In 1925 a debate erupted in the correspondence columns of the British Medical Journal concerning the effectiveness of eating raw pancreas as a treatment for diabetes. Enthusiasts were predominantly general practitioners , who claimed success for the therapy on the basis of their clinical impressions. Their detractors were laboratory‐oriented ‘biochemist‐physicians,’ who considered that their own experiments demonstrated that raw pancreas therapy was ineffective. The biochemist‐physicians consistently dismissed the GPs' observations as inadequately ‘controlled’. They did not define the meaning (...) of ‘control’ in this context, although it clearly did not have the term's present‐day meaning of a trial employing an untreated comparison group of patients. Rather, the physician‐biochemists' ‘properly controlled’ experiments involved careful regulation of their patients' diet and other environmental factors, and evaluation of the therapy's success through biochemical, rather than just clinical, criteria. However, my analysis suggests that these factors alone are inadequate to account for the biochemist‐physicians' dismissal of the GPs' work as ‘uncontrolled’. I suggest that the biochemist‐physicians were deliberately exploiting the powerful rhetorical connotations of the term ‘control’. Ultimately, they implied that only a trial which they themselves had conducted could be deemed ‘adequately controlled’. (shrink)

Overmedication is nowadays a serious problem in health care due to influences from the pharmaceutical industry and agencies responsible for regulation. The situation has indeed become appalling in psychiatry, where both theories and treatments have deteriorated under the impact of the industry. The overmedication problem is associated with biased biology in medicine. Adequate biological approaches would indicate that drug therapies must yield to diet therapies, particularly treatments involving omega-3 fatty acids, in many cases. To the extent that philosophy of (...) science adapts to mainstream medicine in analyses of the current situation, it may reinforce the existing bias. Thirty years ago, a number of human inborn errors in carbohydrate metabolism were explored with specific enzymatic tests on blood samples (hemolysates). Hereditary galactosemia was the first example. When the inoperative step in galactose metabolism was specified, the basis for the diet therapy used on the galactosemic infants, namely galactose‐free diet, could be shown to be securely founded.As far as galactose metabolism is concerned, the cells of the infant are faced with two problems: (i) the conversion of (...) dietary lactose (galactosyl glucose) to glucose and its catabolites involved in energy metabolism, and (ii) the conversion of dietary glucose or lactose to galactosyl units of glycolipids and glycoprotein cell structures.Subsequent studies on microorganisms revealed several types of hereditary defect in galactose metabolism. One type which permits the bacteria to develop a normal carbohydrate pattern in their cell walls includes an enzyme defect, like that described in the cells of the galactosemic infant. Two other types, with the inability to synthesize UDPGlc or UDPGal from glucose, do not permit the bacteria to build the fabric of the normal bacterial cell wall. This is the subject for discussion. Issues arising in connection with genes and nutrition policy include both nutrigenomics and nutrigenetics. Nutrigenomics considers the relationship between specifc nutrients or diet and gene expression and, it is envisaged, will facilitate prevention of diet-related common diseases. Nutrigenetics is concerned with the effects of individual genetic variation on response to diet, and in the longer term may lead to personalised dietary recommendations. It is important also to consider the surrounding context of other issues such as novel and (...) functional foods in so far as they are related to genetic modification. Ethical issues fall into a number of categories: why nutrigenomics? I (...) felt blindsided, intensely vulnerable, and dumbstruck—completely unable to respond—and later, when the shock wore off, incandescent with impotent rage.Four years later, I still don’t have words to adequately describe my outrage at this comment. It was only the second time I’d see this medical professional. The first time was the previous year, when she had been filling in for my regular doctor who was on maternity leave. At neither appointment did this physician ask me any questions about my lifestyle, whether or not I saw a primary care physician, or even if I had any concerns that day. She was very brisk, efficient and did not engage in conversation. Such a statement coming from a health care provider, who did not seek to understand or gather more information, surprised and dismayed me.Other incidents where physicians’ statements were not helpful:• Pediatrician: “Well, my wife has had great success with Weight Watchers.”—I was age 12, about 5’5 and weighted about 145 lbs. at that time. I celebrated the American Medical Association’s classification of obesity as a disease for several reasons. First, obesity as a disease involves other medical complications of which I have none, so finally perhaps I can say I am not obese, I am just (...) fat. I am fat, as in my weight is caused by Lipedema and Lymphedema. Lipedema is a congenital condition, which causes my body to produce and accumulate abnormal amounts of adipose tissue in my legs, hips, thighs, and arms. There is not much known about Lipedema in the medical profession, even though it was identified in 1940 by doctors at the Mayo Clinic, and not many treatment options exist. Abnormal Lipedema tissue does not respond to restrictive caloric diet or exercise. Lymphedema is a condition of localized fluid retention and tissue swelling caused by a compromised lymphatic system. The second reason I celebrated obesity being classified as a disease was the hope that more research would be conducted on the actual causes and treatment of obesity and that research might include adipose tissue disorders. My vital health statistics are all normal, yet I still face challenges getting proper treatment in healthcare.My StoryAs I sat staring at the metal beast, I decided to make one final attempt to mount it. I placed my right foot on the step and used both hands to boost myself up, my lower legs were so large there was no room for my left foot. The gut microbiota is an important component of the human body and its immune‐modulating and metabolic activities are critical to maintain good health. Gut microbes, however, are sensitive to changes in diet, exposure to antibiotics, or infections, all of which cause transient disruptions in the microbial composition, a phenomenon known as dysbiosis. It is now recognized that microbial dysbiosis is at the root of many gastrointestinal disorders. However, the mechanisms through which bacterial dysbiosis initiates disease are not fully understood. The reason is that when patients care about their health status, they are inclined to establish a therapeutic relationship with physical therapists. Hence, the care for self may bridge the world of the patient's private experiences and the world of the healthcare system together with its interventions, which is represented by the physical therapist In this framework, (...) care means not only the patient's choice to undergo therapy but also his/her commitment to sustaining its effects in private life. Background: Meditation is gaining recognition as a tool to impact health and well-being. Samyama is an 8-day intensive residential meditation experience conducted by Isha Foundation requiring several months of extensive preparation and vegan diet. The health effects of Samyama have not been previously studied. The objective was to assess physical and emotional well-being before and after Samyama participation by evaluating psychological surveys and objective health biomarkers.Methods: This was an observational study of 632 adults before and after the Isha Samyama (...) retreat. All participants were invited to complete surveys. Controls included household significant others. Surveys were completed at baseline, just before Samyama, immediately after Samyama, and 3 months later to assess anxiety, depression, mindfulness, joy, vitality, and resilience through validated psychometric scales. Voluntary blood sampling for biomarker analysis was done to assess hemoglobin, HbA1c, lipid profile, and C-reactive protein. Primary outcomes were changes in psychometric scores, body weight, and blood biomarkers.Results: Depression and anxiety scores decreased from T1 to T3, with the effect most pronounced in participants with baseline depression or anxiety. Scores at T4 remained below baseline for those with pre-existing depression or anxiety. Vitality, resilience, joy, and mindfulness increased from T1 to T3. Body weight decreased by 3% from T1 to T3. Triglycerides were lower from T2 to T3. Participants had lower HbA1c and HDL at T2, and lower CRP at all timepoints compared with controls.Conclusions: Participation in the Isha Samyama program led to multiple benefits. The 2-month preparation reduced anxiety, and participants maintained lower anxiety levels at 3 months post-retreat. Physical health improved over the course of the program as evidenced by weight loss and improved HbA1C and lipid profile. Practices associated with the Samyama preparation phase and the retreat may serve as an effective way to improve physical and mental health. Future studies may examine their use as an alternative therapy in patients with depression and/or anxiety.Clinical Trial Registration:www.ClinicalTrials.gov, Identifier: 1801728792. This article concerns the problem of assessing selected body compo- sition parameters after completion of antitumor therapy and comparing them with the same parameters of healthy children. A high percentage of overweight and obesity, as well as abnormal fat distribution in convalescents with cancer shows a significant adverse effect of therapy on body composition and suggests the need for early intervention in terms of diet and exercise, which would help patients to quickly achieve the proper parameters of (...) body composition. Two main problems will be mentioned during the presented data analysis. Firstly, in each group there was a small number of observations. Because of this, the real differences between examined subgroups may have been omitted. Secondarily, many variables are correlated and are not normally distributed. Therefore, be- side the standard statistical tests to compare two groups, principal component analysis was applied to reduce the dimensions of the attribute space and to attempt to classify two groups of patients. Metabolic dysfunction is a ubiquitous underlying feature of many neurological conditions including acute traumatic brain injuries and chronic neurodegenerative conditions. A central problem in neurological patients, in particular those with traumatic brain injuries, is an impairment in the utilization of glucose, which is the predominant metabolic substrate in a normally functioning brain. In such patients, alternative substrates including ketone bodies and lactate become important metabolic candidates for maintaining brain function. While the potential neuroprotective benefits of ketosis have been recognized for (...) up to almost a century, the majority of work has focused on the use of ketogenic diets to induce such a state, which is inappropriate in cases of acute disease due to the prolonged periods of time (i.e., weeks to months) required for the effects of a ketogenic diet to be seen. The following review seeks to explore the neuroprotective effects of exogenous ketone and lactate preparations, which have more recently become commercially available and are able to induce a deep ketogenic response in a fraction of the time. The rapid response of exogenous preparations makes their use as a therapeutic adjunct more feasible from a clinical perspective in both acute and chronic neurological conditions. Potentially, their ability to globally moderate long-term, occult brain dysfunction may also be relevant in reducing lifetime risks of certain neurodegenerative conditions. In particular, this review explores the association between traumatic brain injury and contusion-related dementia, assessing metabolic parallels and highlighting the potential role of exogenous ketone and lactate therapies. We find that the Ethics surrounds the idea that man can be egoless in the Buddhist sense of that term. This concept provides a criterion of mental health. Further, according to Spinoza's theory of the Affections, those which are passive include some which are based on pain. These he 'enumerates (...) among the diseases'. And for them he provides, in Part V, specific 'remedies'. This in turn leads him to equate 'Mental Freedom or beatitude' with a 'healthy Mind'. We thus have in Part V additional possible criteria of mental health. Finally, there is the suggestion that philosophy for Spinoza was a kind of therapy.' This paper explores evolving treatments for hysteria in the eighteenth century by examining a selection of works by both physician-writers and educated literary women. The treatments I identify—which range from aggressive bloodlettings, diets, and beatings, to exercise, fresh air, and writing cures—reveal a unique culture of therapy in which female sufferers and doctors exert an influence on one another's notions of what constitutes appropriate management of women's mental illness. A scrutiny of this exchange of ideas suggests that female patients (...) were not simply oppressed and silenced by male practitioners; rather, their collective voice, intellect, and expertise helped to form progressive treatments for eighteenth-century hysteria. This paper asks whether we can identify a neutral explanandum for theories of phenomenal consciousness, acceptable to all sides. The ‘classic’ conception of qualia, on which qualia are intrinsic, ineffable, and subjective, will not serve this purpose, but it is widely assumed that a watered-down ‘diet’ conception will. I argue that this is wrong and that the diet notion of qualia has no distinctive content. There is no phenomenal residue left when qualia are stripped of their intrinsicality, ineffability, (...) and subjectivity. Thus, if we reject classic qualia realism, we should accept that all that needs explaining are ‘zero’ qualia – our dispositions to judge that our experiences have classic qualia. Infants, children, adolescents, and pregnant and lactating women have been described as groups with special needs. Regardless of diet chosen, these groups are at higher risk for nutritional deficiencies than adult males. Vegan diets can be safely used by these groups if foods, and in some instances supplements, are selected which provide a healthful and nutritionally adequate diet. Guidelines have been developed for those choosing to follow vegan diets. In many instances vegan diets offer health benefits. Studies of (...) vegans are limited by factors such as heterogeneity of diets, the size and extent of this type of study, and difficulty in subject identification. There is a scarcity of studies of newer vegetarians who are often more mainstream than the vegetarians of the 1960s and 1970s. In some instances this has led to assumptions about today's vegans which are based on out-dated information. Individuals exposed to dieting-related environmental cues have been repeatedly shown to be better able to resist tempting food. This especially applies to restrained eaters who hold a chronic dieting goal. Thus far, mainly short-term effects of environmental dieting cues have been examined and the individuals were typically unaware of being influenced. The present longitudinal study applied (...) a 2 × 2 between-subjects design for 6 months. Our results provide preliminary indications that cue, awareness, and restrained eating interact. The results suggest that high restrained eaters could deliberately apply environmental dieting cues for themselves to facilitate losing weight. The Western diet pattern characterized by high daily intake of saturated fats and refined carbohydrates often leads to obesity and overweight, and it has been linked to cognitive impairment and emotional disorders in both animal models and humans. This dietary pattern alters the composition of gut microbiota, influencing brain function by different mechanisms involving the gut–brain axis. In addition, long-term exposure to highly palatable foods typical of WD could induce addictive-like eating behaviors and hypothalamic-pituitary-adrenal axis dysregulation associated with chronic (...) stress, anxiety, and depression. In turn, chronic stress modulates eating behavior, and it could have detrimental effects on different brain regions such as the hippocampus, hypothalamus, amygdala, and several cortical regions. Moreover, obesity and overweight induce neuroinflammation, causing neuronal dysfunction. In this review, we summarize the current scientific evidence about the mechanisms and factors relating WD consumption with altered brain function and behavior. Possible therapeutic interventions and limitations are also discussed, aiming to tackle and prevent this current pandemic. (shrink)

Autoimmune diseases such as rheumatoid arthritis and gastrointestinal disorders such as stomach ulcers are often treated with drugs. NSAIDs, a common treatment in rheumatoid arthritis, may cause stomach ulcers which call for additional medications, notably antacids in the sense of drugs that suppress acid secretion by the stomach. Infection with Helicobacter pylori also plays a role in the ulcers. The infection is typically treated with antibiotics added to antacids. Considering NSAIDs and antacids, we suspect that overmedication is common to the (...) extent that particular diets are a better option. Current research and current treatments with these drugs are also problematic since circadian rhythms are mostly disregarded. All the processes involved in the disorders treated show marked variations in the course of the day. Hence experiments conforming to the guidelines of evidence-based medicine, and treatments in line with them, have outcomes strongly depending on the time factor. This calls for reforms in medicine with fresh inputs from biology. (shrink)

The diet of expectant mothers was a significant issue of social, political and scientific concern between 1930 and 1960. However, while histories of maternity services and nutritional science are independently available, no existing study addresses the nutrition of expectant mothers in this period. Between 1900 and 1930, maternal mortality rates were rising despite improving clinical antenatal provisions. Breakthroughs in nutritional science resulted in the identification of key dietary components, while changing social attitudes meant hunger was increasingly being seen as (...) a humanitarian issue requiring a modern solution. As a result, the diet of expectant mothers first began to be addressed as a social concern in the 1930s. It subsequently made a transition into official policy as part of wartime rationing. The government entirely changed its attitude to maternal nutrition in a beneficial and successful way during the war; conferring to modern historians another example of war being a generator of change. Diet in pregnancy finally became the concern of the scientific community post-war, as illustrated by the work of the Royal College of Obstetrics and Gynaecology Nutrition Committee for Pregnancy, whose papers are considered for the first time here. This paper charts the rise of diet in pregnancy as a concern and considers the contributions of different communities involved between 1930 and 1960. It also notes that the actual recommended diet stayed the same. Thus while diet is the subject here, discussion focuses on why there were such distinct rearrangements of social, political and scientific forces around an issue that itself remained unchanged. For over fifty years, scholars have argued that a therapeutic ethos has begun to change how people think about themselves and others. There is also a growing concern that the therapeutic ethos has influenced educational theory and practice, perhaps to their detriment. This review article discusses three books, The Dangerous Rise of Therapeutic Education (by Kathryn Ecclestone and Dennis Hayes), Aristotle, Emotions, and Education (by Kristján Kristjánsson), and The Therapy of Education (by Paul Smeyers, Richard Smith and Paul Standish), (...) that point to the problematic assumptions and outcomes of therapeutic educational practices. The authors of the three books, however, disagree about whether a focus on emotions or therapy in education is necessarily an unwelcome intrusion into education. (shrink)

An earlier version of it won Adrienne Rose Bitar the Society for Utopian Studies' Eugenio Battisti Award for the best essay published in the society's journal in 2015. "The Paleo Diet and the American Weight Loss Utopia, 1975–2014" was among several in a special issue that featured essays and book reviews on utopian foodways.The book chapter that emerged from that award-winning (...) essay is even richer within the context of Bitar's engaging longer study. Major depressive disorder is not only the most widespread mental disorder in the world, it is a disorder on the rise. In cases of particularly severe forms of depression, when all other treatment options have failed, the use of electroconvulsive therapy (ECT) is a recommended treatment option for patients. ECT has been in use in psychiatric practice for over 70 years and is now undergoing something of a restricted renaissance following a sharp decline in its use in the 1970s. (...) Despite its success in treating severe depression there is continued debate as to the effectiveness of ECT: in some studies, it is argued that ECT is marginally more effective than sham ECT. In addition, there is still no clear explanation of how ECT works; among the range of hypotheses proposed it is claimed that ECT may work by harnessing placebo effects. In light of the uncertainties over the mechanism of action of ECT and given the risk of serious side effects that ECT may produce, I contend that the process of informed consent must include comprehensive accounts of these uncertainties. I examine the possible consequences of providing adequate information to potential ECT patients, including the consideration that ECT may still prove to be effective even if physicians are open about the possibility of it working as a placebo. I conclude that if we value patient autonomy as well as the professional reputation of medical practitioners, a fuller description of ECT must be provided to patients and their carers. (shrink)

Se realizó un estudio descriptivo con el objetivo de caracterizar la dieta cetogénica como una alternativa en el tratamiento de la epilepsia refractaria en niños para mejorar su calidad de vida, con la consiguiente disminución de su costo económico. La epilepsia catastrófica o refractaria es un problema social de salud que la padecen pacientes pediátricos, generalmente en los primeros días de vida, de difícil control y con múltiples crisis pese al empleo de fármacos antiepilépticos en dosis tóxica, e incluye diferentes (...) síndromes y enfermedades con características propias. Esta dieta ofrece una esperanza para aquellos pacientes que se resisten al tratamiento con medicamentos y su solución representa un valor social porque su beneficio supera el costo. A descriptive study with objective to characterize the ketogenic diet as alternative in the treatment of the refractory epilepsy in children for elevating their quality of life, with the consequent decreasing of its economic cost, was carried out. The catastrophic or refractory epilepsy is one health social problem that the pediatric patient suffers, generally in the first days of life, the hard control and with numerous crisis despite of the use of antiepileptic drugs in toxic doses, and include different syndromes and diseases with own characteristics. This diet offers one hope, and its solution represents a social value because the benefit overcomes the cost for those patients that resist the drug treatment. (shrink)
