Background Inadequate knowledge among health care providers (HCPs) and parents of affected children limits the understanding and utility of secondary genetic findings (SFs) in under-represented populations in genomics research. SFs arise from deep DNA sequencing done for research or diagnostic purposes and may burden patients and their families despite their potential health importance. This study aims to evaluate the perspective of both groups regarding SFs and their choices in the return of results from genetic testing in the context of orofacial (...) clefts.Methods Using an online survey, we evaluated the experiences of 252 HCPs and 197 parents across participating cleft clinics in Ghana and Nigeria toward the return of SFs across several domains.Results Only 1.6% of the HCPs felt they had an expert understanding of when and how to incorporate genomic medicine into practice, while 50.0% agreed that all SFs should be returned to patients. About 95.4% of parents were willing to receive all the information from genetic testing (including SFs), while the majority cited physicians as their primary information source (64%).Conclusions Overall, parents and providers were aware that genetic testing could help in the clinical management of diseases. However, they cited a lack of knowledge about genomic medicine, uncertain clinical utility, and lack of available learning resources as barriers. The knowledge gained from this study will assist with developing guidelines and policies to guide providers on the return of SFs in sub-Saharan Africa and across the continent. (shrink)

Dr Sydney Brenner has played a major, and unique, role in biology during the past 40 years. His contributions have ranged from key work on the structure of the genetic code and the existence of mRNA through the development of Caenorhabditis elegans as a key model system in developmental biology to genomic analysis and function in vertebrates. While the benefits of both an external focus of attention (FOA) and of a longer quiet eye (QE) duration have been well researched in a wide range of sporting activities, little is known about the interaction of these two phenomena and how a potential interaction might influence performance. It was this study’s aim to investigate the interaction and potential effect on performance by using typical FOA instructions in a dart throwing task and examining both the QE and performance outcome. The (...) results replicate neither the benefit of an external FOA nor the benefit of a longer QE duration. However, an interaction was observed, as QE was prolonged by an earlier onset and later offset in the internal focus condition only. As the typical effect of a performance benefit due to an external focus could not be replicated, the interaction must be interpreted with caution. The results are discussed and interpreted in light of the inhibition hypothesis and possible avenues for future research are suggested. (shrink)

In Physical Realization, Sydney Shoemaker considers the question of how physicalism can be true: how can all facts about the world, including mental ones, be constituted by facts about the distribution in the world of physical properties? Physicalism requires that the mental properties of a person are 'realized in' the physical properties of that person, and that all instantiations of properties in macroscopic objects are realized in microphysical states of affairs. Shoemaker offers an account of both these sorts of (...) realization, one which allows the realized properties to be causally efficacious. He also explores the implications of this account for a wide range of metaphysical issues, including the nature of persistence through time, the problem of material constitution, the possibility of emergent properties, and the nature of phenomenal consciousness. Sydney Shoemaker is one of the most influential philosophers currently writing on philosophy of mind and metaphysics. Any property has two sorts of causal features: “forward‐looking” ones, having to do with what its instantiation can contribute to causing, and ldquo;backward‐looking” ones, having to do with how its instantiation can be caused. Such features of a property are essential to it, and properties sharing all of their causal features are identical. Causal necessity is thus a special case of metaphysical necessity. Background Diversity in Institutional Review Board (IRB) membership is important for both intrinsic and instrumental reasons, including fairness, promoting trust, improving decision quality, and responding to systemic racism. Yet U.S. IRBs remain racially and ethnically homogeneous, even as gender diversity has improved. Little is known about IRB chairpersons’ perspectives on membership diversity and barriers to increasing it, as well as current institutional efforts to promote diversity, equity, and inclusion (DEI) within IRB membership.Methods We surveyed IRB chairpersons leading U.S. boards registered (...) with the Office for Human Research Protections. Here, we focus exclusively on responses from a subset of 388 chairpersons of IRBs at universities and academic medical centers (AMCs).Results Board chairs were predominantly white and evenly split between men and women. Only about half reported that their boards had at least one member who is Black or African American (51%), Asian (56%), or Hispanic (48%), with 85% of university/AMC boards comprised entirely (15%) or mostly (70%) of white members. Most IRB chairpersons (64%) reported satisfaction with the current diversity of their membership. Participants largely agreed that considering diversity in the selection of IRB members is important (91%), including to improve the quality of IRB deliberation (80%), with an emphasis on racial/ethnic (85%) and gender diversity (74%). Most participants (80%) reported some type of active DEI effort regarding board membership at their university/AMC and just over half (57%) expressed satisfaction with these efforts.Conclusions Our national survey found that although university/AMC IRB chairpersons report valuing diversity in board membership, it may be lacking in key areas. Going forward, it will be important to specify clear reasons for diversity in the IRB context, as well to establish targets for acceptable levels of board diversity and to match DEI efforts to those targets. This paper explores a philosophical argument about the use of natural light in films. I argue that directors use sunlight as a visual metaphor to induce a sublime experience from the viewer to elevate the narrative. While it is more efficient in terms of time management and finances to use electric lights, sunlight creates a successful emotional effect on the viewer placing them in contemplation of the relationship between humanity, nature, and humility. Essentially, I am concerned with what it is (...) that the capturing of sunlight accomplishes in these works of art that a light bulb can’t. In this paper, I consider how natural light is used to guide the viewer’s eye and how natural light makes us feel. I believe motion pictures are the perfect artistic medium to explore our emotional understanding of the sun as it not only captures objects in motion in the light of the sun it also captures the sun itself in motion. My intention is to provide a better understanding of the powerful psychological effect that the use of sunlight has on the viewer of a film. (shrink)

Despite the importance of Suárez’s defense of the freedom of the will at the threshold of early modern philosophy, his account has received scant recent attention. This paper aims partially to redress that neglect. Suárez’s position can be understood as a balancing act between desiring to attribute libertarian freedom to agents and desiring to maintain the will’s status as a rational appetite. Hence, he rejects an intellectualism that says that choices are necessitated by the intellect’s judgements (since he does not (...) think that the judgements themselves can be directly free), but affirms that only what is judged good can be chosen. Two kinds of epistemological sceptical paradox are reviewed and a shared assumption, that warrant to accept a proposition has to be the same thing as having evidence for its truth, is noted. 'Entitlement', as used here, denotes a kind of rational warrant that counterexemplifies that identification. The paper pursues the thought that there are various kinds of entitlement and explores the possibility that the sceptical paradoxes might receive a uniform solution if entitlement can be made to reach sufficiently far. Three (...) kinds of entitlement are characterized and given prima facie support, and a fourth is canvassed. Certain foreseeable limitations of the suggested antisceptical strategy are noted. The discussion is grounded, overall, in a conception of the sceptical paradoxes not as directly challenging our having any warrant for large classes of our beliefs but as crises of intellectual conscience for one who wants to claim that we do. This paper aims to address when the wise person should participate in politics. The question is addressed through engagement with the Analects. Rather than provide interpretations of key terms in the Analects, we provide an account of wisdom that draws from themes in the Analects. The case is made that the wise person is committed to participating in politics primarily because of the connection between wisdom and benevolence (ren 仁 in the Analects). We address challenges to the Confucian approach from (...) the Zhuangzi concerning the potential harms of political participation. General political disengagement is not considered exemplary on the Confucian approach to wisdom, although when political engagement may be eschewed is explored. Finally, the case is made that while ren motivates the wise to participate politically, the wise also have a sense of timeliness that provides guidance as to when to refrain or withdraw from political participation. Clinical red blood cell transfusion guidelines have been widely adopted in clinical practice, resulting in standardized transfusion practices in hospitalized patients with anemia. Standardization of transfusion practice has been welcomed by clinicians and health systems as a mechanism for reducing unnecessary, harmful, and costly practice variation that results in healthcare disparities. However, overzealously applied guidelines can have deleterious consequences for individual patients, ultimately resulting in and/or exacerbating healthcare disparities, rather than resolving them. This article provides empirical examples of the adverse (...) consequences from the well-meaning attempt to standardize transfusion practice based on clinical practice guidelines and discusses the ethical implications of standardized transfusion practice. […] One view I hold about the nature of phenomenal character, which is also a view about the relation between phenomenal character and the introspective belief about it, is that phenomenal character is “self intimating.” This means that it is of the essence of a state’s having a certain phenomenal character that this issues in the subject’s being introspectively aware of that character, or does so if the subject reflects. Part of my aim is to give an account which makes (...) it intelligible that this should be so. A more substantive view I hold about phenomenal character is that a perceptual state’s having a certain phenomenal character is a matter of its having a certain sort of representational content. This much I hold in common with a number of recent writers, including Gil Harman, Michael Tye, Bill Lycan, and Fred Dretske. But representationalism about phenomenal character often goes with the rejection of “qualia,” and with the rejection of the possibility of spectrum inversion and other sorts of “qualia invesion.” My version of representationalism embraces what other versions reject. It assigns an essential role to qualia, and accepts the possibility of qualia inversion. A central aim of the present paper is to present a version of this view which is free of the defects I now see in my earlier versions of it. (shrink)

Thanks to Barbeyrac, Pufendorf and others, there is a long-familiar picture of Grotius as offering a groundbreaking account of natural law. By now there is also a familiar observation that there is no agreement what makes Grotius’s account innovative. Sometimes this leads to skepticism about how innovative Grotius’s account of natural law really is. Some scholars suggest that Grotius’s account of natural law resembles Suárez’s account. But others continue to argue that Barbeyrac is right to see Grotius as breaking the (...) ice of previous philosophy and laying the groundwork for a distinctively modern moral philosophy. I plan to contribute to the debate by arguing that, properly understood, Grotius’s position is similar to Suárez’s on a range of fundamental questions, and, furthermore, that seeing Grotius as making a radical break with the past violates his own self-conception. 1.
