The commodification of health care, particularly primary care, presents challenges to care and knowledge development. The purpose of this study is to examine how nurses perceive and develop their knowledge in a commodified context. A mixed‐methods study was conducted that included a closed‐question survey and in‐depth interviews with nurses in public primary care in Catalonia. There were 104 valid responses to the questionnaire and 10 in‐depth interviews. The main findings of the survey were related to workload and limited time for (...) nursing care. Six themes emerged from the in‐depth interviews: (1) limited time for nursing, (2) feelings of burnout, (3) awareness of patient and family satisfaction, (4) organizational factors that favor nurses' needs, (5) organizational factors that hinder nurses' needs, and finally (6) public administration requirements. Participants perceive excessive workload and time constraints and feel that this affects their nursing care and their physical and mental health. However, nurses purposefully use knowledge patterns to cope with the problems associated with commodification. Nurses have multidimensional, contextualized, and integrated knowledge that allows them to optimize their care based on the needs of their patients. This research examines many challenges related to nursing practice and the nursing discipline and opens the door for further research that encompasses all areas of nursing. (shrink)

El pensamiento ético y bioético es una de las áreas de conocimiento recurrentes en el ejercicio reflexivo del profesional de la salud en Cuba; sin embargo, se detectan insuficiencias relacionadas con imprecisiones y falta de claridad en el pensamiento ético-moral por parte de este personal, según se observa en materiales evaluados para su publicación, en los ejercicios para el proceso de obtención de categorías docentes y en la presentación de las evaluaciones de posgrados. The ethical and bioethical thought is one of the recurrent areas of knowledge in the reflexive exercise of health professionals in Cuba. However, according to the observations of evaluated materials for their publication, in the exercises for the process of obtaining educational categories and in the presentation of postgraduate evaluations, inadequacies related with imprecision and lack of clarity are detected in the ethical-moral thought on the part of health professionals. A virtuous person has a distinctive grasp of what is important in the light of which she chooses what to do. In what does this grasp consist? According to the intellectual tradition, moral virtue requires you always to be able to have an explicit, conscious grasp of the reasons why your action is right. Recently, this view has been defended by Julia Annas. I do not think that her argument establishes her conclusion, and I provide further defense of intellectualism, finishing (...) with some conclusions about the importance of moral philosophy. My project here is to argue for situating moral judgments about Indian surrogacy in the context of Reproductive Justice. I begin by crafting the best picture of Indian surrogacy available to me while marking some worries I have about discursive colonialism and epistemic honesty. Western feminists' responses to contract pregnancy fall loosely into two interrelated moments: post-Baby M discussions that focus on the morality of surrogacy work in Western contexts, and feminist biomedical ethnographies that focus on the lived dimensions of (...) reproductive technologies and how they are embodied and negotiated in specific cultural contexts. Both approaches have their shortcomings. Uncritically extending Western moral frameworks (for example, liberal feminist political values) to Indian surrogacy work raises the specter of discursive colonialism; with it, worries arise about how Western normative traditions can distort, erase, or misread non-Western subjects' lived experiences. Feminist biomedical ethnographic approaches correct this, but raise the specter of a weak moral absenteeism; with it, concerns arise about under-theorizing the structural harms and injustices shaping surrogacy worker's lives. I suggest that we might reduce these shortcomings by framing normative and ethnographic engagement with global surrogacy as questions of reproductive justice. (shrink)

Successfully navigating the norms of a society is a complex task that involves recognizing diverse kinds of rules as well as the relative weight attached to them. In the United States, different kinds of rules—federal statutes and regulations, scientific norms, and professional ideals—guide the work of researchers. Penalties for violating these different kinds of rules and norms can range from the displeasure of peers to criminal sanctions. We proposed that it would be more difficult for researchers working in the U.S. (...) who were born in other nations to distinguish the seriousness of violating rules across diverse domains. We administered a new measure, the evaluating rules in science task, to National Institutes of Health-funded investigators. The ERST assessed perceptions of the seriousness of violating research regulations, norms, and ideals, and allowed us to calculate the degree to which researchers distinguished between the seriousness of each rule category. The ERST also assessed researchers’ predictions of the seriousness that research integrity officers would assign to the rules. We compared researchers’ predictions to the seriousness ratings of 112 RIOs working at U.S. research-intensive universities. U.S.-born researchers were significantly better at distinguishing between the seriousness of violating federal research regulations and violating ideals of science, and they were more accurate in their predictions of the views of RIOs. Acculturation to the U.S. moderated the effects of nationality on accuracy. We discuss the implications of these findings in terms of future research and education. (shrink)

This essay serves as both a response and embellishment of Marilyn Frye's now classic essay " Oppression." It is meant to pick up where this essay left off and to make connections between oppression, as Frye defines it, and the privileges that result from institutional structures. This essay tries to clarify one meaning of privilege that is lost in philosophical discussions of injustice. I develop a distinction between unearned privileges and earned advantages. Clarifying the meaning of privilege as unearned structural (...) advantage makes visible the role white privilege plays in maintaining complex systems of domination such as racism, sexism, heterosexism and classism. Using a critical reading of both Frye and Young's accounts of oppression as a springboard, I develop a definition of privilege as a particular class of unearned advantages. -/- I distinguish my account of privilege from standard legal and philosophical definitions of privilege. The general distinction I make between privileges and advantages rests on three interrelated claims: that benefits granted by privilege are always unearned and conferred systemically to members of dominant social groups; that privileges granted to members of dominant groups solely on the basis of their membership in these groups is never justifiable; and, that privileges have an unconditional value that can be explained not only in terms of immunities, but also in terms of additional benefits. ArgumentThis paper explores the relationship between the medium of motion-picture film and the representation of autobiographical memory during the middle decades of the twentieth century. The paper argues that a reciprocal relationship developed between film and memory, in which film was understood as an externalized form of memory, and memory an internalized record of personal experience similar in many respects to film. Memory was often represented as an object-like entity, preserved in stable form within the body, and able to be (...) extracted by the right stimulus or trigger. A particularly important community in which this representation was developed was psychotherapeutic practitioners with psychoanalytic orientations, particularly during and shortly after the Second World War. In special circumstances, therapists and others claimed, records of past life events could be projected, film-like, onto the screen of an individual's conscious, replaying previous experiences in real time. The paper develops a social historical account of this relationship, and reflects on its significance for the history of selfhood in the twentieth century. (shrink)

It has become one of the world's most highly respected research centers in part because of its efforts over the years to provide the research community with leadership in both the ethical and scientific parameters of research involving humans. Three major aspects of G1 regulation acting at START in fission yeast are discussed in this review. Firstly, progression towards S phase in the mitotic cycle. This is controlled by the activation of transcription complexes at START which cause cell cycle‐dependent activation of genes required for DNA synthesis. The second aspect is the regulation of developmental fate occurring during G1. Passage through START appears to inhibit sexual differentiation because the meiotic and mitotic pathways are mutually exclusive. This is brought about (...) because the meiotic pathway is inhibited by the same gene functions that are required for S phase onset. Thirdly, distinct checkpoint, or dependency, controls operate both pre‐ and post‐START in the mitotic cycle to inhibit mitosis in the absence of replicated DNA, and also to limit rounds of DNA replication to one per cell cycle. This essay discusses the yoked history of witnessing in science and the law and examines the history of attempts, over the past century, to use science to improve the surety of witness testimony. It examines some of these projects, particularly in the 1960s and 1970s. The essay argues that modern psychology offers a particularly problematic form of expertise because its focus is a task central to the jury’s mandate, the evaluation of a witness and his or her testimony. It concludes (...) that a key feature of modern disputes over the legal relevance of psychological techniques is not the quality or character of the expertise being offered to the court but, rather, the question of whether expertise is needed at all and under what circumstances it should impinge on the jury’s decision making. The terms “ideal theory” and “nonideal theory” are used in contemporary Anglophone political philosophy to identify alternative methodological approaches for justifying normative claims. Each term is used in multiple ways. In this article Alison M. Jaggar disentangles several versions of ideal and nonideal theory with a view to determining which elements may be helpful in designing models of real-world justice that are contextually relevant, morally plausible, and practically feasible.

The most popular uniting theme in feminist peace literature grounds women's peace work in mothering. I argue if maternal arguments do not address the variety of relationships different races and classes of mothers have to institutional violence and/or the military, then the resulting peace politics can only draw incomplete conclusions about the relationships between maternal work/thinking and peace. To illustrate this I compare two models of mothering: Sara Ruddick's decription of "maternal practice" and Patricia Hill Collins's account of racial-ethnic women's (...) "motherwork.". (shrink)

BackgroundMost end-of-life decisions are made by family members. Current ethical guidelines for family decision making are based on a hierarchical model that emphasizes the patient’s wishes over his or her best interests. Evidence suggests that the model poorly reflects the strategies and priorities of many families.MethodsResearchers observed and recorded 26 decision-making meetings between hospital staff and family members. Semi-structured follow-up interviews were conducted. Transcriptions were analyzed using qualitative techniques.ResultsFor both staff and families, consideration of a patient’s best interests generally took (...) priority over the patient’s wishes. Staff generally introduced discussion of the patient’s wishes for rhetorical purposes, such as persuasion. Competing moral frameworks, which de-emphasized the salience of patients’ autonomy and “right to choose,” played a role in family decision making.ConclusionsThe priority given to the patients’ wishes in the hierarchical model does not reflect the priorities of staff and families in making decisions about end-of-life care. The present issue of Philosophical Topics is devoted to global gender justice. In this introduction to the volume, I sketch the emergence of global gender justice as a field of philosophical inquiry and identify some of the philosophical challenges that its emergence raises. The easiest way to explain the distinctiveness of this field is to situate it in the context of earlier philosophical inquiries into justice.

In this work we will analyze the main features of Sartre’s ontological-phenomenological theory in Being and Nothingness, linking them with some key historical precedents. By clarifying the connections of this theory with the former philosophical tradition on the subject of Being, its role in existence, reality’s nature and its relation to the phenomenon, the actual contribution of Sartre in this matter is demonstrated.

This paper considers a specific aspect of a practice-as-research project—the Kingston University Big Read. It explores how to achieve optimum attractiveness and perceived value among students and staff for a free book circulated for the purposes of a pre-arrival shared reading scheme. After consideration of the academic literature relating to the distribution of free books and the theft of books, there follows a detailed examination of marketing practice in the publishing industry relating to the dissemination of free and promotional items (...) through collaborative endeavours. The solutions adopted are described, along with the outcomes perceived in the market, and recommendations are made for the future. (shrink)

Alternative food movements work to create more environmentally and economically sustainable food systems, but vary widely in their advocacy for social, racial and environmental justice. However, even those food justice activists explicitly dedicated to equity must respond to the unintended consequences of their work. This paper analyzes the work of activists in Oakland, CA, who have increasingly realized that their gardens, health food stores and farm-to-table restaurants play a role in what scholars have called green gentrification, the upscaling of neighborhoods (...) through the creation of environmental amenities. Gentrification has had grave consequences for the low-income communities of color that food justice activists seek to serve. Activists are reflexive about this dynamic, and have developed strategies to push back against displacement. Most commonly, non-profit organizations and individual social entrepreneurs found businesses that seek to raise the profile of people of color in the trendy Oakland food scene while employing long-term residents in well-paying, green jobs. However, while these efforts are an essential component of a broader agenda to create both food justice and development without displacement, even these relatively high paying “good food jobs” cannot keep up with escalating rents. For this reason, we also highlight the direct action and policy-oriented strategies engaged by a smaller number of food justice activists, and argue that these are necessary compliments to a market-based approach. Kant and Kierkegaard are two philosophers who are not usually bracketed together. Yet, for one commentator, Ronald Green, in his book Kierkegaard and Kant: The Hidden Debt , a deep similarity between them is seen in the centrality both accord to the notion of freedom. Kierkegaard, for example, in one of his Journal entries, expresses a ‘passion’ for human freedom. Freedom is for Kierkegaard also linked to a paradox that lies at the heart of thought. Subjective feelings of familiarity associated with a stimulus tend to be strongest when specific information about the previous encounter with the stimulus is difficult to retrieve . When a stimulus has been encountered previously and the circumstances of the encounter cannot be recollected, additional cognitive resources may be directed toward recollection processes; this resource allocation is accompanied by autonomic arousal [Dawson, M. E., Filion, D. L., & Schell, A. M. . (...) Is elicitation of the autonomic orienting response associated with allocation of processing resources?. Psychophysiology, 26, 560–572]. One easily measurable index of autonomic arousal is the skin conductance response . In the present study, participants studied lists of words and then gave recognition ratings to briefly displayed and masked studied and nonstudied test words while their SCRs were monitored. Results revealed a relationship between recognition ratings and the temporal characteristics of the SCR, supporting the idea that feelings of familiarity are indeed “feelings” in that they stem from autonomic arousal associated with cognitive resource allocation.
