GPs usually care for their patients for an extended period of time, therefore, requests to not only discontinue a patient’s treatment but to assist a patient in a suicide are likely to create intensely stressful situations for physicians. However, in order to ensure the best patient care possible, the competent communication about the option of physician assisted suicide as well as the assessment of the origin and sincerity of the request are very important. This is especially true, since patients’ requests (...) for PAS can also be an indicator for unmet needs or concerns. Twenty-three qualitative semi-structured interviews were conducted to in-depth explore this multifaceted, complex topic while enabling GPs to express possible difficulties when being asked for assistance. The analysis of the gathered data shows three main themes why GPs may find it difficult to professionally communicate about PAS: concerns for their own psychological well-being, conflicting personal values or their understanding of their professional role. In the discussion part of this paper we re-assess these different themes in order to ethically discuss and analyse how potential barriers to professional communication concerning PAS could be overcome. Advance directives are written documents that give patients the opportunity to communicate their preferences regarding treatments they do or do not want to receive in case they become unable to make decisions. Commonly used pre-printed forms have different formats. Some offer space for patients to appoint a surrogate decision maker, and/or to determine future medical treatments and/or give a statement of personal values. So far it is unknown which forms GPs preferably use and why they decide to do so. 23 (...) semi-structured interviews with GPs were analysed using content analysis. Interviewees mainly use short templates and medium length templates with checkboxes to indicate patients’ preferences in regards to life prolonging measures. Especially when patients faced the progression of a disease, participants use the latter version. Only then, the interviewees remarked, patients are capable to rate concrete situations reliably. GPs also realize the importance of the verbal assessment of patients’ preferences; however they rarely keep a written form of the conversation. Some GPs hand out one or more templates and ask their patients to read and think about them at home with the option to talk to them about it later on, while others prefer their patients to fill them out alone at home. Regardless of template usage, most GPs emphasize that ADs require regular updates. GPs tend to see standardized advance directives mainly as a tool to start a conversation with their patients and to identify their real preferences and values. When the patient is still not facing the progression of an already existing disease it could be sufficient to only appoint a surrogate decision maker instead of creating a full AD. However, in all other situations, appointing a surrogate decision maker should be backed up by a written statement of a patient’s general values. Patients and their relatives should always have the opportunity to ask their GP for medical advice when drafting an AD. BackgroundThe proper and ethical inclusion of PWLHIV and their young children in research is paramount to ensure valid evidence is generated to optimize treatment and care. Little empirical data exists to inform ethical considerations deemed most critical to these populations. Our study aimed to systematically review the empiric literature regarding ethical considerations for research participation of PWLHIV and their young children.MethodsWe conducted this systematic review in partnership with a medical librarian. A search strategy was designed and performed within the following (...) electronic databases: Ovid MEDLINE, Embase and CINAHL. We screened titles and abstracts using the following inclusion criteria: (1) a study population of PWLHIV or children under 5 years of age; and (2) collection of qualitative or quantitative data regarding ethics of research participation. Excluded were reviews, commentaries, policy statements, clinical care-related ethics concerns, abstracts, case studies, or studies unrelated to HIV research. Studies were appraised for quality, data were extracted, and studies were qualitatively analyzed using a principle-based ethical framework within the Belmont Report.ResultsOf the 7470 titles identified, 538 full-text articles were reviewed for eligibility and only three articles met full criteria for inclusion within this review. While we allowed for inclusion of studies involving young children born to mothers with HIV, only articles focused on PWLHIV were identified. Within the results of these studies, four themes emerged: (1) adequacy of informed consent; (2) consideration of paternal involvement; (3) balancing risks; and (4) access to research and treatment. A strength of this review is that it included perspectives of international research investigators, community leaders, and male partners. However, only two studies collected empiric data from PWLHIV regarding their experiences participating in researchConclusionResearchers and funding agencies should be aware of these considerations and appreciate the value of and critical need for formative research to ensure clinical trials involving PWLHIV promote ethical, well-informed research participation and, ultimately, improve care outcomes. Background Palliative care is an integral part of health care, which in term has become increasingly technologized in recent decades. Lately, innovative smart sensors combined with artificial intelligence promise better diagnosis and treatment. But to date, it is unclear: how are palliative care concepts and their underlying assumptions about humans challenged by smart sensor technologies (SST) and how can care benefit from SST? -/- Aims The paper aims to identify changes and challenges in palliative care due to the use of (...) SST. In addition, normative guiding criteria for the use of SST are developed. -/- Methods The principle of Total Care used by the European Association for Palliative Care (EAPC) forms the basis for the ethical analysis. Drawing on this, its underlying conceptions of the human and its socio-ethical aspects are examined with a phenomenological focus. In the second step, the advantages, limitations, and socio-ethical challenges of using SST with respect to the Total Care principle are explored. Finally, ethical-normative requirements for the application of SST are derived. -/- Results and Conclusion First, SST are limited in their measurement capabilities. Second, SST have an impact on human agency and autonomy. This concerns both the patient and the caregiver. Third, some aspects of the Total Care principle are likely to be marginalized due to the use of SST. The paper formulates normative requirements for using SST to serve human flourishing. In this paper, I investigate the relation of Kant's theory of biology to epigenetic accounts of organic generation and development. In the literature, a dispute about similarities between Blumenbach's epigenetic account and Kant dominated the debate for many years (see Lenoir 1980, 1981, and 1982, 17–34, Richards 2000; 2002, 207–37; Look 2006, and van den Berg 2009). Some more recent interpreters claim that Wolff's, more than Blumenbach's account plays the pivotal role in the development of a vitalistic conception of epigenesis (...) in Kant (see Dupont 2007 and Huneman 2007). Although I myself hold the view that Kant's position contains preformistic and epigenetic characteristics, in the current paper I focus solely on an investigation of epigenetic elements in Kant's account and compare them to the corresponding epigenetic elements in Wolff's theory. Section I of the paper is devoted to an analysis of Wolff's most important epigenetic theorems: the notion of the essential power (vis essentialis) and the conception of the part-whole composition of organized matter. Although Wolff describes the essential power vitalisticly, as a principle of life, he understands it as the cause of mechanical motions explaining the generation, nourishment, and the growth of an organism. Wolff's model of the part-whole composition of organic matter is subtle, but committed to fundamental mechanistic assumptions, such as that the organism as a whole is composed of inorganic parts. In section II, I analyze the corresponding elements in Kant's theory: the notion of the formative power and the conception of the whole-part composition of organized materials. Kant describes the formative power as a principle that causes the purposive form of an organized being such that matter and mechanism are the means to the purpose of the being as their end. The purpose of the whole is a functional unit which is in principle superior to the form and matter of the subordinate parts. The parts are combined into such a whole in being mutually cause and effect of each other and in being related to the superior whole. In section III, I respond to the debate in the literature. Against Dupont (2007) and Huneman (2007) I argue that, according to Wolff, the vis essentialis accounts for mechanic effects in matter, whereas, according to Kant, the formative power explains the intentional order (form, end, purpose) of an individual organized being, its parts, and its species. Since this view is closer to Blumenbach than to Wolff, the ongoing comparison between Kant and Blumenbach in the literature is justified. However, the emphasis on the specific part-whole composition that Kant considers to be the determining feature of an organized being can be found only in Wolff and not in Blumenbach—though Wolff and Kant describe it in opposing ways. This increases the systematic importance of Wolff for Kant. Thus, a fresh look on the historical debate is required. (shrink)

We report specific heat and μSR measurements on Th and/or B substituted UBe13. The specific heat data show that either Th or B substitution reduces the Kondo temperature TK and increases the entropy at the superconducting transition by almost 20%, indicating an enhanced density of states. However, whereas μSR shows clear evidence for magnetic correlations for Th substitutions, no magnetism is observed for B substitutions. The enhanced specific heat jump in the B-substituted material is associated with a change in the (...) superconducting properties as TK is reduced. (shrink)

Higgins-Biddle's new edition of the work Locke published anonymously in 1695 is another fine entry in the Clarendon series. It (...) is the first critical edition and the only one to include the revisions Locke made in a personal copy. Higgins-Biddle provides a set of Locke's manuscripts related to the text (including manuscript indices), a table of Biblical references, and a bibliography. In his meticulously researched introduction, Higgins-Biddle sets out the background and main themes of The Reasonableness by considering the controversies that surrounded it, particularly the accusations of Deism, Socinianism, and Hobbism.Locke's title is apt to mislead contemporary readers by encouraging them to expect a rationalistic defense of religious belief. What we have instead is a treatise largely devoted to discovering the basic beliefs one must hold in order to be a Christian. Christianity is 'reasonable' according to Locke because its fundamental doctrines are perspicuous and accessible to all, not because it is in every respect rationally demonstrable. Locke argues that belief in two propositions, viz., that God exists and that Jesus Christ is the Messiah, is sufficient to make one a Christian. Early critics, as Higgins-Biddle shows, mistakenly took this to mean that other doctrines, especially that of the Trinity, formed no part of Locke's brand of Christianity. Instead, Locke suggests that although belief in the fundamental doctrines is sufficient to make one a Christian, one is subsequently obligated to receive "all the parts of Divine Revelation, with a docility and disposition prepared to imbrace" (169). Locke grants, however, that a Christian may be ignorant of or even disbelieve a large portion of scriptural truths. His failure to specify which particular 'truths' he had in mind seems to have led to the charge of Socinianism, which persists to the present day.It is not clear what this charge amounts to, and Higgins-Biddle does a fine job of sorting through the religious controversies of Locke's age. One core plank of Socinianism or Unitarianism seems to have been the denial of the Trinity. Nowhere in The Reasonableness does Locke so much as mention that doctrine; however, his position does make it a secondary one, since assent to it is not required of all Christians. What is most important here is the continuity between Locke's project in this work and his thoughts on toleration. The chief aim of those who, like Locke, advocated the 'way of fundamentals' was to achieve toleration for non-conformist religious sects and to make possible a more harmonious and unified Church of England.Locke is often regarded as a founding father of English Deism, a view Locke understood as entailing the denial of all religious claims except those supported by natural theology. But Locke himself saw The Reasonableness as partly directed against the temptation to reject the claims of revelation. Such claims can be objects of faith, accepted by virtue of Biblical authority. Opposition to Deism is particularly evident in Chapter XIV, where Locke argues for the necessity of Christ as saviour. Since uncertainty is a major challenge in medicine and bears the risk of causing incorrect diagnoses and harmful treatment, there are many efforts to tackle it. For some time, AI technologies have been increasingly implemented in medicine and used to reduce medical uncertainties. What initially seems desirable, however, poses challenges. We use a multimethod approach that combines philosophical inquiry, conceptual analysis, and ethical considerations to identify key challenges that arise when AI is used for medical certainty purposes. We identify several (...) challenges. Where AI is used to reduce medical uncertainties, it is likely to result in (a) patients being stripped down to their measurable data points, and being made disambiguous. Additionally, the widespread use of AI technologies in health care bears the risk of (b) human physicians being pushed out of the medical decision-making process, and patient participation being more and more limited. Further, the successful use of AI requires extensive and invasive monitoring of patients, which raises (c) questions about surveillance as well as privacy and security issues. We outline these several challenges and show that they are immediate consequences of AI-driven security efforts. If not addressed, they could entail unfavorable consequences. We contend that diminishing medical uncertainties through AI involves a tradeoff. The advantages, including enhanced precision, personalization, and overall improvement in medicine, are accompanied by several novel challenges. Although a fascination with language is a familiar feature of 20th-century empiricism, its origins reach back at least to the early modern period empiricists. John Locke offers a detailed (if sometimes puzzling) treatment of language and uses it to illuminate key regions of the philosophical topography, particularly natural kinds and essences. Locke's main conceptual tool for dealing with language is 'signification'. Locke's central linguistic thesis is this: words signify nothing but ideas. This on its face seems absurd. Don't we need (...) words to signify things as well? But its very absurdity – our inclination to dismiss Locke as a 'linguistic idealist'– should signal to us that we have not yet understood Locke. Doing so must begin with an analysis of signification. Each of the three main interpretations on offer allows Locke to escape the charge of linguistic idealism, although they do so in very different ways. Locke's text also offers an influential account of linguistic particles, words like 'is', 'and' and 'if'. These signify, not ideas, but acts of the mind. These acts can either take place within a proposition, uniting its constituent ideas into a thought that admits of a truth-value, or they can take propositions as their objects, in which case they express attitudes like doubt, assertion and so on. Even this seemingly innocuous sketch of Locke's view is controversial, and many writers, from J.S. Mill onwards, have argued that Locke cannot make sense of propositional attitudes. Apart from the intrinsic interest of these questions, understanding how Locke thinks language works is a prerequisite for understanding his arguments against scholastic essentialism. Education is important in shaping professional identity, including how one approaches norms and normalisation. In the analysis presented in this study, nursing students' own constructions of norms and normality from the outlook of their education are highlighted and problematised. To deepen the understanding of these matters, the aim of this study was to explore constructions of norms and normality among students in nursing education. Students studying in a nursing department at a Swedish university college were approached and asked to consider (...) open survey questions targeting their views on norms and normality; 154 of them replied. After a discourse analytic approach to the data, we could see how the students constructed norms and normality as (a) instrumental instructions, consisting of easy‐to‐digest statements grounded in the profession's obvious moral and ethical values, (b) limiting and frustrating obstacles for personal freedom that were important to challenge, (c) rules to be obeyed for the stability of society and (d) a matter of reflection, with each individual being responsible for understanding differences in norms, perspectives and opinions. We conclude that nursing education would benefit from norm‐critical perspectives, problematising students' own positions to norms, power and privilege. (shrink)

The target of this criticism is the possibility of universal transworld depravity, which is crucial to Plantinga’s defence. In this paper, we argue that there is a simpler and more plausible free will defence that does not require the possibility of universal transworld depravity or the truth of counterfactuals of creaturely freedom. We assume only (...) that libertarianism is possibly true and that God’s existence is consistent with the existence of free agents who never go wrong. We conclude the paper by explaining how our defence may be able to succeed without assuming, in a way that is consistent with compatibilism. (shrink)

This study of Heidegger is a remarkably lucid general introduction to his thought. Macquarrie considers in turn Heidegger's "life", "thought", and "significance." Macquarrie accepts for the most part Heidegger's own self-interpretation of his development--that his thought represents a continuous progressive path, all along guided by the problem of being. As one might expect, (...) there are a number of illuminating comparisons between Heidegger and other theological literature, including gnosticism, Barth, Bultmann, Tillich, and Heinrich Ott. The account of Heidegger on God is among the most meritorious features of the book. The references to Heidegger are to the English translations. There is no bibliography, but there is an excellent glossary which explicates clearly, simply, and accurately many important words in Heidegger's vocabulary.--J. D. C. (shrink)

Kockelmans’ book is an anthology of readings which he has collected together and in most cases translated. Eight of the eleven selections have already been published—although not all have appeared in English. Part I contains two essays with no evident connection—one by Kockelmans and a 1961 article from a Dutch periodical by Jan Aler on the view of language in Being and Time. Part II is the heart of the book and contains the translated texts of the six lectures given (...) at the Colloquium at Penn State in 1969 on "Heidegger’s Conception of Language," of which Kockelmans was the director. It also includes the panel discussion among the participants: Walter Biemel, Otto Poeggeler, Henri Birault, Heinrich Ott, Werner Marx and Kockelmans. These papers are extremely high level Heideggerian scholarship and very penetrating studies. They will repay careful study. Part III contains papers of a linguistic nature by Johannes Lohmann and Erasmus Schoefer. The essay by Schoefer is interesting; it is excerpted from his valuable book Die Sprache Heideggers which studies Heidegger’s own use of the German language. Michael Choniates (c. 1138–c. 1222), a pupil of Eustathius of Thessalonica, who was Greek Orthodox Metropolitan of Athens for some 25 years up to that city's capture by Frankish crusaders ina.d.1205, is best known to classical scholars as the possessor of probably the last complete copy of Callimachus'HecaleandAetia. He had brought with him from Constantinople many books of all kinds, and added to his collection when in Athens. Although an immense task, it would be well worth trying to identify all (...) The philosophical theory of scientific explanation proposed here involves a radically new treatment of causality that accords with the pervasively statistical character of contemporary science. Wesley C. Salmon describes three fundamental conceptions of scientific explanation--the epistemic, modal, and ontic. He argues that the prevailing view is untenable and that the modal conception is scientifically out-dated. Significantly revising aspects of his earlier work, he defends a causal/mechanical theory that is a version of the ontic conception. Professor Salmon's theory furnishes a robust (...) argument for scientific realism akin to the argument that convinced twentieth-century physical scientists of the existence of atoms and molecules. To do justice to such notions as irreducibly statistical laws and statistical explanation, he offers a novel account of physical randomness. The transition from the "reviewed view" of scientific explanation to the causal/mechanical model requires fundamental rethinking of basic explanatory concepts. (shrink)

Reichenbach's principles of a probabilistic common cause of probabilistic correlations is formulated in terms of relativistic quantum field theory, and the problem is raised whether correlations in relativistic quantum field theory between events represented by projections in local observable algebrasA(V1) andA(V2) pertaining to spacelike separated spacetime regions V1 and V2 can be explained by finding a probabilistic common cause of the correlation in Reichenbach's sense. While this problem remains open, it is shown that if all superluminal correlations predicted by the (...) vacuum state between events inA(V1) andA(V2) have a genuinely probabilistic common cause, then the local algebrasA(V1) andA(V2) must be statistically independent in the sense of C*-independence. Carnap and Quine first met in the 1932-33 academic year, when the latter, fresh out of graduate school, visited the key centers of mathematical logic in Europe. In the months that Carnap was finishing his Logische Syntax der Sprache, Quine spent five weeks in Prague, where they discussed the manuscript “as it issued from Ina Carnap’s typewriter”. The philosophical friendship that emerged in these weeks would have a tremendous impact on the course of analytic philosophy. Not only did the meetings (...) effectively turn Quine into Carnap's disciple, they also paved the way for their seminal debates about meaning, language, and ontology---the very discussions that would change the course of analytic philosophy in the decades after the Second World War. -/- Yet surprisingly little is known about these first meetings. Although Quine has often acknowledged the impact of his Prague visit, there appears to be little information about these first encounters, except for the fact that the Quines “were overwhelmed by the kindness of the Carnaps” and that it was Quine’s “most notable experience of being intellectually fired by a living teacher”. Neither their correspondence nor their autobiographies offer a detailed account of these meetings. In this paper, I shed new light on Carnap’s and Quine’s first encounters by examining a set of previously unexplored material from their personal and academic archives. Why did Quine decide to visit Carnap? What did they discuss? And in what ways did the meetings affect Quine’s philosophical development? In what follows, I address these questions by means of a detailed reconstruction of Quine’s year in Europe based on a range of letters, notes, and reports from the early 1930s. (shrink)

In this article I try to refute the so-called "libertarian" theory of free will, and to examine how our conclusion ought to modify our common attitudes of praise and blame. In attacking the libertarian view, I shall try to show that it cannot be consistently stated. That is, my dscussion will be an "analytic-philosophic" one. I shall neglect what I think is in practice an equally powerful method of attack on the libertarian: a challenge to state his theory in such (...) a way that it will fit in the modern biology and psychology, which are becoming increasingly physicalistic. An edition utilizing all three sources seemed to be called for. 2. The Crapulli edition offers (a) a careful introductory study of the three sources; (b) the Regulae in the newly established Latin text, with the Dutch translation on facing pages and the variants in the footnotes; (c) notes containing the editorial apparatus; and (d) (...) an Appendix giving the contemporary texts relating to the Regulae more fully than does AT. 3. The Crapulli edition is superior by virtue of the meticulous care and balanced judgment that went into it, and because of its use of all three sources. One unexpected result is a much smoother, more idiomatic and therefore more readable Latin text. However, meaning is affected only in a few instances, despite the large number of new readings. Excepting trivial emendations (changed word order, better grammar, etc.) I find some twenty-two materiM changes. 0nly seven or eight of them alter the meaning significantly; none of them affects a vital text. Three examples must suffice (I cite page and line in AT, Volume X, and italicize CrapuUi's reading): 368.13 inductio/deductio; 448.26 relatum/rotatum; 46725 discursu/ decursu. 4. Scholars and translators will henceforth have to work with Crapulli's version, using their judgment where he, too, had to resort to conjecture. But Scholars are unlikely to abandon the habit of citing AT as well--it would make the use of earlier work difficult. Unfortunately Crapulli does not give a tabular concordance between his edition and AT (though he indicates the AT pagination), and the complex organization of his book makes correcting one's AT volume extremely time-consuming and tedious. It would be appropriate for the editors of the revised AT edition to provide such a concordance. Meanwhile a short list of the more significantnew readings may be obtained from this reviewer. Most important among these (with presumed dates) are : De Principlls Cognitionis et Actionis (1636); Logica (1645); Philosophia Prima (De Motu, Loco, et Tempore) (1646); First Draft of De Corpore (1648-49); Revision of this version (1651); Published version, De Corpore (1655). The first of these is known only through an English summary, discovered by Mario M. Rossi and discussed by him in his L'evoluzione del pensiero di Hobbes aUa luee di un nuovo manoo scritto, in "Civilt~ Modero~", 1941. Of the many accounts of the Battle of the Milvian Bridge ina.d.312 written soon after the conflict, only those of Eusebius of Caesarea have Maxentius cross the Tiber on a bridge of boats to face the forces of Constantine. This detail, it is here argued, suggests that Maxentius may be seen as a latter-day Xerxes, the Persian emperor who, in preparation for his invasion of Greece in 480b.c., famously spanned the Hellespont with a pair of boat-bridges. The article first reviews (...) the seminal accounts of Xerxes’ feat in Aeschylus’Persiansand Herodotus’Histories, and next discusses the story's long afterlife in subsequent Greek (and Latin) authors, including those of Late Antiquity. Close analysis of Eusebius’ battle narratives in hisEcclesiastical History(9.9.3–8) and in hisLife of Constantine(1.38) reveals that their vocabulary echoes the distinctive language used by Aeschylus, Herodotus and later writers in reference to Xerxes’ achievement. The article concludes by exploring the implications of this identification of Maxentius with Xerxes. It exemplifies two venerable tactics in Roman political propaganda: that of portraying a native rival as a foreign enemy and that of mapping the Persian Wars onto contemporary events. As Xerxesrediuiuus, Maxentius is cast as the quintessential barbarian tyrant, an Eastern despot resident in Rome. (shrink)

Disaster response is a highly collaborative and critical process that requires the involvement of multiple emergency responders (ERs), ideally working together under a unified command, to enable a rapid and effective operational response. Following the 9/11 and 11/13 terrorist attacks and the devastation of hurricanes Katrina and Rita, it is apparent that inadequate communication and a lack of interoperability among the ERs engaged on-site can adversely affect disaster response efforts. Within this context, we present a scenario-based terrorism case study to (...) highlight the challenges of operational disaster command and response. In this article, which is based on the French emergency response doctrine, we outline a semantics-based common operational command system that is designed to guarantee an efficient information flow among ERs. Our focus is on offering to all ERs, a real-time operational picture of the situation in order to enable multilevel coordination among firefighters, police, healthcare units, public authorities, and other stakeholders. Our approach consolidates information to promote timely sharing of data among ERs. The proposed system is based on an ontology that has been developed to represent the different types of knowledge on the part of ERs, providing a shared vocabulary that covers a variety of interoperability concerns. In our papers on the rationality of magic, we distinghuished, for purposes of analysis, three levels of rationality. First and lowest (rationalitYl) the goal directed action of an agent with given aims and circumstances, where among his circumstances we included his knowledge and opinions. On this level the magician's treatment of illness by incantation is as rational as any traditional doctor's blood-letting or any modern one's use of anti-biotics. At the second level (rationalitY2) we add the element of rational thinking (...) or thinking which obeys some set of explicit rules, a level which is not found in magic in general, though it is sometimes given to specific details of magical thinking within the magical thought-system. It was the late Sir Edward E. Evans-Pritchard who observed that when considering magic in detail the magician may be as consistent or critical as anyone else; but when considering magic in general, or any system of thought in general, the magician could not be critical or even comprehend the criticism. Evans-Pritchard went even further: he was sceptical as to whether it could be done in a truly consistent manner: one cannot be critical of one's own system, he thought. On this level (rationalitY2) of discussion we have explained (earlier) why we prefer to wed Evans Pritchard's view of the magician's capacity for piece-meal rationality to Sir James Frazer's view that magic in general is pseudo-rational because it lacks standards of rational thinking.
