Cet article compare les différentes réformes adoptées par plusieurs pays européens ces dernières décennies, et montre la manière dont se dessine aujourd’hui une politique européenne de retour à l’emploi des mères qui élèvent seules leurs enfants – ce que les Anglo-Saxons qualifient de Workfare ou de Welfare to work.

There is a traditional conception of sensory experience on which the experiences one has looking at, say, a cat could be had by someone merely hallucinating a cat. Disjunctivists take issue with this conception on the grounds that it does not enable us to understand how perceptual knowledge is possible. In particular, they think, it does not explain how it can be that experiences gained in perception enable us to be in ‘cognitive contact’ with objects and facts. I develop this (...) chal- lenge to the traditional conception and then show that it is possible to accommo- date an adequate account of cognitive contact in keeping with the traditional conception. One upshot of the discussion is that experiences do not bear the explanatory burden placed upon them by disjunctivists. (shrink)

The paper explains what disjunctivism is and explores its implications for skepticism. Following an account of Paul Snowdon’s conception of a disjunctivist account of perceptual experience the the focus is on how disjunctivism has figured in the epistemological work of John McDowell. A conception of recognitional abilities is deployed to expand on McDowell’s position. Finally, there is consideration of whether McDowell offers a satisfactory response to skepticism, taking account of criticism’s made by Crispin Wright.

It is argued that seeing that P is a mode of knowing that P that is to be explained in terms of the exercise of visual-perceptual recognitional abilities. The nature of those abilities is described. The justification for believing that P, when one sees that P, is provided by the fact that one sees that P. Access to this fact is explained in terms of an ability to recognize of seen objects that one is seeing them. Reasons for resistance to (...) such an account are considered. Informed consent is an essential step in helping patients be aware of consequences of their treatment decisions. With surgery, it is vitally important for patients to understand the risks and benefits of the procedure and decide accordingly. We explored whether a written IC form was provided to patients; whether they read and signed it; whether they communicated orally with the physician; whether these communications influenced patient decisions. Adult postsurgical patients in nine general hospitals of Italy’s Campania Region were interviewed via (...) a structured questionnaire between the second and seventh day after the surgery at the end of the first surgical follow up visit. Physicians who were independent from the surgical team administered the questionnaire. The written IC form was given to 84.5% of those interviewed. All recipients of the form signed it, either personally or through a delegate; however, 13.9% did not know/remember having done so; 51.8% said that they read it thoroughly. Of those who reported to have read it, 90.9% judged it to be clear. Of those receiving the written consent form, 52.0% had gotten it the day before the surgery at the earliest 41.1% received it some hours or immediately before the procedure. The written IC form was explained to 65.6% of the patients, and 93.9% of them received further oral information that deemed understandable. Most attention was given to the diagnosis and the type of surgical procedure, which was communicated respectively to 92.8 and 88.2% of the patients. Almost one in two patients believed that the information provided some emotional relief, while 23.2% experienced increased anxiety. Younger patients and patients with higher levels of education were more likely to read the written IC form. The written IC form is not sufficient in assuring patients and making them fully aware of choices they made for their health; pre-operative information that was delivered orally better served the patients’ needs. To improve the quality of communication we suggest enhancing physicians’ communication skills and for them to use structured conversation to ensure that individuals are completely informed before undergoing their procedures. Decisions to use antibiotics require that patient interests are balanced against the public good, that is, control of antibiotic resistance. Patients carry the risks of suboptimal antibiotic treatment and many physicians are reluctant to impose even small avoidable risks on patients. At the same time, antibiotics are overused and antibiotic-resistant microbes are contributing an increasing burden of adverse patient outcomes. It is the criteria that we can use to reject the use of antibiotics that is the focus of this paper. (...) Scanlon's contractualism explains why antibiotics should not be used to gain small benefits, even when the direct costs of antibiotics are low. We know that some individuals now (and probably more in the future will) carry a burden of irretrievable harm as a consequence of treatment- (antibiotic-) resistant infection. If we accept that the dominant justification for use of antibiotics is to prevent irretrievable harm to an individual or contact, then the use of antibiotics for self-limiting conditions, or for the treatment of individuals with conditions for which antibiotics do not substantially impact on outcomes (eg, in the latter stages of terminal illness), or for access based on preference or willingness to pay (internet or over-the-counter access), or the use of antibiotics as animal growth promoters can be rejected. Scanlon's approach also suggests that, with few new antibiotics in the pipeline and an increasing burden of disease attributable to resistant microbes, control of the spread of antibiotic-resistant microbes should be given increasing priority. Birds are not commonly admired for emotional expression, and when they are, the focus is typically on negative states; yet vocal behavior is considered a direct reflection of an individual’s emotional state. Given that over 4000 species of songbird produce learned, complex, context-specific vocalizations, we make the case that songbirds are conspicuously broadcasting distinct positive emotional states and that hearing songs can also induce positive states in other birds. Studies are reviewed that demonstrate that that the production of sexually motivated (...) song reflects an emotional state of anticipatory reward-seeking (i.e., mate-seeking), while outside the mating context song in gregarious flocks reflects a state of intrinsic reward. Studies are also reviewed that demonstrate that hearing song induces states of positive anticipation and reward. This review brings together numerous studies that highlight a potentially important role for the songbird nucleus accumbens, a region nearly synonymous with reward in mammals, in positive emotional states that underlie singing behavior and responses to song. It is proposed that the nucleus accumbens is part of an evolutionarily conserved circuitry that contributes context-dependently to positive emotional states that motivate and reward singing behavior and responses to song. Neural mechanisms that underlie basic emotions appear to be conserved and similar across vertebrates. Thus, these findings in songbirds have the potential to provide insights into interventions that can restore positive social interactions disrupted by mental health disorders in humans. (shrink)

‘Zero tolerance’ of avoidable infection events is explicit in UK and international policy documents describing strategies for the control of healthcare-associated infection. I consider what principles governing avoidable infections acquired in healthcare institutions might be reasonably rejected from the contractualist perspective of Thomas Scanlon. Many hospital infections can be cost-effectively avoided. There would seem to be additional reasons to take the prevention of avoidable infection acquired in hospitals seriously in addition to optimizing the cost-effectiveness of healthcare. These include the irretrievable (...) nature of the harm of some of the infections; the reasonable rejection of avoidable harm when the patient could not rationally consent to the risk of harm and the social consequences such as loss of trust in healthcare providers. Despite these reasons, a principle of ‘zero tolerance’ has implausible implications, promotes unrealistic patient expectations and can be reasonably rejected from the perspective of those who will suffer unacceptable opportunity costs. However, the contractualist perspective would also seem to reject a principle of tolerance of harm when there is a feasible reconfiguration of institutional arrangements that reduces the risk of harm without a redistribution of a proportionate or greater burden to others. Attribution of responsibility to individuals for outbreaks of infectious disease is challenging even with the most sophisticated microbial typing techniques. Typing methods can help to elucidate potential transmission pathways but there are additional conditions required before responsibility for the spread of infection can be attributed to individuals. These conditions include the knowledge and opportunity to undertake alternative actions. Governmental and institutional obligations arise from the requirement for concerted collective action(s) which, by contrast with individuals, have the knowledge and resources to (...) investigate outbreaks, and design and implement control measures. Individuals should not be the presumptive targets for the ascription of responsibility for outbreaks of infection. Indeed, our focus ought to be on the institutional obligations that fall upon public health providers as the primary agents with responsibility for assuring the concerted actions required to protect the public’s health. My concerns are about the phrase “the nature of imagery,” and the interpretation of findings with blind people. This discussion considers reports of imagery by congenitally totally blind people, and what should not be inferred from comparing efficiency levels of blind and sighted people in spatial tasks.

The Australian HREC system is now highly centralised and subject to national guidelines in an attempt to assure consistency of decision-making and of ethical standards. The penalty has been greatly increased paperwork and reporting requirements which many committees feel burdensome. In addition, many Committees may be under-resourced. However, I argue that though it is timely to reassess the current directions it is an exaggeration to claim that the system is in danger of collapse.

Much contemporary philosophy of religion is preoccupied with highly general problems about the nature of religious belief and of religious language, rather than with how to interpret, in detail, specific religious beliefs or forms of religious discourse. Among the matters of dispute there seem to be two of overriding importance. The first concerns the relation between religious beliefs and experience and centres on the question, what sorts of experience are relevant to the acceptance or rejection of religious beliefs. The second (...) concerns whether or not religious beliefs have an explanatory function. Discussion of both these themes in relation to theistic belief is still largely dominated by conceptions of God and of his relation to the world which have been developed by natural theologians, particularly, though not exclusively, those who have worked within traditions significantly influenced by Thomas Aquinas. Thus the idea that religious beliefs have an explanatory function is commonly associated with the view that they present answers to questions raised by those alleged traits or features of the world which have been the concern of natural theologians and which have been described by means of concepts of ‘contingency’, ‘purposiveness’, ‘order’, and ‘design’. Consequently, the sort of experience often held to be relevant to the acceptance or rejection of theistic belief is that which is relevant to the application of these problematic concepts. (shrink)
