When weighing up which inhaler to prescribe, a doctor may prioritise a patient’s preferences over the expected harms from the associated carbon emissions. Parker argues that this is wrong.1 Doctors have a pro-tanto duty to switch from a high-carbon metered-dose inhaler (MDI) to a low-carbon dry-powdered inhaler (DPI)—even though this provides no direct patient benefit—unless switching would undermine trust or significantly worsen a patient’s health. He goes on to state that even if DPIs are more expensive for the National Health (...) Service (NHS) then this is justified so long as it does not ‘significantly threaten’ the NHS’ ability to protect and promote health. This may appear to be a radical proposal, challenging the ethical principles of autonomy, health entitlements and justly distributed resources. However, we will claim that it is only radical in so far as one perceives (A) patient autonomy and healthcare entitlements as existing within a vacuum, unrelated to other foundational ethics concerns and (B) the health consequences of a given healthcare budget are limited solely to the designated recipients rather than all affected parties. _Disaster Psychiatry: Intervening When Nightmares Come True_ captures the state of disaster psychiatry in the aftermath of the terrorist attacks of September 11, 2001. This emergent psychiatric specialty, which is increasingly separated from trauma and grief psychiatry on one hand and military psychiatry on the other, provides psychotherapeutic assistance to victims during, and in the weeks and months following, major disasters. As such, disaster psychiatrists must operate in the widely varying locales in which natural and man-made disasters occur, and they (...) must establish their role among the chaotic array of organizations involved in direct disaster response. Editors Anand Pandya and Craig Katz have captured the challenge and promise of disaster psychiatry through first-person narratives. We hear from psychiatrists who have encountered disasters at various stages of their career and in widely varying social, political, and personal contexts. Accounts of psychiatric involvement with adults and children during and after 9/11 have understandable pride of place in this collection. But they are balanced by richly informative narratives about other domestic and international disasters. Fraught with the drama attendant to the events they describe, these essays delineate the dizzying array of challenges that confront the disaster psychiatrist. They range from the intense emotional responses that are part of the aftermath of any disaster, to the need to legitimize a psychiatric presence within diverse cultural and medical contexts, to the subtle task of providing therapeutic boundaries at a time when all rules seem to be suspended. Special attention is given to the daunting task of working with children whose parents' are disaster victims. What emerges from these testimonies is compelling documentation of skilled and compassionate psychiatrists at the outer limits of their specialty, pursuing their calling into uncharted realms of therapeutic engagement. Modal epistemology has been dominated by a focus on establishing an account either of how we have modal knowledge or how we have justified beliefs about modality. One component of this focus has been that necessity and possibility are basic access points for modal reasoning. For example, knowing that P is necessary plays a role in deducing that P is essential, and knowing that both P and ¬P are possible plays a role in knowing that P is accidental. Chalmers (2002) (...) and Williamson (2007) provide two good examples of contrasting views in modal epistemology that focus on providing an account of modal knowledge where necessity and possibility are basic access points for modal knowledge, and Yablo (1993) provides a good account of how we have justified beliefs about modality. In contrast to this tradition I argue for and outline a modal epistemology based on objectual understanding and essence, rather than knowledge or justification and necessity and possibility. The account employs a non-modal conception of essence and takes objectual understanding of essence, rather than knowledge of essence to be basic in modal reasoning. I begin by articulating Kvanvig’s (2003) account of objectual understanding, on which objectual understanding of Φ is not equivalent to propositional knowledge of Φ. I then argue that an epistemology of essence that uses property variation-in-imagination is better construed as a model that delivers objectual understanding of essence rather than knowledge of essence. I argue that this is so, since the latter and not the former runs into a version of the Meno paradox. I show how this account can be applied to two issues in modal epistemology: the Benacerraf problem for modality, and the architecture of modal knowledge. (shrink)

There are three theories in the epistemology of modality that have received sustained attention over the past 20 years: conceivability-theory, counterfactual-theory, and deduction-theory. In this paper we argue that all three face what we call the problem of modal epistemic friction. One consequence of the problem is that for any of the three accounts to yield modal knowledge, the account must provide an epistemology of essence. We discuss an attempt to fend off the problem within the context of the internalism (...) versus externalism debate about epistemic justification. We then investigate the effects that the PMEF has on reductive and non-reductive theories of the relation between essence and modality. (shrink)

The aim of this paper is to develop the theory of groups definable in the p-adic field ${{\mathbb {Q}}_p}$, with “definable f-generics” in the sense of an ambient saturated elementary extension of ${{\mathbb {Q}}_p}$. We call such groups definable f-generic groups.So, by a “definable f-generic” or $dfg$ group we mean a definable group in a saturated model with a global f-generic type which is definable over a small model. In the present context the group is definable over ${{\mathbb {Q}}_p}$, and (...) the small model will be ${{\mathbb {Q}}_p}$ itself. The notion of a $\mathrm {dfg}$ group is dual, or rather opposite to that of an $\operatorname {\mathrm {fsg}}$ group (group with “finitely satisfiable generics”) and is a useful tool to describe the analogue of torsion-free o-minimal groups in the p-adic context.In the current paper our group will be definable over ${{\mathbb {Q}}_p}$ in an ambient saturated elementary extension $\mathbb {K}$ of ${{\mathbb {Q}}_p}$, so as to make sense of the notions of f-generic type, etc. In this paper we will show that every definable f-generic group definable in ${{\mathbb {Q}}_p}$ is virtually isomorphic to a finite index subgroup of a trigonalizable algebraic group over ${{\mathbb {Q}}_p}$. This is analogous to the o-minimal context, where every connected torsion-free group definable in $\mathbb {R}$ is isomorphic to a trigonalizable algebraic group [5, Lemma 3.4]. We will also show that every open definable f-generic subgroup of a definable f-generic group has finite index, and every f-generic type of a definable f-generic group is almost periodic, which gives a positive answer to the problem raised in [28] of whether f-generic types coincide with almost periodic types in the p-adic case. Cognitive architectures are unified theories of cognition that take the form of computational formalisms. They support computational models that collectively account for large numbers of empirical regularities using small numbers of computational mechanisms. Empirical coverage and parsimony are the most prominent criteria by which architectures are designed and evaluated, but they are not the only ones. This paper considers three additional criteria that have been comparatively undertheorized. (a) Successful architectures possess subjective and intersubjective meaning, making cognition comprehensible to individual cognitive (...) scientists and organizing groups of like-minded cognitive scientists into genuine communities. (b) Successful architectures provide idioms that structure the design and interpretation of computational models. (c) Successful architectures are strange: They make provocative, often disturbing, and ultimately compelling claims about human information processing that demand evaluation. (shrink)

Sen’s capabilities approach offers a radical generalisation of the conventional approach to welfare economics. It has been highly influential in development and many researchers are now beginning to explore its implications for health care. This paper contributes to the emerging debate by discussing two examples of such applications: first, at the individual decision making level, namely the right to die, and second, at the social choice level. For the first application, which draws on Nussbaum’s list of capabilities, it is argued (...) that many capabilities are ambiguously or indirectly related to the right to die, but the ability to form a concept of the good life and plan one’s own life provides a direct justification for such a right. In the second application, the focus is specifically on healthcare rationing and it is argued that, although not committed to age based rationing, the capabilities approach provides a more natural justification of age related access to health care than the fair innings argument, which is often used to justify the alleged ageism inherent in quality adjusted life years maximisation. An explicit formal-ontological representation of entities existing at multiple levels of granularity is an urgent requirement for biomedical information processing. We discuss some fundamental principles which can form a basis for such a representation. We also comment on some of the implicit treatments of granularity in currently available ontologies and terminologies (GO, FMA, SNOMED CT).

Since January 2004, violent conflicts in the deep South of Thailand have caused 4,453 deaths, 7,239 injuries in 10,386 violent incidents. The numbers are increasing everyday. Myriads of studies, strategies and proposals have been put forth to address and redress this deep-rooted problem. This paper is a modest attempt to find analysis and inspiration from the rich cultural resources of Buddhism to address the question of conflict and conflict transformation in Thai society.The Buddhist “Genesis” or The Agganna-sutta has been analyzed (...) by Thai and Western Buddhist scholars as offering an allegorical tale which details a long, inter-dependent process of human beings’ moral degradation on the one hand, and socio-political evolution on the other. This is considered to be the Buddhist equivalent of the Genesis without God.This paper offers a re-reading of this important sutta as a narrative of conflict and conflict transformation. It argues that the personal conflict of the two young Brahmins aspiring to be fully ordained is transcended and transformed by the Buddha’s allegorical tale of human socio-political evolution. This paper offers an analysis of the narration and demonstrates that in the narrating process, the Buddha was “substituting” the Hindu creation myth with the Discourse on What is Primary (Agganna-sutta). This narrative replacement subverts the deep-rooted cultural force of the Hindu creation myth which serves as a cosmological justification for an hierarchy of classes/castes in the social world. Once the creation Hindu myth is de-mythologized, the structure of meaning for the personal conflict of the two Brahmins evaporates. In this way, the Buddha’s approach to conflict transformation addresses three dimensions simultaneously, namely, the personal, the socio-political and the cosmological.This re-reading is one paradigmatic example of how to revitalize Buddhism to function as an agent of social transformation by offering a Buddhist creative approach to conflict transformation. In the last fifty years, average overall health status has increased more or less in parallel with a much celebrated decline in mortality, attributed mostly to poverty reduction, sanitation, nutrition, housing, immunization, and improved medical care. It is becoming increasingly clear, however, that these achievements were not equally distributed. In most countries, while some social groups have benefited significantly, the situation of others has stagnated or may even have worsened.If health is a prerequisite to a person functioning as an agent, (...) inequalities in health constitute inequalities in people's capability to function -- a denial of equality of opportunity. So why should a concern with health equity be singled out from the pursuit of social justice more generally? Can existing theories of justice provide an adequate account of health equity? And what ethical problems arise in evaluating health inequalities? These are some of the important questions that this book addresses in building an interdisciplinary understanding of health equity. With contributions from distinguished philosophers, anthropologists, economists, and public-health specialists, it centres on five major themes: what is health equity?; health equity and social justice; responsibilities for health; ethical issues in health evaluation; and anthropological perspectives. We introduce and study the notions of a PAC-substructure of a stable structure, and a bounded substructure of an arbitrary substructure, generalizing [10]. We give precise definitions and equivalences, saying what it means for properties such as PAC to be first order, study some examples (such as differentially closed fields) in detail, relate the material to generic automorphisms, and generalize a "descent theorem" for pseudo-algebraically closed fields to the stable context. We also point out that the elementary invariants of pseudo-algebraically (...) closed fields from [6] are also valid for pseudo-differentially closed fields. (shrink)

We study type-definable subgroups of small index in definable groups, and the structure on the quotient, in first order structures. We raise some conjectures in the case where the ambient structure is o-minimal. The gist is that in this o-minimal case, any definable group G should have a smallest type-definable subgroup of bounded index, and that the quotient, when equipped with the logic topology, should be a compact Lie group of the "right" dimension. I give positive answers to the conjectures (...) in the special cases when G is 1-dimensional, and when G is definably simple. (shrink)

Modal rationalism includes the thesis that ideal primary positive conceivability entails primary possibility. Modal monism is the thesis that the space of logically possible worlds is coextensive with the space of metaphysically possible worlds. In this paper I explore the relation between the two theses. My aim is to show that the former thesis implies the latter thesis, and that problems with the latter make the former implausible as a complete picture of the epistemology of modality. Analytic Panpsychism has been brought into contact with Indian philosophy primarily through an examination of the Advaita Vedānta tradition and the Yogācāra tradition. In this work I explore the relation between Rāmānuja, the 12th century father of the Viśiṣṭādvaita Vedānta tradition, and analytic panpsychism. I argue that Rāmānuja’s philosophy inspires a more world affirming form of cosmopsychism where there are different kinds of reality, rather than one fundamental reality of pure consciousness and an ordinary wrold that is illusory from the (...) perspective of fundamental reality. Approximately two billion people lack access to medicines globally. People living with HIV, cancer patients, those suffering from tuberculosis or malaria, and other populations in desperate need of life-saving medicines are increasingly unable to access existing preventative, curative, and life-prolonging treatments. In many cases, treatment may be unavailable or inaccessible for even some of the most common and readily treatable health concerns, such as hypertension. In the developing world, many of the factors that contribute to making the world’s most vulnerable (...) and marginalized populations particularly susceptible to illness also operate to restrict their access to medicines. As a result of dramatic economic inequities and widespread poverty, it is not profitable for most originator pharmaceutical companies to develop new medicines for sale in developing markets or to lower the cost of existing drugs so that they are affordable for the majority of these populations.
