In this article, we provide a comprehensive analysis and a normative assessment of rationing through inconvenience as a form of rationing. By “rationing through inconvenience” in the health sphere, we refer to a nonfinancial burden that is either intended to cause or has the effect of causing patients or clinicians to choose an option for health-related consumption that is preferred by the health system for its fairness, efficiency, or other distributive desiderata beyond assisting the immediate patient. We argue that under (...) certain conditions, rationing through inconvenience may turn out to serve as a legitimate and, compared to direct rationing, even a preferable tool for rationing; we propose a research agenda to identify more precisely when that might be the case and when, alternatively, rationing through inconvenience remains ethically undesirable. After defining and illustrating rationing through inconvenience, we turn to its moral advantages and disadvantages over other rationing methods. We take it as a starting assumption that rationing, understood as scarce-resource prioritization, is inevitable and, in a society that has goals beyond optimizing health care for individual patients—such as improving societal health care, education, or overall welfare—prudent and fair. Based on multiDEVS formalism, we introduce multiPDEVS, a parallel and nonmodular formalism for discrete event system specification. This formalism provides combined advantages of PDEVS and multiDEVS approaches, such as excellent simulation capabilities for simultaneously scheduled events and components able to influence each other using exclusively their state transitions. We next show the soundness of the formalism by giving a construction showing that any multiPDEVS model is equivalent to a PDEVS atomic model. We then present the simulation procedure associated, usually called (...) abstract simulator. As a well-adapted formalism to express cellular automata, we finally propose to compare an implementation of multiPDEVS formalism with a more classical Cell-DEVS implementation through a fire spread application. We read with interest the extended essay published from Riisfeldt and are encouraged by an empirical ethics article which attempts to ground theory and its claims in the real world. However, such attempts also have real-world consequences. We are concerned to read the paper’s conclusion that clinical evidence weakens the distinction between euthanasia and normal palliative care prescribing. This is important. Globally, the most significant barrier to adequate symptom control in people with life-limiting illness is poor access to opioid analgesia. (...) Opiophobia makes clinicians reluctant to prescribe and their patients reluctant to take opioids that might provide significant improvements in quality of life. We argue that the evidence base for the safety of opioid prescribing is broader than that presented, restricting the search to palliative care literature produces significant bias as safety experience and literature for opioids and sedatives exists in many fields. This is not acknowledged in the synthesis presented. By considering additional evidence, we reject the need for agnosticism and reaffirm that palliative opioid prescribing is safe. Second, palliative sedation in a clinical context is a poorly defined concept covering multiple interventions and treatment intentions. We detail these and show that continuous deep palliative sedation is a specific practice that remains controversial globally and is not considered routine practice. Rejecting agnosticism towards opioids and excluding CDPS from the definition of routine care allows the rejection of Riisfeldt’s headline conclusion. On these grounds, we reaffirm the important distinction between palliative care prescribing and euthanasia in practice. (shrink)

There is only one “race”: the human race; to think otherwise is like still believing that the earth is flat. But “racism” is a different matter. It exists as a system of beliefs and prejudices that people differ along biological and genetic lines and (...) that one’s own group is superior to another group. When these beliefs and prejudices are coupled with power—especially the power to negatively affect the lives of those perceived to be inferior—we have a serious problem. And no one should downplay or underestimate the harm that such an ideology inflicts upon everyone who participates in it. According to one African American professor of social work, “America’s history is inextricably bound to this racist ideology. From the codifying of slavery, to the belief in ‘Manifest Destiny,’ to the treatment of ‘illegal immigrants,’ many of America’s actions continue to conflict with its creed that ‘All men are created equal.’”1Over the past decade or so, I have written a number of pieces that focus upon racism in America and racism in so-called American Buddhism.2 Being African American myself, my reasons for this focus may perhaps be clear though not necessarily inevitable. I have written on a number of other topics: Buddhist philosophical discourses and life stories of Buddhist “saints,” for example. In my writings on racism, I have tried to communicate something of the emotional and psychic toll it takes to live in this country as a person of color. In a number of these writings, I have called myself a “Baptist-Buddhist,” and thus this is also one of my “dual belongings,” one of my “multiple identities.”When I speak of racism, I am not concerned with garnering pity, or playing the “race card” of guilt, or telling you, from some assumed position of superiority on the matter, that there is simply no way for you to understand this toll. I certainly cannot speak for all African Americans, or even for most of them. Early on in the process of writing this paper, I emailed Jung and Paul to say that I found the title of the panel both much too broad and much too narrow. “Too broad” because one person cannot possibly channel and/or verbalize the countless instances of suffering endured by African Americans (and more broadly, by persons of color) throughout space and over centuries in this country owing to slavery and its aftermath. And “too narrow” [End Page 97] because, ultimately, we want to do more than just rehash our many sufferings. We don’t want to wallow in victimhood. We want to move on—from the recognition of that immense suffering, seen and unseen—and to go out into the world and act to bring about social, economic, and ecological justice. That’s what we are called upon to do: to form a new community wherein there is compassion and caring for one another, where there is love and peace, nonviolence, reconciliation, and justice.So, even though I am quite frankly tired—as are so many people of color—of being the “token” African American who can add visible “diversity” to a group in order to show that it is now more “inclusive” than before, nevertheless, in this so-called postracial society where we have a black president and the demographic of “person of color” now comprises an actual majority of the United States’ population, I must still rise to say that, sadly, racism is not dead in this country! Its painful vestiges exist, and those vestiges are far from being either innocent or unharmful.I truly believe that—for African Americans, in particular—it is the trauma and the legacy of slavery that haunts us in the deepest recesses of our souls. Montesquieu's Considérations sur les causes de la grandeur des Romains et de leur décadence (1733/1734) is a methodological exercise in causal explanation on the meso-level applied to the subject of the military rise and fall of Rome. Rome is described as a system with contingent initial conditions that have a strong path-determining effect. Contingent and plastic initial configurations become highly determining in their subsequent operation, thanks to self-reinforcing feedback loops. Montesquieu's method seems influenced by the ruthless commitment to efficient causality (...) and the reductionism of seventeenth-century mechanicist philosophy; but in contrast to these predecessors, he is more interested in dynamic processes than in unchangeable substances, and his use of efficient causality in the context of a system approach implies a form of holism that is lacking in his predecessors. The formal and conceptual analysis in this article is in many ways complementary with Paul Rahe's recent predominantly political analysis of the Considérations. At the same time, this article points to a problem in the works on the Enlightenment by Jonathan Israel: his account stresses a one-dimensional continuum consisting of Radical, Moderate and Counter-Enlightenment. This invites Israel to place the combined religious, political and philosophical views of each thinker on one of these three points. His scheme runs into trouble when a thinker with moderate religious and political views produces radical philosophical concepts. Montesquieu's Considérations is a case in point. (shrink)

That, however, he read him carefully and thoroughly time and again there can be no doubt, and it is also clear that he couches his argument both in his Considerations on the Causes of the Greatness of the Romans and their Decline and in his Spirit of Laws as an appropriation and critique of the work of (...) the predecessor whom he termed ‘this great man’. Blanchot took Mallarmé’s “Book” as the paradigm for an artwork that aspired to such excess it could not exist. And yet it partly did, in the form of the poem Un coup de dés jamais n’abolira le hasard. For Blanchot, this ultimate literary work acted as a model for a relentless deconstructing not just of what existed but also of that which did not. His emptying theoretical perspective is ideally suited to analyse the phenomenon that is harsh noise wall music. (...) This type of music aims to be both total and static, an extreme stilling of the music impulse, and even of noise itself. This stilling will of course fail, in the shape of recordings, concerts, recognizable “pieces,” but this failing puts it into the same realm as the Book, as glossed and mobilized by Blanchot, in that it exists but it should not. It cannot happen and yet it unfolds. Rather than failure, then, I argue that this type of noise music is one realization of Blanchot’s thinning, dissipating and disappearing of Mallarmé’s project. This completion, present in these writers and in harsh noise wall, notably in the work of Romain Perrot is not just resemblance but is something that enables a reading, a return completion of Blanchot. Bien qu’importante, cette recherche, d’ordre exégétique, doit déboucher sur l’herméneutique, entrer en dialogue avec les contextes pluriels d’aujourd’hui. C’est ce à quoi l’auteur s’attache dans ce troisième d’une série de quatre articles sur «l’homosexualité et la parole de Dieu». L’objet de cet article est le texte de Romains 1,26-27, le seul texte (...) du Second Testament qui semble traiter de façon explicite de l’homosexualité. Suivant une procédure établie dans les articles précédents, l’auteur fait suivre des considérations exégétiques sur le passage de Romains de réflexions de type herméneutique soulevées par une lecture contemporaine de ce texte.
