Recently, Brukner and Zeilinger 3354) have claimed that the Shannon information is not well defined as a measure of information in quantum mechanics, adducing arguments that seek to show that it is inextricably tied to classical notions of measurement. It is shown here that these arguments do not succeed: the Shannon information does not have problematic ties to classical concepts. In a further argument, Brukner and Zeilinger compare the Shannon information unfavourably to their preferred information measure, I , with regard (...) to the definition of a notion of “total information content.” This argument is found unconvincing and the relationship between individual measures of information and notions of “total information content” investigated. We close by considering the prospects of Zeilinger's Foundational Principle as a foundational principle for quantum mechanics. (shrink)

PfizenmaierHistorians of Newton's thought have been wide ranging in their assessment of his conception of the trinity. David Brewster, in his The Life of Sir Isaac Newton (1831), was fully convinced that Newton was an orthodox trinitarian, although he recognized that "a traditionary belief has long prevailed that Newton was an Arian."1 Two reasons were used to defend his conclusion that Newton was (...) orthodox. The first was a letter from John Craig, a friend of Newton, written shortly after Newton's death to John Conduitt, the husband of Newton's niece. In this letter Craig remarked that Newton's theological opinions "were sometimes different from those which are commonly received" but that he hoped Conduitt would publish Newton's theological papers, "that the world may see that Sir Isaac Newton was as good a Christian as he was a mathematician and philosopher."2 The second reason with which Brewster defended his conclusion was his acknowledgment that the doctrine of the trinity itself had variations.I had no hesitation when writing the Life of Sir Isaac Newton in 1830, in coming to the conclusion that he was a believer in the Trinity; and in giving this opinion on the creed of so great a man, and so indefatigable a student of scripture, I was well aware that there are various forms of Trinitarian truth, and various modes of expressing it, which have been received as orthodox in the purest societies of the Christian Church.3L. He proceeded to say that Newton was not only an Arian but, because of the manner in which he understood Jesus' role as prophet, a Unitarian.5More recently the famous British economist John M. Keynes, who considered it a great "impiety" that the Portsmouth family's collection of Newton's non-scientific manuscripts were dispersed at the Sotheby's auction in 1936 and who recovered about half of them creating the "Keynes" collection at Cambridge, provided yet a third interpretation of Newton's thought.Very early in life Newton abandoned orthodox belief in the Trinity. At this time the Socinians were an important Arian sect amongst intellectual circles. It may be that Newton fell under Socinian influences, but I think not. He was rather a Judaic monotheist of the school of Maimonides. He arrived at this conclusion, not on so-to-speak rational or skeptical grounds, but entirely on the interpretation of ancient authority. He was persuaded that the revealed documents give no support to the Trinitarian doctrines which were due to late falsifications. The revealed God was one God.6Among contemporary scholars, the consensus is that Newton was an Arian rather than a Socinian.7 This view is clearly expressed by Richard [End Page 58] Westfall who writes, "Well before 1675, Newton had become an Arian in the original sense of the term. He recognized Christ as a divine mediator between God and man, who was subordinate to the Father who created him."8 Yet Frank Manuel, in his The Religion of Isaac Newton (1974), advises that we not try to push Newton into a theological categorization too quickly. He reminds us that Newton was, if nothing else, an original thinker.It is an error to seize upon his antitrinitrianism in order to pigeonhole him in one of the recognized categories of heresy—Arian, Socinian, Unitarian, or Deist.9So how did Isaac Newton conceive of the trinity? In exploring his writing on the subject we will be guided by two principles mentioned above. The first is Brewster's insight that the trinitarian doctrine has in fact been understood with a certain degree of variety within the sphere of orthodoxy. I defend Christopher Peacocke's and Robert Hopkins's experienced resemblance accounts of depiction against criticisms put forward by Gavin McIntosh in a recent article in this journal. I argue that, while there may be reasons for rejecting Peacocke's and Hopkins's accounts, McIntosh fails to provide any.

In the majority of academic institutions nursing and medical students receive a traditional education, the content of which tends to be specific to their future roles as health care professionals. In essence, each curriculum design is independent of each course. Over the last decade, however, interest has been accumulating in relation to interprofessional and multiprofessional learning at student level. With the view that learning together during their student training would not only encourage and strengthen future collaboration in practice settings but (...) also enhance patient care, the University of Dundee decided to run a pilot study to explore shared teaching in ethics between medical and nursing students. This article presents a report on the reasons for selecting health care ethics as a precursor for shared teaching, the educational tool used for the sessions, and the results of student and facilitator evaluation of the short course. Overall, despite problems such as poor attendance by some students, and facilitation and timetable difficulties, most of the feedback from students and facilitators has been positive. In essence the ‘idea’ has gone from strength to strength and there are now three levels of shared teaching in ethics between nursing and medical students, with plans to include further sessions with students from other disciplines. Within the text, ‘health care ethics’ will be referred to as ‘ethics’; nursing students/nurses encompasses midwifery students/midwives. (shrink)

This article critically explores the notion of those sociopolitical spaces that are 'disability', 'holism' and 'genetics', arguing from the perspectives of someone who identifies as having a disability. Medical genetics is seen to reflect the ideology and dominant biomedical reductionist thought. In contrast with this, it is proposed that disability and health are inherently social. A nursing approach is seen to recognize the social and holistic nature of the human person and to present a critical reflection on the reductionistic applications (...) of medical genetics. (shrink)

How do we know which institutions provide good care? Some scholars argue that the best way to think about care institutions is to model them upon the family or the market. This paper argues, on the contrary, that when we make explicit some background conditions of good family care, we can apply what we know to better institutionalized caring. After considering elements of bad and good care, from an institutional perspective, the paper argues that good care in an institutional context (...) has three central foci: the purpose of care, a recognition of power relations, and the need for pluralistic, particular tailoring of care to meet individuals? needs. Aside from satisfying technical requirements like statistical reliability, results yielded by such a test must conform to at least some accepted pretheoretical (...) desiderata; for example, determinations of competence, as measured by the test, must capture a minimal core of accepted basic intuitions about what competence means and what a theory of competence is supposed to do. The concepts of "face validity" and "content validity" are both important here. Face validity "indicates that an instrument appears to test what it is supposed to and that it is a plausible method for doing so" (Portney and Watkins 2000, 82). Content validity "means that the test contains all the elements that reflect the variable being studied" (Portney and Watkins 2000, 83). An adequate instrument for measuring mental competence must at minimum satisfy both of these requirements.What Tan and her colleagues (2006) have shown is that the MacArthur Competence Assessment Tool—Treatment (MacCAT-T) test for mental competence may not be an empirically valid measure of mental competence for some cases of anorexia. The data they report show that "competence," as measured by the MacCAT-T, does not conform sufficiently well with what is wanted of a clinical medical theory of competence. More specifically, the MacCAT-T is deficient in both face and content validity. This result is extremely significant; previous challenges to the validity of the MacCAT-T have often been largely theoretical in nature. My own objection that the MacCAT is inadequate because it omits the influence of emotion is based on a combination of empirical theory (Charland 1998a) and philosophical argument (Charland 1998b). However, it is entirely theoretical in nature. So is my argument that in ignoring emotion the MacCAT misses a crucial contributing ingredient in competence, namely, value (Charland 2001, see especially 136 note 2). Tan's study provides valuable empirical evidence for the second of these challenges, which deals more specifically with value. On the one hand, the study clearly demonstrates that patients sometimes do [End Page 283] base their treatment decisions on reasons derived from values. And on the other hand, it also nicely illustrates the centrality of values—"utilities" or "preferences"—in the decision-making process generally; a basic assumption of standard decision theory for which, paradoxically, there appears to be no satisfactory counterpart in the MacCAT-T model of reasoning and decision making.Thus, values figure among the reasons that patients actually use in reasoning when they weigh the risks and benefits of proposed treatment options. The MacCAT-T must be amended to incorporate this fact if it is to be a satisfactory clinical medical tool. Without such a correction, the concept of mental competence it measures is empirically invalid. At the root of the problem is a clash of intuitions. Tan's data provide convincing evidence that some of the patients who are deemed to be mentally competent by MacCAT-T standards should not in fact be considered competent. Note that the language in which this clash is expressed—"should not in fact"—involves a combination of descriptive and prescriptive elements, reminding us that the concept of competence is dual in nature: it is both prescriptive and descriptive (Freedman 1981). These problems underscore the need for greater clarity about what pretheoretically we want a clinical theory and test of competence to capture. It is of course reasonable to say that such a test must be consistent with major tenets of the relevant case law (Appelbaum 1998). But a legally valid concept of competence of this sort is too thin and arbitrary a foundation on which to build a clinical medical theory of competence. An empirically valid theory and test of the latter sort must incorporate the role of values if it is be truly helpful in medical practice.Value and the MacCAT-TI agree with Tan and her colleagues that some notion of "pathological value" needs to be... The mathematical concept of pragmatic truth, first introduced in Mikenberg, da Costa and Chuaqui (1986), has received in the last few years several applications in logic and the philosophy of science. In this paper, we study the logic of pragmatic truth, and show that there are important connections between this logic, modal logic and, in particular, Jaskowski's discussive logic. In order to do so, two systems are put forward so that the notions of pragmatic validity and pragmatic truth can be (...) accommodated. One of the main results of this paper is that the logic of pragmatic truth is paraconsistent. The philosophical import of this result, which justifies the application of pragmatic truth to inconsistent settings, is also discussed. (shrink)

Current challenges in medical practice, research, and administration demand physicians who are familiar with bioethics, health law, and health economics. Curriculum directors at American Association of Medical Colleges-affiliated medical schools were sent confidential surveys requesting the number of required hours of the above subjects and the years in which they were taught, as well as instructor names. The number of relevant publications since 1990 for each named instructor was assessed by a PubMed search.In sum, teaching in all three subjects combined (...) comprises less than two percent of the total hours in the American medical curriculum, and most instructors have not recently published articles in the fields they teach. This suggests that medical schools should reevaluate their curricula and instructors in bioethics, health law, and health economics. (shrink)

The fundamental problem on which Ilya Prigogine and the Brussels–Austin Group have focused can be stated briefly as follows. Our observations indicate that there is an arrow of time in our experience of the world (e.g., decay of unstable radioactive atoms like uranium, or the mixing of cream in coffee). Most of the fundamental equations of physics are time reversible, however, presenting an apparent conflict between our theoretical descriptions and experimental observations. Many have thought that the observed arrow of time (...) was either an artifact of our observations or due to very special initial conditions. An alternative approach, followed by the Brussels–Austin Group, is to consider the observed direction of time to be a basic physical phenomenon due to the dynamics of physical systems. This essay focuses mainly on recent developments in the Brussels–Austin Group after the mid-1980s. The fundamental concerns are the same as in their earlier approaches (subdynamics, similarity transformations), but the contemporary approach utilizes rigged Hilbert space (whereas the older approaches used Hilbert space). While the emphasis on nonequilibrium statistical mechanics remains the same, their more recent approach addresses the physical features of large Poincare systems, nonlinear dynamics and the mathematical tools necessary to analyze them. Human subject research involving brain imaging is likely to reveal signifcant incidental fndings of abnormal brain morphology. Because of this fact and because of the fduciary relationship between researcher and subject, board-certi-fed or board-eligible radiologists should review the scans to look for any abnormality, the scans should be conducted in accordance with standard medical practice for reviewing the clinical status of the whole brain, and the informed consent process should disclose the possibility that incidental fndings may be revealed and what (...) consequences will follow. In the event such fndings are revealed, qualifed physicians should explain to the subject the signifcance of the fndings and the alternatives available. (shrink)

In 2000, the Center for Disease Control and Prevention’s National Center for Environmental Health issued a report that explored some of the ways in which “sprawl” impacts public health. The report has generated great interest, and state health officials are beginning to discuss the relationship between land use and public health. The CDC report has also produced a backlash. For example, the Southern California Building Industry Association labeled the report “a ludicrous sham” and argued that the CDC should stick to (...) “fighting physical diseases, not defending political ones.”In retrospect, it is probably unfortunate that this report was funded by an organization called “Sprawl Watch.” “Spraw” is a word that has no clear meaning but is applied to a huge range of issues involving suburban development. But under the surface, as I think the papers in this special issue demonstrate, the civilized heat comes from a good deal of passion. One way in which philosophy can contribute to the debate is by making plain the sources of this passion, (...) so that the motivating forces are clearer.One motivating force is this: There will be people who do not have dementia but who have symptoms or signs that might predict dementia, which cause them concern, whom we might help by early intervention. The intervention might involve advice and support, or it might involve some form of drug treatment; and in the utopia of our dreams, it might involve prevention. All of this is very motivating for clinical scientists. It also offers a laudable motivation to pharmaceutical companies, who simultaneously must have regard to the possibility of profits for their shareholders.Here, however, is an alternative motivation: The effects of aging must not be ignored if help can be given, but older people must not be stigmatized by a medical process that thrusts disabling labels on them. People age, but they age differently, and it is better to recognize difference, both from a biological and from a philosophical perspective. Thus, a passionate commitment to the cause of individual agency in old age and a desire for authentic autonomy for older people provide strong motivation too.At root, then, the conflicting passions over MCI reflect the age-old struggle in the philosophy of psychiatry between the natural sciences (Naturwissenschaften) and the human or cultural sciences (Geisteswissenschaften): the tension is between a tendency to approach the problem with the aim of eliciting a scientific explanation and a tendency to try to achieve a human and cultural understanding of the phenomena (Phillips 2004). So, although MCI can be regarded as an explanatory diagnostic entity with some predictive value, it can also be regarded as a label that might cause harm or as a human phenomenon that requires understanding. Furthermore, MCI raises ontological questions about the nature of disease entities. And there is a wider issue as well about how aging itself should be regarded.From a biological perspective, it turns out there is no clear cutoff between what is normal and what [End Page 1] is not. Hence, in a number of ways, values seem to be as important as facts (Sadler 2005). Perhaps philosophy should content itself with saying no more than this: MCI ought not to be considered or debated as if it lies outside the realm of evaluative judgment. The very possibility of its existence as a clinical entity is a matter worthy of moral and social discourse.The direction of thought indicated here and in the papers that follow, however, is more radical than the easily won point that the "diagnosis" of MCI is not value free. On the back of the biological point, that there is a continuum between physiological and pathological aging, is the broader philosophical point that aging can neither be understood solely in terms of physiology nor in terms of pathology, nor even in terms of both. Ageing represents, inter alia, a social and cultural phenomenon manifest in the context of particular psychological and spiritual responses to a mixture of physiological adaptations and pathological predispositions. Without the broader picture, we tend to characterize old age, as Michael Bavidge has suggested elsewhere, in terms of deteriorations: "We should think of old age as offering alternative rather than impaired ways of experiencing life" (2006, 49). The difficulty is that any diagnosis seems to suggest impairment in one form or another. The radical suggestion, therefore, might be to commend descriptions that encourage shared understanding over and above the giving of names and labels.In a brief critique of the scientific approach to "banal memory disorders and MCI", Jean-Marie Léger, like Bavidge, regrets the extent to which people with cognitive impairment "are described in terms of what they have lost, with no regard for the wealth of faculties which are still present." What exactly is a genetic disease? For a phrase one hears on a daily basis, there has been surprisingly little analysis of the underlying concept. Medical doctors seem perfectly willing to admit that the etiology of disease is typically complex, with a great many factors interacting to bring about a given condition. On such a view, descriptions of diseases like cancer as geneticseem at best highly simplistic, and at worst philosophically indefensible. On the other hand, there is clearly some practical (...) value to be had by classifying diseases according to theirpredominant cause when this can be accomplished in a theoretically satisfactory manner. The question therefore becomes exactly how one should go about selecting a single causal factor among many to explain the presence of disease. When an attempt to defend such causal selection is made at all, the standard accounts offered (Koch's postulates, Hill's epidemiological criteria, manipulability) are all clearly inadequate. I propose, however, an epidemiological account of disease causation which walks the fine line between practical applicability and theoretical considerations of causal complexity and attempts to compromise between patient-centered and population-centered concepts of disease. The epidemiological account is the most basic framework consistent with our strongly held intuitions about the causal classification of disease, yet it avoids the difficulties encountered by its competitors. There is a story about the connection between the rise of consent theories of political obligation and the fall of natural law theories of political obligation that is popular among political philosophers but nevertheless false. The story is, to put it crudely, that the rise of consent theory in the modern period coincided with, and came as a result of, the fall of the natural law theory that dominated during the medieval period. Neat though it is, the story errs doubly, (...) for it supposes both that consent did not play a key role in natural law theories of political authority offered in the medieval period (a supposition falsified by close inspection of the view of Aquinas, perhaps the paradigmatic natural law theorist) and that natural law theory did not play a key role in the consent theories of political authority offered in the modern period (a supposition falsified by close inspection of the views of Hobbes and Locke, perhaps the paradigmatic consent theorists).Footnotes* I owe thanks to Pat Káin, Paul Weithman, Bob Roberts, and Henry Richardson for instructive criticisms. Public health emergencies have occurred throughout history, encompassing such events as plagues and famines arising from natural causes, disease pandemics interrelated with wars, and industrial accidents such as the 1986 Chernobyl disaster, among others. Law and legal tools have played an important role in addressing such emergencies. Three prime U.S. examples are Congressional authorization of quarantine as early as 1796, legally mandated smallpox vaccination upheld in a landmark 1905 U.S. Supreme Court ruling, and the President's 2003 executive order adding SARS (...) to the federal government's list of “quarantinable communicable diseases.”The public health emergencies of the present — both actual and potential — pose equally serious threats but do so in the context of greatly magnified expectations that stem directly from the attacks of September 11, 2001, and the immediately following anthrax attacks. Persons think. Bodies, time-slices of persons, and brains might also think. They have the necessary neural equipment. Thus, there seems to be more than one thinker in your chair. Critics assert that this is too many thinkers and that we should reject ontologies that allow more than one thinker in your chair. I argue that cases of multiple thinkers are innocuous and that there is not too much thinking. Rather, the thinking shared between, for example, persons and their bodies is (...) exactly what we should expect at the intersection of part sharing and the supervenience of the mental on the physical. I end by responding to the overcrowding objection, the personhood objection, the personal-pronoun reference problem and the epistemic objection. (shrink)

We formulate Suppes predicates for various kinds of space-time: classical Euclidean, Minkowski's, and that of General Relativity. Starting with topological properties, these continua are mathematically constructed with the help of a basic algebra of events; this algebra constitutes a kind of mereology, in the sense of Lesniewski. There are several alternative, possible constructions, depending, for instance, on the use of the common field of reals or of a non-Archimedian field (with infinitesimals). Our approach was inspired by the work of Whitehead (...) (1919), though our philosophical stance is completely different from his. Physicians have long aided their patients in dying in an effort to ease human suffering. It is only in the nineteenth and twentieth centuries that the prolongation of life has taken on new meaning due to the powers now available to physicians, through new drugs and high technology interventions. Whereas earlier physicians and patients could readily acknowledge that nothing further could be done, today that judgment is problematic.Most often, aiding the dying took the form of not doing anything further to (...) prolong life. Morally, this act of restraint was not difficult, because few options could have been considered. However, some physicians, rarely, would help patients die either by hastening death directly or by increasing pain control with the risk of hastening death, the so-called double effect. When directly hastening death, physicians either directly caused the death of the patient through euthansia, or supplied the means to the patient to bring about death with or without encouragement to use these means. The latter is physician-assisted suicide. (shrink)

Libertarians typically object to having the state deal with law and order for several general reasons: it is inefficient; it is carried out at the expense of taxpayers; and it punishes so-called victimless crimes. Exactly what the observance of liberty implies with respect to the treatment of tortfeasors and criminals is more controversial among libertarians. A pure theory of libertarian restitution and retribution is mainly what is attempted here, without becoming involved in general moral anti-state arguments. However, the pure theory (...) alone will raise practical problems that require immediate response if the theory is to appear at all plausible. (shrink)

De Interpretatione is among Aristotle's most influential and widely read writings; C. W. A. Whitaker presents the first systematic study of this work, and offers a radical new view of its aims, its structure, and its place in Aristotle's system. He shows that De Interpretatione is not a disjointed essay on ill-connected subjects, as traditionally thought, but a highly organized and systematic treatise on logic, argument, and dialectic.

Richard Jeffrey and Michael Goldstein have both introduced systematic approaches to the structure of opinion changes. For both approaches there are theorems which indicate great generality and width of scope. The main questions addressed here will be to what extent the basic forms of representation are intertranslatable, and how we can conceive of such programs in general.
