach to our knowledge of objective moral facts. After clarifying the ways in which moral knowledge is to be similar to scientific knowledge, I claim that the analogy fails, but for little-noticed reasons. A (...) preliminary conclusion of the article will be that this positive comparison to scientific knowledge hurts, rather than helps, the realist position. Yet, rather than spell trouble for moral realism altogether, I suggest that the apparent failure of Cornell realist moral epistemology points to a better way forward for moral realism. (shrink)

In his article ‘Saints and Heroes’, Urmson argues that traditional moral theories allow at most for a threefold classification of actions in terms of their worth, and that they are therefore unsatisfactory. Since the conclusion of his argument has led to the widespread use of the term ‘acts of supererogation’, and since I do not believe that such acts exist, I propose to argue that the actions with which he is concerned not only can, but should, be contained within the (...) traditional classification. A goal of living as well as possible is central to practice and research with young adults living with home mechanical ventilation (HMV). Significant effort has been put into conceptualizing and measuring the quality of life (QOL) as a proxy for living well. Yet, dominant understandings of QOL have been influenced by normative, ableist, and biomedical discourses about what constitutes a good life that, when applied in practice and systems with those living with HMV, can contribute to exclusion and constrain (...) opportunities to live well. Inquiry into what certain understandings of living well can do is critical to opening up possibilities to reimagine living well with HMV. This paper draws on findings from a critical narrative inquiry that explored the experiences of five young adults (ages 18–40 years) living with HMV. Data were co‐constructed virtually through an initial interview and photo‐elicitation using participant‐generated photographs. A critical narrative analysis of participants' stories made visible the ideological effects of ableist, biomedical, and individualist discourses and how the young adults reproduced and resisted these dominant discourses. Their stories further opened up possibilities for nurses and other healthcare providers to see living well and QOL differently. (shrink)

In this response article, I focus on two issues. First, I discuss the problem, raised by the commentators, of ‘categorial ambiguity’ in membership categorization analysis, and make suggestions about how to approach it. Second, I argue that, as conversation analysts have demonstrated the ‘systematics’ of interactional practices, membership categorization analysis should also begin to build a robust corpus of studies of ‘categorial systematics’.

Under many circumstances, children and adult rats reorient themselves through a process which operates only on information about the shape of the environment (e.g., Cheng, 1986; Hermer & Spelke, 1996). In contrast, human adults relocate themselves more flexibly, by conjoining geometric and nongeometric information to specify their position (Hermer & Spelke, 1994). The present experiments used a dual-task method to investigate the processes that underlie the flexible conjunction of information. In Experiment 1, subjects reoriented themselves flexibly when they performed no (...) secondary task, but they reoriented themselves like children and adult rats when they engaged in verbal shadowing of continuous speech. In Experiment 2, subjects who engaged in nonverbal shadowing of a continuous rhythm reoriented like nonshadowing subjects, suggesting that the interference effect in Experiment 1 did not stem from general limits on working memory or attention but from processes more specific to language. In further experiments, verbally shadowing subjects detected and remembered both nongeometric information (Experiment 3) and geometric information (Experiments 1, 2, and 4), but they failed to conjoin the two types of information to specify the positions of objects (Experiment 4). Shifts in funding and a worldwide trend towards marketising higher education have led to a new emphasis on the quality of the student experience. In the UK this trend finds its strongest expression in recent policy proposals to simultaneously increase student fees and student choice so that students themselves become the drivers of higher education. We trace the policy developments of this shift over recent years and rehearse some of the criticisms against it. Accepting that there is good reason to (...) support improving the student experience, we then consider ways in which this might be achieved that foreground the relation of university teachers with their subject matter and students' engagement in this. A higher education, we argue, should expose its students to disciplines whose standards are rightly contested and to a form of learning that intensifies or unsettles desires rather than simply aspiring to satisfy them. This article examines patients’ calls to three different GP services in the United Kingdom. Using conversation analysis, combined with coding of 447 calls, we studied the role of thank you in closing sequences, focusing on their timing and order in relation to service outcome. We show first how patients withhold thank you in orientation to an absent summary or specification of service: patients are more likely to initiate thank you if the receptionist volunteers such a summary. Second, we show there (...) is variation in how appropriately participants project the termination of calls using thank you. Finally, while thank you serves a primary role in managing the termination of calls, the timing, order and design of thank you can also display patient satisfaction. We discuss our findings in terms of service encounters more generally, including implications for larger scale analysis. (shrink)

Growing opioid misuse in the United States has resulted in more children living with an adult with an opioid use history. Although an abundance of research has demonstrated a link between opioid misuse and negative parenting behaviors, few intervention efforts have been made to target this underserved population. The Family Check-Up has been tested in more than 25 years of research, across multiple settings, and is an evidence-based program for reducing risk behavior, enhancing parenting skills, and preventing the onset of (...) substance use. It is designed to motivate parents to engage in positive parenting practices and to change problematic parenting and has been tested across a variety of ages including early childhood and adolescence. It is highlighted in NIDA’s Principles of Substance Use Prevention for Early Childhood: A research-based guide as one of only three effective selective prevention programs for substance abuse among families with young children. Recently, we developed an online version of the FCU that has now been adapted for early childhood and families with opioid use histories. The online platform and telehealth model allow for wide-scale dissemination, ease of training with community providers, and increased public health reach for families in remote, rural areas. This is particularly important when targeting families with opioid misuse and addiction because there are high rates of addiction in remote areas, yet few services available. In this article, we describe the FCU Online and review new content in the model that targets a population of young adult parents with substance abuse histories, including opioid use. New modules include content focused on harm reduction for this high-risk population of parents, such as safety in the home, substance use while parenting, and managing conflict with partners and friends. Modern science is predicated on an objective stance which demystifies the cosmos and allows for a technological mastery of nature which has improved human life in innumerable ways. The modern conception of the self is grounded in a new conception of the autonomy and dignity of the individual, and modern political theory begins with the notion that society is an artifact to be constructed in the service of the very individuals who constitute the body politic. We are made uneasy by the nagging suspicion that life in this world which we have remade for ourselves is somehow hollow, an artificial construction without depth or solidity, a merely superficial order which threatens to dissolve into meaninglessness. The technological advances made possible by modern science have also brought the atomic bomb, global warming, and acid rain. Power over nature has led to the power to destroy nature. The modern world has seen an unprecedented rise in individual freedom, and yet the autonomy of the individual has all too often given way to the anomie of the individual. It has become quite common in attempting to give an account for this perplexing state of affairs to speak of a process of secularization in the transition from the medieval to the modern world, a process in which many of the central ideas and principles of the modern age are taken to be secular variations on medieval theological themes. Thus, for example, faith in the progress of science or of history is taken to be a secularized version of belief in divine Providence, the political equality of all individuals under the law is said to be a secular reiteration of the understanding of the equality of all human beings before God, the transcendental subject is exposed as a secular version of the divine or angelic knower, and the communist state and other modern utopias marking the end of history are said to be secularizations of Christian eschatological expectations of the end of the world and the coming reign of God.This reading of the epochal transition explains the worldly character of modern reality in terms of the "immanentization" or secularization of religious attitudes and expectations. It also serves to explain the uncanny sense of worldlessness characteristic of much of modern life.
