BackgroundDepressive disorders are a common form of psychiatric illness and cause significant disability. Regulation authorities, the medical profession and the public require high safety standards for antidepressants to protect vulnerable psychiatric patients. Ketamine is a dissociative anaesthetic and a derivative of a hallucinogen. Its abuse is a major worldwide public health problem. Ketamine is a scheduled drug and its usage is restricted due to its abuse liability. Recent clinical trials have reported that ketamine use led to rapid antidepressant effects in (...) patients suffering from treatment-resistant depression. However, various flaws in study designs, and possible biased reporting of results, may have influenced those findings. Further analyses of ketamine use are needed to ensure patient safety.DiscussionThe use of ketamine in research and treatment of depressive disorders is controversial. Recently, mental health professionals raised ethical concerns about an ongoing ketamine trial in the UK. Also, a Canadian agency reviewed the existing evidence and did not recommend prescribing ketamine to treat depressive disorders. Findings obtained from tightly controlled research settings cannot be easily translated to clinical practice as substance abuse is commonly comorbid with depressive disorders. An effective antidepressant should reduce severity of depressive symptoms without liability problems. Although the US FDA has not approved the use of ketamine to treat depressive disorders, some psychiatrists offer off-label repeat prescription of ketamine. Prescribing ketamine for treating depressive disorders requires substantial empirical evidence. Clinicians should also consider research findings on ketamine abuse. Depressive disorders can be chronic conditions and the current evidence does not rule out the risk of substance abuse after repeat prescription of ketamine. Off-label ketamine use in treating depressive disorders may breach ethical and moral standards, especially in countries seriously affected by ketamine abuse. This article presents two real-world clinical vignettes which highlight ethical principles and theories, including autonomy, nonmaleficience, fidelity and consequentialism, as related to off-label ketamine use.ConclusionWe urge clinicians to minimise the risk of harming patients by considering the empirical evidence on ketamine properties and attempting all standard antidepressant therapies before considering the off-label use of ketamine. We are big fans of propositions. But we are not big fans of the proposed by Mitchell et al. The authors ignore the critical role played by implicit, non-inferential processes in biological cognition, overestimate the work that propositions alone can do, and gloss over substantial differences in how different kinds of animals and different kinds of cognitive processes approximate propositional representations.

In Mills’ sociological analysis, a central notion is the ‘social milieu’ which encapsulates ‘the social setting of a person that is directly open to his personal experience’. For Mills, sociology should entail an investigation of the set of relations and practices that are a feature of human experience. Understanding the significance of Mills’ approach, we argue, requires grasping the way the notion of ‘milieu’ or ‘setting’ itself draws upon spatial and topological notions – notions that have become prominent in much (...) contemporary sociological thinking. From this perspective, Mills’ work turns out to be relevant as a corrective, both to the undue emphasis on empirical particularity that is evident in some contemporary sociology and to what Mills viewed as ‘abstract’ theorisation. A large part of the relevance of Mills’ work for contemporary sociological problems and challenges is thus to be found in the way his emphasis on situation (or ‘place’), as given through the idea of ‘milieu’, allows a more complex and encompassing approach. Work has become so de-skilled and fragmented that the Marxian working class has been decisively replaced by a " non-class of non-workers." Work, now thoroughly absorbed and shaped by technocratic industry, is beyond hope of humanization. The radical program he recommends for the " non-class" is to maximize leisure which is already a realizable possibility within the technocratic economy. With that pronouncement, whatever dignity homo faber may have possessed is lost as he vanishes into the depths of a selfreproducing technocratic order.2 In this paper I wish to examine an alternative to Marxism for approaching work as a subject worthy of philosophical attention. \Vhile classical and scholastic thought may not appear at first to be promising grounds on which to consider the topic, the work of two contemporary Thomists-Jacques Maritain and Joseph Pieper-offers a significant and unique contribution in this area. However, I hesitate to say that either of these thinkers has accomplished the construction of a full-scale philosophy of work.3 In all fairness, that is not the intent of either one of them. This paper is an extended exploration of Mead's phrase the emergence of the novel. I describe and characterize emergent systems-complex dynamical systems that display behavior that cannot be predicted from a full and complete description of the component units of the system. Emergence has become an influential concept in contemporary cognitive science [A. Clark Being there, Cambridge: MIT Press], complexity theory [W. Bechtel & R.C. This book is part of Prentice-Hall's new Central Issues in Philosophy series, and seems a welcome addition. The editor's introduction does little more than state the problem and review some of the ways with which it has been dealt. We are then brought immediately to the meat: the first section of the book contains selections from Descartes, Spinoza, and Hobbes intended to acquaint us with some of the more classical solutions to the problem. The second part, entitled "The Identity Thesis," (...) contains one of J. J. C. Smart's influential essays for this thesis and two replies, one by Jerome Shaffer and one by James Cornman. Part three, entitled "Theoretical Materialism," contains essays by Jaegwon Kim, in which he denies the validity of having only physical property descriptions; and Thomas Nagel and Keith Gunderson, who both argue that the case for materialism is stronger than the case against it. The fourth part, containing essays by Jerry Fodor, Hilary Putnam, and David Lewis, focuses on functional materialism. The fifth section of the book is devoted to the rather radical view of the eliminative materialists. There is a short note by Paul Feyerabend, a full length essay by Richard Rorty, a reply by Richard Bernstein and a rejoinder by Rorty. All of the essays in this book are well-written pieces of solid philosophy, but none is so technical that it can not be understood by a relative beginner. After finishing this book and thoroughly digesting its contents, the reader will be well versed in modern thought on this problem and well capable of continuing his research in any of several directions. The goal of this paper is to present a succinct overview, from a clinician’s perspective, of the importance and implications of research on heart failure in African Americans. It first gives a brief outline of the rationale and results of the African-American Heart Failure Trial, which showed evidence for the effectiveness of fixed-dose combination of isosorbide dinitrate and hydralazine, marketed as BiDil, in this population. Finally, it underscores the necessity of treating African Americans with evidence-based medicine given that humanistic physicians (...) and other clinicians strive to provide holistic treatment to their patients.Most researchers and clinicians recognize the difficulty with utilizing race as a category for inclusion in trials. While race lacks any true biologic definition, it is potentially beneficial as a consideration in clinical studies, based on prior evidence of certain potential differences across self-identified racial/ethnic groups in morbidity and mortality and responses to medicine. Even though use of race as a classification is a problematic distinction, it should not be ignored. If clinical trials demonstrate significant improvement with targeted treatment in African Americans, clinicians should not withhold life-saving drugs that result from such studies. (shrink)
