It is commonly asserted that Boethius defined free will as the judgment of the will or a rational choice. Accordingly, sin or evil is identified with ignorance or vice of the intellect, which prevents or distorts rational deliberation. However, Boethius adopted a more complex understanding of the self-motion of the soul and, consequently, articulated a more nuanced account of sin and the healing effects of Providence. Boethius treated human freedom as a complex including a natural motion, identified as the desire (...) for happiness, the determination of reason following the judgment of deliberation, and the sovereignty of the will over its own acts and, to some extent,over other acts of the soul. Sin, therefore, involves mistaken ideas about reality but also deformations in the affective orientation of the will to the world and in the exercise of the will’s control over the soul. (shrink)

BackgroundFear of cancer recurrence is an important cause of suffering for cancer survivors, and both empirical evidence and theoretical models suggest that prognostic uncertainty plays a causal role in its development. However, the relationship between prognostic uncertainty and FCR is incompletely understood.ObjectiveTo explore the relationship between prognostic uncertainty and FCR among patients with ovarian cancer.DesignA qualitative study was conducted utilizing individual in-depth interviews with a convenience sample of patients with epithelial ovarian cancer who had completed first-line treatment with surgery and/or (...) chemotherapy. Semi-structured interviews explored participants’ understanding of their prognosis; experiences, preferences, and attitudes regarding prognostic information; and strategies for coping with prognostic uncertainty. Inductive qualitative analysis and line-by-line software-assisted coding of interview transcripts was conducted to identify key themes and generate theoretical insights on the relationship between prognostic uncertainty and FCR.ResultsThe study sample consisted of 21 participants, nearly all of whom reported experiencing significant FCR, which they traced to an awareness of the possibility of a bad outcome. Some participants valued and pursued prognostic information as a means of coping with this awareness, suggesting that prognostic uncertainty causes FCR. However, most participants acknowledged fundamental limits to both the certainty and value of prognostic information, and engaged in various strategies aimed not at reducing but constructing and maintaining prognostic uncertainty as a means of sustaining hope in the possibility of a good outcome. Participants’ comments suggested that prognostic uncertainty, fear, and hope are connected by complex, bi-directional causal pathways mediated by processes that allow patients to cope with, construct, and maintain their uncertainty. A provisional dual-process theoretical model was developed to capture these pathways.ConclusionAmong patients with OC, prognostic uncertainty is both a cause and an effect of FCR—a fear-inducing stimulus and a hope-sustaining response constructed and maintained through various strategies. More work is needed to elucidate the relationships between prognostic uncertainty, fear, and hope, to validate and refine our theoretical model, and to develop interventions to help patients with OC and other serious illnesses to achieve an optimal balance between these states. In Religion and the Obligations of Citizenship Paul J. Weithman asks whether citizens in a liberal democracy may base their votes and their public political arguments on their religious beliefs. Drawing on empirical studies of how religion actually functions in politics, he challenges the standard view that citizens who rely on religious reasons must be prepared to make good their arguments by appealing to reasons that are 'accessible' to others. He contends that churches contribute to democracy by enriching political (...) debate and by facilitating political participation, especially among the poor and minorities, and as a consequence, citizens acquire religiously based political views and diverse views of their own citizenship. He concludes that the philosophical view which most defensibly accommodates this diversity is one that allows ordinary citizens to draw on the views their churches have formed when voting and offering public arguments for their political positions. This study examined the historical perspectives of eleven emergent bilingual and bilingual students at two middle schools. Data analysis revealed that the participants’ perspectives on U.S. history reflected three schematic narrative templates focused on nation-building, equality, and discrimination. The participants primarily employed the (in)equality narratives when discussing aspects of U.S. history directly linked to their identities. The findings add to the extant research on student historical perspectives and use of schematic narrative templates. The findings further suggest that engaging (emergent) bilingual (...) students in examining multiple perspectives and conducting critical history inquiry can contribute to notions of culturally and linguistically responsive social studies instruction. Focusing on the self as a normative construct, I consider how and why the self, not the group, is ontologically fundamental. Selves live in communities or societies of selves. The intrinsic normativity of the self and the actions the self performs constitute the grounds on which people fashion coherent narratives about themselves and seek to display themselves to others in ways that conform to their narratives. People bond with, care for, and invest in each other as distinctive selves that they (...) can coordinate with in a vast range of different activities and practices together with the conflicts and tensions between these activities and practices that define their political nature. The persistence or otherwise of the activities and projects that selves participate in is dependent on other activities and practices that lend their support and cooperation (or otherwise) to them. Activities and projects are therefore interdependent in ways that are more or less democratic. Selves-in-activities actively maintain, contest, and change these activities and projects. Political activity is thus seen as the processes of maintaining, contesting, and changing both selves-in-activities and the relations between activities. (shrink)

This paper explores some of the basic similarities and differences between fault-based and no-fault systems of automobile accident insurance from the perspective of a U.S. state and the laws of Macedonia. The majorities of U.S. states have adopted an at-fault system of compensation and therefore share more similarities with Macedonia than the laws of a dozen U.S. states that have adopted a no-fault system. Whereas Macedonia employs a system of nearly universal health care, such is not the case in the (...) United States. The respective states have adopted either a fault-based system or a no-fault system, but in either case issues of coverage for automobile accidents is more complicated for the lack of a nationalized health care system that at minimum acts as a safety net for treatment of injuries related to automobile accidents. It is posited that complications relating to the lack of universal health care coverage in the United States has led to significantly heightened regulatory intervention by the several states such that the similarities between the free-market approach of the states and of civil law (code) systems are greater than the differences. The paper is not based on empirical data of the legal systems or the medical systems, nor is the undeniable interrelation between a medical system and the corresponding legal system with regard to insurance coverage of primarily health-based issues considered other than to highlight how both may play a part in regulation of the automobile accident insurance industry in light the involvement of public health, safety and welfare. In an effort to create new synergies to fill gaps in evaluation of value, assessment of quality, and definition of roles in clinical ethics programs we convened a meeting entitled Innovations in Clinical Ethics: A Working Un-Conference (the Un-Conference) in August 2018. The Un-Conference was conceived to be a working event aimed at promoting cross pollination and idea generation for innovative practices in clinical ethics. The event was attended by 95 individuals from 62 institutions, representing a wide diversity of healthcare (...) systems, who believed in the concept and brought their enthusiasm and expertise to share with others. As a product of the Un-Conference, whitepaper groups developed summaries and broad overviews of areas that need to be further addressed within our field. The whitepapers are being published in this issue of The Journal of Clinical Ethics. The first three whitepapers highlight the broad themes of demonstrating value to the institution, quality assessment, and emerging roles for clinical ethics programs and ethicists. The final whitepaper offers guidance to clinical ethicists engaged in pediatric ethics work and, in conversation with the emerging roles article, thoughtfully reflects on the role of pediatric ethicists. Elevations in brain stimulation reward (BSR) thresholds have been observed in rats undergoing nicotine withdrawal and have been proposed as a sensitive measure of the negative affective state associated with nicotine withdrawal. mGluR are presynaptic autoreceptors that decrease glutamate release when stimulated. The aim of this study was to examine the role of glutamate neurotransmission in nicotine dependence. The mGluR agonist LY314582 (2.5–7.5 mg/kg) precipitated nicotine withdrawal as measured by elevations in BSR thresholds in nicotine-treated rats but not in controls. (...) It was hypothesized that LY314582 precipitated nicotine withdrawal by decreasing glutamatergic tone at postsynaptic glutamate receptors. Therefore, the effects of MPEP (0.5–2 mg/kg), an mGluR antagonist, and MK-801 (0.01–1 mg/kg), an NMDA receptor antagonist, were examined. MPEP elevated BSR thresholds by an equal magnitude in control and nicotine-treated rats. At low doses, MK-801 (0.01–0.2 mg/kg) lowered BSR thresholds to a similar extent in control and nicotine-treated rats. At higher doses, MK-801 (0.25–1 mg/kg) disrupted performance in nicotine-treated and control rats. These data indicate that mGluR and NMDA receptors regulate BSR in opposite directions in non-dependent animals, and chronic nicotine treatment does not alter these effects. Most importantly, the data demonstrate that the mGluR is involved in nicotine dependence, but mGluR and NMDA receptors do not mediate mGluR actions in nicotine dependence. (shrink)

A good portion of his work has, during the last two decades, been translated into English, and there are now half-a-dozen books and many essays by him available in that language. The book here under review was published in French in 2009.The book’s title aptly indicates one of its main themes, which is the deep and distinctive difference between the God of the Christians and other gods, especially the God of Islam. Brague does not like—and wishes to place under the ban—ways of characterizing Christianity and Islam that suggest or imply that Christians and Muslims worship the same God, and that they are each monotheistic, Abrahamic, and a people of the Book. Furthermore, he advocates such a ban, sometimes in excitable language, because of deep differences in Christian and Muslim ways of understanding, depicting, and analyzing God.Brague seems to think that difference in predicatively identifying, describing, and responding to something requires, if it reaches a certain level, the conclusion that what is being identified, described, and responded to is not the same thing. Such difference makes, for example, shared reference impossible. If the predicate list I supply for something I wish to talk about is sufficiently different from the predicate list you supply for something you wish to talk about, the conclusion Brague prefers is that we are not talking about the same thing—that we are not referring to the same thing.But there are problems here. Imagine the following case. I speak of someone who is female, sixty-eight years old, lives in Bangalore, writes in English, and has published seventy-four essays and thirteen books. You speak of someone who is male, seventy-three years old, lives in Montréal, writes in French, and has published thirty-nine books and more than one hundred essays. The predicates have little in common. It might seem reasonable to say [End Page 463] that we are and must be referring to different people because no one person could share this predicate list: the list contains noncompossibles. Robin Lovin’s An Introduction to Christian Ethics and Kent Van Til’s The Moral Disciple are impressive and worthy additions to that growing collection. Both are intended for undergraduate students—an audience, these authors realize, whose upbringing, history, and experiences are very different from their own. How can professors guide students through the thicket of choices and decisions that confront them every day, especially when they are bombarded with seemingly endlessly diverse answers about what is true, good, worthwhile, or fulfilling? When so much is constantly changing, how can professors help students understand what it means to live wisely, responsibly, and with integrity? How can they use the manifold traditions and resources of Christian ethics to help students discern the contours of a life that is truly worth living as well as the attitudes, dispositions, and choices that will enable them to achieve that? And, perhaps most challengingly, how can they do this when their students come to the study of ethics, as Lovin observes, with little more than “mere fragments of a moral vocabulary that has lost its meaning”? (238) With these questions in mind, Lovin and Van Til, wise and compassionate guides, lead the reader through the history of Christian ethics to illustrate how those who have gone before us, in all the different ways they understood the moral life, can assist us on our journey today.Both texts are structured around the customary focal points of goals and consequences (teleology), laws and norms (deontology), and character and virtue (areteology). In investigating each of these models for understanding ethics, [End Page 213] Van Til and Lovin give special attention to the influence of Aristotle, Augustine, Aquinas, and Kant. And both offer an insightful, balanced overview of the natural law, the moral and theological virtues, and the abiding but sometimes overlooked influence of utilitarianism on our moral reasoning. Moreover, both texts offer ample examples or case studies along with clear, succinct definitions that help students connect the material to their lives and appreciate important differences among varied accounts of the moral life.But there are also interesting differences between these two books. Because Van Til is primarily concerned with what ethics means in a life of faithful discipleship, he places how one’s relationship with Christ shapes her identity, understanding of the world, and sense of moral agency more at the forefront of his analysis than does Lovin in his Introduction to Christian Ethics. This is not a shortcoming of Lovin’s text, but it indicates an important difference in their methodologies. For example, because Van Til emphasizes that the lives of Christians are primarily guided by the biblical narrative, he moves from asking in the opening chapter “What is ethics?” to considering in chapter 2 the role of the scriptures for illuminating how Christians understand God, themselves, and their place in the world. “Christians believe that the Bible is God’s true and complete revelation,” Van Til writes, “and we also trust that the moral guidance we receive from Scripture is the truest and most complete moral guidance available to us” (18). Nonetheless, Van Til rightly cautions that because the Bible is “not an ethics textbook … problems may arise if we try to make the Bible guide our morality in ways that the Bible itself does not intend” (18). Abstract:The Analects and the Mencius are among the most influential early Confucian texts. They emphasize the importance of moral self-cultivation. The individual is expected to identify what is good, and freely choose it regardless of their internal predispositions or external conditions. Curiously, in their philosophical frameworks they do not posit anything outside of contingencies. This means there is no non-contingency-based notion of "good" or "agency." This paper contributes to the current discourse by explaining how morality and agency can be possible (...) in a completely contingent system. Instead of positing a pregiven principle, self, will, or even capacity for agency, I will argue that Confucian texts assume that morality and agency emerge out of contingencies. As such, morality and agency are categorically distinct from contingencies, and are thereby able to reflect on them, without being ultimately sourced from any power outside of contingencies. This study examines the effects of demographic characteristics on ethical perceptions. While earlier research has produced conflicting results regarding the predictive power of these variables, significant and definite insights were obtained with proper controls. The following predictors of ethical attitudes are examined: age, gender, marital status, education, dependent children status, region of the country and years in business, while controlling for job status. A nation-wide random sample of employees was used in obtaining a response rate of fifty-three percent (total n (...) of 423). Indices of aspects of business ethical attitudes were constructed using factor analysis. Linear multiple regression analysis indicated the significant predictive variables. Age was found to be a most-significant predictor. Older workers had stricter interpretations of ethical standards. Gender and region predicted attitudes about job-discrimination practices only, with women and persons from the Midwest most strongly opposed to the practice. All the other variables proved to be unreliable ethics predictors. (shrink)

This paper offers four contrasting perspectives on the role of the nurse ethicist from authors based in different areas of world, with different professional backgrounds and at different career stages. Each author raises questions about how to understand the role of the nurse ethicist. The first author reflects upon their career, the scope and purpose of their work, ultimately arguing that the distinction between ‘nurse ethicist’ and ‘clinical ethicist’ is largely irrelevant. The second author describes the impact and value that (...) a nurse in an ethics role plays, highlighting the ‘tacit knowledge’ and ‘lived experience’ they bring to clinical ethics consultation. However, the second author also warns that the ‘nurse ethicist’ must be cautious in their approach to avoid being viewed as a resource only for nurses. The third author questions the introduction of additional professional distinctions such as ‘nurse ethicist’ on the basis that distinctions threaten the creation of egalitarian healthcare systems, while also acknowledging that clinical ethicists ought not strive for objective attachment in their work. In direct contrast, the final author suggests that the nurse ethicist can play a pivotal role in highlighting and addressing ethical challenges that are specific to nurses. In this paper we take Luce Irigaray’s idea of fluid feminine subjectivity as productive for the project of rethinking agency in a collaborative feminist-Confucian context. We discuss how diffused agency in the Analects can be used alongside Irigaray’s work to critique contemporary notions of atomic agency. Our argument employs the notions of fluidity and agency in Irigaray and the Analects with concentrations on: 1) similarities between Irigaray’s “philosophy of breath” and Confucian ritual; 2) parallels between fluid feminine subjectivity and diffused (...) agency; and 3) a shared concern for the ethical life. We thus see how Confucianism can contribute to contemporary feminist discourse. (shrink)

This paper examines the strengths and weaknesses of one of science's most pervasive and flexible metaprinciples;optimalityis used to explain utility maximization in economics, least effort principles in physics, entropy in chemistry, and survival of the fittest in biology. Fermat's principle of least time involves both teleological and causal considerations, two distinct modes of explanation resting on poorly understood psychological primitives. The rationality heuristic in economics provides an example from social science of the potential biases arising from the extreme flexibility of (...) optimality considerations, including selective search for confirming vidence, ex post rationalization, and the confusion of prediction with explanation. Commentators are asked to reflect on the extent to which optimality is an organizing principle of nature, a set of relatively unconnected techniques of science, a normative principle for rational choice and social organization, a metaphysical way of looking at the world, or something else still. In philosophical interpretations of the Laozi the function of the ‘sage’ is a relatively under concentrated on topic. Although nearly every scholar does have something to say about the sage, comments are usually brief and often revolve around the sage as some particular character-type; for example highlighting the sage as a ‘sage-ruler’. In this article we will argue that the sage serves as a tool for understanding the major concepts, thinking, and logic of the Laozi. While the sage does often (...) refer to the sage-ruler this does not mean all references are politically orientated, and even those that are can easily be applied to a host of social situations. This paper thus seeks to appreciate the context for what the Laozi says by looking at how the text itself envisions the embodiment of its ideas through reference to the sage. (shrink)

In “Potential Subjects’ Responses to an Ethics Questionnaire in a Phase I Study of Deep-Brain Stimulation in Early Parkinson’s Disease,” Finder, Bliton, Gill, Davis, Konrad, and Charles undertake informed consent research on what they describe as a Phase I trial of deep brain stimulation (DBS) for Parkinson’s disease. We argue that the authors should have more carefully characterized the nature of the DBS study at the start of their clinical study.
