Magnetic resonance imaging is a noninvasive imaging tool that utilizes a strong magnetic field and radio frequency waves to visualize in great detail organs, soft tissue, and bone. Unlike conventional x-rays, there is no exposure to ionizing radiation and at most field strengths the procedure is considered safe for nearly every age group. Because it is non-invasive and possesses excellent spatial resolution, the use of MRI as a research tool has increased exponentially over the past decade. Uses have ranged from (...) add-ons to a clinical study to studies of brain development in typically developing children. In addition, a major effort has been made in recent years to use MRI to study brain function. Because the clinical utility of fMRI has not yet been realized, fMRI is still considered highly exploratory, and we cannot yet identify incidental findings of a functional nature. (shrink)

The amount of biomedical research being conducted around the world has greatly expanded over the past 15 years, with particularly large growth occurring in low- and middle-income countries. This increased focus on understanding and responding to disease burdens around the world has brought forth a desire to help LMIC institutions enhance their own capacity to conduct scientifically and ethically sound research. In support of these goals the Johns Hopkins-Fogarty African Bioethics Training Program has, for the past six years, partnered with (...) three research institutions in Africa to support research ethics capacity. Each partnership began with a baseline evaluation of institutional research ethics environments in order to properly tailor capacity strengthening activities and help direct limited institutional resources. Through the course of these partnerships we have learned several lessons regarding the evaluation process and the framework used to complete the assessments. We believe that these lessons are generalizable and will be useful for groups conducting such assessments in the future. (shrink)

No consensus yet exists on how to handle incidental fnd-ings in human subjects research. Yet empirical studies document IFs in a wide range of research studies, where IFs are fndings beyond the aims of the study that are of potential health or reproductive importance to the individual research participant. This paper reports recommendations of a two-year project group funded by NIH to study how to manage IFs in genetic and genomic research, as well as imaging research. We conclude that researchers (...) have an obligation to address the possibility of discovering IFs in their protocol and communications with the IRB, and in their consent forms and communications with research participants. Researchers should establish a pathway for handling IFs and communicate that to the IRB and research participants. We recommend a pathway and categorize IFs into those that must be disclosed to research participants, those that may be disclosed, and those that should not be disclosed. (shrink)

As moral injury is a still-emerging concept within the area of military mental health, prevalence estimates for moral injury and its precursor, potentially morally injurious events (PMIEs), remain unknown for many of the world’s militaries. The present study sought to estimate the prevalence of PMIEs in the Canadian Armed Forces (CAF), using data collected from CAF personnel deployed to Afghanistan, via logistic regressions controlling for relevant sociodemographic, military, and deployment characteristics. Analyses revealed that over 65% of CAF members reported exposure (...) to at least one event that would be considered a PMIE. The most commonly PMIEs individuals reported included seeing ill or injured women and children they were unable to help (48.4%), being unable to distinguish between combatants and non-combatants (43.6%), and finding themselves in a threatening situation where they were unable to respond due to the rules of engagement under which they were required to operate (35.4%). These findings provide support for both the presence of exposure to PMIEs in CAF members and the need for formal longitudinal data collection regarding PMIE exposure and moral injury development. This study aimed to explore experiences and practices of key research team members in obtaining informed consent for pharmacogenetics research and to identify the approaches used for enhancing understanding during the consenting process. Data collection involved 15 qualitative, in-depth interviews with key researchers who were involved in obtaining informed consent from HIV infected individuals in Uganda for participation in pharmacogenetic clinical trials. The study explored two prominent themes: approaches used to (...) convey information and enhance research participants’ understanding and challenges faced during the consenting process. Several barriers and facilitators for obtaining consent were identified. This chapter focuses on one of the common fallacies in Western philosophy called the 'is/ought fallacy (IOF)'. Some philosophers conclude that the IOF is not a logical problem but an epistemological one, meaning that even if inferences like this one are logically valid, they cannot be used epistemologically to warrant anyone's real‐life moral beliefs. Arguments do not warrant their conclusions unless the premises of those arguments are themselves warranted, and in the real world, they say, no one would ever be (...) warranted in believing premise. Charles Pigden argues for the illegitimacy of the move from is to ought, saying that we should understand David Hume as pointing to the conservativeness of logic. According to Pigden, logic is conservative in that “the conclusions of a valid inference are contained within the premises. You do not get out what you have not put in”. (shrink)

This paper examines the increasing trend by nursing scholars such as Patricia Benner to conceptualize ethics as a contextual and embodied ‘way of knowing’, embedded in nursing expertise. The intellectual origins of this development and its debt to neo‐Aristotelian thinkers such as philosopher Charles Taylor are discussed. It will be argued that rather than revealing a truth about ethical expertise, the emergence of the ‘expert’ nurse as a moral and ethical category is the result of the elaboration of neo‐Thomist (...) discourses in the educational and professional shaping of nurses. These discourses act on and are enacted by the individual nurse through his or her participation in specific ethical exercises that result in the constitution of the desired subjectivity – or ‘expertise’. Central to this shaping are particular notions of ‘the good’ and its relationship to knowledge, skill and practice. Critiques of these neo‐Aristotelian perspectives are discussed and applied to the notion of moral expertise in nursing, and the claims made by its proponents concerning the ‘ethical’ distinction between experienced and expert nurses. Finally, a call is made for a more pluralistic approach to ethics. (shrink)

Rhythm is generally taken to refer to a temporal pattern of events. Yet in recent years, across diverse fields in the arts, humanities, and social sciences, it has come to serve as the conceptual marker for a wide range of new approaches to understanding relations and relationality, following most explicitly from the late work of Henri Lefebvre. This article explores the temporal aspect of such relational thinking, in particular asking how time is implicated in relations, and how it can be (...) meaningful in those relations. The sense of time is surveyed from the perspectives of both cognition and metaphysics as well as with reference to music scholarship and social theory, and key threads are drawn, from the writings of Gaston Bachelard and Charles Sanders Peirce in particular, in order to suggest that time can be a sign, and that the signifying properties of rhythm have critical social consequences. Existing as man, the Son experiences all that pertains to historioolly conditioned humanity. In the Incarnation the (...) "·who", the person, remains the same, but the manner of the experience of the " Who " is distinctly different-divine and human-without confusion, change, division, or separation. The mystery obviously remains, but it is meaningful and coherent. As stated at the outset, Morris presents a thorough and provocative study which should be read and studied by scholars. While he aptly critiques his opponents, he fails to challenge their understanding of the incarnational notion of "become" and the ensuing union. Rather, he himself presupposes that the Incarnation implies a compositional union of natures which in turn leads to an unresolvable incompatibility between the divine and human attributes in Christ. Morris may have been aided had he been more acquainted with the early Christological controversies and councils. Nevertheless, Morris has clearly raised many important contemporary Christological issues and has admirably attempted to provide answers which are faithful both to the Christian tradition and to human i~ea..'\On and logic. In recent years the pathology of atomistic individualism has been variously described by the Council Fathers of Vahlcan II, liberationist and feminist theologians, Stanley Hauer.was, and countless other theological and secular critics. According to Frank Kirkpatrick, therapeutic intervention will not be effective in the long run unless it is based upon an alternative conceptual "model" of human oommunity. Of course, Kirkpa.tric.k has something specific in mind. Not just any alternative will do. Marxian models of human association, for example, are prone to BOOK REVIEWS 373 totalitarianism and the denial of proper individuality. Thus, he offers what he calls "the mutual/personal model of community" as "a substantial grounding for a Christian vision of community congruent with liberationist and feminist concerns " (vii). Western culture has generated a welter of perspectives on the nature and ground of human association. Within this motley, Kirkpatrick discerns three models of oommunity which have shaped Western social philosophy and religious thought. The book de~crihes these traditional models by identifying their controlling metaphors, accounts of human nature and its fulfillment, and the social, economic, and political practices and institutions they undergird. The author appraises the strengths and weaknesses of the models and attempts to reconcile their positive features within the Christian vision of Koinonia. The first model is "the atomistic/contractarian" wherein persons are regarded as "independent atoms rationally contracting with each other for the terms of their enforced relationship" (2). Kirkpatrick locates its sources in the thought of Hobbes, Locke, Bentham, James Mill, Adam Smith, and Rousseau. Observing that the individualism of this tradition has always flourished in America, Kirkpatrick surveys some of its c001temporary expressions in the work of George Gilder, F. A. Hayek, Michael N·ovak, Robert Nozick, and John Rawls. The fruits of the atomistic/ contracta.rian model are, to Kirkpatrick's taste, mostly bad-isolation, alienation, oppression, injustice-and are ineongruent with Christian Koinonia. His second model, " the organic/functional," views persons as "organs, interdependent and functionally related to each other within a larger organism" (2). Conceived by Edmund Burke, this model became fully formed and viable in the thought of Hegel and Marx, and was instrumental in the analyses of sociologists such as Ferdinand Tonnies and Charles H. Cooley. Kirkpatrick is troubled by this model's tendency to view human relationships exhaustively in terms of co-operativ.e function. Charles Pigden has argued for a logical Is/Ought gap on the grounds of the conservativeness of logic. I offer a counter-example which shows that Pigden’s argument is unsound and that there need be no logical gap between Is-premises and an Ought-conclusion. My counter-example is an argument which is logically valid, has only Is-premises and an Ought-conclusion, does not purport to violate the conservativeness of logic, and does not rely on controversial assumptions about Aristotelian biology or 'institutional facts.'.

In his final paragraph Nelson says that one could as well assert the possibility of God's nonexistence as of his existence. I can understand this only in the following sense: a positivist may well say that, for all we know at least, there may be nothing divine, since the idea of divinity cannot be shown to have consistent cognitive meaning. But an atheist, as I have defined the word, can only mean by the "possibility of God's nonexistence" that God (...) might in contingent fact not exist, and this is excluded by step 6 of the argument. Hence the non-positivistic opponent of theism must quarrel with one or more of the first six steps of the proof. He is free to do so. But some of us find these steps persuasive. (shrink)

In her anchor-piece on historicizing the canon, Li-Hsiang Lisa Rosenlee appeals to professional philosophers to develop several tools that can be implemented in historicizing the canon. Amy Donahue, David H. Kim, Nelson Maldonado-Torres, and Kris Sealey tessellate different aspects of this call. Donahue augments Rosenlee’s argument by braiding together Dharmakīrti’s “anyāpoha” theory and Charles Mills’ ruminations about “white ignorance”; Kim explores some of the nuances of Rosenlee’s account for a post-Eurocentric philosophy; Maldonado-Torres ruminates about the larger social context (...) in which thinking can be decolonized; and Sealey uses the work of Kristie Dotson to acknowledge the possibility of multiple canons. In putting on the table a number of questions, concepts, and approaches to canon-building, the symposium aims to contribute to what is by now a large array of similar reflections and engagements in different parts of the world. Over the past decade, a series of studies have found that physicians-in-training who work extended shifts are at increased risk of experiencing motor vehicle crashes, needlestick injuries, and medical errors. In response to public concerns and a request from Congress, the Institute of Medicine conducted an inquiry into the issue and concluded in 2009 that resident physicians should not work for more than 16 consecutive hours without sleep. They further recommended that the Centers for Medicare & Medicaid Services and the (...) Joint Commission work with the Accreditation Council for Graduate Medical Education to ensure effective enforcement of new work hour standards.The IOM's concerns with enforcement stem from well-documented non-compliance with the ACGME's 2003 work hour rules, and the ACGME's history of non-enforcement. In a nationwide cohort study, 84% of interns were found to violate the ACGME's 2003 standards in the year following their introduction. (shrink)

In this article, we discuss current legal restrictions governing the use of race in medical research. In particular, we focus on whether the use of race in various types of research is presently permitted under federal law and the federal constitution. We also discuss whether federal restrictions on the use of race in research ought to be expanded, and whether federal policies that encourage the use of race ought to be abandoned.

Through an engagement with Adorno's critical theory of society, Charles Prusik advances a novel approach to understanding the origins and development of neoliberalism. Offering a corrective to critics who define neoliberalism as an economic or political doctrine, Prusik argues that Adorno's dialectical theory of society can provide the basis for explaining the illusions and forms of domination that structure contemporary life. Henry Nelson Wieman’s most distinctive philosophical contributions are his identification of creative interchange as the ultimate process in human experience through which people and their institutions are able to create, sustain, improve, and cor­rect their value perspectives and, equally important, his description of creative inter­change in psychological, sociological, histor­ical, religious, and institutional contexts as subject inquiry and the experimental test of consequences. This massive collection, thirty-three orig­inal essays with an appendix and index, rep­resents the first formal attempt to consider (...) fully the interdisciplinary implications of creative interchange. Following an introduc­tion, the book is structured into six sections, beginning with historical studies on the de­velopment of Wieman’s philosophy of creativity. With this information as a base the subsequent five sections treat: _ _his metaphysics and theory of knowledge; _ _his the­ory of value; _ _creative interchange in the context of rhetoric and literary criticism; _ _creative interchange as a guide for social phi­losophy; and creative interchange as re­lated to current philosophic trends. Around the turn of the twentieth century, microbiologists in Western Europe and North America began to organize centralized collections of microbial cultures. Collectors published lists of the strains they cultured, offering to send duplicates to colleagues near and far. This essay explores the history of microbial culture collections through two cases: Johanna Westerdijk’s collection of phytopathogenic fungi in the Netherlands and Ernst Georg Pringsheim’s collection of single-celled algae at the German University in Prague. Historians of science have tended to look (...) at twentieth-century biological specimen collections as either repositories of communal research materials or storehouses of economically important biological variation. An examination of Westerdijk’s and Pringsheim’s collections illustrates how collectors, researchers, and patrons ascribed different kinds of value to collections featuring distinctive microbial life forms. This essay argues that characteristics of cultivated microorganisms, such as a fungus’s propensity to infect crops or an alga’s amenability to experimentation, shaped the trajectories of Westerdijk’s and Pringsheim’s collections as these collectors developed relationships with colleagues and patrons. Letters between Westerdijk and Pringsheim open a window onto divergences in their approaches to collecting cultures, while also shedding light on the aspirational internationality of the collections that resulted. In this pathbreaking book, which includes a powerful new translation of Hesiod's Works and Days by esteemed translator David Grene, Stephanie Nelson argues that a society's vision of farming contains deep indications about its view of the human place within nature, and our relationship to the divine. She contends that both Hesiod in the Works and Days and Vergil in the Georgics saw farming in this way, and so wrote their poems not only about farming itself, but also about (...) its deeper ethical and religious implications. Hesiod, Nelson argues, saw farming as revealing that man must live by the sweat of his brow, and that good, for human beings, must always be accompanied by hardship. Within this vision justice, competition, cooperation, and the need for labor take their place alongside the uncertainties of the seasons and even of particular lucky and unlucky days to form a meaningful whole within which human life is an integral part. Vergil, Nelson argues, deliberately modeled his poem upon the Works and Days, and did so in order to reveal that his is a very different vision. Hesiod saw the hardship in farming; Vergil sees its violence as well. Farming is for him both our life within nature, and also our battle against her. Against the background of Hesiods poem, which found a single meaning for human life, Vergil thus creates a split vision and suggests that human beings may be radically alienated from both nature and the divine. Nelson argues that both the Georgics and the Works and Days have been misread because scholars have not seen the importance of the connection between the two poems, and because they have not seen that farming is the true concern of both, farming in its deepest and most profoundly unsettling sense.
