BackgroundAlthough the Life-Sustaining Treatment (LST) Decision Act was enforced in 2018 in Korea, data on whether it is well established in actual clinical settings are limited. Hospital-acquired pneumonia (HAP) is a common nosocomial infection with high mortality. However, there are limited data on the end-of-life (EOL) decision of patients with HAP. Therefore, we aimed to examine clinical characteristics and outcomes according to the EOL decision for patients with HAP.MethodsThis multicenter study enrolled patients with HAP at 16 referral hospitals retrospectively from (...) January to December 2019. EOL decisions included do-not-resuscitate (DNR), withholding of LST, and withdrawal of LST. Descriptive and Kaplan–Meier curve analyses for survival were performed.ResultsOf 1,131 patients with HAP, 283 deceased patients with EOL decisions (105 cases of DNR, 108 cases of withholding of LST, and 70 cases of withdrawal of LST) were analyzed. The median age was 74 (IQR 63–81) years. The prevalence of solid malignant tumors was high (32.4% vs. 46.3% vs. 54.3%, P = 0.011), and the ICU admission rate was lower (42.9% vs. 35.2% vs. 24.3%, P = 0.042) in the withdrawal group. The prevalence of multidrug-resistant pathogens, impaired consciousness, and cough was significantly lower in the withdrawal group. Kaplan–Meier curve analysis revealed that 30-day and 60-day survival rates were higher in the withdrawal group than in the DNR and withholding groups (log-rank P = 0.021 and 0.018). The survival of the withdrawal group was markedly decreased after 40 days; thus, the withdrawal decision was made around this time. Among patients aged below 80 years, the rates of EOL decisions were not different (P = 0.430); however, mong patients aged over 80 years, the rate of withdrawal was significantly lower than that of DNR and withholding (P = 0.001).ConclusionsAfter the LST Decision Act was enforced in Korea, a DNR order was still common in EOL decisions. Baseline characteristics and outcomes were similar between the DNR and withholding groups; however, differences were observed in the withdrawal group. Withdrawal decisions seemed to be made at the late stage of dying. Therefore, advance care planning for patients with HAP is needed. (shrink)

We used functional magnetic resonance imaging to examine the interactive effects of perceived competence and task interest on the cognitive and affective responses to negative feedback. Twenty-four undergraduates performed both interesting and uninteresting tasks and received failure feedback. The participants’ perceived competence in the task was manipulated between subjects prior to scanning with bogus feedback. The results showed that negative feedback processing was contingent upon both perceived competence and task interest. The most adaptive coping mechanism, indicated by activation in the (...) cognitive control network and attenuation in the negative affect region, was identified for the high-competence and high-interest combination. When either competence or interest was low, signals in the cognitive control network were weaker. The most detrimental activation patterns were observed for the combination of low-competence and high interest. Our results reveal the combination of task and learner characteristics that best harnesses the potential benefits of negative feedback and illustrate the neuroscientific mechanisms underlying this observation. This study examines why individuals who are deeply embedded in the organization may engage in unethical pro-organizational behavior (UPB). Drawing from social identity theory and self-affirmation theory, we propose that deeply embedded employees may engage in UPB as a way of promoting or maintaining their status in the organization. We further propose that this positive relationship between organizational embeddedness and UPB, mediated through status perceptions, is stronger for employees working under managers who display low levels of ethical leadership. Using data (...) gathered in a two-wave survey from 224 working adults, the results of the study lend support for the hypothesized relationships and remained significant even after controlling for affective commitment. The study advances our understanding of both contextual and individual factors motivating employees to engage in UPB and the boundary conditions under which UPB can be reduced. (shrink)

Previous studies have used various external variables and parameters as well as moderator variables such as emotional intelligence have been to understand emotional labor and its related problems. However, a comprehensive model to study such variables’ correlations with each other and their overall effect on emotional labor has not yet been established. This study used a structural equation model to understand the relationship between employees’ expression of emotional labor and perception of customer feedbacks. The study also looked at how the (...) perception of customer feedback affects emotional exhaustion in order to understand how emotional exhaustion affects job satisfaction and turnover intentions. Further, in order to fully understand the effects of emotion on emotional labor at the service contact points, this study developed and tested a model of emotional labor with 4 factors of emotional intelligence as moderating factors. 579 emotional labor workers in service industries in the U.S. were collected and 577 valid survey results have been analyzed. The result shows that there exists moderating effects of emotional intelligence on how employees’ Deep Acting and Surface Acting recognize customers’ reactions, both positive and negative, that would affect employees’ Emotional Exhaustion and Job Satisfaction, and hence, Turnover Intention. The result suggests that employees with better understanding of their own emotions, although they are more likely to recover from emotional exhaustion, experience a greater negative effect when there is a discrepancy between what they feel and how they should act. Background: Infectious disease outbreaks such as COVID-19 and MERS pose a major threat to healthcare workers' physical and mental health. Studies exploring the positive changes gained from adapting to traumatic events, known as post-traumatic growth, have attracted much attention. However, it is unclear which factors or experiences lead to PTG among HCWs. The purpose of this mixed-method study was to investigate factors associated with PTG among HCWs who experienced the MERS outbreak in South Korea, and fully describe their experience of (...) developing PTG.Methods: Quantitative data from 78 participants were collected using psychometric tools for Psychological distress, Resilience, and Support for coping, and Post-traumatic growth. Qualitative interviews were conducted with seven nurses. Data were analyzed using the qualitative content analysis method according to the sub-themes of resilience, which was the main factor associated with PTG.Results: We found resilience to have a significant impact on PTG. Thus the qualitative interviews were analyzed using the core concepts of resilience. Qualitative interviews with nurses illustrated how participants experienced the development of resilience in terms of its sub-factors: hardiness, persistence, optimism, and support.Conclusion: HCWs who endured the MERS outbreak showed high levels of PTG, and the analysis of the interview data provided a fuller understanding on the experience of remaining resilient and developing PTG. These results provide practical and pragmatic information helpful for developing intervention strategies and protocols that can help HCWs transform adversity into growth and development. (shrink)

Our main result is that for any η-like computable linear ordering, such that has no interval of order type η, and such that the order type of is determined by a -limitwise monotonic maximal block function, there exists computable such that has no nontrivial automorphism.

Background Conventional consent practices face ethical challenges in continuously evolving digital health environments due to their static, one-time nature. Dynamic consent offers a promising solution, providing adaptability and flexibility to address these ethical concerns. However, due to the immaturity of the concept and accompanying technology, dynamic consent has not yet been widely used in practice. This study aims to identify the facilitators of and barriers to adopting dynamic consent in real-world scenarios. Methods This scoping review, conducted in December 2022, adhered (...) to the PRISMA Extension for Scoping Reviews guidelines, focusing on dynamic consent within the health domain. A comprehensive search across Web of Science, PubMed, and Scopus yielded 22 selected articles based on predefined inclusion and exclusion criteria. Results The facilitators for the adoption of dynamic consent in digital health ecosystems were the provision of multiple consent modalities, personalized alternatives, continuous communication, and the dissemination of up-to-date information. Nevertheless, several barriers, such as consent fatigue, the digital divide, complexities in system implementation, and privacy and security concerns, needed to be addressed. This study also investigated current technological advancements and suggested considerations for further research aimed at resolving the remaining challenges surrounding dynamic consent. Conclusions Dynamic consent emerges as an ethically advantageous method for digital health ecosystems, driven by its adaptability and support for continuous, two-way communication between data subjects and consumers. Ethical implementation in real-world settings requires the development of a robust technical framework capable of accommodating the diverse needs of stakeholders, thereby ensuring ethical integrity and data privacy in the evolving digital health landscape. BackgroundExperience with open disclosure and its study are restricted to certain western countries. In addition, there are concerns that open disclosure may be less suitable in non-western countries. The present study explored and compared the in-depth perceptions of the general public and physicians regarding open disclosure in Korea.MethodsWe applied the COREQ checklist to this qualitative study. We conducted 20 in-depth interviews and four focus group discussions with 16 physicians and 18 members of the general public. In-depth interviews and focus group (...) discussions were performed according to semi-structured guidelines developed according to a systematic review of open disclosure. We conducted a directed content analysis by analyzing the verbatim transcripts and field notes in accordance with the predetermined guidelines.ResultsOpen disclosure perceptions were summarized in terms of the “five Ws and one H”. All physician and general public participants acknowledged the normative justifiability of open disclosure. The participants mostly agreed on the known effects of open disclosure, but the physicians had negative opinions on its expected effects, such as decreased intention of the general public to file lawsuits and increased credibility of medical professionals. Generally, the participants thought that open disclosure is required for medical errors causing major harm. However, the physicians and general public had conflicting opinions on the need for open disclosure of near misses. Most physicians did not know how to conduct open disclosure and some physicians had bad experiences due to inappropriate or incomplete open disclosure.ConclusionPhysicians and the general public in Korea acknowledge the need for open disclosure. Guidelines according to the type of patient safety incident are required to encourage physicians to more readily conduct open disclosure. Furthermore, hospitals need to consider organizing a dedicated team and hiring experts for open disclosure. (shrink)

Acknowledging separable factors underlying negative symptoms may lead to better understanding and treatment of negative symptoms in individuals with schizophrenia. The current study aimed to test whether the negative symptoms factor (NSF) of the Positive and Negative Syndrome Scale (PANSS) would be better represented by expressive and experiential deficit factors, rather than by a single factor model, using confirmatory factor analysis (CFA). Two hundred and twenty individuals with schizophrenia spectrum disorders completed the PANSS; subsamples additionally completed the Brief Negative Symptom (...) Scale (BNSS) and the Motivation and Pleasure Scale—Self-Report (MAP-SR). CFA results indicated that the two-factor model fit the data better than the one-factor model; however, latent variables were closely correlated. The two-factor model’s fit was significantly improved by accounting for correlated residuals between N2 (emotional withdrawal) and N6 (lack of spontaneity and flow of conversation), and between N4 (passive social withdrawal) and G16 (active social avoidance), possibly reflecting common method variance. The two NSF factors exhibited differential patterns of correlation with subdomains of the BNSS and MAP-SR. These results suggest that the PANSS NSF would be better represented by a two-factor model than by a single-factor one, and support the two-factor model’s adequate criterion-related validity. Common method variance among several items may be a potential source of measurement error under a two-factor model of the PANSS NSF. (shrink)

L’objectif de cet article est de proposer une analyse de deux conceptions distinctes de la causalité chez Spinoza. Selon la première la nature-dieu serait la cause directe de toute action qui a lieu au niveau des choses finies; tandis que, selon la seconde toute action finie ferait partie d’une chaîne infinie de causes qui est répandue dans la durée. Je montre que cette causalité-ci n’est ni illusoire ni simplement derivative, contre les suggestions de plusieurs travaux récents sur Spinoza. En deuxième (...) lieu nous proposons un modèle pour la causalité consécutive qui assure la possibilité que les êtres finis puissent fonctionner comme des causes adéquates au sens spinoziste dans le champ physique. Ici l’analyse s’oppose à plusieurs chercheurs qui soutiennent qu’il existe une rupture entre les attributs de la pensée et de l’étendue dans le système spinoziste.This study addresses the problem of analyzing two distinct conceptions of causality in Spinoza. According to the first god-nature is the direct cause of every action which occurs at the level of finite things; while, according to the second , every finite action would be part of an infinite chain of causes extended into duration. I show that sequential causality is neither illusory nor simply derivative, contrary to the suggestions made by several recent studies of Spinoza. Secondly, I offer a model for sequential causality which assures the possibility that finite beings may function as adequate causes within the physical field. Here my analysis is opposed to those commentators who argue for a rupture in Spinoza between the attributes of thought and extension. (shrink)

Qu'est-ce que la "non-politique"? Est-elle pensable? Pourquoi l'évidence de la politique est-elle si forte? L'histoire de la philosophie occidentale est une tentation toujours renouvelée de "penser la politique depuis la non-politique". Nous pouvons indiquer qu'au moins, dans la modernité, les théoriciens du contrat social inventent déjà une idée révolutionnaire de "l'état de nature" qui est une notion non politique par excellence. Si nous admettons qu'il y a quelque chose de non politique, il y a au moins trois possibilités de penser (...) la non-politique qui nous conduisent à étudier trois théories radicalement distinctes : la "philosophie de l'avenir" de Nietzsche, la "psychanalyse" de Freud et la "non-philosophie" de François Laruelle. Non seulement ce sont les trois possibles conceptions de la non-politique, mais elles mettent en jeu le devenir historique de la notion non politique chez ces trois penseurs d'immanence. Nietzsche est le représentant du primat de la politique : la non-politique finit toujours par être surmontée et absorbée par le politique. Freud observe une différence irréductible entre politique et non-politique : l'une ne domine pas l'autre et vice versa. Enfin, Laruelle démontre que c'est la non-politique qui détermine en dernière instance la politique ; c'est l'Un qui fait un usage unilatéral du matériau occasionnel qu'est l'Autorité dont le coeur est la Pensée-Monde. ABSTRACT: Proponents of the ‘Threshold View’ have held that to believe a proposition is to be sufficiently confident of the proposition’s truth, but that there is no sharp cutoff between degrees of confidence that constitute belief and degrees of confidence that do not. Brian Weatherson has objected that no plausible account of vagueness can support this view. In this paper, I reply to Weatherson’s objection. Along the way, I identify a way in which one might hope to maintain the Threshold (...) View without a fuzzy threshold, and I reformulate the Threshold View to accommodate fuzzy thresholds without begging substantive questions about vagueness. -/- RÉSUMÉ : D’après la «théorie du seuil», croire une proposition, c’est être suffisamment sûr de la vérité de cette proposition, bien qu’il n’y ait pas de limite nette entre les degrés de certitude qui constituent une croyance et ceux qui n’en constituent pas une. Il y a, par exemple, de bonnes raisons de rejeter la thèse de Sellars selon laquelle le référent ultime d’une saisie perceptive est une impression sensible. ABSTRACT: Ever since Hume raised the problem of induction, many philosophers have tried to solve this problem; however, there still is no solution that has won wide acceptance among philosophers. According to Wilfrid Sellars, the reason is mainly that these philosophers have tried to justify induction by theoretical reasoning rather than by practical reasoning. In this paper I offer a sort of Sellarsian proposal. On the basis of the instrumental principle and the constructivist view of the concept of epistemic justification, (...) I argue that it is reasonable to accept induction.RÉSUMÉ: Depuis que Hume a soulevé le problème de l’induction, nombreux ont été les philosophes qui se sont attaqués à cette question. Aucun, pourtant, n’y a jusqu’à présent apporté de réponse satisfaisante et susceptible d’entraîner l’adhésion générale. Selon Wilfrid Sellars, la raison de cet échec est que les tentatives de justification se fondaient sur des raisonnements théoriques plutôt que pratiques. In the second Critique, Kant claims that the moral law is given as a fact of reason. In this paper, contra the standard view, I argue that there is a non-dogmatic way of defending this claim. And Kant’s principle of morality is widely taken to be a formal principle. How then can such a formal principle be reconciled with our substantial moral end? In this paper, I also argue that Kant’s principle of morality can be construed as a formal principle (...) in the sense that it provides correctness conditions for moral laws, rather than providing specific moral laws.Dans sa deuxième Critique, Kant affirme que la loi morale se donne comme un fait de raison. Dans cet article, contrairement à la position standard, nous prétendons qu’il existe une autre façon — non dogmatique — de défendre cette affirmation. Par ailleurs, le principe de moralité de Kant est largement considéré comme étant formel. Comment un tel principe formel peut-il être réconcilié avec notre fin morale substantielle? Nous soutenons également que le principe de moralité de Kant peut être interprété comme un principe formel dans le sens où il fournit des conditions d’acceptabilité pour les lois morales, et non des lois morales spécifiques. Robert Brandom argues for a “pragmatic phenomenalist account” of knowledge. On this account, we should understand our notion of justification in accordance with a Sellarsian social practice model, and there is nothing more to the phenomenon of knowledge than the proprieties of takings-as-knowing. I agree with these two claims. But Brandom's proposal is so sketchy that it is unclear how it can deal with a number of much-discussed problems in contemporary epistemology. The main purpose of this article is to develop (...) and defend a pragmatic phenomenalist account of knowledge by resolving those problems. I argue, in particular, that this account can accommodate both the lesson of the Gettier problem and the lesson of reliabilism simultaneously. -/- RÉSUMÉ: Robert Brandom soutient une «théorie pragmatique et phénomenaliste» de la connaissance. Selon cette théorie, nous devons comprendre notre notion de justification d'après un modèle sellarsien de la pratique sociale, et il n'y a rien d'autre dans phénomène de connaissance que la pertinence de «prendre-pour-connaître». Je suis tout à fait d'accord avec ces deux opinions. Mais la thèse de Brandom est si sommaire qu'il n'est pas évident de savoir comment elle peut trailer les nombreux problèmes discutés en épistémologie contemporaine. Le but principal de cet article est de développer et défendre une théorie pragmatique et phénomenaliste de la connaissance en résolvant ces problèmes. Je soutiens, en particulier, que cette théorie peut s'accorder en même temps à lerçon du problème de Gettier et au «fiabilisme». (shrink)

BonJour affirme qu’il a trouvé un moyen de sortir du dilemme sellarsien de la justification non-doxastique. Dans cet article, je soutiens trois thèses afin de montrer que sa solution alléguée échoue. Tout d’abord, il y a deux exigences pour qu’une raison soit bonne, et la notion de prise de conscience non-conceptuelle de l’expérience sensorielle développée par BonJour donne lieu à un dilemme sérieux en ce qui concerne ces exigences. Deuxièmement, il tire son idée de la soi-disant «prise de conscience constitutive (...) du contenu» de sa conception de la croyance consciente et occurrente. Celui-ci fut défendu par Kitcher : afin de rendre compte d’une action réussie, il faudrait expliquer pourquoi une et une même action peut être à la fois psychologiquement et effectivement pertinente. Pour lui, seule la vérité-adéquation permet de répondre à cette exigence. Dans cet article, je soutiens qu’afin de rendre compte d’une action réussie, il convient d’expliquer en quoi (...) l’action d’un agent lui est un moyen approprié d’atteindre ses fins, et que cette explication même, dans le cadre d’une conception déflationniste, suffit à répondre à l’exigence théorique de Kitcher. In this paper I introduce questions and discussions concerning the philosophy of Taekwondo and argue that it contains sufficient value to be a field of philosophy, like all other philosophies. For this purpose, I begin from the conception that Taekwondo is a martial art, which consists of the training of skilled techniques utilized in such a way to protect myself and, in the process, subjugate my opponent. I also discuss three philosophical components of the study of Taekwondo. The first is: (...) "What are the contents of the philosophy of Taekwondo, such as the principles of Samjae and Kang‐Yu?" The second is: "Why should we train lethal techniques that are used to injure, and even kill people?" The third, and final, is: "How can we perfect these techniques?" In this course I aim to explain how the spirit of Taekwondo and the epistemology of “unification of me and things” (Mul-A-Il-Che) and other components are based on the reflection of Taekwondo as a martial art. La question philosophique du cosmopolitisme – comment établir par le biais de la morale universelle l’association des « citoyens du monde » libres et égaux – ne fonde qu’un horizon normatif d’attentes qui attire le regard sur une réalité irrationnelle. La réalité irrationnelle se caractérise par la notion de « société mondiale du risque », introduite par Ulrich Beck pour (...) faire le diagnostic de notre temps frappé par toutes les catastrophes envisageables. Celles-ci montrent à quel point les fondations sur lesquelles nous avons bâti l’État national et la démocratie sont instables. Plus les risques sont globalisés, plus nous sommes contraints à une « conscience civile mondiale ». Malgré la connaissance des risques globalisés, nous sommes en premier lieu « citoyens d’État » et non « citoyens du monde ». Il est néanmoins indéniable que les frontières de l’État national perdent leur importance. Partant de l’hypothèse que la société mondiale est plus qu’un marché mondial, je souhaiterais souligner le « moment cosmopolite » de la délimitation transnationale afin de développer la morale universaliste de la société civile mondiale à l’égard de l’avenir de la démocratie. This paper propounds a new reading of Berkeley's account of the activity of finite spirits. Against existing interpretations, the paper argues that Berkeley does not hold that we causally contribute to the movement of our bodies. In contrast, our volitions to move our bodies are but occasions for God to cause their movement. In answer to the question of wherein then consists our activity, the paper proposes that our activity consists in the dual powers to produce (1) our volitions ? Is a human more conscious than an octopus? In the science of consciousness, it’s oftentimes assumed that some creatures (or mental states) are more conscious than others. But in recent years, a number of philosophers have argued that the notion of degrees of consciousness is conceptually confused. This paper (1) argues that the most prominent objections to degrees of consciousness are unsustainable, (2) examines the semantics of ‘more conscious than’ expressions, (3) develops an analysis of what it is for a (...) degreed property to count as degrees of consciousness, and (4) applies the analysis to various theories of consciousness. I argue that whether consciousness comes in degrees ultimately depends on which theory of consciousness turns out to be correct. But I also argue that most theories of consciousness entail that consciousness comes in degrees. (shrink)
