The power of the application of bioinformatics across multiple publicly available transcriptomic data sets was explored. Using 19 human and mouse circadian transcriptomic data sets, we found that NR1D1 and NR1D2 which encode heme‐responsive nuclear receptors are the most rhythmic transcripts across sleep conditions and tissues suggesting that they are at the core of circadian rhythm generation. Analyzes of human transcriptomic data show that a core set of transcripts related to processes including immune function, glucocorticoid signalling, and lipid metabolism is (...) rhythmically expressed independently of the sleep‐wake cycle. We also identify key transcripts associated with transcription and translation that are disrupted by sleep manipulations, and through network analysis identify putative mechanisms underlying the adverse health outcomes associated with sleep disruption, such as diabetes and cancer. Comparative bioinformatics applied to existing and future data sets will be a powerful tool for the identification of core circadian‐ and sleep‐dependent molecules. (shrink)

What do we mean by theory in the arts, and what role does it play in that sense? Theory appears as an active, transformative, fluid, and communicative element of art practices, and research in the arts in particular. Theory is the fluid that perfuses and connects the territory of fine art and design, as well as other art forms such as music, architecture, writing, moving images, dance, and theatre. At the heart of this publication is the new field of research (...) in theory and the way the ArtEZ Theory in the Arts research group investigates its possibilities and extends its boundaries. The goal of this paper is twofold. First, we argue that the understanding one has of a proposition or a propositional content of a representational vehicle is a species of what contemporary epistemologists characterise as objectual understanding. Second, we demonstrate that even though this type of understanding differs from linguistic understanding, in many instances of successful communication, these two types of understanding jointly contribute to understanding a communicated thought.

BackgroundJunior medical doctors have a key role in discussions and decisions about treatment and end-of-life care for people with dementia in hospital. Little is known about junior doctors’ decision-making processes when treating people with dementia who have advance care directives, or the factors that influence their decisions. To describe among junior doctors in relation to two hypothetical vignettes involving patients with dementia: their legal compliance and decision-making process related to treatment decisions; the factors influencing their clinical decision-making; and the factors (...) associated with accurate responses to one hypothetical vignette. MethodA cross-sectional survey of junior doctors, including trainees, interns, registrars and residents, on clinical rotation in five public hospitals located in one Australian state. The anonymous, investigator-developed survey was conducted between August 2018 and June 2019. Two hypothetical vignettes describing patients with dementia presenting to hospital with an ACD and either: bacterial pneumonia; or suspected stroke were presented in the survey. Participants were asked to indicate whether they would commence treatment, given the ACD instructions described in each vignette.ResultsOverall, 116 junior doctors responded. In Vignette 1, 58% of respondents selected the legally compliant option. Participants who chose the legally compliant option perceived ‘following patient wishes’ and ‘legal requirements to follow ACDs’ as equally important reasons for complying with the ACD. The most common reason for not selecting the legally compliant option in Vignette 1 was the ‘ACD is relevant in my decision-making process, but other factors are more relevant’. In Vignette 2, 72% of respondents indicated they would commence treatment and 18% selected they would not commence treatment.. Similar reasons influenced participant decision-making in Vignette 2, a less legally certain scenario.ConclusionsThere are critical gaps in junior doctors’ compliance with the law as it relates to the implementation of ACDs. Despite there being differences in relation to the legal answer and its certainty, clinical and ethical factors guided decision-making over and above the law in both vignettes. More education and training to guide junior doctors’ clinical decision-making and ensure compliance with the law is required. (shrink)

Objective: Ethical guidelines are designed to ensure benefits, protection and respect of participants in clinical research. Clinical trials must now be registered on open-access databases and provide details on ethical considerations. This systematic survey aimed to determine the extent to which recently registered clinical trials report the use of standard of care and post-trial obligations in trial registries, and whether trial characteristics vary according to setting. Methods: We selected global randomized trials registered on http://www.clinicaltrials.gov and http://www.controlled-trials.com. We searched for intervention (...) trials of HIV/AIDS, malaria, and tuberculosis from 9 October 2004, the date of the most recent version of the Helsinki Declaration, to 10 April 2007. Results: We collected data from 312 trials. Fifty-eight percent (58%, 95% CI = 53 to 64) of trial protocols report informed consent. Fifty-eight percent (58%, 95% CI = 53 to 64) of trials report active controls. Almost no trials (1%, 95% CI = 0.5 to 3) mention post-trial provisions. Most trials measure surrogate outcomes. Twenty percent (20%, 95% CI = 16 to 25) of trials measure patient-important outcomes, such as death; and the odds that these outcomes are in a low income country are five times greater than for a developed country (odds ratio (OR) 5.03, 95% CI = 2.70 to 9.35, p = < 0.001). Pharmaceutical companies are involved in 28% (CI = 23 to 33) of trials and measure surrogate outcomes more often than nonpharmaceutical companies (OR 2.45, 95% CI = 1.18 to 5.09, p = 0.31). Conclusion: We found a large discrepancy in the quality of reporting and approaches used in trials in developing settings compared to wealthier settings. (shrink)

There is a sense in which it is trivial to say that one accepts intention- (or convention-) based semantics.[2] For if what is meant by this claim is simply that there is an important respect in which words and sentences have meaning (either at all or the particular meanings that they have in any given natural language) due to the fact that they are used, in the way they are, by intentional agents (i.e. speakers), then it seems no one should (...) disagree. For imagine a possible world where there are physical things which share the shape and form of words of English or Japanese, or the acoustic properties of sentences of Finnish or Arapaho, yet where there are no intentional agents (or where any remaining intentional agents don’t use language). In such a world, it seems clear that these physical objects, which are only superficially language-like, will lack all meaning. Furthermore, it seems that questions of particular meaning are also settled by the conventions of intentional language users: it’s nothing more than convention which makes the concatenation of letters ‘a’^‘p’^‘p’^‘l’^‘e’ mean apple, rather than banana, in English.[3] So, understood as the minimal claim that intentional agents, who have a practice of using certain physical objects (written words, sounds, hand gestures, etc) to communicate certain thoughts, are a prerequisite for linguistic meaning, the idea that semantics is based on both intention and convention seems indisputable. I will label a theory which recognises this preconditional role for speaker intentions an A-style intention-based semantics and we will explore one such account in §1. (shrink)

If asked for an example of a rigid designator it is likely that one would suggest a name, like ‘Aristotle’ or ‘Tony Blair’, or a demonstrative, like ‘that book’ said whilst pointing at a certain text. Intuitively, what these expressions have in common is the central role they accord to perception of an object: you can see the book you want to talk about, there are (...) people around in our community who have bumped into Tony, and, although no one alive today perceived Aristotle directly, it seems plausible to claim that our ability to use the name now relies on the fact that someone, sometime, did perceive him directly. However, as anyone at all familiar with rigid designation knows, not all such expressions follow this model. Kripke himself stressed that certain definite descriptions have a constant extension across all possible worlds (for example, ‘the smallest prime number’, ‘the actual prime minister of Great Britain now’) and thus meet the criterion for being rigid designators; while Kaplan emphasized the role of a descriptive rule in determining the referent for a token utterance of an indexical, like ‘I’ or ‘tomorrow’. It is often held that a correct semantic theory should assign a semantic content, p, to a given sentence, s, just in case a speaker who utters s says that p – thus ‘what is said’ is taken to be a semantically significant notion. This paper explores what exactly such a claim amounts to and offers five versions of the relationship between a semantic theory and judgements of what is said. The first three of these versions embody the central claim (...) of semantic significance; however, I argue that none of these versions are feasible. Thus, contrary to the initial proposal, I claim that ‘what is said’ is not a semantically significant notion. Assignments of semantic content do not turn on evaluations of what a speaker uttering a sentence says. (shrink)

This paper investigates the challenges of implementing antibiotic stewardship – reducing and optimizing the use of antibiotics – in agricultural settings of Low- and Middle-Income Countries (LMICs) as a strategic part of addressing the global problem of antimicrobial resistance (AMR). It does so through analysis of the rapidly transforming yet diverse Kenyan poultry sector, characterized by growing commercial operations alongside traditional smallholder farming. Our research involves interviews with farmers, processors, policymakers, and agro-veterinary stores in these settings. We blend Chandler’s ( (...) 2019, p. 8) notion of “antibiotics as infrastructure” with value chain frameworks to understand how the structural role of antibiotics in agriculture plays out through contrasting value chains, with different implications for stewardship. Weak regulation and intense market-based pressures are shown to drive widespread antibiotic use in poultry value chains involving small- and medium-sized farms supplying open markets. Antibiotic stewardship through adherence to agricultural and food safety standards is more evident, though unevenly observed, in value chains involving large commercial farms and processors supplying corporate buyers. Our findings reveal the complex structural roles of antibiotics in maintaining producer livelihoods in an intensely competitive and heterogeneous Kenyan poultry sector. This highlights challenges with applying global AMR policy to transforming food systems in LMICs without appropriate translation. We argue that attempts to reduce and optimize the use of antibiotics in agriculture must be informed by nuanced understandings of the roles of antibiotics in food systems in specific places including where very different scales and models of farming and value chain co-exist. This paper has two central aims. First, we motivate a puzzle. The puzzle features four independently plausible but jointly inconsistent claims. One of the four claims is the sufficiency leg of the knowledge norm of assertion (KNA-S), according to which one is properly epistemically positioned to assert that p if one knows that p. Second, we propose that rejecting (KNA-S) is the best way out of the puzzle. Our argument to this end appeals to the epistemic value of intellectual humility (...) in social-epistemic practice. Given the surprising cross-cultural robustness of our findings, we suggest that the types of evidence for the ascription of a belief are, at least in some circumstances, lexicographically ordered: assertions are first taken into account, and when an agent sincerely (...) asserts that p, non-linguistic behavioral evidence is disregarded. In the seventeenth century the new science was introduced through the works of Bacon, Hooke, Boyle, Power, and others. The advocates of the new science promised to divulge the inner workings of nature and to help man overcome his painful fallen state by means of controlling nature. The new sciences of mechanism and corpuscularism were to be based on objective experiments that would reveal the secret inner natures of minerals, vegetables, animals, the sun, moon, and stars. These experiments were done (...) with new and improved telescopes and microscope with magnifications of up to 100 times. One early critic of the new science was Margaret Cavendish. Cavendish was skeptical of the ambitious claims, methodology, instruments, and institutions of the new science. In her work, Observations Upon Experimental Philosophy, Cavendish argued against “experimental and dioptrical writers,” provided her own account of the natural world, investigated aspects of chemistry, medicine, and the nature of heat and color, as well as many other topics in natural philosophy (OEP 10). While many think Cavendish landed on the wrong side of history with respect to her skepticism regarding microscopes and telescopes, her criticisms of the new science were wide ranging and she was by no means the only one to question the value of such experiments and instruments. While several commentators, like Eve Keller (1997), have argued that Cavendish was against all things experimental, several recent commentators, Emma Wilkins (2014) and Deborah Boyle (2018), have tried to show the much more complicated relationship between Cavendish and the Royal Society and medical studies, respectively. In addition, some commentators, such as Lisa Sarasohn (1984, 2010) and Eve Keller (1997), have argued that Cavendish’s criticisms of the new science are based on her belief that Nature, as a representation of the feminine, was under attack by the experimentalists desire to “penetrate” and “manipulate” her for their own ends. While it is certainly true that Cavendish and many of the experimentalists personified nature as a woman, and that Cavendish does portray the men as trying to make her into something she is not, I agree with Deborah Boyle (2004) that these descriptions are not the focus of her objections to experimentalism. Rather than hold that Cavendish is concerned with, as Sarasohn claims, “the sexual implications for both women and nature of the new philosophy” (2010: 147), it seems that Cavendish’s objections were largely based on her philosophical commitments. However, I believe there is one aspect of the new science that Cavendish does critique from a feminist perspective, and that is what she sees as its institutional nature and its exclusion of women on the basis of sex, and to this I will turn in the last section of the chapter.1 My aim is to address Cavendish’s three major critiques of the new science. The chapter is divided as follows: The first section provides a brief overview of Cavendish’s views on the nature of bodies and perception. The second regards her critique of the methods and aims of the new science as represented by Bacon and Boyle. The third section examines her critique of Hooke and the instruments of experimentalism. The final section lays out her feminist critique of the institution of the new science. (shrink)

Both patients and clinicians frequently report problems around communicating and assessing pain. Patients express dissatisfaction with their doctors and doctors often find exchanges with chronic pain patients difficult and frustrating. This chapter thus asks how we could improve pain communication and thereby enhance outcomes for chronic pain patients. We argue that improving matters will require a better appreciation of the complex meaning of pain terms and of the variability and flexibility in how individuals think about pain. We start by examining (...) the various accounts of the meaning of pain terms that have been suggested within philosophy and suggest that, while each of the accounts captures something important about our use of pain terms, none is completely satisfactory. We propose that pain terms should be viewed as communicating complex meanings, which may change across different communicative contexts, and this in turn suggests that we should view our ordinary thought about pain as similarly complex. We then sketch what a view taking seriously this variability in meaning and thought might look like, which we call the “polyeidic” view. According to this view, individuals tacitly occupy divergent stances across a range of different dimensions of pain, with one agent, for instance, thinking of pain in a much more “bodycentric” kind of way, while another thinks of pain in a much more "mindcentric” way. The polyeidic view attempts to expand the multidimensionality recognised in, e.g., biopsychosocial models in two directions: first, it holds that the standard triumvirate— dividing sensory/cognitive/affective factors— needs to be enriched in order to capture important distinctions within the social and psychological dimensions. Second, the polyeidic view attempts to explain (at least in part) why modulation of experience by these social and psychological factors is possible in the first place. It does so by arguing that because the folk concept of pain is complex, different weightings of the different parts of the concept can modulate pain experience in a variety of ways. Finally, we argue that adopting a polyeidic approach to the meaning of pain would have a range of measurable clinical outcomes. (shrink)

This chapter examines the way the two main philosophies of Heidegger and Derrida, come into critical contact with each other. Derrida represents his own thinking as a development of Heideggerian thought. The chapter discusses Derrida's engagement with Heidegger spanned nearly his entire philosophical career. Derrida's exploration of Heidegger's spiritual idiom, while somewhat unusual, is not unconnected with his other writings on Heidegger. Through tracing Heidegger's spiritual idiom, Derrida seeks to bring out what is at stake in those aspects of Heidegger's (...) thought he finds uncertain or, to put it more bluntly, questionable. In tracing Derrida's reading of Heidegger in Of Spirit, then, one shall in effect be crossing two pathways. One uses the medium of spirit to introduce certain critical questions for Heideggerian philosophy, specifically questions that relate to the political and geo political aspects of Heidegger's work. Scientists, philosophers, and policymakers disagree about how to define microaggression. Here, we offer a taxonomy of existing definitions, clustering around (a) the psychological motives of perpetrators, (b) the experience of victims, and (c) the functional role of microaggression in oppressive social structures. We consider conceptual and epistemic challenges to each and suggest that progress may come from developing novel hybrid accounts of microaggression, combining empirically tractable features with sensitivity to the testimony of victims.

In this article, we aim to map out the complexities which characterise debates about the ethics of vaccine distribution, particularly those surrounding the distribution of the COVID-19 vaccine. In doing so, we distinguish three general principles which might be used to distribute goods and two ambiguities in how one might wish to spell them out. We then argue that we can understand actual debates around the COVID-19 vaccine – including those over prioritising vaccinating the most vulnerable – as reflecting disagreements (...) over these principles. Finally, we shift our attention away from traditional discussions of distributive justice, highlighting the importance of concerns about risk imposition, special duties, and social roles in explaining debates over the COVID-19 vaccine. We conclude that the normative complexity this article highlights deepens the need for decision-making bodies to be sensitive to public input. (shrink)

It would be desirable to have some notion of strong reflection to remedy this, and we have proposed Global Reflection Principles based on a somewhat Cantorian view of the universe. Such principles justify the kind of cardinals needed for, inter alia , Woodin’s Ω-Logic.

Minimal Semantics asks what a theory of literal linguistic meaning is for - if you were to be given a working theory of meaning for a language right now, what would you be able to do with it? Emma Borg sets out to defend a formal approach to semantic theorising from a relatively new type of opponent - advocates of what she call 'dual pragmatics'. According to dual pragmatists, rich pragmatic processes play two distinct roles in linguistic comprehension: as (...) well as operating in a post-semantic capacity to determine the implicatures of an utterance, they also operate prior to the determination of truth-conditional content for a sentence. That is to say, they have an integral role to play within what is usually thought of as the semantic realm. Borg believes dual pragmatic accounts constitute the strongest contemporary challenge to standard formal approaches to semantics since they challenge the formal theorist to show not merely that there is some role for formal processes on route to determination of semantic content, but that such processes are sufficient for determining content. Minimal Semantics provides a detailed examination of this school of thought, introducing readers who are unfamiliar with the topic to key ideas like relevance theory and contextualism, and looking in detail at where these accounts diverge from the formal approach. Borg's defence of formal semantics has two main parts: first, she argues that the formal approach is most naturally compatible with an important and well-grounded psychological theory, namely the Fodorian modular picture of the mind. Then she argues that the main arguments adduced by dual pragmatists against formal semantics - concerning apparent contextual intrusions into semantic content - can in fact be countered by a formal theory. The defence holds, however, only if we are sensitive to the proper conditions of success for a semantic theory. Specifically, we should reject a range of onerous constraints on semantic theorizing (e.g., that it answer epistemic or metaphysical questions, or that it explain our communicative skills) and instead adopt a quite minimal picture of semantics. Microaggressions are seemingly negligible slights that can cause significant damage to frequently targeted members of marginalized groups. Recently, Scott O. Lilienfeld challenged a key platform of the microaggression research project: what’s aggressive about microaggressions? To answer this challenge, Derald Wing Sue, the psychologist who has spearheaded the research on microaggressions, needs to theorize a spectrum of aggression that ranges from intentional assault to unintentional microaggressions. I suggest turning to Bonnie Mann’s “Creepers, Flirts, Heroes and Allies” for inspiration. Building from Mann’s (...) richer theoretical framework will allow Sue to answer Lilienfeld’s objection and defend the legitimacy of the concept, ‘microaggression’. (shrink)

This chapter offers a brief analysis of the problems facing Bonnett's attempt to articulate a richer conception of thinking. Bonnett states, authentic thinking and understanding require that one become the originators and authors of their own thinking, as against merely reflecting the thoughts of others. In order to deflect the criticism that the account is promoting a self‐centred view of thinking, Bonnett introduces the concept of self referencing, which he describes as the determination to understand what one learns in terms (...) of one's own experiences. Bonnett states, the discipline and rigour of poetic thinking are rooted in the underlying sense of personal responsibility. One of the main flaws with Bonnett's account is that he simply does not engage in a careful enough reading of the philosophical accounts of thinking he draws upon, especially the Heideggerian accounts of being in the world and truth.
